No injury or incident yet. The PTU was diagnosed as failing at 46,500 miles on June 30, 2025. The vehicle is still being driven because the driver has no alternative, but no dealership has been able to confirm whether it is safe to operate in this condition. A failing PTU can result in sudden power loss, drivetrain lockup, or loss of park function—especially in AWD vehicles. These symptoms
... match those described in NHTSA Recall 25V011000 affecting Jeep Cherokees. The lack of clear safety guidance and the delay in repair have created a potentially hazardous situation. This should be investigated as part of the broader PTU defect pattern.Read more

While driving approximately 70 MPH on the highway, the rear passenger-side wheel rim suddenly failed and blew out, causing the tire to also blow out. The incident caused a sudden jolt and I nearly lost control of the vehicle. Thankfully, I was able to steer safely to the shoulder without crashing, but it was an extremely dangerous situation. The failure of the rim caused significant damage to the
... rear suspension. The vehicle required the following repairs: New rim Two new tires Right Rear Control Arm – Forward Right Rear Control Arm – Adjustable Right Rear Trailing Arm Lower Rear Lateral Link Rear Suspension Shock Absorber All of this stemmed from the catastrophic failure of the rim at highway speed, which I believe should never occur on a properly maintained vehicle with no prior wheel damage. The failure occurred without hitting debris or a known pothole and was not due to a slow leak or flat—it was a sudden rim failure. This presents a major safety hazard not only to drivers of Ford Escapes but to others on the road. I urge the NHTSA to investigate whether other similar failures have occurred in 2018 Ford Escape models. A recall or formal review may be warranted.Read more

Speed control became stuck and I had to hit my breaks to stop acceleration. Vehicle stopped shifting while driving down the road periodically. It has completely shut off driving down the road through town and stopped. I had to hurry and turn it back on as there were cars behind me. The transfer case on my 2016 Ford Edge Titanium AWD cracked and failed at approximately 76,000 miles. For months prior, the vehicle showed warning signs including a strong burning smell inside the cabin, jerky movement while reversing, and rubbing or grinding noises during low-speed turns. I sought help multiple times, but the issue was not diagnosed despite repeated service visits. Eventually, t
... he vehicle was taken to another repair facility where, during a test drive, the transfer case fully cracked while the mechanic was driving. According to the technician, the vehicle began smoking and was no longer safe to operate, and he immediately turned around to avoid further risk. The mechanic explicitly told me he was thankful it happened to him and not to me or my family, as it could have been much worse had it occurred on the road. The damaged part has since been replaced, but I have photographs of the failed transfer case attached to this complaint. Safety Risk: The failure made the vehicle inoperable and unsafe to drive, with serious risk of mechanical seizure or loss of control if it had occurred under different conditions (e.g., on the highway or in traffic). There were no dashboard alerts or warning lights—only progressive, difficult-to-pinpoint symptoms. Problem Prevalence and TSB History: Afterward, I discovered that transfer case failures are a known issue on Ford Edge models, and that the 2016 model year has an unusually high number of Technical Service Bulletins (TSBs) related to powertrain and drivability issues. These include: TSB 18-2057, related to strong cabin odors TSB 18-2255, related to transfer case leaks and driveability problems Given the number of complaints online and the pattern reflected in these TSBs, this points to a broader reliability and safety issue with this component in this model year. Inspection History: The vehicle was evaluated multiple times for these issues and was ultimately diagnosed by a certified repair technician afRead more

On Sunday, [XXX] (a very hot day in Michigan. My vehicle registered 100 degrees) I was driving on [XXX] about 70-73 mph when I got an alert that said (to the best of my recollection) TOO HOT - IDLE IMMEDIATELY! and there was a continuous high pitch beeping which I only heard after turning off the radio. I continued at highway speed for a good 2-3 miles to the next exit afraid at any minute my c
... ar would start smoking. I exited and had to stop at a red light before turning left into a Kroger plaza with a fuel station. When the light turned green I accelerated and a grinding sound came from my car. I pulled into the plaza and turned the car off but had to turn it back on in about a minute as it was too hot to sit in car without air conditioning. It started and idled just fine. I googled roadside service and got a nice fellow (on a SUNDAY!) and he arrived in about 40 minutes. He towed me 40 miles away to a recommended auto shop that on Monday told me it was my transmission cooler that was defective and that I needed a transmission shop for a new transmission. It was then towed another 20 minutes away where it stayed for 9 days. Cost: $6564.48 plus $150 for paying with a credit card so totaled $6714.48. My drive train warranty was only good to 60,000 miles/5 years and I have 76,000 miles on my car at the time. Was told the transmission cooler was defective so I asked to have the cooler part and I have it in my garage. I spent over $1000 at the dealership in the months of Feb/March/April 2025 as my car hit the 70,000 mile mark so I had them do all work required by the car manual. The transmission cooler is apparently not a part they check. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I started hearing a scratching/rubbing noise from the brake, and feeling the scratching/rubbing through the brake pedal, making it difficult to push down. I almost crashed in the ally into my garage, but the emergency braking took over and stopped the car very suddenly, snapping my neck a bit, and it is still sore as of me writing this report. Since then brake pedal makes scratching/dry rubbing n
... oise with heavy rubbing and grabbing feel through pedal. It only happens at 20mph or below during quick stops (not smooth stop, press pedal quickly), at 15-25% pedal position. When holding pedal at that position while moving, rubbing/grabbing through the pedal feels rhythmic like it's related to wheel rotation. If car is creeping in Neutral or Drive (no gas pedal pressed), and you lightly press the pedal, you can hear and feel like something is grabbing/releasing/grabbing/releasing. Sometimes it the brake pedal resists and there is a very loud rubbing or grinding noise, and I have to press the pedal very hard. This can also be replicated when the car in in Park. Lightly pressing on the brake pedal causes the braking pump to make a whirring sound to build pressure (this is normal), but at the end there is a scratching sound and grinding feeling in the pedal. I took it to the dealer 3 days after purchasing it on June 25th, 2025 and the assistant service manager reproduced the issue in front of me, and thought it was the brake actuator going bad or something with ABS. However, the master tech at the dealer just drove it around, DID NOT CHECK ANY COMPONENTS except the thickness of the brake pads, and declared everything fine. I fear for my safety in this car. If I am involved in an accident, it will be because the braking system failed in this car and the dealer refused to fix it.Read more

While driving on May 10, 2025, with Dynamic Radar Cruise Control (DRCC) and Lane Trace Assist (LTA) active, my 2025 Toyota Camry Hybrid unexpectedly veered off the road during a curve. I immediately applied full brake pressure, but the vehicle failed to decelerate and continued rolling 600–800 feet. The braking system, including Brake Override and Automatic Emergency Braking, did not engage as d
... esigned. After the incident, the car displayed warnings: Brake System Malfunction, Smart Stop Malfunction, Electronically Controlled Brake System Failure, and Low Brake Fluid. No warning lights appeared prior to the incident. My safety was severely at risk as the vehicle left the roadway and struck rocks and brush. Had another car or pedestrian been present, this could have caused serious injury or death. Toyota’s Product Liability Team and a contracted engineering analyst inspected the vehicle and retrieved Event Data Recorder logs showing 100% brake pedal application with inadequate deceleration. Toyota confirmed the brake master cylinder was low and the left rear caliper was leaking fluid but denied responsibility, citing system limitations. The vehicle was declared a total loss by my insurance company (Progressive) and inspected by an authorized body shop. I have extensive documentation, including Toyota’s denial letter and the diagnostic report, which are available for review. I am reporting this defect because I believe a safety-critical failure occurred and the braking system did not function as intended. Tires are failing on the year model at an alarming rate. We have just turned 30,000 miles and have had 7 failures. 2 were deemed unrepairable and had to be replaced, the others were patched or plugged. The concern is sidewall failures on this year model, seems as if it a poorly engineered wheel size for this car that will result in catastrophic failures. I’ve called Toyota and there are appar

Facebook Prius groups indicate this is a widespread issue not exclusively to me. I’ve been driving for 45 plus years and have never had such an experience as this. Keep in mind, Toyota sends these vehicles out without spares or tire changing equipment and the acceptable replacement tires are not readily available and have to be ordered. Again I’m making this issue known as these vehicles are creating dangerous conditions for inexperienced drivers and our motoring public that Toyota is not interested in correcting.Read more

URGENT SAFETY CONCERN: Intermittent Total Loss of All Vehicle Audio Alerts This issue has occurred multiple times and poses a serious safety concern. Temporary Workaround (While Driving): The only way to restore functionality is by performing a forced reboot of the infotainment system. This requires the driver to press and hold the home button until the system power cycles—a distracting and unsafe action that must be done while the vehicle is in motion. Please note that while rebooting does not disable the vehicle’s ability to drive, it temporarily resets multiple core systems, including the climate control and infotainment interface. This creates a significant distraction and impairs the driver’s ability to safely monitor vehicle status and surroundings. This malfunction poses a significant safety risk and must be investigated immediately.Read more

My 2020 Tesla Model Y Long Range/AWD vehicle has 46,000 miles and can no longer recharge its battery (engine) with an approved charger (L2) due to a commonly failed part inside the battery. This part is called PCS (Power Conversion System). This device attached to the Tesla battery is used to convert power from a charger which is AC (household alternating current) into DC (Direct Current) the bat
... tery can understand. This error appeared for the first time on [XXX] at [XXX]. This critical part has failed in thousands of Tesla vehicles, some multiple times easily verified by thousands of customer complaints online. The PCS is NOT a consumable part - such as a tire, wiper blades, brake pads, etc. There is nothing the vehicle Owner can do to prevent the PCS failure. The Cost to repair/replace the PCS part(s) is ~ $3,000 and may fail again after replacement. As required by Tesla, I scheduled a service appointment through their phone app, and received an estimate of ~$280.00 to repair my vehicle on June 26, 2025 at the Irvine, CA Tesla Service Center 2801 Barranca Pkwy. After diagnosis, I was informed the cost to fix/repair the PCS part in my vehicle so that I could charge and drive it, would be ~$3,000. The Manager Ryan Sutherland (949) 404-2989, further stated the repair would not be covered under my Vehicle Tesla Warranty (Tesla battery or drive unit warranty that does not expire until [XXX]). I tried to contact Tesla corporate for assistance at [email protected] and did not receive a response but instead was contacted by service manager Ryan Sutherland via email confirming Tesla's warranty position. Tesla is essentially the only service/repair resource available for these vehicles. This repair is obviously due to a faulty/defective PCS or faulty design by Tesla affecting thousands of vehicles and should covered under my vehicle warranty so that it can be driven. I own a 2018 Ford Mustang EcoBoost with a Gasoline Direct Injection (GDI) engine. At around 100,000 miles, the engine developed severe carbon buildup, resulting in cylinder scoring and catastrophic engine failure. Despite following Ford’s recommended maintenance schedule (including a 100,000-mile GDI cleaning service performed by the dealership), my car now has one cylinder with only 70 psi comp
... ression (confirmed by borescope and mechanical test). Other cylinders measure 135–160 psi. I have learned Ford is aware of this issue and has made design changes in later models (adding extra fuel pumps/dual injection systems) to address it, but no recall or owner warning has been issued for earlier EcoBoost Mustangs. My research and discussions with BG Products and independent experts show this is a common flaw with GDI-only systems, and Ford has failed to inform owners or provide a lasting solution. The dealership and Ford have offered limited goodwill, but the engine is beyond repair and requires a full replacement. I believe Ford’s failure to warn owners about this well-known defect constitutes a safety and reliability risk. I am requesting NHTSA to investigate this widespread issue and require Ford to take responsibility for affected vehicles.Read more

We’ve been stranded in traffic, unable to accelerate, and fearful for our safety. Despite repair after repair, the problem continues. At this point, I do not believe this truck can be fixed, and I am unwilling to keep driving it—or trade it in knowing another family could end up in the same dangerous situation. Had a rear door open on the vehicle and secured my [XXX] into his car seat. Remote started the vehicle though VW carnet app and placed my things inside then closed the door. Command from the app then locked the doors and did not start it. my [XXX] was trapped in the vehicle during a heatwave 98 on that day. called 911 and my partner after getting help for someone near by and lucky he was able to
... unlock it with his phone remotely. My partner is a service tech for a different auto manufacturer and he was able to isolate and duplicate this unsafe concern. Vehicle will not lock or start with front doors or hatch open. But with only rear doors open it will lock and not start. Brought vehicle to local VW dealer and confirmed concern with them. They said car shouldn't lock if it isn't starting. They contacted technical assistance and they had them try this concern on other like vehicles and they verified that all VWs will lock rear doors when open and not start when using app. VW tech support said it must be normal operation. They advised me to contacted customer care and they said if no faults were found then no problem exists. Clearly the problem is children and animals can get trapped in the car but they said it was working as designed.
