... o get to the side safely thinking any minute vehicle could catch fire. Assuming it was a oil leak, had it towed to the mechanic shop where I was informed it was transmission fluid everywhere. After looking up systems I came across a forum where owners of this vehicle are calling for recall for the exact same problem. Mechanic stated take to Ford dealership bc these edges have had transmission issues and he didn't want to replace converter or any other part then months later need new transmission which he quoted at 9600. Not only my safety was put at risk but every person on the road that day, it was smoking so bad couldn't see clearly out winshield, people honking telling me vehicle was on fire underneath. After all the chaos, fluid everywhere, smoke, burnt smell, jerking, ect.. still NO alert on dash of any kind!Read more

While driving under normal conditions, the air conditioning system in my Toyota Grand Highlander suddenly stopped working. The vehicle was inspected by a Toyota dealership, which determined that the AC condenser had a hole/puncture that caused a refrigerant leak and complete AC failure. Toyota denied warranty coverage, classifying the damage as “road hazard,” despite acknowledging that this i
... s a common issue with the Grand Highlander. There was no collision, no unusual road event, and no warning prior to failure. The condenser appears to be positioned in a highly exposed and vulnerable location, allowing small road debris to puncture it easily. Based on discussions with the dealership and reports from other owners, this issue is occurring repeatedly and results in expensive repairs (approximately $1,500–$2,000). This appears to be a design-related vulnerability, not an isolated incident or abnormal use. A failure of the air conditioning system significantly impacts vehicle safety and operability, particularly in extreme heat conditions. I am reporting this issue to document a pattern of premature AC condenser failure in Toyota Grand Highlander vehicles and to request further investigation.Read more

My 2021 Genesis G90 has experienced repeated failures of the main infotainment / viewing display system, which has been replaced multiple times by authorized Genesis dealers. Despite these repairs, the defect continues to recur. On December 19, 2025, while the main display was nonfunctional, I was involved in a rear-end collision while proceeding from a stop at a traffic light. During this event,
... the vehicle did not provide any forward collision warning and did not engage autonomous emergency braking, despite the presence of a vehicle directly in front of me. I am concerned that failure of the main display system or its associated control modules disables, suppresses, or prevents proper operation or driver notification of Advanced Driver Assistance Systems (ADAS), including Forward Collision Warning, Forward Collision-Avoidance Assist, and Autonomous Emergency Braking. Due to the display failure, no visual warnings or system status messages could be seen, and there was no indication that collision-avoidance systems were disabled or degraded. The absence of both warnings and braking intervention raises serious concerns regarding system integration, fault handling, and fail-safe design. This vehicle has undergone multiple unsuccessful repair attempts for the same defect and is currently subject to a formal warranty demand under the Magnuson-Moss Warranty Act. This complaint is submitted to request investigation into whether recurring display failures in the 2021 Genesis G90 can impair ADAS functionality or driver awareness, creating an unreasonable safety risk.Read more

The contact owns a 2016 Honda Pilot. The contact stated that when he had parked the vehicle at work, he had heard an abnormal clicking sound from the engine compartment, and the vehicle was idling roughly. The contact stated that as he drove the vehicle back to his residence at approximately 60 MPH, the check engine warning started flashing on the instrument cluster. The contact drove the vehicle
... to an auto parts store, and the store employee used a handheld scanner and drew voltage and engine communication codes. The contact then had the vehicle towed to a local dealer. The dealer did not perform a detailed diagnosis, but when the contact picked the vehicle up, he was still hearing the abnormal clicking sound. The dealer had a mechanic try to evaluate the abnormal noise by listening and thought it sounded like a failing timing belt tensioner. The contact then drove the vehicle to an independent mechanic who diagnosed the vehicle and determined that the rod bearings had failed and the crankshaft was scored. The mechanic recommended replacing the engine. The vehicle was not repaired. Both the front AND REAR exhaust pipes are cracked and broken with holes in them where the SINGLE bracket that secures both pipes mounts. There was special coverage issued to repair, but the special coverage only identifies the front pipe. The rear pipe is essentially dangling from the upstream catalytic converter, which also appears to be cracked due to the same underlying issue. If the vehicle do
... ors are open, exhaust fumes will enter, rather than be expelled to the rear of vehicle. This pipe can essentially finish cracking and fall off into the roadway. The check engine light did not come on until these components broke. The problem is vast across many of these vehicles as reported to me by an owner of an independent service center as well as technicians who work at certified GM dealerships. The vehicle is available for inspection upon request. I currently still drive it, but its just a matter of time before a heaping hunk of metal pipe falls onto the roadway and causes an accident. The rear pipe is almost completely separated from the remaining exhaust pipe between the upstream and downstream catalytic converters at the bracket location before the flex pipe.Read more

My 2018 Tesla Model X experienced repeated over-the-air software update failures starting in late December 2025. Each update attempt would run for hours and then fail with a timeout error. This pattern continued for several weeks with the same error every time. After these repeated failed update attempts, the Autopilot ECU (computer) completely failed. Tesla service diagnosed the issue on January
... 6, 2026 as a failed Autopilot ECU 3.0 requiring full replacement at a cost of $1,808. The failure has disabled Full Self-Driving, adaptive cruise control, lane keeping assistance, automatic emergency braking, and all driver-assistance safety features. I am concerned this represents a systemic defect where Tesla's software updates are damaging or exposing failures in Autopilot hardware. Multiple other Tesla owners have reported this same pattern in 2024-2025: update timeout errors followed by complete Autopilot computer failure requiring expensive replacement. Tesla is charging customers to fix problems created by their own update process. The loss of critical safety features occurred solely due to Tesla's software update attempts.Read more

I am submitting this letter to formally notify the NHTSA of a safety critical defect in my 2025 Chevrolet Equinox Activ starting on 12/9/25 involving the Telematics Control Module (TCM), GM part number 85669062. This defect results in intermittent or complete failure of essential OnStar safety functions, including Automatic Crash Response, emergency communication, and accurate GPS location reporti
... ng. During normal driving, the vehicle experiences the following failures: •Loss of telematics connectivity •Inability of OnStar services to initialize or maintain a connection •GPS location drift or inaccurate positioning •Failure to reliably transmit vehicle location or contact emergency services These failures occur without user action and under normal operating conditions. The defect directly impairs systems intended to provide crash detection and emergency communication. If a collision occurs, the vehicle may not contact emergency services or may transmit an incorrect location. This represents a loss of a safety system that General Motors advertises as part of the vehicle’s crash response capability. On December 9, 2025, my Chevrolet dealer confirmed that the Telematics Control Module had failed and requires replacement. General Motors has placed this part on national backorder with no estimated availability, leaving the vehicle in an unsafe condition with no available remedy. General Motors has not issued a recall, service bulletin, or customer satisfaction program addressing this defect. The absence of available replacement parts suggests a potential supplier level issue affecting multiple vehicles. Given the safety implications, part unavailability, and potential for a broader pattern of failures, I respectfully request that NHTSA: •Review this defect as a potential systemic issue •Evaluate whether similar failures are occurring in other GM vehicles •Consider opening a Preliminary Evaluation (PE) if warranted. Thank you.Read more

I am submitting this as a formal and urgent complaint regarding serious safety issues with a vehicle I recently purchased. The trunk opens on its own while I am driving, which creates an immediate safety hazard. In addition, the backup camera is malfunctioning, further increasing the risk when reversing or parking. Shortly after reporting the trunk issue, the radio also stopped working. I have c
... ontacted the dealership regarding these concerns, but no corrective action has been taken. Given that this vehicle was recently purchased and involves active safety risks, this lack of response is unacceptable. I am requesting immediate inspection and repair of these issues, as well as written confirmation of how the dealership plans to resolve this matter promptly. Please consider this issue urgent due to the safety risks involved. I look forward to your immediate response.Read more
