Despite braking, the vehicle continued forward and struck the exterior of the building. I was able to avoid the window area where the children were standing. The airbag deployed, and the front passenger side of the vehicle was damaged. The building was not structurally damaged, and no one was injured. There were no obstructions such as floor mats interfering with the accelerator, and there was no unintended pedal input. My foot was on the brake at the time of the incident. This appears to be a case of sudden unintended acceleration. The issue has not been reproduced or confirmed by a dealer or service center, and there were no prior warning signs before the event. The vehicle is available for inspection upon request. It has been towed to the body shop. The problem has not been reproduced or confirmed by a dealer or independent service center at this time. The vehicle was inspected by insurance and they did not total it. It is going to be repaired. The vehicle has been inspected by insurance representatives. While I am able to stay alert and correct these issues as they are constantly arising, they should not be available for use on public roads as they at random cause the vehicle to operate in an unpredictable manner and create a significant hazard for other drivers on the road who may not be paying attention - it's not if someone will collide with the back of the vehicle but rather when and how severe - this vehicle should not be hitting the brakes when there is nothing to stop for and a system like cruise control should not constantly and randomly stop working while in use.Read more

The issue is particularly concerning because it creates a driver distraction and potential safety risk, as the system behaves unpredictably and requires attention while driving to manage or disable unintended inputs. I reported these issues multiple times to an authorized Mazda dealership while the vehicle was still under warranty. However, I was told no issue could be found or that software updates would resolve the problem. These updates did not fix the issue. On August 9, 2025, another system update was performed under the Mazda Connect Class Action Settlement, but the problem persists. The dealership is now requiring a paid diagnostic fee before proceeding further, despite the known nature of this defect. Based on my research and widespread owner reports, this issue is commonly associated with infotainment system defects, including screen delamination and/or failure of the Connectivity Master Unit (CMU). I am submitting this complaint to document the defect and request further review, as it affects safe operation and driver attention.Read more

Occurred immediately after authorized service, suggesting procedural or system-related defect. Noted in Mazda Technical Service Bulletin 07-007/19 as a known issue. Attempts at Resolution: Mazda offered partial goodwill assistance (~$775–$875) and assigned partial responsibility to the customer. Multiple attempts to escalate with Mazda Customer Experience (Case manager Betty) have not resulted in a satisfactory resolution. Both recalls were issued within 3 weeks of the deletion. This was the third of three consecutive failed appointments — all failed because El Segundo could not provide a promised loaner vehicle (January 21, February 10, March 14, 2026). The only surviving record [XXX] ) was altered by Rivian Service — they inserted the statement that ride height appears to be as expected and does not show variance before any inspection, directly contradicting my written report and a video I submitted through the Rivian app on January 2, 2026 at 9:34am documenting the defect. Rivian also added an all-caps requirement to replicate the issue on a test drive prior to intake despite existing written and video documentation. Rivian has a federal obligation to complete FSAM-1795 and FSAM-1794. Neither has been reinstated as of April 1, 2026. Formal complaint delivered to Rivian CEO via USPS Certified Mail March 18, 2026. ... f the incident, I was parking at a café and had my foot firmly on the brake while the vehicle was in Drive. As I was preparing to shift the gear into Park (without having actually moved the gear yet), the vehicle suddenly behaved as if it had shifted into Reverse, despite the gear still being in Drive. The rearview camera activated, and the car began rolling backward. This occurred while I was still pressing the brake, which should have prevented any movement. In response to this dangerous situation, I attempted to lift my foot off the brake and press it again, but the brakes did not respond. I also tried shifting between Reverse and Drive in an attempt to regain control and restore normal braking function, but this did not resolve the issue. As the vehicle continued to slowly roll backward, I was ultimately forced to shift the gear into Park while the car was still in motion and turn off the engine to stop the vehicle. This action successfully stopped the car. After restarting the vehicle, I was able to park safely without further issue. This was a highly dangerous malfunction that could have easily resulted in a serious accident. Additionally, there were two passengers in the vehicle at the time—one in the front passenger seat and one in the rear—both of whom are licensed drivers and can confirm that the vehicle began reversing while still in Drive and that the brakes were not functioning as expected. The 9-speed ZF automatic transmission (9HP) in the 2019 Pilot is the same or substantially similar unit. I called the dealer and they are having me tow the truck potentially at my expense for them to look at and decide if it falls into the category of the recall after they did the test, and at one point told me that cause they did the test it is no longer recalled. I am being told people who have had the engines replaced because of the recall that the replacement engines have failed as well. I have a 9 month old baby and this left me stranded and unable to pick him up from daycare I had to scramble to find somone able to do so. I'm concerned that this could have happened while driving and having him with me or being left stranded somewhere.I'm glad it didn't happen when I was dropping him off that morning and while I was driving And meanwhile while they look at the issue and decide weather this falls under there recall or not and while it gets repaired I'm left without a vehicle and to be honest I don't feel safe even if they do repair it as there seems to not be an actual fix to the situation. this is unsafe and unacceptable for the price we have to pay for these rigs and the safety regulations and standards we have, how is this even an issue.Read more
