I am submitting this complaint regarding Toyota’s decision to exclude hybrid vehicles from an active safety recall affecting non-hybrid Toyota Tundra models equipped with the same twin-turbo V6 engine architecture. Toyota has stated that hybrid vehicles are excluded because they retain motive power through the electric drive system in the event of an internal combustion engine failure. This rat
... ionale does not address real-world safety risk and does not reflect the mechanical realities of towing or highway operation. The hybrid version of the Toyota Tundra uses the same base V35A-derived engine architecture, block casting, bearing design, and oiling system as the recalled non-hybrid vehicles. Toyota has not demonstrated that hybrid engines were manufactured differently or are immune to the defect that prompted the recall. In towing conditions, an internal combustion engine failure creates an immediate safety hazard even if limited electric propulsion remains available. Electric-only operation provides severely reduced power, limited speed, and restricted torque, making highway merging, grade climbing, or maintaining safe traffic flow unsafe while towing. Retaining minimal electric motion does not equate to maintaining safe propulsion under load. Additionally, a catastrophic engine failure may introduce metal debris, oil pressure loss, or thermal imbalance that can compromise shared drivetrain and hybrid coupling components. The presence of an electric motor does not mitigate the underlying defect risk or the secondary damage potential. Toyota’s distinction between hybrid and non-hybrid vehicles appears to be regulatory and financial rather than engineering-based. The same defect risk exists regardless of the hybrid system. I request that NHTSA evaluate whether Toyota’s exclusion of hybrid vehicles is based on actual engineering differences or solely on regulatory interpretation.Read more

There have been several times that I have been driving and either turning onto a 4-lane road (2 lanes going each way) or driving on the parkway, where my car has slammed on the brakes by itself. Not only was I not prepared for this to happen, but neither were the cars behind me. I tend to drive with at least 1.5 car-length space between myself and the car in front of me. Last week, I was making a
... right hand turn (going north) into the right lane from a residential street. The only other car near me was the one driving in the left lane (also going north). My car drove about 100 ft and maybe realized that there was a car next to me and just slammed on the brakes. I was suddenly thrown forward. My seat belt locked and I was thrown back into my seat. I had a bruise from my seat belt and came too close to slamming my head on my steering wheel. Thankfully, the car behind wasn't tailgating. About a month ago, I was driving on the parkway going east. I was originally in the right lane. I put on my blinker and when it was clear, I changed lanes to the middle lane. After I was clear and my blinker was off, my car started screaming to tell me that there was a car in my driver's blind spot (this happens EVERY time I switch lanes). I drove for maybe 1.5 - 2 miles and then suddenly, my car slammed on the brakes. Someone must be watching over me because the Escalade behind me (definitely tailgating) was somehow able to swerve into the left lane and then swerve around me to cut me off in revenge. They were screaming and honking at me, while I was screaming and yelling at my car. They could have easily have plowed into me (we were driving at least 60mph) and could have caused a horrible accident that could have killed myself or others around me. I have notified Hyundai of this issue and have read many similar complaints online. Hyundai claims they cannot "replicate the situation" and therefore, Hyundai Corp is unable to fix the problem.Read more

My 2022 VW ID.4 Pro S has experienced a total failure of the GPS and telematics system (J949 module), causing a dangerous 'GPS Drift' where the vehicle reports its location miles away from its actual position. This creates two severe safety hazards: Automated System Malfunction: The Travel Assist and Dynamic Road Sign systems are receiving conflicting data. The camera correctly identifies a spee
... d limit (e.g., 40 mph), but the faulty GPS overrides it with incorrect map data (e.g., 25 mph). This causes the vehicle to unexpectedly and dangerously decelerate while in active traffic. Emergency Service Failure: The eCall (Emergency Call) system is reporting the wrong location. In the event of a serious crash, emergency responders would be directed to a location miles away from the actual accident site. My dealer provided written documentation (attached) stating there is 'NO FIX' currently for 2021-2023 models, as they are hardware-locked to a failing Verizon-based data protocol that VW has not patched. The manufacturer has acknowledged the defect but offers no remedy for this safety-critical system.Read more

I am reporting a safety-related defect involving the vehicle’s infotainment and forward safety systems. When this occurs, the vehicle also shows a “Check Forward Safety System” warning message. Diagnostic trouble code C162078 is triggered, indicating a front radar out of alignment. As a result, critical safety features such as Forward Collision-Avoidance Assist, Smart Cruise Control, and ot
... her radar-based driver assistance systems are disabled. This issue creates a significant safety risk because the driver is not alerted to potential forward collisions and cannot rely on active safety systems. The blank infotainment screen also prevents access to vehicle settings, warnings, and camera functions. In South Korea, this same defect has resulted in an official recall and free front radar replacement and calibration. However, in the United States, customers are not covered under a recall and are required to pay out of pocket for radar realignment or replacement, even though the issue is identical and a lot of time the issue came back 3 months later and customer have to pay out of pocket once again to fix the same issue. Many owners report receiving official recall notices in other markets, while U.S. owners must independently diagnose the issue. Dealers often state that the problem may be caused by radar misalignment after a software update or system fault. In some cases, cleaning or recalibration temporarily resolves the issue, but in other cases, full radar replacement is required. This inconsistent treatment places U.S. consumers at a financial disadvantage and allows a known safety defect to remain unresolved. I am requesting that NHTSA investigate this defect and require the manufacturer to issue a U.S. recall with free repair, calibration, or replacement, consistent with actions already taken in other countries.Read more

While driving, the vehicle has repeatedly experienced sudden loss of power and electrical failures without warning. During these events, the dashboard flickers, warning messages appear, and the vehicle shuts off or goes into reduced power mode. When this occurs, steering becomes extremely difficult, braking becomes stiff, and the vehicle cannot accelerate. These incidents have happened multiple t
... imes in different driving situations, including: At stoplights While pulling into intersections Backing out of parking spaces Driving on public roads, including dark rural roads Driving with my three minor children in the vehicle On at least one occasion, the vehicle shut off while pulling onto a road with oncoming traffic, leaving us stopped in the roadway with no power and limited steering/braking. This created an immediate and serious risk of collision. The vehicle eventually restarted after several attempts. The problem has occurred repeatedly over many months. Multiple independent repair facilities have attempted to diagnose and repair the issue, but no definitive cause has been identified, and the failures continue. The vehicle has had numerous electrical, sensor, engine, and powertrain-related repairs, yet the loss-of-power events persist. Because the vehicle can suddenly shut down while in motion and compromise steering and braking, I believe this is a serious safety defect that could result in a crash, injury, or worse.Read more

Shortly after purchase, my 2025 Ford Maverick Hybrid AWD experienced a sudden and severe electrical burning or melting odor while driving at freeway speed (approximately 75 mph). The odor was extremely strong and clearly coming from my vehicle. At the same time, the vehicle produced a loud, continuous whirring or growling noise and experienced a sudden loss of power, as if under heavy load or brak
... ing. I exited the freeway as soon as safely possible. No visible smoke or fire was observed, but the odor raised serious concern for overheating electrical or hybrid drivetrain components Since the initial incident, the vehicle has experienced multiple similar but less severe events, including intermittent electrical odors, abnormal hybrid noises, hesitation or lag when switching between electric and gasoline operation, and a significant and permanent reduction in fuel economy. The vehicle has been taken to a Ford dealership multiple times for inspection. I requested inspection for signs of overheating or electrical damage. No diagnostic trouble codes were found and the vehicle was returned without repair. I am concerned this condition may involve overheating or failure of hybrid or electrical drivetrain components, such as the inverter, electric motor, or wiring, which may not immediately trigger diagnostic trouble codes but could pose a safety risk. I am also aware of other 2025 Ford Maverick Hybrid owners reporting similar electrical odors, abnormal hybrid noises, and power loss symptoms in public owner forums, suggesting this may not be an isolated incident. I am submitting this complaint to document a potential safety-related defect involving overheating or electrical failure in the hybrid drivetrain system.Read more

While the system was engaged and operating normally moments before, the vehicle accelerated forward as if the lane was clear, despite the semi being directly ahead. I had to apply the brakes manually to prevent a collision. A short time later in the same traffic situation, the vehicle did it again—it accelerated toward the same type of lead vehicle and I again had to brake to avoid rear-ending it. During the second event, I observed the instrument cluster / driver display showing the lead semi incorrectly identified as a passenger car instead of a truck, and the display showed that “car” accelerating away, which was inaccurate and did not match real conditions. This suggests the system may have misclassified or failed to track the lead vehicle, leading to unintended acceleration.Read more

A structural chassis component (V-brace) was permanently removed from my vehicle by Tesla Service during a service visit. The vehicle was sold and delivered with this component installed, and its removal was not disclosed or approved prior to service. The noise/rattle was present prior to the service visit and prompted the service appointment. The structural component removal occurred during the service visit. The problem was reproduced and confirmed by Tesla Service and they: •Acknowledged the noise/rattle concern. •Performed a structural retrofit involving removal of the V-brace. •Marked the concern as “resolved”, despite the noise still being present. •Later stated the noise is considered “normal”, even though it is not present in all Model Y vehicles, including the Juniper variant. No warning lights or system messages appeared. The only symptom was a persistent rattle/noise originating from the vehicle chassis The vehicle was inspected and modified by Tesla Service, acting on behalf of the manufacturer.
