My 2015 BMW Z4 vehicle has a coolant pump safety recall. It reportedly could cause a short circuit and thermal event or fire. I bought the vehicle while living elsewhere and am now in a small community with 4 major automobile dealers but not BMW. The nearest BMW dealer is about 220 miles away. I contacted that dealer. The service rep said it is 1.5-2 day repair and a loaner vehicle may be availabl
... e. The repair time sounds wrong but is what she said. She said the dealership would not work with any facility close to me to make the repair so I must drive the 440 plus round trip miles for BMW dealer repair. Repair of 1.5 to 2 days means I would either need lodging and waste least 2-3 days of my time or drive back home with a 2nd trip to the dealer to return a loaner and get my car, 880 miles and many hours driving. Even a 1 day repair means 440 miles and many hours. Called BMW Recall Hotline at [XXX] & explained my situation. They declined help saying the dealer decides how the repair is handled. I called BMW Customer Service [XXX] . They declined help & said a BMW dealer must repair unless it wants to work with another repair facility. I submitted a web-form message on BMW's corporate website online message portal outlining the problem and requesting help to get the safety recall fix completed in a convenient manner; asked to escalate if possible. They replied 3 days later declining to help saying: "The recall would have to be performed at a BMW dealership. Unfortunately, towing assistance is not available for this recall. If you would like to have the repair performed at another mechanic, It would be a non-reimbursable out of pocket expense. I would not be able to submit an escalation in this matter because it does not meet the criteria to be escalated. I apologize about an inconvenience this may cause." This seems unreasonable for a safety recall that could cause a car fire. Can NHTSA please help me obtain a reasonably convenient repair from BMW? The affected component is the engine/rod bearing (Theta II 2.4L). My 2011 Hyundai Sonata stalled and the engine locked up twice in live traffic (March 2025 and July 2025). The vehicle is available for inspection if needed. This created a serious safety risk to me and others on the road, as the stall occurred on [XXX] at highway speeds. This failure could have been catastrophic had it happened on
... the DC/Baltimore Beltway or another high-traffic roadway. Hyundai’s system shows Recall 132 was marked complete at very low mileage (around 32,000 miles), but my vehicle later developed oil consumption and stalling consistent with rod bearing failure. Capitol Heights Hyundai noted a warranty pre-authorization (PA) in March 2025 and told me Hyundai needed my mailing address “for the claim,” but later said it was a typo and that no record exists. The dealership charged me for a diagnostic but did not inspect the vehicle for rod bearing failure, despite symptoms consistent with Recall 132. Annapolis Hyundai, where the car was purchased as a CPO, has admitted they have no documentation that Recall 132 was ever performed (no repair order, mileage, or technician notes). Yet Hyundai’s system shows Recall 132 marked “complete.” Without proof that the KSDS software or bearing inspection was done, I belive this recall should be considered open. Hyundai Consumer Affairs has stated that I must pay for towing and diagnosis before they will investigate, with possible reimbursement later. This conflicts with federal law (49 U.S.C. § 30120), which requires recall diagnosis and repair at no cost to the consumer. Prior to the failures, my vehicle had a history of oil consumption and “no oil on dipstick” findings, which are known precursors to rod bearing failure. No warning lamps appeared before the stalls. I am requesting NHTSA’s assistance in requiring Hyundai to provide documentation or reopen Recall 132 and perform a no-cost inspection/repair to ensure this vehicle is safe to operate INFORMATION REDACTED PURSUANT TO THE FREEDOM OFRead more

I am reporting a safety defect in my 2020 Honda Pilot EX-L ~86,300 miles. The rearview camera and electrical/infotainment system intermittently fail, showing static, disconnecting Bluetooth, and sometimes leaving the camera blank. This creates a serious safety risk when reversing, driving in tight spaces, or transporting my children. These issues began within the first year of ownership (2020–2
... 021) and continued after Recall 23-047, during which cables were replaced and the system later rewired in August 2023. Despite these repairs, the defects persist. The dealership Pohanka Honda Fredericksburg confirmed the faults multiple times but now refuses to repair, quoting over $3,000. The vehicle has also experienced three dead batteries since 2020, leaving me stranded multiple times, including once during out-of-state travel. There were no warning lights for the battery failures, but infotainment and camera issues were noticeable immediately. The vehicle has been out of service for approximately three cumulative months, impairing reliability and my ability to perform daily work and family responsibilities. These defects have significantly impaired the use, value, and safety of the vehicle, especially given my responsibilities as a single mother, caregiver to a blind parent, and emergency department practitioner. The vehicle has been inspected by the dealer and reviewed by Honda corporate. I am seeking a proper remedy for a federally mandated safety feature (rear camera, FMVSS 111). The vehicle and affected components are available for inspection upon request. This complaint documents ongoing safety risks, repeated battery failures, and unresolved recall-related defects that began under warranty. Honda’s failure to properly repair these defects continues to create unsafe driving conditions and a serious risk of injury to myself, my passengers, and the public.Read more

I am reporting a serious safety concern regarding the 2025 Kia Niro Hybrid and its adaptive cruise control system. When the cruise control is activated and set to maintain a certain speed, the vehicle continues at that speed regardless of whether there are other vehicles in front of it. In multiple instances, the car approached slower-moving vehicles at 45 mph without reducing speed or giving any
... warnings. There was no automatic braking or even a prompt to alert the driver of the imminent danger. This is extremely dangerous and could easily result in a rear-end collision if the driver is not fully alert. It is especially surprising because the same vehicle features intelligent climate control functions—such as adjusting fan speed during phone calls and preventing windshield washer fluid odors from entering the cabin while the A/C is running. Given the level of attention to such minor details, the lack of basic forward collision mitigation behavior is unacceptable. The issue was reported to a Kia service center, but no clear explanation or resolution was provided. I believe this may be a software bug or a design oversight that has not been properly acknowledged. Recommendation to other drivers: always keep your eyes on the road and be ready to intervene, even if the adaptive cruise system is engaged. The risk of collision is high if you rely on the system to slow down for vehicles ahead. This needs to be investigated immediately before it leads to serious accidents. I urge the NHTSA to review this issue and, if necessary, request a recall or software update from the manufacturer.Read more

Component or system that failed / malfunctioned The brake system (pads, calipers, hydraulic fluid) failed three times in less than 90 days. The right-side tie rod end also separated from the steering assembly. Both components are available for inspection upon request. How safety was put at risk The brakes repeatedly lost all stopping power while driving, with the pedal going to the floor and the
... reservoir emptying within minutes. On one occasion, the brake pads and caliper fell off the wheel assembly, confirmed by the dealer’s own shop manager. The tie rod failure caused uneven tire wear and a highway blowout, forcing an emergency tow. These issues placed me, my child, and other drivers at risk of a serious crash. Has the problem been reproduced or confirmed by a dealer or service center? Yes. Premier Auto (the dealer’s affiliated garage) confirmed and attempted repairs three times. On the third visit, the shop manager (Mike) stated that the brake pads were so low they had fallen off, causing the calipers to overextend and leak. Has the vehicle/component been inspected by others? Yes. Premier Auto inspected and repaired the brakes multiple times. Dobbs Tire & Auto later inspected after the blowout and diagnosed additional electrical issues. Documentation, invoices, and photos are available. Were there warning lamps, messages, or other symptoms? The brake warning light illuminated and the pedal sank to the floor during failures. No prior warning lamps appeared before the tie rod separation or blowout. The failures occurred suddenly while driving.Read more

Subject: Complaint Regarding 2021 Volkswagen Tiguan – Premature Engine Failure at 46,000 Miles To Whom It May Concern, I am filing a complaint regarding my 2021 Volkswagen Tiguan (VIN: [XXX] ) which suffered a catastrophic engine failure at only 46,000 miles. •I purchased the vehicle used in August 2023 through CarMax Auto Finance. •At the time of failure, the car had fewer than 50,000 mil
... es and had been properly maintained. •Despite the extremely low mileage, Volkswagen has refused to cover the repair under warranty because the vehicle is slightly past the 4-year time limit. I believe this is unfair and unreasonable. A major component such as the engine should not fail this early, and a vehicle sold under these conditions is not fit for its ordinary purpose. Because of this premature failure, I was left with no functional car but still obligated to pay the outstanding loan balance. This has caused severe financial hardship and forced me to dispose of the vehicle below value. On [XXX], while driving ~65 mph, my 2020 Chevrolet Silverado 2500HD Duramax experienced a sudden transmission lock-up, nearly causing a crash. The event caused severe internal transmission damage, leaving the truck unsafe and inoperable. GM refuses to repair my vehicle under Special Coverage N242454441 (15 years/150,000 miles) because code P0747 did not set, even though my failure clearly matches the defect. I requested a hydraulic valve body leak test, the only way to confirm the defect, but the dealer refused, saying they only scan codes. GM must perform this test at no cost, as required by federal recall laws. GM’s denial leaves a known safety defect unrepaired, violating federal recall obligations and putting drivers at risk. NHTSA action is needed to require GM to repair vehicles like mine, regardless of which DTC is stored. I own a 2021 Ford Bronco Sport that has had repeated premature failures of critical components. At ~52,000 miles the water pump failed and was replaced along with coolant and belts. Less than 21,000 miles later (at ~73,000 miles) the water pump has failed again, with coolant leakage and a burning coolant smell. Ford has issued TSB 23-2104 acknowledging coolant leaks at the water pump/hose connecti
... on, yet no recall or extended coverage has been offered. This is not the only issue. My alternator has also failed twice and required multiple replacements in under 4 years of ownership. Having two major components (water pump and alternator) fail repeatedly within such a short period clearly shows a systemic defect, not normal wear and tear. Safety risks: coolant leaks can drip or vaporize onto hot engine components, creating a fire hazard. Alternator failure risks sudden loss of power and stalling in traffic. Both are unacceptable in a vehicle this new. The dealership confirmed the latest water pump diagnosis and quoted over $1,000 in labor alone, placing an unreasonable financial burden on the owner. This is the second water pump and second alternator within 73,000 miles. Ford and its dealers know this is an ongoing issue in Bronco Sports, as confirmed by widespread owner reports and TSBs, but continue to deny responsibility. A recall or customer satisfaction program should be issued to address these repeated failures.Read more

Subject: Suspension Defect Report — Rear Shock Spring Rate / Stability Hazard To Whom It May Concern, I am submitting a safety defect report concerning my 2025 Honda Hornet SP1000. The motorcycle is equipped with Öhlins suspension, and from new, the rear suspension has exhibited unsafe behavior due to an inappropriate spring rate. Safety-related issues observed: The rear shock frequently to
... ps out and unloads over standard road bumps, causing instability and momentary loss of traction. Tire temperature measurements after a long ride confirm a load imbalance: rear tire 99 °F (underheated), front tire 124 °F (overheated). This indicates the rear suspension is not carrying proper load, forcing excessive stress onto the front end. The motorcycle becomes unpredictable mid-corner and during throttle transitions because the rear fails to remain planted. The resulting instability poses a clear risk to rider safety on normal public roads. I weigh approximately 150 pounds, and at this weight the stock rear spring rate is excessively stiff. The suspension is effectively tuned for a much heavier rider, which makes the motorcycle unsafe for a significant portion of its intended market. Requested action: That NHTSA investigate this suspension tuning defect to determine whether it constitutes a broader safety concern requiring manufacturer remedy or recall. Respectfully submitted, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Vehicle reverse camera will stay on while driving. Leak in gasket. Leak in right mount. Radiator issue undiagnosed. ... lower cost - all repair and service records also available) has advised faulty parts as listed above. Forgive me, I am not a car person so I do not know what exact things, but can provide. Have been on the phone all morning with FCA, manufacturing, and back and forth until this information was received. I have pulled a recall list by VIN and found that some issues with my Jeep are on there; however, apparently my specific VIN is not associated with the recalls listed (can provide report), I am responsible for repairs. Only warning lamp I have received was for a "cylinder 1 misfire" that was diagnosed at Jeep of Fontana in CA, where I was told to take it elsewhere. Now, after having a tuneup last week, I started my Jeep on Sunday and it began smoking. Was told at local shop, Mountain View Tire in Rialto/Fontana, they cannot fix. Please assist!Read more

Bought this vehicle Aug 2020 and backup camera worked fine. Shortly after purchase the backup camera started flickering and would either be upside down or black. To this day it is 99% of the time upside down. Every once in awhile
