Vehicle: 2021 Audi Q5 I am submitting this complaint regarding a serious safety defect involving unintended emergency braking. On September 10, 2025 and again on January 8, 2026, my vehicle suddenly applied emergency braking while I was driving and there were no vehicles, pedestrians, or obstacles in front of me. The dashboard displayed “Auto Pre Sense” during both incidents. These unexpect
... ed braking events created a high risk of a rear-end collision and placed me and other drivers in immediate danger. This is not a driver error, but a malfunction of the safety system. Because this has occurred more than once, I am concerned that the Audi Pre Sense system in my vehicle may operate unpredictably and cause a serious accident. I respectfully request that this issue be investigated as a potential safety defect.Read more

I am reporting a series of safety concerns regarding my 2017 Chevy Volt, which have persisted despite multiple attempts to address them with the dealership. These issues pose significant risks to my safety and that of others on the road. On January 18th, 2024, I brought my vehicle to the dealer after experiencing reduced propulsion power while driving on the highway, coupled with the illumination
... of the check engine light. The mechanic performed a quick inspection and used a GM scan tool to diagnose the issue, revealing the code P1E00 for the Hybrid Powertrain Control Module 2, which was cleared without a thorough investigation. I was advised to return if the issue reoccurred. Despite the reset, the problem persisted, and on February 9th, 2024, I scheduled another appointment due to my Chevy Volt's High Voltage battery charging only to 65% capacity despite indicating a full charge. Subsequently, on February 11th, 2024, the check engine light illuminated again, and on February 12th, 2024, November 25th, 2025, and January 11th, 2026, while driving to work, both times I experienced a loss of propulsion power on the highway, placing my safety at risk and lost all power in a traveling lane of traffic. These ongoing issues with propulsion power and the high-voltage battery charging system are not only inconvenient but also potentially dangerous. Despite multiple visits to the dealership and attempts to diagnose and resolve the problem, it persists, indicating a systemic issue that requires immediate attention. The reduced propulsion power while driving on the highway, coupled with the illumination I urge the National Highway Traffic Safety Administration (NHTSA) to investigate this matter thoroughly to ensure the safety of Chevy Volt drivers and passengers. It is imperative that General Motors addresses these issues promptly and implements appropriate measures to prevent further incidents.Read more

I bought this truck used from a dealer in 2022. From the start the resting voltage of the car was a problem getting 12.7v and when I was driving it would often only get up to 13.4v. I took it to the dealer and they replaced it completely in 2023. It still shows low resting voltage and driving voltage. I told them and they said it was fine and not to worry. Now for the past 6 months it has been mak

It still hasn't gotten better so I took it to a different mechanic and they said its a bad alternator and that they are very expensive and on back order. I tried setting up an appointment with the dealer to look at it again and they said I am now out of warranty and will charge me a diagnostic fee and if it is the alternator it again isn't covered by the warranty. I have been searching online and this is something thousands of other customers have been experiencing and there is a class action lawsuit about this as well. I am kindly asking that you look into this and if appropriate issue a recall for this. I shouldn't have to pay thousands of dollars to fix an alternator for a truck that only has 60k miles on it. My truck is only worth $29k so having to pay for a repair that is about 7% of my vehicle value is ridiculous.Read more

To avoid going straight and hit the shops at the other end of the parking lot, she turned the wheel to one side. She hit a light post and then hit something else. The vehicle flipped over and landed on its roof. The vehicle was totaled. She was trapped inside the car unable to free herself until the ambulance, tow truck and the police arrived. By God's grace, she did not hit any other car or person and she only suffered minor injury. She was taken to the emergency room at Peace Health, examined and released after 3 hours. If you look at the RAV4, you cannot believe that she only suffered minor injury. Praise the Lord. She is still mentally shaken up by this traumatic event. Looking at the pictures of the damage to the vehicle, the car must have been traveling at a very high speed and there is no way that the driver stepped on the gas pedal intentionally. The insurance Company is Liberty Mutual Claim# [XXX] -XX The vehicle is considered total loss by the insurance company. EDR was inspected but data in the EDR was not collected. I suspect this incident was not caused by the driver, The data in the EDR will tell the true story. INCIDENT SUMMARY: On January 9, 2026, at approximately 1:30 PM, while traveling at 50 MPH in North Carolina, the vehicle experienced a sudden, catastrophic engine seizure without any prior "Limp Mode" warning or Check Engine Light. This resulted in an immediate, total loss of motive power in active traffic. I was forced to navigate a dead multi-ton vehicle across lanes without power steering or va
... cuum-assisted braking, narrowly avoiding a high-speed collision. TIMELINE & NEGLIGENCE: This vehicle was purchased from an independent retail dealer on December 20, 2025—less than 21 days prior to this life-threatening failure. The vehicle was sold with an "Open" status for Safety Recall 23V-707. The failure occurred exactly as described in the recall’s warning: a catastrophic stall at speed. TECHNICAL EVIDENCE: A non-invasive borescope inspection of the cylinders has confirmed the presence of metallic "silver chunks" (sheared bolt heads) and "half-moon" impact marks on the piston crowns, indicating a total timing failure and internal mechanical collision (piston-to-valve contact). REQUIRED REMEDY: This incident is a direct manifestation of the safety risk documented in Recall 23V-707. Per BMW Service Bulletin SIB 11 09 23, the presence of sheared bolt heads and seized internals constitutes "Catastrophic Consequential Damage." I am filing this report to ensure federal oversight of BMW North America’s obligation to provide a complete engine replacement, as the safety defect was present at the time of sale and resulted in a near-fatal incident within 300 miles of ownership.Read more

Unexpected rear wheel lock-up at 29000 miles. While engine braking from highway speed onto an off-ramp at approximately 50 mph, the rear wheels unexpectedly locked up. Due to prior rear brake actuator issues, I suspected the emergency brake had activated without my input. It was only momentary, and no fault codes or warnings were displayed. It happened again under the same circumstances two months
... later, with no fault codes. On May 9, 2025, the check engine light came on, and I scheduled service. The dealer addressed a recall regarding the transmission by installing software. In August 2025, I received a letter from GM regarding recall N242454440 for excessive wear in the transmission control valve, which was causing rear-wheel lockup. On October 31, 2025, while towing a small enclosed trailer at 70 mph on the highway with cruise control engaged, the truck suddenly decelerated, and I was nearly rear-ended by a semi-truck (no brake lights to warn them). The truck would not go over 50 mph unless I was going downhill. The transmission would not shift into higher gears. I returned it to the dealer, who checked the computer and found no record of the incident, so they said there was no problem. They informed me that the new software was designed to limit the truck's speed if a potential lockup was detected. This recall did not fix the transmission problem or the safety issue. It was simply a way to avoid costly repairs. There is a problem with the physical transmission valve that, for me, resulted in an extremely dangerous situation. The NHTSA needs to take this issue seriously and require GM to replace the prematurely worn valves in the affected vehicles.Read more

In 2018, my 2010 Toyota Corolla was taken to the dealership for the Takata Airbag Recall. After the repair the airbag warning light would intermittently display and go away only to come back later. In 2021, I took the vehicle to the dealership in which at the time the li

Currently the airbag warning light has been solid. Today 1/9/2026, the dealership has indicated they would not remediate this situation under workmanship warranty work despite previously resetting the code. The dealership insists the customer should pay for this remediation work. I have currently declined the work to seek external advise. The dealership work invoice states the code B1806 for the front passenger side squib circuit, this continued error puts passengers at risk in a car accident.
