This poses a safety risk that can result in a crash and/or death. Cars are require

Incident date: Apr 4, 2026
The contact owns a 2019 Ford F-150. The contact stated that after starting the vehicle, the transmission hesitated to shift into gear. Additionally, the vehicle shuddered and stalled while driving. The engine revved and experienced rapid acceleration. The transmission shifted into reverse(R) one time, and the contact reversed into a parking space and turned off the vehicle. The vehicle was able to

DESCRIPTION OF SAFETY ISSUE: The Concentric Slave Cylinder (CSC) on my 2019 Toyota Corolla Hatchback 6-speed manual failed during normal driving at around 60mph, creating an immediate and uncontrolled safety hazard. The clutch pedal depressed fully to the floor while I was shifting from 5th gear to 6th gear and the vehicle became unable to be shifted out of gear, leaving me unable to properly con
... trol vehicle speed or movement. The only way to release the transmission was to physically reach down and lift the clutch pedal up with my foot while attempting to drive. CRITICAL SAFETY CONCERN - SHARED BRAKE AND CLUTCH FLUID RESERVOIR: On this vehicle, the brake fluid reservoir and clutch hydraulic system share a single reservoir. When the CSC fails and leaks fluid, brake fluid is consumed directly from the same reservoir that supplies the braking system. A CSC failure does not only result in loss of clutch function — it results in loss of brake fluid, and with sufficient fluid loss, partial or complete brake failure. This is not a theoretical risk. This vehicle can fail in a way that simultaneously removes the driver's ability to control vehicle speed via the transmission AND reduces or eliminates braking capability — both at the same time, without warning I am requesting that NHTSA investigate this as a safety defect and consider whether a recall or service campaign is warranted given the risk of simultaneous clutch and brake failure on these vehicles.Read more

The vehicle's engine has suffered a catastrophic internal failure at approximately 56,000 miles. The engine is the 6.4-liter HEMI V8. The issue began with a loud ticking noise from the engine. I brought the vehicle to Stoneridge Chrysler Jeep Dodge of Pleasanton for diagnosis. At that visit, the dealership lubricated an unspecified component and instructed me to continue driving. The ticking nois
... e temporarily subsided. Shortly after, the check engine light illuminated. I returned the vehicle to the same dealership, where the technician retrieved diagnostic trouble code P0303 (cylinder 3 misfire). Upon further inspection, the technician found scoring on the cylinder 3 wall, a persistent loud ticking noise from the engine, and metal contamination in the engine oil sample. The dealership diagnosed the failure as requiring a complete engine long block replacement at a quoted cost exceeding $23,000. The powertrain warranty has expired based on the 3-year time limit; however, the vehicle has fewer than 56,000 miles, which is well below the warranty's mileage threshold and exceptionally low for a vehicle of this age. The vehicle has been properly maintained throughout ownership with regular oil changes and scheduled service intervals. No aftermarket modifications have been made; the engine and all related components remain factory stock. I contacted Stellantis customer care to request assistance. They stated that because the vehicle is no longer under warranty, there is nothing they can do, and that the dealership should have submitted a goodwill assistance request on my behalf. The dealership said they would look into it. A complete engine failure at this mileage on an engine marketed as a high-performance powerplant suggests a manufacturing or material defect rather than normal wear and tear.Read more

It worked okay for a short time and then I had the same issues and now added to those were it wouldn't start for at least 15 minutes and several tries. Cruise Control could not be used and the turn signals would not work while all of the messages were displaying. The windshield wipers would only work at the highest setting and the rear wipers would not work at all. I took it to another Ford Dealership and they said it was an electrical wiring issue that they found. The High Speed Cam wiring is what they found a break in and they fixed it and cleared the codes. I paid $450 for this repair. Just 4 days later the issues have returned. Messages about components not working, this time the right turn signal would not shut off. I am concerned about the safety issues while driving with my turn signal stuck on. I called the same Ford dealership and they can get it in on Friday so that I can have a loaner car. I just don't feel safe driving the Bronco. I don't have the money to loose on trying to trade it in with these issue. Help!!!!Read more

My car was parked at home and hadnt been driven in hours. My neighbor contacted me, saying that my car was on fire. I went outside, and it was on fire, and the fire department was called. After BMW conducted an investigation, they deemed the car fire to be related to the active recall. The manufacturer (BMW) is violating the National Traffic and Motor Vehicle Safety Act (49 U.S Code Section 3012
... 0) by refusing a "remedy without charge" for recall 24V-536. A manufacturer is required to provide a remedy for a safety recall "without charge." BMW is offering a loan payoff (21,851.37). This current offer violated this federal statute by forcing me to absorb the loss of my $2500 down payment and accumulated equity. Additionally, Case Management is threatening to revoke my essential transportation (loaner) unless I sign a settlement that omits a verbally promised $4,000 credit buy with non-complaint conditions: it is a restricted discount only valid for future BMW purchases, it was omitted from the legal contract that I am being pressured to sign, and it places burden on me, the victim, to find a replacement vehivle while threaten to revoke my loaner transportation. This is a clear attempt to bypass the federal requirement for a free and complete remedy for a dangerous safety defect. I have attached a picture of the fires along with the fire department report, and the email thread between Case Management of BMW and me. On March 25, 2026, I was offered a loyalty goodwill of $4,000 after the purchase of a BMW from their dealership. March 26, 2026, I was told to contact my loan finance company to inquire about a collateral substitution. After I emailed the CEO and VP of BMW executives, I was then asked about what car I was interested in as a substitute for collateral. March 31, 2026, I am told to coordinate with the dealership as well because the BMW corporation doesn't coordinate with the dealership about the substitution of collateral.Read more

Our 2018 Honda Odyssey repeatedly and without warning completely loses acceleration power, creating extremely dangerous driving conditions. In both situations, pressing the

On Interstate 285 in Atlanta, Georgia — one of the busiest and highest-speed highways in the country — our vehicle has failed to accelerate during lane changes on multiple occasions, nearly causing serious accidents. Most recently, within the past week, my husband was driving on I-285 when he pressed the accelerator and the vehicle had absolutely zero acceleration response. He narrowly avoided a collision. We have also experienced complete loss of acceleration when turning from a stop sign into traffic and when merging — situations where immediate acceleration is critical to safety. The vehicle lags for several seconds before any power is delivered to the wheels, leaving us dangerously exposed in oncoming traffic. All manufacturer recalls have been completed, including the fuel pump recall. The problem persists and has not been resolved by any dealer service or recall repair performed to date. We are aware that this is a known, widespread defect affecting 2018 Honda Odysseys equipped with the ZF 9-speed automatic transmission. Hundreds of other owners have reported identical symptoms to NHTSA and to Honda directly. Honda has acknowledged the transmission's behavior to some owners but has stated it is "how the car is supposed to drive" — a response that is wholly unacceptable given the documented near-accidents this defect is causing. We are requesting that NHTSA open or expand its investigationRead more
