The “Consider Taking a Break” notification while driving is incredibly loud and distracting even when on the lowest setting and can repeat up to every 20’ while driving. The vehicle began accelerating and decelerating erratically without driver input, creating immediate risk of rear-end collision or loss of control. No warning lights or messages appeared before the incident. The vehicle displayed "Thermal Fault Detected" and "Coolant Level Low" messages only after the erratic behavior began. This incident occurred with my family in the vehicle and created serious risk of injury or death. I was forced to have the vehicle towed. PRIOR RELATED INCIDENTS: This is not the first safety issue with this vehicle. Service Request SR-RAB3DU1AM documents "Emergency Braking Mistakenly Triggered on Regular Roadway" from August 29 2025 that remains unresolved in Rivian's system. The vehicle has required multiple service visits for thermal and cooling system failures at only 15000 miles. MANUFACTURER RESPONSE: Rivian service center claims rodent nesting caused thermal system failure leading to loss of vehicle control. If correct, this indicates critical design flaw - environmental factors should not be able to compromise vehicle control systems without warning or failsafe mechanisms. Vehicle lacks adequate: - Warning systems for thermal degradation before catastrophic failure - Failsafe mechanisms to prevent loss of control - Protection of critical systems from environmental factors SAFETY CONCERN: If thermal system damage can cause sudden loss of vehicle control without warning, this represents unreasonable safety risk. Now Dealership did full diagnostics and tells me the car needs PDM (Power Distribution Module) replaced. Nissan previously issued Service Campaign P9351 for this exact issue, but my VIN is not included. I believe my vehicle has the same defective PDM and poses a safety risk due to sudden loss of drive power. Nissan has declined coverage. I am requesting NHTSA investigate whether the recall should be expanded to include additional affected vehicles. Also there is open Recall R24B2 regarding Lithium Battery getting too hot durinig Fast DC charging for which they don't have the Remedy yet. I recall that only 3-4 weeks before above incident, I had my previous Fast DC charging session, after which I got a message on dashboard saying something like "Battery is very hot". I waited for 10 minutes before driving. Each time I would drive from standstill position, it will accelerate very slowly, but eventually drive OK after speeding up. Driver's side headlight adjuster screw breaks very easily, and makes the headlight beam be too low with inability to adjust it. The ADAS camera has failed. It is extremely common. When it fails, all the lights for EVERY system of ADAS comes on. None of them work. And it is not possible to turn off. It will drive the vehicle, unassisted, down to 20 mph, then just shuts off and lets the vehicle coast, unguided. Also, if the cruise is set and lane keeping on, and it can’t find lane lines, particularly where lanes merge, it will often veer left, into oncoming traffic. I have tested both of these, when the system was functioning “correctly”. The cruise and lane keeping just shuts off, at any speed, and lets the car go where ever it would without input. I contacted General Motors Consumer Department and sent a letter to Mr. Mark Reuss, President of GM requesting that the vehicle be replaced or the money I paid be refunded. They refused to replace the vehicle or refund the money I had already paid. Without any warning and after about 7 minutes of driving, Jeep turned off while driving at about 40 mph. I only say that to let you know that I have a lot of experience with Jeeps, was able to exclude certain causes and have not encountered thisnwith any of my previous Jeeps. Jeep is in show condition and meticulously cared for. The day after vehicle shut off, all dash lights and windows also stopped working so issue also caused batteries to fully drain. Dealership says it is needs a new PDC. I strongly suspect this is due to manufacturer defect related to PCM and some of manufacturing defects associated with COVID-era manufacturing issues. Uncertain if it could also be related to other open recall. Vehicle has less than 50K on it. It really doesn't feel right that just a large component would go out on such a new Jeep and definitely isn't in accordance with the quality I've come to know and expect when owning other Jeeps. Would appreciate anything you can do to assist. On another note, about half of the Gladiators I see have the 3rd brake light out (mine included). The pinion and carrier bearings failed on a less than 3 year old truck with 66k miles. This is way to early for this type of repair and was just out of warranty. This could have caused the truck to lock up while driving possibly injuring myself and other drivers. The noise caused from this was heard months before but was believed to be tire related.

Rear view mirror backup camera flickers and goes on and off while in reverse, sometimes with the screen remaining blank.
