Vehicle: 2023 Toyota RAV4 Prime Plug-In Hybrid Component/System: Digital instrument cluster / dashboard display Description of the problem: The digital instrument cluster intermittently and unpredictably goes completely blank while the vehicle is being driven. When this occurs, all dashboard information is lost, including vehicle speed, fuel and battery level, warning indicators, cruise control s
... tatus, and tire pressure information. The screen remains entirely black and provides no information to the driver. Safety risk: This failure makes the vehicle unsafe to operate. The driver cannot see speed or critical warning information, and multiple safety features become functionally disabled due to the lack of display feedback. The screen has gone black while driving, which significantly increases the risk of an accident and poses a serious safety hazard to the driver, passengers, and others on the road. Timeline and progression: The issue first occurred in October 2024. Initially, it happened infrequently (about once per month) but has progressively worsened and now occurs nearly every other drive. Restarting the vehicle does not reliably restore the display. Cold weather may be a contributing factor, but the failure also occurs under normal conditions. Reproduction and inspection: The problem has occurred repeatedly and with multiple drivers. The vehicle has been taken to a Toyota dealership twice for this issue. No repair was performed, and the concern was dismissed. The vehicle is available for inspection upon request. Warnings or alerts: There are no warning lights, messages, or alerts prior to the failure. The display turns completely black without warning.Read more

Vehicle: 2015 Hyundai Genesis sedan (3.8L) Issue Type: Exterior Lighting / Electrical System Description of the Problem: I am reporting an intermittent electrical failure affecting the rear lighting and backup safety systems on my 2015 Hyundai Genesis. The vehicle’s reverse lights, tail lights, and backup camera intermittently fail and flicker, especially when the trunk is opened or moved. T
... his behavior has worsened over time. The failure appears to be related to wiring in the trunk area, as opening or closing the trunk causes the lights and camera to flicker or stop functioning. This issue affects multiple systems simultaneously, including: •Reverse (backup) lights •Tail lights •Backup camera License plate lighting The reverse lights and backup camera sometimes do not function at night, and the tail lights may flicker or go out. Safety Incident / Near Miss: At night, while backing up, I nearly struck a pedestrian because: •I could not clearly see behind the vehicle due to the backup camera and reverse lights not functioning, and •The pedestrian did not realize the vehicle was backing up because the reverse lights were not illuminated. No injury occurred, but this was a serious near-miss involving a pedestrian at night. Additional Information: I have since learned that Hyundai has issued a Technical Service Bulletin (TSB) for 2015–2016 Genesis vehicles related to damage or failure of the trunk wiring harness, which can cause loss of rear lighting and backup camera function. My vehicle’s symptoms match the conditions described in the bulletin. This issue appears to be a design-related wiring failure caused by normal trunk operation, not misuse or modification. Current Status: The problem is ongoing and worsening. Rear lighting and backup safety systems are unreliable and fail intermittently without warning.Read more

When driving with the adaptive cruise control on, the Prologue throws out an alert after it suddenly disengages the cruise control that states that the forward camera is blocked and the cruise control will disengage. The cruise control also fails to notice cars in front of the Prologue and will speed up, then will suddenly "see" the car, jam on the breaks, then the accelerator again. Also, the ada
... ptive cruise control cannot gauge distance between cars and all following distances set for the car, whether 1, 2, or 3, are exactly the same. This has been reproduced for the dealer and confirmed as an issue with this Prologue. The dealer then shrugged and said there were no fixes from Honda yet, so there was nothing they could do. This is a huge safety issue because a sudden deceleration at highway speeds has nearly caused a half dozen accidents in the past 2 weeks alone. This issue has been reoccurring for over 1 year with multiple congerences with the dealership, all ending with the statement that Honda has not issued a fix yet.Read more

Ford is attempting to block warranty claims related to this issue. I believe they are doing this so they will not have to issue yet another recall! While driving my 2021 GMC Yukon XL, the vehicle unexpectedly lost acceleration. After lifting off the gas pedal, it jolted forward, triggered a “Reduced Performance” dashboard warning, and remained in what felt like second gear, refusing to shift. I immediately pulled into a parking spot; when I attempted to reverse, the vehicle would not move backward. As you may imagine, this was a major s
... afety concern, as I had my [XXX] and [XXX] children in the vehicle with me. I had the Yukon inspected at a GMC dealership, where a technician confirmed the issue originated from a faulty transmission valve body. My independent research has revealed this to be a widespread defect in the 2021 Yukon XL, prompting GM to issue Special CoverageXXX, which explicitly covers valve-body replacements for 2021 Yukon XLs experiencing reduced performance. Despite meeting these criteria and receiving identical diagnostic alerts, I was informed that my VIN is not covered. I am formally requesting that this safety-related defect—widespread among this model year—be recognized and my VIN included under Special Coverage XXX. The valve body causing the loss of propulsion in my Yukon XL is exactly the same defective component GM has acknowledged and is covering under this program. The vehicle remained at the dealer for 7 months. The dealer had replaced the two side cameras, the control module and had performed the software update with the assistance of a Jeep factory engineer; however, the failure persisted. The engineer determined that the camera wiring harness needed to be replaced; however, the part had been discontinued. The engineer determined that the wiring harness needed to be bypassed, and a patch cord needed to be installed. The dealer called the contact and informed the contact that the rearview camera was operating as intended after the installation of the cord patch. The vehicle is affected by a manufacturer safety recall related to the engine. According to the recall, machining debris may not have been properly removed during engine production. This can lead to engine knocking, rough running, failure to start, loss of power, or the engine stalling while driving. The manufacturer has stated that the fix will require replacement of the engine assembly, but the
... remedy is not yet available. Because of this, the vehicle continues to be driven with a known engine defect and no repair timeline. The main safety concern is the risk of sudden loss of power or engine stall while driving, which could be dangerous, especially at highway speeds or in traffic. The vehicle has been inspected by a dealer, who confirmed that the recall applies but could not perform a repair, provide a timeline, or confirm that the vehicle is safe to drive while waiting for the remedy. No crash or injury has occurred so far. There have been no warning lights or specific failure events yet, but the concern is based on the acknowledged manufacturing defect and the possibility that the issue could occur without warning. The vehicle is still being driven because there is no available repair, no clear safety guidance from the manufacturer, and no alternative transportation or loaner vehicle provided. The concern remains that the vehicle could unexpectedly lose power or stall, putting the driver and others at risk.Read more

Issue was first noticed in October 2025 - there are roughly 70k miles on the vehicle. It is a 2017 vehicle, and has regular service (oil, inspection). There was a class action lawsuit for this issue (wylie v hyundai), which was settled prior to the purchase of the vehicle. Vehicle stutters when shifting into and out of first gear. A local mechanic and hyundai both confirmed that it was a transmiss
... ion defect. The Hyundai dealership initially thought it was an actuator issue, but has since said that it is a faulty gear assembly. it has a DCT (dual clutch transmission). Vehicle suddenly stops and is erratic when driving. Coming out of a stop risks the vehicle shuddering; going through an intersection or turning cross traffic is extremely dangerous as the vehicle could stop momentarily and cause a collision. Vehicle is unable to maintain flow of traffic. The problem appears to be widespread - there are various forums we found where others have this same issue. There were no warnings on the dashboard prior to the issue. When issue appeared a check engine light was displayed.Read more

On November 10, 2025, my 2016 Hyundai Sonata Hybrid (VIN [XXX] , approximately 150,000 miles) experienced a sudden loss of power while I was turning right and accelerating into traffic. The vehicle immediately entered limp mode with flashing warning lights and audible alerts. The vehicle lost normal propulsion and could not accelerate properly, creating a dangerous situation as it was blocking a
... traffic lane. I was forced to limp the vehicle into a nearby parking lot to get out of traffic. The vehicle was not safely drivable afterward and was towed home. A few days later, the vehicle was towed to Murdoch Hyundai in Lindon, Utah, for diagnosis. The vehicle was found to have diagnostic trouble code P1326, which Hyundai identifies as activation of the Knock Sensor Detection System (KSDS) related to connecting-rod bearing wear. This condition is part of Hyundai’s known engine defect and safety recall/warranty extension programs. During inspection, the dealer observed that the valvetrain did not show signs of neglect or sludge, which is inconsistent with oil starvation or severe maintenance neglect. This further supports that the failure is consistent with the known connecting-rod bearing defect addressed by Hyundai’s KSDS safety recall, rather than a maintenance-related issue. This is a safety issue because sudden loss of propulsion or reduced power while entering traffic creates a high risk of collision. Hyundai customer care has been unwilling to open a new case number or escalate review, and the dealer has indicated possible denial of engine replacement coverage based on incomplete oil-change records, despite the vehicle exhibiting the KSDS safety condition. I am requesting NHTSA review of this safety-related defect and the manufacturer’s handling of KSDS/P1326 failures.
