Incident date: Apr 24, 2026
I am submitting a safety complaint regarding the driver assistance system (ProPILOT Assist) in my 2024 Infiniti QX80. The behavior of this system creates a potentially dangerous driving condition. When ProPILOT Assist is engaged, the vehicle consistently delays braking until the last possible moment, then applies sudden and aggressive braking rather than smooth deceleration. This creates a high r
... isk of rear-end collisions, especially in moderate to heavy traffic where gradual speed adjustments are expected. In addition, the lane-keeping function appears unstable. The vehicle tends to drift or “hug” one side of the lane instead of maintaining a centered position. At highway speeds, the system makes abrupt and overly aggressive steering corrections, even when the vehicle is already within lane boundaries and at a safe following distance. These sudden inputs feel unnatural and reduce driver confidence, requiring constant manual correction. Overall, the system behavior is unpredictable and does not align with reasonable expectations for a driver assistance feature. Instead of enhancing safety, it introduces unnecessary risk by combining delayed braking with aggressive steering inputs. This issue occurs under normal driving conditions on clearly marked roads and does not appear to be an isolated event. Given the size and weight of the QX80, these system responses could lead to serious accidents if not addressed. I request that this issue be investigated, as it presents a significant safety concern for drivers and others on the road.Read more

My 2013 Hyundai Sonata Limited equipped with the Theta II 2.4L GDI engine experienced a sudden connecting rod bearing failure while driving on the expressway in Florida. At the time of failure, the vehicle had approximately 153,000 miles. The Check Engine Light illuminated and the vehicle entered limp mode, causing significant loss of power while in motion at highway speeds. The engine and vehicle
... remain available for inspection upon request. Diagnostic scan confirmed P1326 — the exact code the Knock Sensor Detection System (KSDS) is designed to trigger upon detecting connecting rod bearing wear or failure. Prior to this incident, no warning lamps or symptoms were present; the failure occurred suddenly while the vehicle was being driven normally. The KSDS software update was previously completed at an authorized Hyundai dealership. The vehicle has been maintained with regular oil changes and complete maintenance records are available. No engine sludge was found upon inspection at the dealership. The failure was confirmed by an authorized Hyundai dealership, which diagnosed connecting rod bearing failure consistent with the known Theta II GDI engine defect covered under the class action settlement lifetime warranty. Despite meeting all eligibility requirements — KSDS update completed, P1326 confirmed, full maintenance records, no evidence of neglect — Hyundai Motor America denied the warranty repair citing vehicle age and mileage exceeding 150,000 miles and 8 years. This mileage and age limitation does not apply to Theta II GDI engines under the lifetime warranty class action settlement. This failure represents a serious safety risk as the engine lost power while the vehicle was in motion on the expressway, creating a dangerous situation for the driver and other road users. The vehicle has not been inspected by police or insurance, but is available for manufacturer or NHTSA inspection.Read more

I own a 2021 Kia Seltos with a 2.0L 4-cylinder engine (146 hp) and approximately 38,967 miles at the time of failure. On April 21, 2026, the vehicle began rattling during acceleration and stalling at stops with no warning lights of any kind. On April 22, 2026, my partner and I were driving on the highway when the rattling intensified and the vehicle lost the ability to accelerate properly, requir
... ing my partner to press hard on the gas just to keep up with highway traffic. He safely exited the highway and shortly after turning off the main road the vehicle stalled completely on a residential street and could not be restarted. My partner opened the hood, at which point white smoke was visible and there was a strong burning smell. The vehicle was towed from that location to an authorized Kia dealership. Upon arrival, the service manager noted a burning smell from the starter motor. The engine produced no cranking sound when the key was turned. On April 24, 2026, the dealership confirmed the engine is seized and submitted the case to Kia corporate. The vehicle was properly maintained with oil changes every 3,500–4,500 miles. A full inspection just 3,587 miles prior identified no engine issues. No warning lights appeared at any point before or during the failure. This failure matches the defect described in NHTSA Campaigns 21V259 and 25V099, both citing improperly heat-treated piston oil rings in 2021 Kia Seltos vehicles equipped with the 2.0L engine causing cylinder wall damage, seizure, and risk of fire. My VIN returned 0 unrepaired recalls on April 22, 2026, placing it outside both recall populations. However, my vehicle has the identical engine, model year, and failure mode. I request NHTSA investigate whether both recall populations adequately captured all affected vehicles, including those manufactured outside the stated production date ranges.Read more

Good morning, I am writing to formally escalate a serious and unresolved safety concern involving my vehicle and to follow up on my existing case (Case #XXX). On April 22, 2026, while driving on the highway at approximately 55 mph, my vehicle suddenly performed a hard stop as if it detected an imminent collision. At the time, there were no visible obstructions or vehicles directly in front of me
... that would have warranted automatic emergency braking. This is a repeat issue. A nearly identical incident occurred on August 8, 2025, which was previously reported. Despite that report, the problem remains unresolved and continues to present a significant safety risk. These unexpected braking events create a hazardous driving condition with a clear risk of rear-end collision, particularly at highway speeds. Based on the pattern and circumstances, this strongly suggests a false activation of the automatic emergency braking system or a related safety system malfunction. Given the repeated nature of this issue and the potential safety implications, I am formally requesting: * Immediate escalation of this case to the appropriate engineering or safety review team * A full investigation as a potential safety defect * Identification of any related technical service bulletins, recalls, or known system issues * A clear plan and timeline for diagnosis and resolution at an authorized INFINITI dealership * Confirmation of whether reimbursement or other corrective action will be considered given the ongoing safety risk and repeated service attempts Please confirm receipt of this escalation in writing, along with next steps and assigned case handling details. I expect timely and serious attention to this matter given the safety risk involved. Sincerely, XXX XXXRead more

I am reporting a serious safety concern with my 2016 Infiniti QX60 involving a defective cooling fan system. The vehicle currently experiences a ticking/tapping noise from the cooling fan, and the fan does not operate properly, which has caused the engine to overheat. This condition creates a significant safety hazard, as engine overheating can lead to sudden engine failure while driving, increasi
... ng the risk of an accident. The issue also makes it unsafe to use essential features such as the air conditioner in hot weather and the defroster in cold conditions, which impacts visibility and driver safety. This defect appears to be part of a known issue affecting this vehicle platform, where the cooling fan system fails prematurely. I understand there have been allegations, including in Anderson v. Nissan North America, that the manufacturer may have been aware of this defect but did not adequately disclose it. In addition, I am aware that this vehicle model has been associated with other safety-related recalls, including issues involving the ABS actuator (which may pose a fire risk) and the hybrid powertrain control system (if applicable). These existing concerns further highlight the urgency of addressing this defect. This is not normal wear and tear, but a potential safety-related defect that places drivers, passengers, and others on the road at risk. I am concerned about continuing to operate this vehicle and potentially causing an accident or injury due to overheating or loss of vehicle function. I am requesting that this issue be formally investigated to determine whether a recall or corrective action is necessary to prevent harm.Read more

The vehicle experienced a catastrophic engine failure at approximately 50,230 miles, after which both the engine and turbochargers were replaced under warranty with the cause listed as undetermined. Both the original and replacement engines have now failed in the same manner, with turbochargers leaking oil and producing visible smoke, indicating a likely recurring defect or unresolved underlying issue. The replacement engine failed after only 40,000 miles, further raising reliability concerns. Infiniti quality actions (P4A05 and P4A06) identify turbocharger oil leakage as a known issue, aligning with the current failure. The dealership has confirmed the turbochargers are leaking oil but will only replace them if a full engine replacement is authorized by me at a cost of $15,556.49.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? Two components have failed: the infotainment/Apple CarPlay system, including the USB-C connectivity port, and the front collision detection/crash sensor system. The entire A/V unit was replaced on August 1, 2025, yet the infotainment defect persisted. Both systems are currently installed in my vehicl
... e and are available for inspection upon request. How was your safety or the safety of others put at risk? The front collision detection system malfunction directly compromises my ability to rely on the vehicle's built-in safety features to prevent or mitigate crashes. Additionally, the infotainment screen has frozen and reset while the vehicle was in motion. I have also been locked out of my vehicle twice while the engine was running, which created a dangerous situation. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. On July 25, 2025, a technician at Georgetown Infiniti attempted and failed to connect their own phone to the CarPlay system, confirming the defect was in the vehicle and not my personal device. The collision system malfunction has been documented across multiple service visits at both Georgetown Infiniti and Grubbs Infiniti. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes. The vehicle has been inspected and serviced by authorized Infiniti dealerships, specifically Grubbs Infiniti and Georgetown Infiniti, both of which are manufacturer-authorized service centers. Most recently addressed 2/4/20226. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The infotainment screen displayed warning messages, froze, and reset while driving. The front collision system displayed malfunction warnings on the dashboard.Read more
