My 2023 Ford F-150 XLT (base 4×4—2H, 4H, 4L only) is experiencing a front-axle/IWE malfunction that causes the hubs to partially engage on their own in cold weather. When temperatures drop below freezing, the truck produces a very loud, physical pop when turning in 2H and a mechanical grinding/rumble noise from the front axle while driving straight. This happens even though the roads are dry a
... nd I have not engaged 4WD. The noise immediately disappears when I switch into 4H, which indicates the integrated wheel ends (IWEs) are partially engaging due to vacuum loss or actuator failure. When the hubs fully lock in 4H, the grinding stops. This confirms the IWEs are not disengaging properly in 2H. This occurs only in freezing temperatures, to date, and it feels as if the front axle is binding or locking during turns. The loud popping feels violent enough that it could lead to CV axle, differential, or IWE gear failure. My Ford dealer stated they have “not heard of this issue,” but this condition matches the symptoms described in Ford TSB 25-2513 for incomplete IWE disengagement on 2021–2025 F-150 trucks. Despite Ford saying this is not a known problem, there is significant owner evidence online showing that many 2021–2024 F-150 owners experience the exact same symptoms. Multiple threads on F150Forum, F150Gen14.com, and Reddit describe identical cold-weather popping and grinding caused by IWEs partially engaging in 2H. In many of those cases, owners report that dealers dismissed the issue or stated it was “normal,” even though the symptoms are widespread and consistent. This is a safety concern because the front axle partially engages without driver input, causing binding, loss of smooth steering, loud mechanical pops, and grinding while in 2WD. This could lead to sudden front drivetrain failure, loss of control, or damage to steering/braking components. I am submitting this complaint so NHTSA is aware that this issue is occurring F-150s and may not be isolated.Read more

The contact owns a 2023 Cadillac Escalade. The contact stated that while driving at various speeds, there was an abnormally loud motor sound coming from the vehicle. There was no warning light illuminated. The contact also stated that the vehicle unexpectedly shut off while in a parking lot and failed to restart after several attempts. The contact was picked by her husband. The contact stated that
... her husband later returned to the vehicle and was able to restart the vehicle. The contact then received notification of NHTSA Campaign Number: 25V274000 (Engine and Engine Cooling). The vehicle was taken to the dealer for the recall repair and passed a vehicle inspection, and the dealer added a higher viscosity oil, installed an oil fill cap, replaced the oil filter, and updated the Owner’s Manual. The contact stated that two weeks later, the check engine warning light illuminated, and the vehicle was taken to an independent mechanic. The mechanic inspected the vehicle and discovered there was no oil in the engine. The contact was informed that the vehicle was unsafe to operate. The dealer that had serviced the recall was notified of the failure and stated that the vehicle was serviced according to the recall. The mechanic informed the contact that the dealer had possibly drained the oil and refilled it with 0W-40 oil to pass the recall inspection and not have to replace the engine. The contact asked the dealer if the practice was performed during the recall repair, and the dealer avoided answering the question. The contact stated that a service member from the dealer arrived at the residence to top off the engine oil. The dealer suggested that an oil consumption test be performed every 1,000 miles to monitor the oil consumption. The contact stated that the recall remedy was insufficient and failed to properly address the issue. The vehicle was not repaired. The contact believed that the engine needed to be replaced. The manufacturer was not notified of the failure. The failure mileage was approximately 45,000.Read more

The rear camera of our vehicle has been intermittently failing for the past year. The VW dealer has not been able to repair as it is an intermittent issue. Today the entire screen wouldn't turn on to even be able to control the HVAC system, and it was a cold day. Luckily the rear has separate controls so my kids weren't freezing in the car. I was going to take it to the dealer today, but the scree
... n started working again and the service advisor said not to bring it in if it's not broken. We have brought this car in several times for the reverse camera not functioning with no repairs done. All we are told is the mechanic resets modules and it should be good. There was a recall for this issue which was performed. The problems occured more frequently since the recall was performed, but I'm always told that they can't fix what's not broken, which I understand. This is a large vehicle not having a reverse camera working properly at all times is a big safety concern that is being dismissed because they can't figure it out. I'm in a hard place. I can't leave them the car forever as it's the car we drive our kids in. They won't give us a loaner as they are not required to. Are we just stuck with a car that has an intermittent safety concern with no remedy? I have video of it not working, but it won't let me upload it.Read more

I purchased this 2024 Volkswagen ID.4 as a Certified Pre-Owned vehicle, and the electrical issues began almost immediately after taking ownership. While driving, the main infotainment display will suddenly reset, freeze, or go completely blank. Sometimes the instrument cluster also flickers or freezes during these events. At the same time, I received a “12V battery limited, please drive with car
... e” warning. When the center screen resets while the vehicle is in motion, I temporarily lose access to important driving information such as the backup camera, vehicle status, sound alerts, and sometimes the speedometer. These failures create a safety concern because they remove visibility of warning indicators and essential driving data. The 12-volt system warnings also raise concern that the vehicle may lose power to critical control modules or experience a sudden electrical shutdown. The issues are intermittent but recurring, and started immediately after purchasing the vehicle CPO, which suggests the defect existed before I acquired the car. This appears consistent with other reported electrical and 12-volt system failures on the ID.4. I am reporting this because I believe the issue may be part of a broader safety defect involving the vehicle’s low-voltage electrical system, displays, or charging circuitry.Read more

... to the GM dealer 4 times and they were unable to repair. My vehicle has a recurring defect with the windshield washer system where the washer fluid does not dispense when activated. This issue occurred on a new vehicle and has persisted despite service attempts. When the washer fluid fails to spray, I am unable to clear dirt, dust, road debris, or moisture from the windshield while driving. This significantly reduces forward visibility, especially at freeway speeds or in conditions where debris from other vehicles hits the windshield. The inability to clear the windshield immediately creates a safety hazard because it can obstruct the driver’s line of sight and increase the risk of a collision. This issue is not cosmetic, it directly affects safe operation of the vehicle and the driver’s ability to maintain visibility during normal driving conditions. I was able to get the car home very slowly, and strangely I had adequate braking power for the remainder of the short trip (although red and yellow brake warning lights remain on). I am happy to provide the car for inspection. This was extremely dangerous to the nearby pedestrians because I could not have stopped the vehicle had one been crossing in front of me. The door lock of the driver's door is stuck in the locked position and the door cannot be unlocked or opened. It will not unlock using the key fob, nor with the manual control inside the door, nor with the emergency key from outside the car. This could trap an inured driver inside the car in the event of an accident. The car has not yet been inspected by anyone. There was no warning lamps or mess
... ages prior to the failure. Earlier that same day the door failed to open while in gear but did so when I shifted to park. As this car was very recently purchased by me and I am still learning about it, I failed to recognize this a symptom of an impending failure. Regardless of that warning, total failure occurred the same day. As a graduate engineer who has worked in engineering related fields for more then 50 years I can recognize that a failure mode such as this is both a health and safety hazard to occupants in the event of an accident and a design defect that should not be allowed to be sold to the general public. I purchased (traded into) a 2021 Jeep Wrangler on October 31, 2024. The vehicle already had an open safety recall (68C) at the time of purchase. Shortly after, the vehicle began experiencing severe and dangerous safety failures. While driving, the Jeep suddenly goes into “turtle mode,” loses power without warning, and drops to 7–12 mph in traffic. The engine light comes on and the vehicle b
... ecomes almost undrivable. This creates an extremely hazardous situation where I could be rear-ended or left stranded on the road. This is a serious safety risk to me and to other drivers. I contacted Jeep/Stellantis Recall Customer Care to report the issue. Only after calling did I receive an email telling me I must pay out of pocket for a rental vehicle, even though the vehicle is unsafe and the recall remedy is not available. I immediately responded saying I cannot pay upfront. Since November 25, Jeep/Stellantis has ignored my messages completely. The vehicle remains unsafe, undrivable, and under a recall with no available parts. Jeep has refused to provide a direct-billed rental or any safe alternative transportation. I have been left without a usable vehicle while the manufacturer fails to communicate or offer assistance. This is a major safety defect, and the lack of manufacturer response puts me in danger. If this vehicle loses power in traffic again, it could cause an accident. I am requesting NHTSA intervention because Jeep/Stellantis is not addressing this safety issue, not providing a remedy, and not honoring their recall responsibilities. Symptoms: •Sudden power loss •Turtle mode activation •Speed dropping to 7–12 mph unexpectedly •Engine warning light on •Vehicle unsafe to operate Manufacturer Failure: •Recall 68C remedy unavailable •Denied direct-billed rental •Required me to pay upfront for rental •No response to my communication since Nov 25 •Left me without transportationRead more

My 2022 Ram 1500 TRX has recurring safety-related failures of the infotainment system. The issues began after forced over-the-air updates from the manufacturer early in 2025. The most concerning defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses all functions controlled through the display, backup camera, vehicle settings, seat controls, suspensi
... on annd steering settings, and performance gauges. This inability to access multiple functions creates an immediate safety hazard, including as a major distraction to the driver while the vehicle is in motion. Another highly concerning problem frequently observed includes a screen “spasm” where all visual displays are jumping around the screen and blinking multiple times per second, rendering the entire system useless. Even when these significant issues are not present, multiple distracting issues continue to persist including wireless CarPlay/Android Auto failures and glitches - again while the car is in motion, causing major distractions. These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

My engine has begun the "Pentastar tick", which is surprisingly well-known, but not recalled. This is a serious safety issue because it can cause engine failure, which would cause an accident.
