On Aug 1st I started noticing a slight shake in the vehicle on the way to work in the morning. I can best describe this shake or shutter like an imbalanced tire or a misfire. ... a slight metallic sound. It was not until I got into town where I was set a slower road speed that I could clearly hear that the metallic sound was a rod knock. I have the hybrid model of the new Tundras, and on that last bit of stretch getting home in town the truck never threw an error or even understood that it was having an engine failure. Towards the end it would kick the engine on and off trying to flip flop between hybrid/EV mode and ICE mode. And some of those instances I think the engine was stalling out. Mine engine never seized up fully, but if it had I question whether or not the ECU would have understood this to disengage the engine from the drivetrain to propel itself only on the MG. As of today (12/5/25) my truck is still down at the shop because of this engine failure. Toyota originally covered a short block replacement under warranty. After the dealership assembled that new engine they are having nothing but one problem after another. After about a month to a month and a half of fighting the new engine Toyota's FTS got involved, were they ordered the new engine to be disassembled for investigation. They found that the new engine had completely destroyed itself, and they claim the cause was from the technician not applying assembly lube on the camshafts. After that Toyota blamed the technician at the dealership and refused to help fix my truck (which is well within the drivetrain warranty). I now have an open arbortation case with the NCDS for this.Read more

My vehicle has the 8F35 transmission and is experiencing severe jerking and shuddering. With the current snow and slick roads, the car feels unsafe to drive. The sudden jerks make me fear I could lose control, slide into another car, or end up in a ditch. Regarding TSB 21-2389, my local Ford dealership told me the reprogramming would not fix the issue and didn’t attempt it. They stated that I need a torque converter replacement, which is a $3,300 repair that Ford should be responsible for. Allowing vehicles with known safety-related transmission problems to remain on the road is unacceptable. I also followed up in June of this year about TSB 25-2154. I have a voicemail from the dealership’s GM saying they would take care of the issue after I contacted him, but nothing came of it. They refused to help unless I paid out of pocket due to the vehicle’s mileage. This is my primary daily driver, so the mileage continues to rise out of necessity, not neglect. I’m doing everything I can to drive as safely as possible, but this is becoming increasingly dangerous. I’m asking Ford to take responsibility for this known defect and properly address it. The vehicle shut off and would not start while at a stop light. It is new: under one year old and still within warranty. The error was a “Restart not possible. As I was on the phone with emergency roadside assistance while out of the vehicle for about 10 minutes, the vehicle was able to start with the warning now displaying “Drivetrain. You can continue driving. Have the problem checked by your local service center.” I hesitantly drove the vehicle home. While parked, I checked about 15 minutes later and the issue resurfaced (as pictured). The problem has not yet been taken to the dealership. It will be tomorrow. The issue has not yet been inspected by any other representatives. There were no warning signs whatsoever before the issue occurred. In September, the dealer reached out to Volvo to have the computer “brain” replaced. They had the car for three days only to be told Volvo denied the replacement because what is occurring is a known issue and an update release was coming in November to fix the issue. November came and went and still no update. We spoke to the dealership and they had no idea. They recommended us contacting Volvo directly. We reached out to express our frustration and was told someone would get back in a few days. About a week later, we received an email from Volvo expressing their sorrows for our inconvenience and to open a case with our dealership. I was extremely disappointed as I responded indicating that’s what we have done time and time again and there’s nothing they can do. It’s a known error and the only way to reset to have the car parked and hold the menu select button. It takes about 3 minutes or so for the system to reboot. The last time this occurred we were driving home and the system shut off. This left us without heat and no safety features. Volvo is clearly aware of the issue and it appears there’s no urgency to get this resolved. 2017 Honda CR V- Hybrid All warning lights on the dashboard—including the Check Engine Light, Oil Pressure Warning, Battery Warning Light, Tire Pressure Monitoring System (TPMS) Light, Brake System Warning, ABS (Anti-lock Braking System) Light, Fuel Level Warning Light, and Engine Temperature Warning Light—activated intermittently. ... started, with symptoms lasting from a few days up to several weeks. The warning lights would temporarily resolve, with periods lasting from a few days to several months. The vehicle's battery was replaced twice within one year, which seemed to temporarily alleviate the issue; however, the warning lights recurred afterward. Recently, Honda conducted a thorough diagnosis and identified the root cause, leading to the replacement of the spark plugs and fuel injectors at an approximate cost of $2,000. If unaddressed, this issue could pose safety risks such as rough idling, decreased fuel efficiency, engine misfires, difficulty starting, poor acceleration, unexpected stalling, complete loss of power, and electrical problems. This issue is recognized by Honda and is currently covered under a recall for the 2023-2025 Honda Pilot and the 2023-2025 Honda Accord Hybrid. While the Honda Accord Hybrid and Honda CR-V Hybrid share the same fuel injection system as part of Honda’s two-motor hybrid-electric powertrain, the Honda CR-V Hybrid has not yet been included in this recall. Nonetheless, many Honda CR-V Hybrid owners are experiencing similar issues and are incurring repair costs out of pocket. Summary of Issue: The infotainment system in my 2019 Mazda CX-5 has been malfunctioning for a long period of time, and the issue has now escalated to the point where the system is unsafe and distracting while driving. I previously reported this issue to the dealership, where they attempted a reset and told me to come back if the problem returned. The issue persisted and worsened. I recently returned, and the dealership diagnosed a communication failure between the CMU and the infotainment screen. However, they cannot identify any cause, and the screen itself has no physical damage and has never been struck. Mazda has declined to cover this under warranty, even though this appears to be an internal failure of components that should not fail at this mileage. Additionally, infotainment failures in Mazda vehicles have been the subject of prior complaints and service bulletins. Safety Concern: The malfunction causes the screen to operate unpredictably while driving, which distracts from the road and creates a risk of an accident. Because the system controls access to vehicle settings, Bluetooth, audio, navigation, and backup camera interfaces, the failure presents a potential safety defect. Requested Action: I am requesting that NHTSA investigate whether this infotainment / CMU communication failure is a broader defect affecting 2019 Mazda CX-5 vehicles, as dealerships cannot identify the cause and the failure poses a clear distraction hazard.Read more

This incident happened twice in one week first with 2023 Nissan Sentra then replaced with a 2024 Nissan Altima both rented from Enterprise. I was traveling down the street from my house, a two-lane, steep hillside road, which I’ve traveled up and down for 30 years. I stopped behind a parked car on the right side of the road to let another car come up the hill and pass by before I continued down.

As I continued down the hill, the car just began to accelerate and I could not stop it, I tried to break, but no response and the car kept accelerating for about 50 feet before crashing into. the granite hillside, lifting the car so it landed perpendicular to the road, setting off all airbags and blocking all traffic completely. Thankfully there was no other damage to other cars, homes or people, which was a minor miracle.To reach such an impact speed in such short distance suggests some kind of mechanical failure. My insurance paid for a replacement rental, a 2024 Altima and on my first drive down the hill, in the exact same spot, the steepest of the road, the exact thing happened, rapid acceleration and no breaks.Read more

This suggests further deterioration of the electrical system. The combination of: • an open fire-risk recall with no fix available, • fluid ingress damaging telematics components, • multiple electrical and safety-system failures, and • new warning lights appearing after service makes this vehicle unsafe to operate, especially for long-distance highway travel. I am requesting that NHTSA document this as a safety-defect case and assist in ensuring BMW provides a safe resolution.Read more

I believe my vehicle is experiencing the recall noted earlier this year (N182182760 in 2025) for power steering unit failures in Corvettes manufactured in my car's timeframe. The recall is still expanding the list of cars associated due to its nascent nature, and I believe mine is one of them. My car was manufactured in 2016 and the manufacturing defect in the power steering unit circuitry was not corrected until 2017. None resolved the issue, and the vehicle became unsafe to drive due to rising temperature and smoke. A second dealer, Mesquite Kia, performed a proper diagnosis. Kia Corporate approved an engine replacement under WTY039 on 11/6/25, and the repair was completed on 11/29/25, confirming the overheating was caused by a covered engine defect. Kia Corporate later denied reimbursement for prior repair attempts by blaming “3rd party repairs,” ignoring that the dealership’s misdiagnosis forced me to seek outside help. This defect created safety hazards including overheating while driving, risk of stalling, coolant intrusion, and visible smoke. I am requesting NHTSA review Kia’s handling of this defect and the failure of Central Kia of Plano to diagnose or escalate a known engine issue, which allowed a safety defect to worsen. The recall notice states that the battery cells can develop excessive lithium deposits, which increases electrical resistance and can cause rapid battery heating during Level 3 fast charging. Nissan instructs owners not to use Level 3 charging at all until a “remedy” is available. The issue is that the remedy describ
... ed by Nissan is not an actual repair of the battery defect. Nissan states it will install software that monitors for “state-of-charge fluctuation” and, if detected, will prevent the vehicle from restarting or recharging in order to avoid a thermal incident. This means the defect inside the battery cells is still present, and the car may disable itself if the defect begins to appear. The recall materials also state there is no warning before overheating occurs. This creates multiple safety concerns: • The underlying battery defect remains uncorrected. • The vehicle can become immobilized (unable to restart or recharge) if the software detects the condition. • Loss of Level 3 charging capability affects the ability to travel safely or plan charging when needed. • A battery fire risk exists during Level 3 charging if the defect is not detected in time. Nissan’s documents state that the software is meant only to prevent the “progression” of a thermal incident, not to repair the defective battery. I am concerned that my vehicle contains a known hazardous defect that has not been physically repaired and that the software-only response is inadequate to ensure safety. I am filing this complaint so NHTSA is aware that the remedy being offered does not address the internal battery defect itself and may leave owners exposed to continued safety and reliability risks.Read more
