I am writing to report a malfunction in the active headrest deployment system in my vehicle. While driving, the passenger’s side headrest unexpectedly deployed without any indication of an impact or collision, posing a potential safety risk. My wife was in the seat at the time when the unit deployed jarring her head forward. Upon my investigation, it appears that the one of the failure points we
... re internal plastic components. This malfunction caused the headrest to deploy randomly, despite there being no collision or triggering event. While the headrest is designed to deploy during a rear-end collision to prevent neck injuries, its unintended activation while driving can cause discomfort, distraction, or even injury, especially if it occurs suddenly. If this happened during an emergency maneuver or while driving at high speed, it could also interfere with my ability to maintain control of the vehicle. At this time, the vehicle has not been inspected by the manufacturer, police, or insurance representatives. However, I believe a more thorough inspection of the headrest system, and perhaps the vehicle’s onboard diagnostics, could provide insight into the issue. There were no warning lamps, error messages, or other noticeable symptoms prior to the headrest deploying unexpectedly. This malfunction occurred without any prior indication of a problem. The deployment was a sudden event while driving, with no alerts or warnings beforehand.Read more

The vehicle’s Start/Stop system began displaying warning messages and malfunctioning prior to the expiration of the factory warranty. The issue first appeared before 50,000 miles and reappeared at 49,997 miles, when a warning was displayed on the dashboard. The vehicle was taken to an authorized dealer for inspection. A diagnostic review confirmed the need to replace the Start/Stop auxiliary bat
... tery and repair the Park Assist system. A diagnostic fee was charged, and both system failures were documented. The Start/Stop system’s failure raises a potential safety concern because the vehicle may not reliably restart after automatically stopping at intersections or in traffic. This could lead to dangerous situations if the vehicle stalls unexpectedly or fails to re-engage power at a critical moment. Although the issue has not caused a crash or injury, it introduces unpredictable behavior that could compromise safe operation. The Park Assist system also failed, triggering system warnings and disabling parking assistance features. If a driver is relying on these systems during low-speed maneuvers, a sudden or silent failure may increase the risk of collision with objects, pedestrians, or other vehicles. The dealership confirmed the problem through inspection and diagnostic testing. The manufacturer was contacted and declined to cover the repairs under warranty, despite the vehicle being within the mileage limit when the issue was first documented and presented for service.Read more

The backup up camera screen stops working when the car is warm. The screen divides into three columns. As it warms up the columns fail. They become electronically distorted beyond recognition and eventually go black. This backup camera screen is also the touch screen that controls lots of functions of the vehicle. Defroster, climate controlls, radio, hands free phone usage, car settings for l
... ights, horns, and much more. The car is available for inspection upon request. There is a large blind spot behind the vehicle when backing up. We only buy a car if it has a backup camera. We do not ever want to back over a child. The camera and viewing screen must work to help prevent this. On this vehicle it does not work. Unacceptable. We called the dealership. They are familiar with the problem. It happens to many jeeps they said. They quoted us $1050 dollars to fix it. I went online and found that many people that had paid to have this repaired at the dealership, had the screens fail again soon after the repair. We have not had it repaired yet. Because we don't have confidence that Jeep has a viable solution yet. It has not been inspected by anyone yet. The problem starts to appear as a separation of the screen into three columns. One by one, those columns begin to fail. They fail rather quickly. As the vehicle gets warmer, the screen fails faster. Often the screen doesn't work at all when you first turn the car on. There have not been any warning signs or lights warning us of a failure. The failure started around August 2024. The vehicle had only 24500 miles.Read more

Rear brake becomes ineffective after a period of time. The brake becomes soft, spongy and does not work properly. I've had the dealer check the brakes, bleed them, replaced the fluid and it will work ok for a short time but then the problem returns. The pedal needs a lot of pressure to actuate the rear brake and eventually it doesn't really work at all.

The harness on my child's Nuna RAVA loosened during a 30 minute drive home (highway and downtown driving) in May of 2025. I learned later that month that there is a recall on Nuna Ravas for that exact reason. It is available for inspection. My child was not properly secured, and I did not know it until we got home; he could have been killed or seriously injured had we gotten into a collision on
... that drive. The car seat has not been inspected by anyone. Nuna offered me a “remedy kit” with a screwdriver, a brush, and a mismatched fabric cover, but I am not a safety technician and do not have the time or expertise to take the seat apart myself and still trust it to secure my child. The problem with the harness straps we have is intermittent, and does not replicate when the car is parked and I simply “tug” on the straps. I do not feel that this is an adequate safety test anyway, as it doesn’t replicate the forces or conditions in an accident. I requested a replacement seat, but Nuna denied my request on 7/21 and again on 7/28. Nuna has not supplied crash test results on layperson “repaired” car seats. This defect is the subject of NHTSA actions DP24003 and PE24026. Other RAVA owners have complained that their repaired RAVA still malfunctioned even after using Nuna's "remedy kit." I would like a replacement car seat to ensure my child's safety.Read more

Incident date: Jul 22, 2025
Absolutely. Here is the revised, anonymous version of your NHTSA complaint about the acceleration and incline issue—with all identifying info removed: ⸻ Component/System: Engine performance and drivetrain. My 2020 Nissan Armada experiences repeated loss of power when accelerating, especially on inclines or under load. The RPMs spike erratically, but the vehicle does not gain speed and often
... slows down unexpectedly. This condition has persisted despite a prior repair and continues to occur. The vehicle is available for inspection upon request. ⸻ Safety Risk: This issue places me and my children at serious risk while driving. The vehicle slows down when acceleration is needed most—such as when merging onto highways or climbing hills—creating a real danger of rear-end collisions or failed merges. The erratic RPM surges add instability and make it unsafe to rely on the vehicle in critical traffic situations like passing or freeway driving. ⸻ Confirmed by Dealer or Service Center: Yes. I brought the vehicle in for service in early 2025 after experiencing this issue. It was diagnosed with code P0025 and the exhaust valve timing control solenoid was replaced at a cost of over $1,000. However, the issue was not resolved and continues exactly as before. I returned for follow-up and was told an engine replacement might be needed, without receiving documentation or a clear explanation. ⸻ Inspected by Manufacturer, Police, or Insurance: No. The vehicle has not been inspected by Nissan corporate, police, or insurance. It has not resulted in a crash, but it remains an ongoing safety concern. ⸻ Warning Lamps or Symptoms: There were no consistent dashboard warnings. The issue began in early 2025 and occurs frequently—particularly during acceleration on hills or when the vehicle is under load. The RPMs surge, but the vehicle slows or struggles to maintain speed.Read more

Here’s a full response you can paste into the NHTSA.gov form text box that directly answers all of the questions shown in your screenshot: ⸻ Component/System: The battery and electrical charging system on my 2020 Nissan Armada have repeatedly failed. The vehicle experiences sudden and unexplained battery drain even after regular driving. The vehicle is available for inspection upon request.
... Safety Risk: This issue has caused my vehicle to fail to start on at least five separate occasions over the past month. I have been stranded in public places, including with my children, and now must carry a battery jump starter at all times. This is a safety concern, especially if the failure happens at night, in a remote location, during extreme weather, or in an emergency. The vehicle cannot be trusted to start reliably, putting me and others at risk. Confirmed by Dealer or Service Center: Not yet. I have not reported this specific battery issue to the dealership yet, as I have been tracking the frequency and pattern of failures before pursuing service. However, based on research, this appears to be a recurring issue with 2020 Nissan Armadas and may relate to an electrical or alternator defect. Inspected by Manufacturer, Police, or Insurance: No. The vehicle has not yet been inspected by Nissan corporate, law enforcement, or insurance. It has not been in a crash. Warning Lamps or Symptoms: There were no dashboard alerts, warning lights, or error messages before each battery failure. The issue started about a month ago and has occurred five times since, always resolved temporarily by using a jump box. The battery appears to drain randomly when the car is off, despite recent driving.Read more

I'm just so frustrated because Kia the main company actually has sent me many letters to get it fixed I call them and they told me their wasn't even a diagnostic done, they tried reaching out to them. Should I tow my car to a different dealer? I'm not sure what to do anymore, I'm almost afraid now they'll say they can't cover it for same reason, my trust is gone at this point with the dealership. I recently had a full knee replacement and getting in and out of my daughter's car has been very hard on me, I asked for a loaner and they said they don't provide them there and I have to pay. I've already put in so much money on this car at this point I feel cheated or targeted for some reason. They treat me like I'm trying to get something for free, they aren't paying out of their pocket. The A/C compressor produced a loud knocking noise and was found to have leaked black debris (burned oil/metal), contaminating the entire system. I was informed the system required full replacement, totaling $4,500. The defect has disabled the ability to cool the vehicle cabin
... , which poses a risk to drivers with health conditions. It also affects window defogging and visibility, especially in humid, rainy conditions. This matches hundreds of similar complaints and class action lawsuits filed against Honda for known A/C system defects in 2016–2021 CR-V and Civic models. My car is outside the standard compressor warranty, and Honda refuses to provide assistance. This failure should be considered a safety hazard, particularly for individuals who require temperature control for medical reasons.Read more

Subject: Tire Safety Concern – BMW OEM Pirelli Cinturato P7 All Season (UTQG 500) 245 50 R19 I am writing to raise a serious vehicle safety concern involving the BMW OEM Pirelli Cinturato P7 All Season tires that came factory-installed on my leased 2022 BMW X3. These tires are labeled with a UTQG 500 treadwear rating, which typically implies a lifespan of 40,000 to 50,000 miles. Yet, all four of
... my tires were found to be severely worn and unsafe at just 23,456 miles. More troubling, I began experiencing significant handling and traction issues as early as 18,000 miles. During normal driving—without aggressive braking, cornering, or high-speed use—the car exhibited: Hydroplaning in moderate rain Slipping and sliding under light acceleration Noticeable vibration and poor stability on dry roads These symptoms were initially confusing until a routine service appointment at Motorwerks BMW in Bloomington, MN revealed that all four tires were nearly bald and posed a safety risk. The dealership confirmed the tread depth was between 1 and 2 mm—well below the safe limit—and told me, alarmingly, that this level of premature failure is typical for this tire. This raises two critical safety issues: The tires were functionally unsafe well before 20,000 miles, long before I was aware of the degradation. The UTQG 500 label creates a false sense of security. A consumer would not expect traction loss or hydroplaning risks within the first 18,000–20,000 miles of normal use from a 500-rated tire. I have since learned that this OEM tire, while labeled identically to the Pirelli Cinturato P7 All Season Plus sold at retail, is actually a different tire made specifically for BMW. It carries no treadwear warranty, despite its 500 label, and reportedly has never been separately tested to confirm its rating. Both BMW and Pirelli have pointed fingers at each other, offering no solution or responsibility—leaving consumers with unsafe tires and no remedy. I urge NHTSA to investigate this issue.Read more

Auto idle system is faulty. I repeatedly experience a failure when this system is not turned off. It causes my engine to shut off when I step on the gas - or (mostly) it causes my car to almost shut off (the dashboard shuts off and flashes back on when I step on gas at a stop sign or other stops, also when I’m driving and accelerate there is a lag of nothing like the car almost turns off followe

I’ve asked Honda dealer in town about this and they said it’s just the transmission and could get it replaced but now I see (via a class action suit I read about) that this is a big problem and it’s not just my personal issue with the car. I’m concerned someone will get hurt if these problems are not recalled and fixed by Honda.Read more
