The contact's daughter owns a 2021 GMC Yukon. While the owner was driving at an undisclosed speed, there was an abnormal ticking sound coming from the engine compartment. The sound was intermittent but frequent. The vehicle stalled intermittently with several unknown warning lights illuminated. The vehicle was taken to an independent mechanic, who advised that the vehicle needed to be taken to a d

The contact researched online and related the failure to NHTSA Campaign Number: 25V274000 (Engine and Engine Cooling); however, the VIN was not included. The manufacturer was informed of the failure and confirmed that the VIN was not associated with the recall. The failure mileage was approximately 154,000.Read more

My vehicle was inspected at an authorized Infiniti dealership under an active recall. During that inspection, metal debris and loose bolts were found in the oil pan, indicating internal engine damage consistent with the recall condition. Despite this, Infiniti declined to repair or replace the engine. the engine failed completely while I was driving at highway speed, causing sudden loss of power
... and creating a serious safety hazard. I was left stranded in active traffic conditions, placing myself and others at risk. This incident raises significant safety concerns, including: Failure of the manufacturer to properly remedy a known recall condition, Continued operation of a vehicle with a known defect, Risk of sudden engine failure during normal driving conditions. Additionally, I did not receive timely notification of the recall. I should have been notified months earlier but only received notice on February 10, 2026. I have documentation from the dealership confirming the presence of metal debris and bolts in the engine, as well as photographic evidence of the damage. I am concerned that other vehicles with this defect may not be properly repaired, potentially leading to similar dangerous failures. Infiniti’s refusal to address a clearly defective engine identified during a recall inspection represents a serious safety issue that warrants investigation. I respectfully request that NHTSA review this matter and determine whether Infiniti is complying with its obligations to properly remedy recall-related defects.Read more

On March 5, 2026, while I and my 2 passengers were traveling on the 405 Freeway at approximately 40–50 mph, the vehicle suffered a catastrophic total loss of power. The engine shut off and all electronic systems failed simultaneously, including the instrument cluster and all interior lighting. Most critically, the hazard lights were inoperable, leaving the vehicle invisible to high-speed traffic
... during the failure. Since I was traveling at highway speeds I was able to safely glide the vehicle to the center shoulder. After approximately 10–15 minutes, the dashboard, infotainment, and climate control systems began flickering intermittently before power temporarily returned. Upon attempting to re-enter traffic to reach an exit, the vehicle stalled a second time while in motion. I returned to the shoulder again. After another 10 minutes the vehicle started and I was eventually able to exit the freeway and reach my residence. The following morning, the vehicle was completely unresponsive and would not start. It was towed to Land Rover Santa Monica dealership on March 6, 2026, where it currently remains for diagnosis and repair. This incident on the 405 freeway marks the third occurrence of a total loss of propulsion and electrical failure while the vehicle was in motion. Despite previous repair attempts, the defect persists, rendering the vehicle fundamentally unsafe for operation. I am gravely concerned for my personal safety, as well as the safety of other motorists on public streets and highways. A vehicle that spontaneously stalls at high speeds—while simultaneously losing the ability to activate hazard lights—presents an unacceptable risk of a high-speed rear-end collision. Given that this is a recurring, catastrophic safety failure, I am requesting an immediate and permanent resolution, as the vehicle in its current state is a liability to public safety and unsafe to be driven by me or anyone.Read more

Vehicle: MINI Cooper Clubman (Model R55) VIN: [XXX] Recall: NHTSA 23V-337 Dealer: Flow MINI of Raleigh, NC 1. COMPONENT FAILED: The Junction Box Electronics (JBE) failed due to moisture/water intrusion, producing electrical fault codes across all core systems including airbags, brakes, and a JBE communication fault. The vehicle is located at Flow MINI of Raleigh and is available for inspection.
... 2. SAFETY RISK: Recall 23V-337 identifies that moisture intrusion into the JBE can lead to corrosion, short circuit, and in rare cases a thermal event (fire). My vehicle's carpet is wet, confirming active water intrusion into the footwell where the JBE is installed. Airbag and brake faults appear exclusively when it rains or when the vehicle is washed, directly and unambiguously linking all symptoms to water intrusion. The vehicle is currently unsafe to operate. 3. DEALER CONFIRMATION: Flow MINI of Raleigh confirmed moisture/water intrusion is causing the JBE faults — directly reproducing the failure during a dealer-performed wash. The dealer declined the recall repair solely because [XXX] is not listed among covered states. 4. MANUFACTURER RESPONSE: MINI USA denied coverage exclusively on the basis that North Carolina is not included in the recall's geographic region. The defect itself has not been disputed. 5. BASIS FOR COMPLAINT: The recall's geographic restriction was established based on road salt usage. North Carolina roads do receive road salt treatment during winter weather events. The recall documentation (RCRIT-23V337-7125.pdf, Page 4, Item 12) explicitly names the JBE as a covered component. I request that NHTSA: (1) treat this as urgent given confirmed airbag and brake faults, (2) review whether NC should be included in the recall's geographic scope, and (3) compel MINI USA to perform the recall repair on VIN [XXX] at no cost. I am writing to file a formal safety complaint against Tesla, Inc. regarding a software update failure that has disabled legally required vehicle functions on my Tesla for over two years. For more than 26 months, my vehicle has been unable to complete a software update through Tesla's over-the-air (OTA) update system. As a direct result of this failure, my vehicle's display screen does not functi
... on for the first several minutes of every drive. This prevents me from accessing my speedometer, turn signals, and other legally required safety features, rendering the vehicle non-compliant with federal and state motor vehicle safety standards during this period. I have made multiple documented attempts to resolve this issue through Tesla's customer support channels over the course of these two years. Despite these efforts, the issue remains unresolved. Tesla has now informed me that resolving the defect will require a paid service visit — despite the fact that the failure originates within Tesla's own software update infrastructure, not from any action or negligence on my part. I am requesting that NHTSA investigate this matter on the following grounds: 1. The software defect disables legally required vehicle safety functions, including the speedometer, turn signals, hazard lights, and more. 2. The defect has persisted for over two years despite repeated good-faith efforts by the vehicle owner to resolve it through the manufacturer. 3. Tesla's OTA update system is manufacturer-controlled infrastructure. A failure within that system constitutes a manufacturer defect, not a maintenance issue. 4. Tesla is requiring payment from the consumer to correct a defect in their own systems. I believe this issue may not be isolated to my vehicle and could affect other Tesla owners experiencing similar OTA update failures. I respectfully request that NHTSA investigate whether a broader safety defect exists and whether a recall or corrective action is warranted.Read more

My vehicle constantly receives an error message while driving stating, "SERVICE HIGH VOLTAGE SYSTEM" and "REDUCED ACCELERATION DRIVE WITH CARE." Error symbols appear on the dashboard that show an orange turtle, a red battery with a big exclamation point through it and and a red car outline with a big exclamation point going through the middle of it. When this happens, the vehicle acceleration is
... significantly reduced and the regenerative braking or "one-pedal" feature automatically becomes disabled. If you're relying on that system to brake, it will not do so when you need it to and you have to manually press the brake to slow and stop the vehicle. This error clears after a few days of driving but never fails to return. The dealer said they found nothing wrong when I brought it in (when the error wasn't being displayed, of course) but it went back today with the error displayed so they could witness it for themselves. Separately, the vehicle makes an incredibly loud knocking noise when accelerating with the wheel turned. It appears this is coming from the CV joints.Read more

Due to the design of the grill located at the front end of the 2025 toyota grand highlander the vehicle poses vulnerability for multiple debris items to fly up into exposed air conditioning condensor. The damage caused by the debris is not covered by the warranty because it is considered an outside occurrence. However the design creates the vulnerability for the ac to become damaged. It is also s
... itting below the radiator posing an additonal risk for damage and should have additonal protection to help prevent damage. I am currently having to dispute the refusal to cover the damage to my own ac condensor done by a rock flying into the grill space causing damage and losing freon, because they state it is an outside occurrence regardless of the fact that the design is faulty. There are multiple people who have experienced this happening to their vehicles some not even at the 2000 mile mark and are responsible for the financial cost. Upon doing research the 2026 toyota grand highlander model was created with an even lower setting bumper that provides better coverage and protection for the air conditioning condensor and all other important mechanical components. Suggesting that Toyota is aware of the issue if they created an additonal part to help protect that area from damage. I am literally at risk each time I drive my car just for the fact that rocks are on every road way as well as other debris. I have called the customer relations department with no resolution given only to be told I am responsible for the cost of the repair. I believe this is something that needs to be addressed as well as fixed for the sake of the buyer and the already large cost of owning and maintaining a vehicle. If it happens more than a few times its not bad luck, it's a flaw in the creation of the product and consumers should not have to be held financially responsible for someone else mistake, whether individual or corporate entity. I have previously owned 4 other toyotas with no issues!Read more

Tires are LT275/65R20. While driving on interstate at approximately the speed limit and low 80 degree temperatures in South Carolina with proper placard pressure tire inflation checked two days prior, the truck developed a faint vibration which got worse over three or four miles. After I could not find anything out of place I got back on the interstate to see if problems still existed. I was only traveling at 45 mph and vibration was getting worse, so I tried to slow and make it to next offramp when the driver side tire blew out causing me to have to quickly react to prevent having an accident. The tire tread and multiple parts of the truck inner fender guards and liner were thrown out into traffic and truck bed side is badly damaged. Prior to blow out there were no warnings on the dash board to to indicate any sort of issue and I verified tire pressures when I stopped after the vibration initially set in. The TPMS system was working properly at the time of the blowout. I have the tire if further inspection is warranted. My safety was at risk when tire blew out on the interstate along with vehicles traveling around me to avoid hitting the flying debris and tire tread. Further this was driver side tire and I had to change the tire on the side of the interstate with traffic right beside my vehicle. I called GMC and they said they would do nothing to help because they see tires as a maintenance item and truck is out of warranty period. I am very familiar with load ratings, tire wear, life expectancies, and I believe theis tires failed prematurely. During operation of the second-row seat adjustment feature, the seat continued moving without adequate resistance detection or automatic stop, indicating a failure of the anti-pinch protection system. This created a hazardous condition where an occupant could become trapped or injured by the moving seat mechanism. This issue is especially concerning because these seat functions are located in areas of the vehicle intended for passengers and children. We have an infant who will be occupying these seating areas, and this defect presents a direct safety risk. Since purchasing the vehicle, I have also become aware that this defect has been linked to multiple injuries and the death of a young child. Additionally, Hyundai has issued a stop-sale on affected vehicles, yet current owners are still expected to operate them without a confirmed repair available. At this time, no remedy has been completed on my vehicle, and I do not believe it is safe to operate given the nature of the defect and its location within the passenger seating area. My vehicle, a 2023 Lexus RZ 450e, has been out of service for over 30 days due to an unresolved HVAC system failure affecting cabin heating. The vehicle was brought in for service on February 20, 2026, and has remained at the dealership since that time. During this period, no clear resolution timeline has been provided, and communication from both the dealership and manufacturer has been limited to general status updates with no substantive progress. The lack of heating functionality raises safety concerns, especially given the vehicle’s reliance on electronic systems for climate control. A loaner vehicle has been provided; however, the extended delay without repair or replacement raises concerns about potential defects in the HVAC or related electrical systems. The inability to resolve the issue within a reasonable timeframe suggests a possible systemic defect that is affecting other vehicles of the same make and model. ... hes the same defect pattern covered under Volkswagen’s recall campaigns for the Atlas and Atlas Cross Sport (water intrusion leading to airbag control module failure), but my VIN is not included. The root cause and safety implications are identical: water entering through a body seam/HVAC area and compromising the airbag system. This is a safety‑critical issue that disables the airbags and creates a risk of injury in a crash. I am requesting NHTSA review this defect for potential recall expansion. Dealership Findings: • Water present at airbag control module • Water leaking through HVAC access hole • Failed seam seal tape at cowl support • Water accumulation damaging the airbag module • Cowl trim cracked and repaired • Recommended replacement of airbag control module and seam sealing tape • Estimated repair cost: $1,377 + tax Safety Concern: Airbag system is disabled due to water intrusion from a factory seam. This creates a risk of injury or death in the event of a collision.Read more

2023 GMC Yukon XL 6.2L V8 experienced progressive engine defects starting in first year. At 7,445 mi (Jan 2023), first dealer visit for engine issues. At 10,315 mi (Apr 2023), Cyl 2 fuel injector failed (internally open), vehicle TOWED to dealer, replaced under warranty. At 14,982 mi (Aug 2023), identical Cyl 7 injector failure (internally open), replaced under warranty. Two identical failures on
... different cylinders within 15,000 mi indicates systemic defect. By 30,409 mi (Aug 2024), dealer performed engine decarbonization due to carbon buildup from fuel injection issues. Dealer recommended additional decarbs at 38,077 mi and 42,577 mi. At 56,028 mi (Mar 2026), engine suffered catastrophic failure: piston ring failure causing excessive oil consumption and engine destruction, requiring full engine replacement. This matches GM's known 5.3L/6.2L V8 piston ring defect (subject of $150M class action settlement). Defective injectors caused incomplete combustion, carbon buildup on piston rings, ring tension loss, and catastrophic engine failure. Vehicle also has unfixed suspension defect (camber bolt/alignment). Vehicle visited dealer 8+ times. Engine failure poses serious safety risk from sudden power loss at highway speeds. Vehicle available for inspection.Read more

Thank goodness my husband is a heck of a driver. I have contacted the Mazda manufacturer and they are stating that the vehicle was already fixed that they will not do a safety remedy. As I found out there was a 8515G recall for the front suspension interior corroding. I just purchased this vehicle on 2/24/2026 it broke on 3/19/2026. The vehicle states to have been fixed in 2017. I have the inspection papers from the previous dealer from whom I bought it from in 2021 and the one from the dealer that I purchased it and that inspection was in 2025. There has been no inspections after I purchased or after the catastrophic failure happened I am in process of this and my insurance is the next I'm contacting.
