The vehicle experienced a failure involving the automatic transmission gear shift cable (recall AOC 2013–2016). The gear selector indicated the vehicle was in “Park,” but the transmission was not fully engaged in Park due to the defective shift cable/bushing. This created a significant safety risk, as the vehicle was capable of rolling unexpectedly after being placed in Park. This condition
... puts the driver, passengers, pedestrians, and surrounding property at risk of injury or damage, especially when parked on an incline or in populated areas. There were no clear or consistent warning indicators prior to the issue. The vehicle appeared to be safely secured, which increases the possibility of an accident or death because the driver may exit the vehicle believing it will remain stationary. This issue is consistent with a known recall involving the shift cable assembly. The defect appears to be intermittent and not always easily reproducible, but it directly impacts the vehicle’s ability to remain securely in Park. At this time, the vehicle has not received a complete or reliable remedy, and the condition raises ongoing safety concerns. The component can be made available for inspection upon request. Because this defect affects a critical safety function and can lead to unintended vehicle movement, it presents a serious hazard. I am submitting this complaint to document the issue and request further investigation and resolution.Read more

The vehicle has a dangerous hesitation and delayed acceleration when I am coasting and then reapplying the accelerator. This occurs in scenarios such as approaching a yield, slowing for a left turn on a green light, or slowing to merge where I reduce speed but do not come to a complete stop. When I press the accelerator to resume speed, the vehicle does not respond for a significant period. The tr
... uck fails to accelerate even with substantial throttle input, including pressing the pedal to the floor. After the delay, power may return suddenly. This delayed response creates a serious safety hazard in traffic. The hesitation occurs at the exact time immediate acceleration is needed to clear an intersection or merge into traffic. This has created near-miss situations due to the vehicle not accelerating when expected. Warning lights and diagnostics: No check engine light is illuminated during the events. The issue occurs without any visible warning indicators on the instrument panel. Conditions: The hesitation is most noticeable during coasting and throttle reapplication at low to moderate speeds, especially when slowing but not stopping. It is not limited to one specific road or location. Service and repair attempts: The vehicle has been presented to a dealer on 06/30/2025, 07/09/2025, 07/17/2025, 08/01/2025, and 02/06/2026. Additionally, I called and confirm there was no software update, recall, or corrective action to be taken on 03/25/2026. The dealer stated they could replicate the issue but could not identify a solution. No permanent fix has been provided.Read more

Incident date: May 2, 2026
The failure involves the AWD drivetrain system, specifically the rear differential assembly and previously the electromagnetic AWD coupler. The vehicle developed significant shuddering and vibration during acceleration from a stop and while turning at low speeds. A Nissan dealership has confirmed metal contamination within the AWD system and recommended replacement of the rear differential. The ve
... hicle has not yet been repaired and is available for inspection upon request. This condition creates unpredictable acceleration and hesitation, particularly when entering traffic or making turns. The vehicle does not respond consistently to throttle input, which poses a safety risk by limiting the driver’s ability to accelerate as expected and avoid potential hazards or collisions. The issue has been confirmed by both an independent mechanic and a Nissan dealership. The independent mechanic identified abnormal debris during fluid service and suspected a drivetrain issue. The dealership later confirmed excessive metal contamination in the AWD system. The vehicle has been inspected by an independent mechanic and a Nissan dealership. The manufacturer has not physically inspected the vehicle but has been contacted through Consumer Affairs. No warning lights or messages appeared. Symptoms began around November 2025 at approximately 80,000 miles and have progressively worsened.Read more

Ford Motor issued two safety recalls regarding this, but has since closed them without providing a safe resolution for consumers. July 8 2019 Ford issued Ford Safety Recall program 19S22 (following 18S32) for stuck open canister purge valve (CPV). This can result in a failure to bleed the vacuum from the gas tank, and deforms the gas tank upon failure. This has been confirmed, reproduced, and r

The issue appears to involve the vehicle’s battery system. The issue is abnormal battery drain and corrosion buildup on the battery terminals. The failure results in the vehicle being unable to start without warning. The battery and affected components are available for inspection upon request. This issue creates a safety risk because the vehicle can become inoperable without warning, leaving me
... stranded. This could occur in unsafe or vulnerable situations, such as being stuck in traffic, in an isolated area, or during extreme weather conditions. The unpredictability of the failure increases the risk, as there is no reliable warning before the vehicle will not start. I've been stranded multiple times. Yes. An independent mechanic can confirm the issue. There were little to no warning indicators prior to failure. The issue primarily presents as unexpected battery drain and corrosion, followed by the vehicle failing to start. Life-threatening, unoccupied rollaway vehicle with personal injury. Possible and/or suspected issues: "Not-P" switch circuit malfunction; "false park"; lack of cross-module redundancy; lack of BCM and PCM redundancy; Diagnostic Blind Spot; lack of Occupancy-Brake interlock; systematic safety failure culminating in unintended, driverless, runaway Zombie vehicle with personal injury. Background
... Info: * Low mileage, mainly in-town errands driving. * Not previously reported quirks, possible latent defects: liftgate opening that was hit-or-miss requiring multiple unlock trunk-unlock doors + get out of the car manual attempts; intermittent off-shutdown double-tap issue to power off car; ill-timed "rear seat alert". * Reported "engine won't start" resulting in three key FOB battery replacements and a locked key FOB. Day of Incident: * Auto-lock and the EBS were not used. * Driver intent was to park and unload, then permanently park. Car was in-park twice before ultimately placed in reverse and backed into position, facing downhill on a slight slope and along a left-side stone retaining wall. * Driver thought vehicle was fully in Park. * Attempted open liftgate via "open trunk, open door" routine, which did not work, so I exited the vehicle to try the manual liftgate button, which also did not work. * Three back-and-forth "unlock trunk/doors + manually press liftgate button" did not work. * Note: Upon opening the door or anytime after, I did NOT hear an audible warning that the car was not fully in Park. Nor did the car move/creep in any direction. * Deducing I must need to shut off the car to open the liftgate -- I leaned in and depressed the on/off button. * This caused the car to move forward, driverless, door open and scraping the stone wall, across a secondary road, over an 11" curb, and down a hill before stopping due to a tree. * I was injured. GM 6.2L L87 was recalled. I took the truck in at 32,000 miles. GM dealer perfromed a Pico Test, motor failed. Motor replaced with "NEW" crate motor. I was without a truck or loaner for 3 weeks. Dealer called me when new motor was almost complete. First they said it would be a couple more days because they started the "NEW" motor and it had no oil pressure. They contacted GM Tec and they tested it
... again, it tested with too-high of oil pressure. GM then instructed them to replace the oil pump on the "NEW" Motor. I got the truck back a week later after being in the shop 3 weeks. I drove a total of 1,200 miles, 400 miles we're highway in one long trip. The balance was local. I went to start the truck one morning after it sat all weekend and the "NEW" motor was knocking like an old diesel. I video'ed it and sent to my service advisior, I was advised to bring it back again. Vehicle has been at dealer now 9 days, dealer took oil sample from "NEW" motor, it was found to have metallic matter in the oil. They contacted GM Tec again, it took GM Tec 3 days to respond, Last Friday GM Tec said pull the oil pan and take pictures. As of the typing of this email I have no loaner because there's a wait list for loaners, GM will only reimburse me $25/day for a rental and a truck rental is $105/day which I refuse to pay for GM's defects out of my pocket.Read more

My head, chest, back, neck, shoulders, right arm, right hand, wrist, and right leg/ankle were all injured due to this accident. If my airbags had deployed, my injuries would’ve been lessened and they could’ve been prevented altogether. I was rushed to the hospital at the scene. I’m still currently receiving treatment for the injuries I sustained from this crash and lack of airbag deployment. No, the problem hasn’t been reproduced nor confirmed by the dealer. No one has inspected the vehicle. An adjuster from the at fault party has only taken pictures of the exterior of the car, but they didn’t inspect the car. Vehicle exhibits persistent, intermittent failure of Bluetooth and Apple CarPlay connectivity. Phone fails to pair, drops all connection mid-drive (all displays including speed, navigation, etc go dead), and disconnects active calls and navigation prompts during operation. Defect requires repeated manual re-pairing attempts that draw driver attention from the road. The same phone pairs reliably wi
... th all other vehicles and devices tested, including a more recent 2025 Countryman dealer loaner and personal home equipment, indicating the defect is in the vehicle's electronic architecture rather than the phone or user settings. Multiple service visits to an authorized MINI dealer have not resolved the issue. The dealership service team has stated verbally that "all the early 2025 Countrymen seem to have these issues." Dealer is now indicating that full head unit replacement may be required, with extended out-of-service time. The defect creates a documented driver-distraction risk and aligns with multiple existing NHTSA complaints and BMW Technical Service Bulletins for the 2025 Countryman platform. The sudden outage meant I had no information while driving on the interstate at speed and didn't know what was happening. I didn't even know if I was indicating to move over to the shoulder of the road. This happened at least 4 other times since I had had the car but thankfully only the once on an actual highway. Other times were in Atlanta city traffic and a couple of times in my driveway. Even more frustrating, I cannot get it to reliably pair with my iPhone 17 Pro. The lack of reliability increased markedly with the last MINI update which occurred about a week ago. I need to travel for work and cannot rely on this car to get me where I need to go and I actually avoid driving it now. The problem has been validated by the Dealer. The Service intake worker noted, "We have seen this in almost all of the early 2025 Countryman models." There was no warning at all when it happenRead more

The contact owns a 2016 Ford Escape. The contact received notification of NHTSA Campaign Number: 25V695000 (BACK OVER PREVENTION). However, the part to do the recall repair was not yet available. The contact stated that the rearview camera would display a distorted image while the vehicle was in reverse(R). The contact stated that the failure had obstructed the driver's visibility, which prevented
... the driver from safely operating the vehicle. In addition, the contact stated that the infotainment screen kept flashing on and off, which caused a distraction. The local dealer was contacted and confirmed that the recall part was not available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The failure mileage was approximately 160,000.Read more

Safety Defect Complaint – Tesla Model Y FSD v14.2.2.5 I am reporting unsafe speed behavior in Tesla Model Y Full Self-Driving (FSD) software v14.2.2.5. FSD uses selectable driving profiles (Sloth, Chill, Standard, Hurry, Mad Max) to control speed, but provides no definition of how these profiles affect speed or limits. Tesla also removed the ability to manually adjust speed incrementally, redu
... cing driver control. While in Standard mode, the vehicle exceeded the speed limit by 12 mph, resulting in a photo radar citation. This was not driver-initiated. I have experienced at least six incidents where the vehicle exceeded the speed limit by 10+ mph without warning. Two design issues create significant safety risk: The right scroll wheel now controls both microphone and FSD profile selection. Its multi-directional design allows unintended inputs that can change speed profiles (e.g., to Hurry or Mad Max) without driver awareness. There is no audible or visual alert when this occurs. The vehicle can accelerate beyond safe/legal speeds before the driver realizes and disengages FSD. The driver-set speed warning (set to 8 mph over limit) failed to activate during multiple overspeed events, including the citation incident, despite being enabled. This removes a critical safety safeguard. Speed increases have occurred both gradually and rapidly, reducing reaction time. Safety Risk: unintended acceleration, loss of predictable speed control, lack of driver alerts, and failure of safety warnings. These conditions could lead to violations or collisions. I request NHTSA investigate whether this constitutes a safety defect.Read more

I, [XXX] was driving home from work on Friday, May 1st on [XXX] in [XXX] during heavy traffic when my vehicle suddenly shut off while traveling approximately 65–70 mph. I initially thought I may not have been pressing the accelerator hard enough, but the vehicle would not respond at all. I then looked at the dashboard and saw a message that said “push brake to start.” At that moment I re
... alized the vehicle had shut down completely while I was in active traffic. I had to coast across three lanes of traffic without any propulsion to reach the shoulder. During this time, I was nearly struck twice from behind because I could not maintain speed with surrounding traffic. Once I reached the shoulder, I was able to restart the vehicle. I was extremely shaken and frightened by the incident. I do not feel safe driving this vehicle again. I was very aware that I could have been seriously injured or killed, and I am thankful I was not struck. I am especially concerned about what could have happened if my grandchild had been in the vehicle with me. This incident occurred after a recall repair had already been performed in September 2025 related to powertrain/hybrid system issues. The failure appears to be the same or similar to the condition the recall was intended to correct. After restarting the vehicle, I drove cautiously to my husband’s workplace and left the vehicle parked. Since the incident, I have experienced ongoing anxiety and distress reliving what happened. This vehicle was intended to be a long-term purchase, and it is extremely concerning that it now appears unsafe to operate. This event represents a serious safety hazard. A sudden loss of propulsion at highway speeds without warning creates a high risk of crash, injury, or death. I am requesting that this issue be investigated, as the prior recall repair does not appear to have corrected the problem. I do not feel this vehicle is safe to operate. I am submitting a safety complaint regarding my Ford vehicle’s infotainment screen failure, which has been identified under a recall but currently has no available remedy or repair timeline. When the screen goes blank or becomes unresponsive, I lose access to critical safety features, including the rearview camera, which is federally required. This significantly increases the risk of accidents
... while reversing, especially in areas with pedestrians. The system also affects access to navigation, hands-free communication, and important vehicle alerts, forcing me to either drive without these features or become distracted trying to compensate. Although Ford has acknowledged this issue, there is no clear timeline for a fix, and no accommodations have been provided. I am expected to continue driving a vehicle that does not meet expected safety standards, which puts both myself and others at risk. I rely on my vehicle daily, including transporting passengers, and the lack of a functioning safety system is a serious concern. The absence of communication and support from Ford regarding a resolution is also concerning. I respectfully request that NHTSA investigate this issue and require Ford to provide a timely remedy or appropriate accommodations, such as a loaner vehicle or interim solution, to ensure driver and public safety while awaiting a permanent fix.Read more
