I am submitting this complaint to report a significant safety defect in my Lexus vehicle. Without any prior warning or history of mechanical problems, the vehicle experienced a sudden and severe loss of engine power while I was merging onto a busy highway. Following this incident, I took the vehicle to a local auto shop, which identified diagnostic code P030002, indicating a misfire in Cylinder 2. I then promptly brought the vehicle to an authorized Lexus dealership. Their diagnostic confirmed the same code and attributed the failure to a “Cylinder 2 direct fuel injector leaking.” Lexus has already acknowledged this defect in TSB L-SB-0007-21, demonstrating that the manufacturer is aware of this recurring issue. My vehicle is five years old with fewer than 100,000 miles, yet this defect resulted in a dangerous loss of engine power under highway conditions. The dealership quoted over $3,000 to replace a single injector, and the remaining injectors may fail at any time, further compounding the safety risk. Despite its knowledge of this defect, Lexus has not issued a recall, implemented preventive measures, or offered assistance, citing that the vehicle is slightly outside the 70,000-mile warranty. I have therefore been left with a vehicle that is unsafe for continued highway operation. Given the severity of the incident, the known recurrence of this defect, and the manufacturer’s lack of corrective action, I respectfully request that NHTSA investigate this matter, as it presents a substantial and ongoing hazard to consumers and the driving public.Read more

The truck has recently and randomly been making a very loud static/poping sound that actually sometimes hurts my ears. After this static continuously plays even when I reset the sync system, cycle the ignition or turn the volume to zero, it still continues. This is followed by a strong electrical burning smell. Finally we had to exit the car because we could no longer take the heat. My husband went back to the car, 5 minutes late, while I was in the store to see if the car had finally shut down. To his surprise the lights and dash where still on. He noticed the start button light was also still on & thought if he hit the button the lights would shut off, but no, the car actually started & again he did not have the key fob with him. This is a serious issue & needs to be addressed & I do not feel that I should be paying for this repair. My husband unexpectantly passed away in Sept 2025 & now I am left to deal with this on my own. I have to get this issue fixed as I can not take the chance someone will take my car someday nor do I want to continue to sit in my car for 10-20 minutes waiting for it to shut down. The dealership I took it to diagnosed it as due to a faulty switch in the shifter which means its poor workmanship & not related to owner usage. Kia refuses to perform a U.S. safety recall due to my being active duty U.S. military stationed overseas. The affected vehicle is U.S.-spec and registered to a U.S. address. The recall (SC250) involves sudden loss of drive power and is a major safety defect. Kia has refused to authorize parts or repair overseas, putting me and my family at risk.

My 2023 Jeep Grand Cherokee 4xe (VIN [XXX] ) is affected by two separate federal safety recalls involving the high-voltage battery: Recall 95B / NHTSA 24V-720 and the newly issued Recall 68C / NHTSA 25V-741. Both recalls warn of a potential vehicle fire and instruct owners to stop recharging the battery and to park the vehicle away from structures and other vehicles due to fire risk. These warning

Recall 95B / NHTSA 24V-720 states that the battery pack may contain cells susceptible to separator damage which, combined with internal cell interactions, may lead to a vehicle fire. Owners are advised not to recharge and to park away from structures. Recall 68C / NHTSA 25V-741 states that under normal vehicle operation, the high-voltage battery may become damaged and lead to a vehicle fire, again instructing owners not to recharge and to park away from structures. These are virtually identical safety defects occurring back-to-back in the same high-voltage battery system. The 68C recall states explicitly that no remedy is currently available and there is no ETA, leaving me with a vehicle that cannot safely be parked at my home, charged, or operated as intended. I am now unable to use the hybrid/electric functionality at all, despite paying a premium for a plug-in hybrid vehicle. I am forced to operate the vehicle only in degraded gasoline mode with significantly reduced functionality and safety. The ongoing fire risk, inability to recharge, and requirement to park away from cars and buildings create a daily hardship and significant safety concern for my family. This is a serious safety defect affecting the same critical system twice in less than a year, and with no available remedy the vehicle remains unsafe to charge, unsafe to park near structures, and unable to function as advertised. My high voltage drive inverter experienced a failure resulting in loss of all power as well as regenerative braking. Mechanical braking and steering remained available. There were no warnings or indications before the failure. After the failure there were warnings that the propulsion system had failed and to pull over immediately.

The backup camera rarely works. It either has a black screen, an upside-down image or the image has lines through it. This puts me, my passengers and pedestrians in danger while trying to reverse out of parking spaces. There is a recall for newer Ford Edges but not for a 2013 so Ford is aware of the issue. I also previously owned a different 2013 Ford Edge and it had the same problems.

Despite this being an extremely dangerous event consistent with the recall symptoms, I was repeatedly told the vehicle is “safe to drive in gas mode,” even though they did NOT inspect it, did NOT ask for diagnostic information, and did NOT offer any safety assessment. They refused to authorize towing, refused to authorize inspection, and told me to pay out of pocket for a rental despite the vehicle being unsafe and unusable. The dealership also stated they have no way to repair the recalls and provided no solution or timeline and refused a loaner vehicle. The vehicle cannot be safely driven the 40+ miles to the dealership due to the prior failure. This vehicle is unsafe to operate, has already experienced the failure described in the open recalls, and the manufacturer is not providing any remedy, inspection, transport, or safe alternative transportation. We paid over $100,000 for this vehicle and currently cannot use it at all. I am reporting this because the defect has already occurred and poses a serious safety hazard to us and other drivers. Ford confirmed an issue on 11/3/25 and another independent mechanic replaced the converter, but stayed the transmission needs to be rebuilt after that did not correct the problem. He states that the torque converter would not cause the reverse issues. Initially, no warning lamps came on, but now the wrench light and check engine light are on. Ford app gave two errors, one on 11/13/25: Powertrain malfunction/reduced power What Is Happening? The transmission control module has detected a concern when trying to shift to 8th gear. A failsafe logic disables 7th gear for the remainder of this key cycle. What Should I Do? Have your vehicle checked by an authorized dealer as soon as possible And one on 11/21/25 Powertrain Malfunction/Reduced Power: What Is Happening? The powertrain control system has detected one of the transmission clutches did not apply within the expected time as expected. Some transmission gears may not being available resulting in delayed or harsh shifts into another gear. Additional alerts that may provide additional information may accompany this alert. Vehicle already had Powertrain issues last year when transaxle assembly was replaced and there was an issue with the exhaust, which ended up falling off the engine after Ford worked on it. The check engine warning light illuminated, and the battery light illuminated intermittently. The contact was able to pull into a gas station. The contact called his daughter, who went to the gas station and jumpstarted the vehicle. The contact stopped by Auto Zone to have the vehicle diagnosed, where it was diagnosed and DTCs: P2191, P0562, C1101, C1102, C1201, C1204, C1207, C1208, C1210, C1211, C1237, C1260, C1513, C1613, and C1688 were retrieved. In addition, the battery showed codes that fluctuated constantly. The vehicle was taken to the dealer pending an inspection. The vehicle remained at the dealer unrepaired. The contact stated that recently the low oil warning light was illuminated, and the fuel level was low; however, the oil level and fuel level were not low. The ABS, traction control, steering, and tire pressure warning lights were illuminated. In addition, the brake warning light failed to function as intended. The contact replaced the brake lights however, the failure persisted. He suggested what I had already done including removing the battery cables. We did all this to no avail. He got the car then took 3 days to get it to release after opening and closing the door 10 times before starting the car again. He said I need a modulator valve. We have been waiting for over 6 months and my car is still waiting for this part. I can not drive the only car I own when I have a safety issue. Should this had happened while driving with my grandson in the car it is unsafe and it isn't what an autistic child needs to face. This incident has happened 4 times since it happened in June. Every time it takes place, I have to turn off the car open and close the door 10 times before the carvwill release and reset the computer system to make the car drivable again. I did check with the dealership yesterday and still other jeeps of this year has the same issues but my car is not in that listing. The idle-stop function would frequently malfunction. I would be at a stop, which would trigger the idle-stop function and the car would turn off, but when I released my foot off the break in preparation for needing to start moving, the car would stall and shut completely off. ... traffic all around me. After the malfunction occurred several times, I would try to remember to turn off the idle-stop function using dynamic mode. I called my local Acura dealership about this problem and the service manager said that he wasn’t sure what was wrong nor did he know how to fix it. He advised me to just wait and hope that Acura issues a recall for the issue. Then, I started noticing that there was a delay in my vehicle starting when pressing the start button. It wasn’t too long of a delay but enough for me to notice. Since the car would ultimately start, I didn’t think too much about it, until my car failed to turn on. It has happened when I turn off my car but then need to turn it back on soon after turning it off. I’ll press the start button and the lights on the dash come on and I can hear the engine trying to turn on but it doesn’t. The engine won’t crank up. I have to let it sit for a few seconds (30 sec?) and then try to turn it on again, at which time I have been lucky that it will turn on, but the initial start-up sounds like a struggle. ... nd. Before we were able to leave the lot, the telematic system broke again. The dealership inspected the vehicle and confirmed that the telematics module had failed. We left the vehicle at the dealership,. There were no warning lights, error messages, or other indications before the failure occurred. After the failure, several critical safety and driver-assistance systems became inoperable or severely impaired. These include, but are not limited to: OnStar emergency services Automatic crash notification Steering assistance Cruise and speed control Lane-change and driver-assist systems GPS and navigation functions Bluetooth, Wi-Fi, and vehicle internet connectivity Although the vehicle is technically drivable, the loss of these systems poses a significant safety risk to occupants and others on the road. The vehicle cannot contact emergency services in a collision, cannot send diagnostic data, and lacks essential driver-assistance and navigation capabilities. The dealer has confirmed the problem, but the replacement telematics module is currently backordered with no clear timeline, and they stated the delay could last weeks or longer. Because the failure appears to have been caused by a GM-issued OTA update, and because essential safety systems are offline, I am concerned about both the defect itself and the extended inability to repair it due to unavailable parts. UNKNOWN whether GM has conducted a broader inspection or analysis of this update or component, but the failure has been reproduced and confirmed by the dealer’s service department.Read more

If you google 73B recall service problems on Chrysler Pacifica - the following appears: Failure to charge and engine light: Some owners have reported that the vehicle no longer charges after the recall service and has a check engine light, with error codes like P0A0A (High Voltage System Interlock Circuit) and P0A7F (Hybrid/EV Battery Pack Deterioration), says Reddit users. Intermittent check engine light: Other owners have noted the check engine light comes on intermittently after the recall service, sometimes disappearing on its own. Battery concerns: Some owners who already had issues with the battery before the recall now report that the recall service did not resolve their problems, and a new, more significant battery issue may have developed. It appears that the issue with the Hybrid battery is not being resolved and the service recall for 73B amplifies issues for the Hybrid Pacifica Models. It may be a ploy from Chrysler for owners to get “rid,” of our vans. I’m contacting the lawyers of the class action lawsuit with this information. Now the dealership is telling me I must pay $1,127 just for them to look inside the transmission, and potentially $2,800–$11,000 for repairs. This is a safety-critical defect that began well within the warranty period and has progressively worsened. A sudden loss of power and inability to control the vehicle is dangerous and could result in a crash, injury, or death. I believe this issue should be investigated for a recall or manufacturer responsibility due to multiple reports of similar failures in Ram 1500 models.
