I have a 2020 Chrysler Pacifica. There was a recall last year and I brought it in to the dealership. At that time, I wasn't even sure what the recall was for. This year, about a month ago, I was driving (I take XXX to work everyday) and the display had a notice that said "Stop safely, the engine will turn off". I pulled over and turned the car off and turned it back on. The check engine light came
... back on but it drove fine. The check engine light eventually went off but the same thing happened again a few days later. I brought it into my mechanic who couldn't read the diagnostics. I brought it back into the dealership and they said I just needed a software upgrade so they did that. Unfortunately, the engine died again on the road. I shut the car off and turned it back on. I brought it back to the dealership and said the issue wasn't fixed. They haven't taken the issue seriously so I started searching and found this issue. It sounds like Chrysler is aware of the issue but I'm nervous even a new transmission won't fix the issue. In the meantime, I've had to rent a car as the dealership doesn't do loaners. This is a pretty big safety issue as now my car has shut down on the middle of the highway multiple times! On March 14 2026 i was driving from [XXX] . To [XXX] to have my tires checked because i felt a strong vibration on my steering column and truck itself, it was getting worse so i took it to my mechanic who does my regular maintenance on truck. The next day i was on my way to purchase the replacement tire when the tire blew out. I was on a heavily traffic road [XXX] . I managed to take control of truck doing about 40 mph at the time and drove it off the road to a safer spot to look at damage. Well the passenger rear tire witch was originally on the front that had the separated radial blew a chunk out of the inner sidewall. I put on the spare and went and replaced the tire with another goodyear new tire. On march 28 as i was coming home from [XXX] Ca late at night the driver side rear tire blew as well again had to put on spare and next day took to local mechanics in [XXX] , and checked tire and again i was told that the tire separated from radial and that was the cause of blew out, they inspected all the tires at this point since this was the second tire from the same DOT set with less then 12000 miles that had separated. Well they found a third tire that had aldo separated and had a flat spot on driver side front tire, now there are 3 tires from the same set, coincidence, i dont think so i replaced the second and third tire. One the third day April 1, 2026 the passenger side front tire blew as well, now i have 4 tires that had radial separation with 2 that had blew out chunks from inner wall, with less then 12000 miles and less then a year old, in a mater of a month. I am filing a safety complaint regarding my 2017 Ford Explorer XLT with the 3.5L V6 engine due to a known internal water pump design defect. The water pump is located internally and driven by the timing chain. When it fails, coolant can leak directly into the engine crankcase and mix with motor oil instead of leaking externally where it would be easier to detect. This creates a serious safety haza
... rd because the engine can suffer catastrophic damage, sudden failure, loss of power, or complete shutdown, sometimes with little or no warning. In my case, the vehicle experienced symptoms consistent with internal water pump failure, including my car overheating out of nowhere, forcing me to coast to the side of the highway in very unsafe conditions. This defect can destroy the engine and create dangerous driving conditions, especially if power loss occurs while driving with children or passengers, thankfully I didn't have any of my three children with me when this happened. This appears to affect many Ford Explorer vehicles from similar model years, and numerous owners have reported similar failures. The repair cost is extremely high because replacement often requires major engine disassembly or full engine replacement. I am currently financing this vehicle with a balance of $14,000 left to pay and have already paid close to $28,000. This is extremely unfair and the cost to repair is close to $5,000. I don't see a way for myself to afford that. I believe Ford should be required to address this issue because: The design can allow coolant contamination inside the engine without obvious external leakage. Failure can occur suddenly and create safety risks. Repair costs are excessive for what appears to be a widespread defect. Owners may not receive sufficient warning before catastrophic failure. I am requesting that NHTSA investigate this defect for safety implications and determine whether a recall or broader manufacturer accountability is warranted. Thank you for your time.Read more

My vehicle has an open PTU recall and when driving down the road my vehicle completely lost power was unable to drive or put vehicle in park in the middle of a turn which resulted in on coming traffic coming towards my vehicle. There were no injuries thankfully. My car was towed to the Jeep dealership in Anchorage, AK to have the PTU recall done as that was the suspected reasoning of the failure.
... After weeks of back and forth I was told the transmission was damaged and needed replaced and the PTU recall was not able to be done due to the recall instructions. I was quoted 8-12 thousand dollars in repairs which was unrealistic to me. I then had vehicle towed to an auto body shop that could replace the transmission for a lesser price. Once at the auto shop they discovered there was a complete PTU failure which caused the transmission damage and told me to take it back to the dealership for repairs to be covered under the recall. The dealership submitted the information and I was denied the recall and repairs. The dealership informed me I would have to pay for a new transmission out of pocket. However once I had the new transmission testing the PTU recall could damage and that I also need to pay out of pocket for the PTU. This has been extremely frustrating and given the pictures and evidence submitted for PTU failure causing the transmission failure and still being denied I find it absolutely appropriate to submit a claimRead more

I'm reporting a safety issue with BMW vehicles equipped with the B58 engine, primarily 2019-2021 models. These vehicles have an oil pump with an internal plastic component that regulates oil pressure, and this part is fracturing in the field, causing sudden loss of oil pressure. The failure typically occurs during cold starts in low temperatures. When it fails, oil pressure becomes erratic or is l
... ost entirely, often with little to no warning before engine damage or vehicle disablement. Warning signs, when present, include inability to measure oil level electronically, abnormal oil pressure, or sudden drive train malfunction warnings. These can rapidly progress to engine shutdown or severe damage from oil starvation. This creates a dangerous situation because the vehicle can become disabled without warning while driving or after startup. This is especially hazardous when cars become immobilized in traffic or during winter conditions when failures occur most frequently, leaving occupants exposed to freezing temperatures while awaiting assistance. Pump replacement requires extensive disassembly and commonly costs $6,000-$8,000, assuming no engine damage. A critical concern: owners cannot determine if their vehicle has the original plastic pump or the later metal revision. BMW's VIN-based parts lookup now shows updated components even when original plastic parts remain installed, and BMW has removed the original plastic part number from dealer networks entirely. Failures are reported across multiple production years, and the scope of affected vehicles remains unclear. Because this condition causes sudden vehicle disablement without adequate warning, especially in cold weather, I believe this warrants investigation as a potential safety-related defect.Read more

The result creates a safety hazard as you lose pedal response and power when the battery reaches below 35% or about 9 miles remaining. On the highway the jeep loses all power and will not accelerate over 62 mph. And the same around t
... own when you need that extra umph, the pedal becomes dead. More Scary when you’re on the highway!! The result? Since the electric power output is unavailable, you have to mash the pedal to the flow to kick it out of electric mode. But this is also unsafe as now you start the engine (often cold as I was running electric mode) and you smash the rpm’s over 6k to launch the gas engine (That can’t be good for the engine long term) . Before battery replacement and recall software updates, this process was very smooth and occurred with 5% or less battery life. This had occurred a few times and could probably reproduce it when this scenario is presentRead more

I am reporting a critical security and safety defect regarding the 2026 Honda Civic Sport Hybrid. The vehicle was stolen only 10 days after purchase. Despite being equipped with factory-installed anti-theft systems and immobilizers, the security was easily bypassed without any physical keys. As a professional engineer, I believe this indicates a significant vulnerability in the vehicle's elec
... tronic security infrastructure or 'smart key' encryption. A security breach of this nature on a brand-new 2026 model, occurring within such a short timeframe, suggests a systemic manufacturing defect that puts other consumers at risk of vehicle theft and unauthorized operation.Read more

There’s a known recall. “2021 Mach-E models produced between Feb 24, 2020, and July 18, 2021, are at risk of the glass panels detaching or leaking.” But my car, a 2021 model, isn’t on the list. I just discovered that my headliner is stained and there’s a leak.
