Agency: National Highway Traffic Safety Administration I am submitting a complaint regarding repeated denial of access to charging infrastructure for my electric vehicle due to fraudulent billing activity within Tesla’s Supercharger network. I own a 2014 Tesla Model S located in Alabama. My Tesla account has repeatedly recorded Supercharging sessions in California that were not performed by my
... vehicle. Tesla customer support representatives have acknowledged that this issue appears to involve VIN cloning or a flaw in their charging identification system. Because I blocked my credit card to prevent fraudulent charges, Tesla’s system repeatedly flags my account as having unpaid balances and automatically disables my vehicle’s access to the Supercharger network until those balances are resolved. This creates a situation where my vehicle can be denied access to charging infrastructure even though the charges are fraudulent and unrelated to my vehicle’s actual activity. The issue has occurred multiple times over several months and requires repeated calls to Tesla customer support to restore charging functionality. Loss of access to charging infrastructure can significantly impact the usability and operational reliability of an electric vehicle. I am requesting that this issue be investigated as a potential defect in the Supercharger billing or authentication system that could affect multiple vehicle owners.Read more

The Belt Integrated Starter Generator (BISG) on my 2023 Ford F-150 PowerBoost intermittently fails, triggering DTC P0A1E-00 and causing complete loss of electric drive mode during normal driving without warning. Vehicle continues running but with significantly reduced fuel efficiency until manually restarted. Fault occurrences are escalating in frequency: 9/13/2025 while running ProPower Onboard u
... nder medium load camping in the Everglades, 11/20/2025, then 3/3/2026, 3/6/2026, 3/10/2026, and 3/12/2026 — two occurrences within one hour during morning commute — seven total occurrences. All documented via Ford App push notifications. Sam Galloway Ford in Fort Myers, FL confirmed DTC P0A1E-00 on 3/4/2026. Repair estimated at $1,900 with part backordered minimum three months. I was charged $266.20 for the diagnostic. Ford Motor Company denied my warranty claim stating the part is not a hybrid unique component despite P0A1E-00 being a hybrid-specific fault code and Ford's own TSB 23-2279 covering this exact condition under warranty. I called Ford customer service 4 times 3/10/2026 and was told my claim ticket was closed on 3/4/2026, the same day I opened it without technical explanation. No one has returned my calls since and I was not provided written denial documents. A BBB complaint has been filed.Read more

My 2022 Honda Odyssey experienced a sudden catastrophic engine failure at approximately 154,000 miles while driving under normal conditions. The failure involved the vehicle’s engine and resulted in a loud knocking noise followed by loss of engine power and the vehicle becoming inoperable. The Honda dealership diagnosed the engine as having internal engine failure with metal contamination prese
... nt in the oil, indicating severe internal engine damage. Based on the symptoms and the diagnosis, the failure appears consistent with possible connecting rod or connecting rod bearing failure. The failed engine remains installed in the vehicle and should be available for inspection upon request. Prior to the failure there were no warning lights or messages, and the oil pressure warning light never illuminated. The vehicle had proper oil levels and no indication of low oil pressure before the failure occurred. A sudden internal engine failure that causes loss of engine power while driving presents a safety concern because it can occur without warning and may leave the driver unable to maintain speed or safely maneuver in traffic, particularly at highway speeds. The problem has been confirmed by a Honda dealership, which documented the condition as internal engine failure with metal contamination in the oil and advised that the engine must be replaced. The estimated repair cost exceeds $6,000. The manufacturer has been notified of the failure through a customer assistance case, but the vehicle has not been physically inspected by the manufacturer, police, or insurance representatives. This complaint is being submitted to document the sudden internal engine failure and possible connecting rod bearing failure in case similar failures are occurring in other vehicles equipped with the same engine design.Read more

Vehicle shut-off while driving on the expressway, around 70-75mph. Upon vehicle turning off, I had no option but to "coast" off to the left side, which thankfully, freed me from traffic for the most part. If anyone who have been a foot over, they would've hit me. I have had numerous electrical issues with this vehicle and have taken it to the dealership but they have been "unable" to duplicate the
... issue. Vehicle was immediately taken to the dealership (and they still have as of this incident report submission). They told me last night that they will keep it again today and test drive it again to see if they can get it to do anything but if it doesn't show anything new, they have nothing to diagnose and will give me my car back. After requesting a copy of repair orders and a list of the history codes in my car, I am beside myself that anyone thinks giving me this vehicle back to drive is ok. See the history code report in documents uploaded. These are MAJOR safety failures in a large vehicle.Read more

I am writing to formally report repeated and serious defects with my 2024 Volkswagen Atlas, VIN [XXX] , which I leased in California. These defects occurred well within the vehicle’s first year and while under factory warranty. Brake System Defect At 8,405 miles, the vehicle received its 10,000-mile maintenance. Dealer performed a “one-time goodwill repair” on the 1-year in-service date, replacing pads and rotors, but refused to document the failure as a warranty-covered defect. This premature failure at 13k miles is not normal wear and reflects a serious manufacturing/material defect that created a safety hazard. Engine and Oil Issues At 13,175 miles, the oil light came on and the vehicle was found to be low on oil despite proper service at 8,405 miles. At 8,405 miles the oil light also came on showing low to no oil. Intermittent Warning and Electrical Issues Throughout this period, the vehicle has experienced recurring warning messages and system malfunctions, including: “Error: Dynamic Road Sign Display” “Lane Assist Not Available” “Front Assist Has Limited Availability” TPMS issues, where displayed tire pressure did not match actual pressure, and the light remained on for days despite correct tire pressures being confirmed. Additional Warranty Repairs At 9,943 miles, the radio required a software repair under warranty. Paint defect required repair under warranty. Summary: In less than 14,000 miles, this vehicle has experienced: Critical brake system failure (pads separating, rotor damage). Engine oil loss triggering warning light. Please be advised that I have been in contact with my local Ford Dealer located in Montrose, CO on numerous occasions in an attempt to have there service department make the necessary repairs to my 2022 Ford F-150, most recently on January 2, 2026. I have been instructed that parts are still not available for them to fix the recall on my truck. It has been (8) months ago that I received an initial recall notice from Ford Motor Co. A few weeks ago I called the Ford Recall Assistance Center at 1-866-436-7332 in hopes of getting a resolution and timeline for a repair work to be completed on my Ford F-150. Unfortunately, this too was simply a waste of my time and energy as nothing was done to complete the necessary recall work to my vehicle. This has been extremely frustrating. I recognize there are safety issues that make it dangerous to drive my Ford-150 without the necessary recall repair work being completed. My wife and I rely solely on our F-150 for transportation as we are a single vehicle family and we are concerned about a potential future accident or vehicle breakdown. It is difficult for me to comprehend how Ford Motor Company can be so negligent and incompetent in getting this recall notice addressed in a timely fashion. In my opinion, Ford Motor Co.’s inaction in dealing with my F-150 recall is nothing short of being totally unacceptable. Thank you in advance for your consideration and assistance in this matter. I am reporting a serious safety issue with my 2021 Ford F-150. This truck has already been subject to multiple recalls, yet the problems have continued. On several occasions while driving, the truck has suddenly shut off without warning, causing a complete loss of power while the vehicle is in motion. This has happened while my family and I were in the vehicle and has nearly resulted in a serious
... crash. Losing power in traffic without warning creates an extremely dangerous situation and has put my family’s lives at risk. A vehicle shutting off while being driven is a major safety hazard. Despite multiple visits for service and ongoing recall issues, the problem has not been resolved, and the truck continues to fail in a way that could easily lead to a catastrophic accident.Read more
