Engine failure due to coolant intrusion. Ford has issued a technical bulletin to its technicians regarding this issue. While my wife was driving the vehicle, a warning light first appeared indicating that the engine temperature was high. Shortly afterward, the warning escalated to a red alert stating that the transmission was blocked. My wife immediately turned off the engine to prevent further d
... amage. The vehicle was then towed to a Ford dealership for inspection. After evaluating the vehicle, the dealership informed us that the engine had suffered severe damage and would need to be replaced due to coolant intrusion into the engine. We have maintained the vehicle responsibly and have documentation of regular oil changes and other maintenance and repairs. When we contacted Ford Motor Company, we were informed that the vehicle was outside the warranty period. We opened a case with Ford’s customer relationship department requesting assistance; however, the case was denied. Although the vehicle is outside the warranty period, Ford has acknowledged this issue in a technical service bulletin, indicating that coolant intrusion is a known problem affecting this engine design in the Ford Escape 2017.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? GM and dealerships have report extensive failures of the Telematics Control Module (TCM) that connects onstar and provides GPS navigation ability. There is currently no issued recall. Yes, this can be provided for inspection. How was your safety or the safety of others put at risk? Unlike 2024 ve
... hicles, where Android Auto is supported, providing a safe, hands free workaround for the failure of the TCM, on 2025 models, GM deliberately removed support for Android Auto to force OnStar subscription services, even for basic navigation. This greed-based decision, along with undisclosed failures of the TCM and months of back order waiting list to fix, puts drivers at significant safety risk when forced to revert to other navigation solutions. It is guaranteed this decision (putting safe navigation behind a paywall, and the mass failure of this hardware) will result in loss of life due to forcing drivers to revert to legacy and unsafe practices, including being forced to use their phone in a non-handsfree method. They also removed basic lane keeping warnings and steering, traditionally free with the vehicle and now only available with a full onstar subscription. This liability is exclusively due to the push towards subscription models at the impact of public safety. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes, dealership confirmed this failure and the extent of the failures across 2025 vehicles. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? All indications were normal except no data connectivity and GPS signal was also lost. As an officer in the military, I can't standby and let this deliberate impact to public safety continuRead more

On the evening of October 29, 2025, my 2025 Lexus RX 450h+ suffered two complete electrical failures within a few hours. The vehicle lost all power and became totally unresponsive — no lights, sounds, or electrical function. During the first failure, both of my young children (ages [XXX] and [XXX] were trapped inside because the doors would not unlock or open manually from the inside or outsid
... e. The failure occurred at night, and I was stranded for nearly five hours. I repeatedly tried to reach Lexus Roadside Assistance, but the calls disconnected multiple times before I reached an agent. A tow truck was dispatched with a 90-minute ETA, but before it arrived, the vehicle suddenly restarted on its own. We finally got home close to 1:00 a.m. I am a single parent and this was a terrifying, unsafe situation that could have resulted in serious harm if my phone had died or if outside help had not stopped to assist us. The vehicle was brand new (leased April 2025) and has approximately 5,000 miles. It was taken to Serramonte Lexus for inspection, and Lexus Corporate is reviewing my request for replacement or buyback. This is an extremely dangerous defect affecting the 12-volt electrical system and electronic locking mechanisms. The vehicle doors should never be inoperable during power loss, especially when children are inside. I am filing this report so NHTSA is aware of this incident and can investigate whether similar failures are occurring in other 2025 Lexus RX 450h+ vehicles. Description of Problem: While driving under normal highway conditions on July 2, 2025, my Tesla suddenly and violently phantom braked — meaning it rapidly and without cause applied the brakes forcefully. There were no obstacles, vehicles, or hazards present. The sudden deceleration caused a severe flare-up of a recent cervical disc replacement surgery I had undergone. I experienced extreme nec

I contacted Tesla requesting the logs from the incident and have received no response. I am in the process of retaining legal counsel and intend to pursue this further as a product liability and injury case.Read more
