On [XXX], I experienced multiple failures of core safety systems in my 2021 Audi Q7 that put my 17-month-old child at serious risk and exposed critical flaws in how the vehicle’s systems operate and record events. On [XXX], the pre sense automatic braking system suddenly engaged while I was driving approximately 35 mph on a clear, empty suburban road. There were no visible obstructions, no warn
... ing lights, and nothing recorded in the vehicle’s logs. This occurred less than 48 hours after the vehicle had received recall-related software updates and routine service at an Audi dealership. Prior to that service, the vehicle had no history of mechanical or safety issues. The following day, the keyless entry and anti-lockout system failed. I placed my toddler in his rear-facing car seat behind the front passenger seat and set our baby bag—with the key fob inside—on the rear seat. After closing the door and walking to the driver’s side, the car unexpectedly locked. The fob and my child were both inside. The outdoor temperature was over 90°F. I attempted to open the vehicle and then to break the window with tools, without success. After approximately seven minutes, I broke the driver’s side window with a sledgehammer and removed my child from the car. The car alarm eventually activated, but no lock-in event, key fob error, or warning was recorded. The vehicle was inspected by the dealership twice and Audi Corporate engineers once. Audi confirmed faults in the emergency call module, comfort system, rear lid sensor, and parking sensors. However, they could not reproduce or explain the braking event or lock-in failure. Audi confirmed that the vehicle should not be able to lock with the key inside, and they do not know why it did and are unable to guarantee the safety feature will work in the future. The vehicle has not been driven since. The failure of both safety function and system visibility placed my child in danger. I am requesting a safety investigation. While driving our screen shut down, autopilot and GPS stopped working and the doors would not open. My kids were stuck on multiple occasions and had to climb through the front seats. Telsa denied coverage because we are out of warranty. I asked whether the known issue has been reported and why it is not being covered as a known safety defect. Because the problem involves loss of a safety system, it should not be treated as routine maintenance or wear and tear. It is a known safety defect and therefore must be reported and remedied at Tesla’s expense. Tesla refuse to address my concern and would only let me talk to store manager. They told me I had to email business resolutions to get more information. I have not heard back. I was forced to spend $2k to get my car fixedRead more

Dear NHTSA, I am submitting a formal safety complaint regarding my 2016 Ford Explorer (VIN: [XXX] .) Build Date: (08/15.), Mileage (115,262.). My vehicle has experienced a steering rack failure, which poses a serious risk of loss of steering control. I am aware of multiple recalls (such as 20v-675 / Ford 20S62 and 21V-537 / Ford 21S32) Addressing rear suspension toe link fractures in 2016 ford e
... xplorers. These recalls cover vehicles built in the same timeframe as mine, but my VIN is not included. This exclusion is concerning, as both the steering rack and suspension defect involve loss of steering control and directly impact driver and passenger safety. I respectfully request that NHTSA investigate: 1. Why certain VIN's, including mine, were excluded despite being manufactured in the recall window. 2. Whether steering rack failures in the 2016 ford explorer should also be subject to an official safety recall. Thank you for your attention to this urgent safety matter. I look forward to hearing from somebody. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Honda installed a VCM (Variable Cylinder Management) in this engine which shuts down the back three cylinders to save fuel when the car warms up. You’ll know when it kicks in when you see ECO on your dash. When the cylinders shut down it starts an abnormal wear and tear on those cylinders. High oil consumption, fouled plugs, the need to repair the back three cylinders and misfiring of the engine
... are some of the problems. Honda did repair some vehicles if they experienced rough running and misfiring on those cylinders because they knew the issue was the VCM. However, when my 2009 Honda Odyssey started having those problems in 2024 they wouldn’t fix the problem and stated it was too late to fix the engine. The timeline, according to Honda, had passed to repair the damaged engine. I never received a recall for this known safety issue. The engine would cut out and the engine light would come on and it sputtered down the road on three of the six cylinders. Honda knew this was a problem but they wouldn’t fix my engine. If I had brought it in years earlier they would have fixed the known problem with the VCM but they wouldn’t deactivate the VCM to prevent it from destroying the repaired engine. The price to fix the engine was over $4200. I feel they should have repaired the engine and then deactivated the VCM by bypassing the the temperature sensor to prevent it from going into ECO MODE VCM. The safety of the occupants was at risk due to the lack of acceleration and the ability to drive the vehicle in a normal manner. I feel a safety recall is justified and the sensor should be deactivated to prevent further damage to the engine.Read more

... e seatbelt. I called Ford and they told me their 5 Year/60K warranty on the car expired. I told them that under Federal Code 49 USC Subsection 30120 the manufacturer is required to repair such safety, they told me they couldn't help me and to go away, despite the clear language of this code. Therefore, I am filing this complaint against the Fiesta/ Ford Motor Company (not sure who exactly, I haven't done this before) to request reimbursement under federal law for defective safety equipment. Do not have both bills. The most recent bill will be uploaded. If this claim can move forward, please let me know and I will henceforth retrieve the earlier invoice for the failed attempt at the independent auto repair shop for evaluation and possible compensation.Read more

Re; Honda CR-V Ex-LVIN: [XXX] Purchased [XXX],From Auto Nation Honda, Dulles, VA I have noticed an issue with the windshield wipers and the timing of the water disbursement, where more than half of the windshield, on the right side, does not get water. When operating, the blades start moving from right to left. The right blade passes by about 55% of the glass before the water begins to disperse
... . The left blade moves to the edge of the glass, and the water is disbursed for the last 2 or 3 inches of the glass and providing no help for the rest of the windshield. This is a Major Safety Issue, especially during the winter months with snow, salt or sand on the road and other cars spraying all that on my windshield while more than half of the windshield is not receiving water to help clean it. I understand this system is fairly new on some models, and I am sure other buyers may have experienced the same issue. I was drying my car after washing it(I only use products approved for washing and drying vehicles) when a piece of paint on the hood just peeled off. It has not been wrecked or damaged in any way on the hood so I feel it must be a defective paint job from the factory.

While driving on June 30, 2025, the engine gave a loud clunking noise, shuttered and shut down. I was able to pull over to the side of the road safely and not impede traffic, however this happened on a public roadway and had the potential of causing different results. I had no warning lights on the dash, no check engine lights came on, no LOW OIl light and/or no other warning signals appeared. I w
... as able to restart the vehicle and it shuttered but was able to drive it at a low reduced speed to get it into a more safer place. The vehicle was towed to the dealership the following morning. The dealership was able to inspect the vehicle and confirmed it was a total engine failure. The dealership has done all the service maintenance since the date of purchase as a new car, with the exception of 3 oil changes throughout the past 8 years. The documentation show all maintenance was done in an approved licensed facility and in the prefered time frames requested by the manufacture. The vehicle has a 150,000 mi warranty and at the time of the incident the mileage was 74,000. Kia is denying the claim and is refusing to replace the engine. I am uncertain of the next steps to take at this point, referring to the Attorney General in my state for advise for product liability claim to hold the manufacture responsible for it's defective engine which has been an issue for this make and model.Read more
