Following the initial impact, while attempting to move the vehicle to a safe location as instructed by insurance/authorities, the engine suddenly stalled and shut down completely while in motion, nearly resulting in a second high-speed collision with the vehicle ahead. CORRELATION WITH RECALL 26V237: In May, I received a Safety Recall Notice (Ford Recall 26S28 / NHTSA Recall 26V237). The description of the defect—an "unexpected downshift to a lower gear causing abrupt wheel speed reduction and rear tire slide"—exactly matches the catastrophic event I experienced in March. The recall also notes that the engine may malfunction or stall, which occurred during my incident. IMPACT & DAMAGES: The insurance company evaluated the damage at approximately $6,000 for structural repairs. However, the vehicle remains unsafe and mechanically compromised. Despite the structural fixes, the core safety defect involving the transmission and engine software remains unresolved as Ford has stated that a remedy will not be available until July 2026. A second Toyota dealership subsequently found cracked motor mounts and transmission mounts in multiple locations on a 3 year old vehicle. Replacement of these components did not resolve the issue. Transmission fluid drained at service was nearly black in color on a Gold Certified vehicle that supposedly passed a 160-point inspection including transmission health check. The vehicle is currently at the dealership unresolved after nearly two weeks. The stalling and RPM surging present a serious safety hazard particularly at highway speeds. Safety Impact: This glare can cause temporary visual distraction and reduced forward visibility. In some cases, the reflected light is intense enough to momentarily obscure portions of the road view or draw the driver’s attention away from driving. The issue is angle-dependent and not constant, but it is repeatable under common sun positions and clear weather conditions. Assessment of Cause: The condition appears to be related to interior material selection and surface finishes that include reflective or semi-gloss components positioned within the driver’s forward field of view. The combination of windshield angle, dashboard geometry, and placement of polished trim elements contributes to a direct reflection path toward the driver. A Ford Field Service Engineer was dispatched April 23, 2026. On April 24, 2026 the dealer located damaged wiring inside a harness behind the BCMC bracket. Prior to that, the dealer escalated through R&I of multiple modules (PSCM, PCM, BCMC, Gateway Module access) over 45 days without locating the actual cause. The vehicle is currently subject to NHTSA recall 26V104 / Ford 26C10 (Trailer Module loss of communication at startup) issued 3/18/2026, status Recall Incomplete. The recall describes the same general failure mode (module communication loss at startup) the customer has been experiencing. ... vent smoke and assess for fire. Once fire was not an issue, I started the ignition sequence to view any warning lights before starting motor. There were no warning lights or sensors indicating loss of fluids or abnormal temperatures noted in oil or transmission. The motor started and then I tried shifting through the transmission. It would engage but not move forward in any of the Drive Train Selector settings. I could travel in reverse and place in neutral, but I could not manually move forward or back when I pushed. I called Ford Roadside Assistance for towing and was flat-bedded to Sterling Korf Ford where catastrophic front end differential and assembly was noted. I had an experience leaving my work parking lot 2/27/2026, and have a malfunction warning on my vehicle phone app. That event was corrected by stopping and rotating through the Drive Train Selector switch out of 4x4 to 2H. I had 100K mile full service at Greeley, CO, Ford in July 2025 with all fluids flushed and filled and multipoint inspection. I feel this is a mechanical design flaw in the Integrated Wheel End (IWE). And with no dashboard warning to hint at impending catastrophic failure, this warrants immediate recall notice. I was fortunate not to be in heavy traffic when catastrophic failure happened, otherwise an accident would have occurred. I am a [XXX] career Emergency Physician and have enough experience to know when there is a system failure and a near miss. I own a 2019 Mazda CX-5 with approximately 36,000 miles. The vehicle developed a strong burning oil smell while driving, particularly through the HVAC/vent system. ... he cylinder head near the exhaust manifold. According to the repair documentation, oil was accumulating on the exhaust due to seepage from the cylinder head. The repair required full cylinder head replacement. This created significant safety concerns because leaking oil was contacting hot engine/exhaust components while the vehicle was being operated. Burning fumes entered the passenger cabin through the vents while driving. I was concerned about the risk of fire, smoke inhalation, sudden oil loss, and possible engine failure while on the road. There were no warning lights prior to the failure. The primary symptom was the persistent burning smell. The dealership replaced the cylinder head. The original repair estimate was approximately $6,000. Mazda later provided partial goodwill assistance, but I was still required to pay approximately $2,500 out of pocket. After researching this issue further, I found numerous similar NHTSA complaints involving 2019 Mazda CX-5 vehicles experiencing cylinder head leaks/cracking near the exhaust manifold, burning smells, smoke or fumes entering the cabin, and major engine repairs at relatively low mileage. Mazda has acknowledged similar cylinder head concerns in CSP11 and TSB 01-002/23 for certain VIN ranges but denied coverage for my vehicle because the leak involved oil rather than coolant and because my VIN was outside the covered range. I am requesting that NHTSA investigate whether this represents a broader cylinder head defect affecting additional 2019 Mazda CX-5 vehicles outside the currently covered VIN range.Read more
