On July 25, 2026, at approximately 7:10 PM, I was a passenger in a fully autonomous Waymo vehicle traveling southbound near 24th Street and Southern Avenue in Phoenix, Arizona. The vehicle entered the intersection to make a left turn and, while still in the active intersection, unexpectedly reversed. I considered this a serious safety concern because of the potential risk to surrounding traffic.

Approximately six weeks have passed, and Waymo has not responded to me regarding the incident. I am concerned that a potentially serious engine/timing-system defect may exist in my 2019 Jaguar XE R-Sport 2.0L Ingenium gasoline engine (64,820 miles). The vehicle then began running noticeably rougher and developed a rattling noise every time the engine is started. The repair facility recommended approximately upper timing-chain-related repairs, which I declined. Jaguar North America referred me to an authorized Jaguar retailer, which has advised me not to drive the vehicle because of the rough running and rattling. The earliest diagnostic appointment is September 21, 2026, and I have been advised repairs could potentially cost approximately $10,000 or more. I request that NHTSA document and investigate whether these VCT/timing-system problems represent a broader safety-related defect in affected Jaguar XE vehicles.Read more

... rapid air loss/blowouts and apparent tread or belt separation. On 8/24/26 this happened spontanously on I-35 going 70 mph causing an almost deadly crash. These have not been ordinary slow leaks. The failures have occurred suddenly and have created a serious safety concern. I am now extremely concerned about operating the vehicle on the fourth remaining matching tire. I have photographs of the remaining tire and can provide photographs via the service rep Joe@Jaguar and documentation regarding the previous failures, dealer repair/replacement records, mileage, tire pressures, and DOT/TIN information. Because three out of four tires have experienced serious failures, I am requesting that this matter be treated as an urgent potential safety-defect issue rather than simply as an individual road-hazard or routine warranty claim. The second failure was treated by Jaguar Dallas Darrick my service rep on July 14th , 2026(approx. 5 weeks earlier) with the first tire instance June 1, 2026. My concern is that this may be related to the H570/H572 high-voltage battery recalls. My vehicle had not yet received the recall-related charging limitation software, and it had previously experienced a significant battery/electrical issue in June 2025 involving Restricted Performance, an isolation fault, and BECM reconfiguration/software that apparently resolved the issue at that time. The dealership is now attributing the 2026 failure to normal age-related battery degradation, but I find it difficult to reconcile that explanation with 31 of 36 modules reportedly failing simultaneously, particularly given the vehicle's prior isolation fault and the current recall's stated concerns involving internal battery faults, short circuits, and thermal overload. The dealership has told me they did not perform a thorough investigation specifically to determine whether the current battery failure is related to H570/H572, yet they have concluded that it is unrelated. I am requesting that NHTSA review this situation because I am concerned that a potentially serious high-voltage battery defect covered by the recall may have caused or contributed to this failure. The transmission, on XXX, we were going to a family dinner and the vehicle started shifting hard and making a clanking noise. We turned around and went straight home. The next morning we took the vehicle to our mechanic who said the motor mounts were broken and a tune-up was needed. We got the vehicle back on Monday, XXX. Tuesday the XXX, I drove about 100 miles for work and noticed the vehicle was making a whining sound from the transmission. Took it to mechanic, by the time we drove about 2 miles to the mechanic, the entire transmission was completely broken. We now have to pay $5402.19 to rebuild the transmission at only 101,703 miles. I know GM is under a lot of scrutiny with the transmission issues right now and I believe my transmission is another one of those affected.Read more

The driver's power seat drive cable on the left-hand track rail snapped internally. The motor only advances the right side, causing the seat to twist violently to the left and jam. This mechanical failure occurred prematurely at 9,000 miles. Component/system: The front passenger Occupant Classification System (OCS) in my 2018 Tesla Model 3. The vehicle is available for inspection on request. Description of the failure: The vehicle intermittently displays "Front passenger safety restraint system fault — service required]" on the center touchscreen. The alert first appeared on or about 4 years ago and has recurred approximately ~50
... times since. The failure is strongly correlated with cabin temperature. It appears after the vehicle has been parked in direct sun in warm weather, and clears on its own once the cabin cools. It has not occurred in cooler ambient conditions. Safety risk: On this vehicle the front passenger seat uses a capacitive Occupant Classification System rather than a simple occupant-detection pad. The restraint control module uses OCS output to determine both whether the passenger airbag deploys and at what force. When the system faults, I have no way to know whether the passenger airbag is suppressed, will deploy at an inappropriate force for the occupant, or will deploy at all. This directly compromises frontal crash protection for anyone in that seat. I have been avoiding seating passengers there as a result. Inspection and confirmation: The vehicle is outside the basic warranty. Per Tesla's own service documentation, the passenger-side OCS sensor is not individually serviceable; the remedy requires replacing the seat cushion assembly and running a proprietary calibration routine that requires a cabin temperature near 68°F. The vehicle has not been inspected by police, insurance, or any independent shop. Prior warning symptoms: The touchscreen alert described above is the only symptom. There was no prior indication. There is no open recall for this condition on my VIN. Given the number of similar owner reports and the cost of the remedy, I believe many affected vehicles remain unrepaired and in service.Read more

A federal safety recall was performed on the vehicle to install a rear subframe reinforcement kit. Before the recall repair, the vehicle operated normally and had no vibration at highway speeds. Immediately after the recall repair, a new vibration developed above approximately 60 mph. The vibration is felt primarily from the rear of the vehicle, occurs only while accelerating, becomes most notice
... able between 60 and 80 mph, and disappears immediately when the accelerator is released while maintaining the same speed. The condition is repeatable and remains unresolved. The vibration creates concern for loss of vehicle stability and control at highway speeds because it occurs while the drivetrain is under load. No warning lights or diagnostic messages appeared before or after the repair. The vehicle has been inspected by both the dealership that performed the recall repair and an independent repair facility. To eliminate other possible causes, the front hub and bearing assembly were replaced and all four tires were remounted and dynamically balanced. Those repairs did not eliminate the rear vibration. The dealership attributed the condition to unrelated maintenance items, including tires, wheels, and brakes, but did not identify a defect with the recall repair or determine whether the rear subframe installation, rear differential mounting, driveshaft, or other components disturbed during the recall procedure contributed to the new vibration. The timing of the failure is significant because the vibration began immediately after completion of the federal recall repair and was not present beforehand. The component is available for inspection upon request. I am requesting that NHTSA investigate whether the recall remedy or repair procedure may have introduced a drivetrain or rear suspension condition affecting vehicle safety.Read more

After approximately 18,000 miles of operation, this restriction completely choked the transmission fluid pressure loop. The extreme thermal stress and back-pressure caused the internal walls of the transmission cooler to suffer a catastrophic structural rupture. This caused the transmission to suddenly overheat and fail while driving, creating an immediate highway safety risk and a total loss of vehicle control. Furthermore, the internal rupture of the cooler walls has caused direct fluid cross-contamination ('SMOD'), forcing transmission fluid and metallic shrapnel directly into the engine's main radiator and cooling passages. This metal shrapnel is currently circulating through the engine's cooling network, creating a severe secondary hazard of catastrophic engine seizure and thermal runaway while traveling at highway speeds. Hyundai Motor America (case #42847594) has refused repair, citing a "branded title"; the only brand on the title is a "Not Actual Mileage" odometer disclosure, not a salvage or rebuilt brand. NHTSA guidance is that title branding is not a valid basis for denying a recall remedy. The defect poses a risk of sudden engine failure and stalling while driving. No crash, fire, or injury. ... o work was done to the seat belt. The seat belt is harder to pull than it was before it went in. Multiple people in the service department agreed it’s much harder to pull. And literally said there’s nothing we can do cause the data did not show a srs notification. It’s my only car, I’m pretty sure the seat belt shot off or partially did and because their computer data says it’s ok they did nothing to actually investigate it. Allowing me their customer to drive around with a broken seat belt. the belt has a permanent, severe mechanical resistance to being pulled out that occurred simultaneously with the unprovoked tightening event. the vehicle manufacturer is refusing to repair a structurally compromised, physically stiffened safety restraint due to a lack of digital software codes. Glitches in software data on cars can easily happen and Volvo is entirely relying on computer data rather then physical investigation. The airbag light on the dash was illuminated when I received the car. I notified the service manager who stated that the light was not on when he initially got in the car after my arrival. The car was returned to the garage to determine the issue. The car was returned 30 – 45 minutes later and the airbag light continued to illuminate. The service manage told me that the driver’s side airbag which was replaced at a different dealership was causing the issue. I got in the car and notice that an oil pressure and voltage gauge that I installed after rebuilding the engine were not working. Therefore, I notified the service manager of the issue. He asked another manager to talk to me. I asked how they replaced the airbag so I would know where to start troubleshooting the cause. He told me that they were NOT in the fuse box because they removed the airbag from the top of the dash. The airbag light has been constantly illuminated since the airbag was replaced at BMW of Fort Lauderdale. It really scared me; I was with my wife and two children. The rear windshield was completely shattered—it had exploded out of nowhere. When I checked online, I realized that many vehicles of this brand—Pathfinders and Rogues—had reported this same incident: exploding spontaneously. Fortunately, my two children were in the second row, but a shard still fell onto my younger son's hand and cut him; he was terrified and crying. I got out of the car, and there was absolutely nothing there—the area was clear. I have photos of the scene and of my son's small cut. Today, June 20, 2026, I took my Pathfinder back to Nissan Downey, but got nowhere; they refuse to take responsibility and don't believe it's a factory defect.
