The passenger airbag system failed, specifically the occupant sensor and the pad beneath the passenger seat. These components are responsible for detecting whether a passenger is present and ensuring the airbag deploys correctly in a crash. I've also been told that I may be at risk of not having my airbag deploy in an accident. My vehicle is available for inspection upon request. I am a single mo
... ther. My child rides in the vehicle regularly, and my elderly parents occasionally ride in the passenger seat . Because the occupant sensor has failed, the passenger airbag may be suppressed entirely in the event of a collision. No passenger in the front seat would have airbag protection in an accident, and it's possible I would not either. We have been driving under these conditions for an extended period of time without a remedy. A trusted independent mechanic inspected the vehicle and confirmed the fault. They diagnosed the issue as a failure of the occupant classification sensor and seat pad, and noted that recalibration was attempted. The recalibration did not resolve the problem, and the airbag warning light remains on. I have not yet taken the vehicle to a Nissan dealership for diagnosis. It has not been inspected by Nissan, law enforcement, or insurance representatives. The airbag warning light on the dashboard has been illuminated continuously for many months. The light is solid, not flashing, which my mechanic indicated reflects a confirmed, persistent fault in the system rather than an intermittent issue. There were no prior symptoms before the light appeared.Read more

Safety Defect: Data Communication Module (DCM) Failure Affecting Hands-Free Microphone and Safety Connect / SOS Emergency System Vehicle: 2019 Toyota RAV4 Hybrid LE The DCM (which controls both the CarPlay/Bluetooth hands-free microphone and the Safety Connect SOS emergency button) suddenly failed without any warning or accident. Symptoms: - No one can hear me during CarPlay or Bluetooth calls
... (microphone inoperative). - Safety Connect / SOS light became intermittent or inoperative (directly impacts emergency calling, crash notification, and roadside assistance). This is a known recurring issue documented in Toyota TSBs T-SB-0112-20 and T-SB-0089-21. Toyota of Hackensack diagnosed it and quoted $1,300 for DCM replacement/update while charging me $200 for diagnosis. They did not disclose during the multipoint inspection that this is a documented safety-related DCM fault. I performed the manufacturer-recommended 12V battery disconnect reset (standard TSB procedure), which fully restored the microphone and returned the SOS light to solid green. However, this is only a temporary fix for a known defect. This failure compromises hands-free calling while driving and the emergency SOS system — both critical safety features. I request NHTSA investigation and urge Toyota to issue a recall or service campaign for all affected 2019–2025 RAV4 Hybrids.Read more

I took my car in to Tomball Hyundai due to intermittent failure of the headlight switch and the wiper switch when both of these features are set to the ‘Auto’ setting. These are very intermittent issues, but as luck would have it, it was sprinkling on my drive to the dealership on [XXX] and I was able to capture the wiper issue via video on my phone which I shared with the service advisor. The
... n, fortunately, 2 days later, the service technician was able to verify the intermittent headlight failure. I was aware of a TSB (TXXH) which is meant to address this problem, however, Hyundai issued a superseded TSB a few months after the original, in which they deleted the wiper switch from the service. The 2015 Genesis multifunction switch is sold as a single integrated unit (P/N 93400-3M031), but Hyundai’s TSB 20-BE-009H-1 mandates a "hack" repair that leaves the failing wiper switch in place. It is my assertion that when Hyundai settled a class action lawsuit regarding this switch, they agreed to extend the warranty on the entirety of the Multifunction Switch Assembly (NOT just the Headlight portion of the switch) for 15 years and Unlimited mileage. The Hyundai National Customer Service Case Manager (Roobika, 714-912-2118 National Consumer Affairs) told me that my request of having the complete switch replaced was denied due to the vehicle being out of warranty and having 73,481 miles. *to be clear here - Hyundai HAS agreed to replace the Headlight switch only - their denial centers on the Wiper switch. Eager to find a solution, I suggested that I could cost-share. This was declined as well. I was only given the option of buying the whole switch myself and allowing them to provide the labor to install it. Furthermore, It is technically illogical to perform a warranty repair by disassembling an integrated unit to replace one half while ignoring the failure of the other half—especially when the labor required to access the part is identical. Since the day of delivery, this 2025 Audi S5 has experienced daily electronic and safety system failures. At least once per day, on startup, the vehicle throws 10 or more warning codes disabling safety-critical systems including blind spot monitoring, lane departure warning, forward collision avoidance, rear traffic alert, and adaptive lighting. These failures occur without warning and persist for
... varying durations before clearing on their own. Additionally, on 5 occasions the adaptive cruise control refused to activate, displaying a false "door open" warning with all doors and trunk confirmed closed. On 4 occasions the power trunk failed to latch using the electronic controls. The 360-degree parking camera system intermittently stops rendering surroundings, displaying only the vehicle graphic with no camera feed. The vehicle was brought to the selling dealer for service. After one hour, the dealer stated no issue was found. A battery diagnostic was requested based on a documented case of an identical 2025 S5 with the same symptoms traced to a faulty 12V battery. The dealer declined to cover the diagnostic under warranty. The service manager later acknowledged these are known platform issues and confirmed a software update is expected from the manufacturer in July 2026. The vehicle already has the latest available software update (TSB 2079690, KD2, version 03.10.00/C) installed and the problems persist. The dealer has confirmed there is no current fix available. These failures put the driver and other road users at risk, as safety systems are non-functional during the error state with no indication of when they will restore.Read more

I am reporting a recurring safety concern involving sudden loss of power and engine stalling while driving. The vehicle wouldn't accelerate during normal driving condition and lost power. After the vehicle stalled, all the warning light came on. My husband retired to restarted the engine but the it made cranking noise. After couple of minutes it barely restarted and was able to drive about a mile
... and then it stalled again. Repeated 4 times to finally make it home. The vehicle has a prior recall history related to the fuel system. The Current symptoms are similar to fuel delivery or fuel system-related failure conditions. I called Honda Corporate to provide flat bed for the towing but they refused. My husband cleared the code (P0087)so the vehicle was free of warning lights so he can drive to the dealer next morning. We brought the vehicle to Honda dealership for diagnosis. However, the issue could not be reproduced during an inspection due to code (P0087) was cleared and couldn't reproduce the code which directly indicates Fuel Rail Pressure Low. Given the safety nature of this condition and the fact that it occurs while driving, I and requesting an investigation into whether this may be related to a known defect of incomplete resolution of prior recall -related repairs. The fuel tank pressurization systems on my 2024 Mazda CX-90 PHEV have failed, and the vehicle is currently available for inspection at Dolan Mazda in Reno. The vehicle was inspected and supposedly repaired by Jerry Seiner Mazda on March 24, 2026, but the problem recurred during the return trip from Las Vegas to Reno on March 29, 2026, when it again spit fuel at the pump. I was driving on the service road of the freeway, driving approximately 40 mph when my main screen started flashing warnings. I was only able to catch 2 before the screen shut off. One was saying front collision sensors not working and the other said side sensors not working then the screen where the radio was went black. Then my brake pedal locked up, the car lost all power steering. The car star
... ted to just roll and I was able to pull into a side parking lot and the car died. I was not able to start the vehicle and called roadside assistance and had my vehicle towed to the car dealership. This was during rush hour traffic with multiple vehicles in the road. I was terrified as my vehicle was slowly starting to stop moving and there were multiple cars surrounding me while I lost all brake function and power steering. At the time of this report, the service department at my dealership was closed and will call them in the morning. The tow truck left my vehicle there as it is inoperable. From the time I noticed the warnings on the screen to when my vehicle came to a complete stop was about 2 minutes.Read more

Third row randomly closes on my kids who are buckled in them. I've reported this to the dealer shortly after getting my van in 2021. Brought it back to them twice where they found "no problem", then I made an additional report here last year after it closed on them an excessive amount of times and I felt helpless. I was forced to move the child who was sitting in the back to front seat after it cl
... osed on them multiple times in one ride. A head on collision occurred a few miles down the road with my child in front seat. He was injured more than he should have ever been, due to needing to be moved from the rear of the van. Furious Chrysler did not take my previous complaints more seriously.Read more
