On 14th, 2026, while traveling at approximately 60 mph on the northbound 55 Freeway near the Katella exit, my vehicle suddenly began to shake violently without any prior warning. In response, I attempted to slow down by applying the brakes; however, the speedometer rapidly increased even as I maintained firm pressure on the brake pedal. Immediately afterward, the vehicle pulled sharply to the left
... , causing a complete loss of steering control and resulting in a collision with the center median barrier. Despite the force of the impact, the airbags did not deploy, raising additional concerns about potential safety system failures. At no point before the incident were there warning lights, alerts, or indications of mechanical or electronic issues. The vehicle had also undergone its regularly scheduled maintenance at the Tustin Mazda dealership on January 2nd, 2026, just prior to the accident. Given the sudden shaking, unintended acceleration, loss of braking effectiveness, steering failure, and non‑deployment of airbags, I believe this incident may involve one or more manufacturer defects affecting critical safety systems. I respectfully request that NHTSA investigate this matter. I also believe the vehicle’s Event Data Recorder (EDR) may contain information relevant to identifying the root cause of these failures.Read more

While driving my 2017 F150 Lariat 4x4 5.0LV8 with 6speed transmission and 48,700 miles on the odometer at roughly 55mph this morning - the transmission suddenly downshifted to 2nd gear. The downshift was sudden enough to "bark" the rear tires. Thankfully, no one was near me, and I was able to maintain control of the vehicle and reach my destination safely.

On multiple occasions, after the vehicle had been parked and not driven for approximately five days or more, the automatic parking brake locked in place and would not release. I attempted standard methods to disengage the brake, including manual release procedures and lightly rocking the vehicle, without success. The only way to release the brake was to apply significant pressure to the accelerato
... r while in reverse, which caused the brake to suddenly unlock. The first incident occurred while the vehicle was parked in my home garage. At the time, my grandson was playing nearby. The sudden release of the brake after applying the accelerator created a dangerous situation and was extremely alarming. A second incident occurred several days later while I was on vacation. In this instance, I made sure no one was near the vehicle before attempting to release the brake. Anticipating the issue, I had my left foot ready to push on the breaks when I accelerated using my right foot which releases the parking. I brought the vehicle to a Mercedes-Benz dealership on January 5, 2026, where the car remained for nearly ten days. The dealership was able to replicate the issue exactly as described. The service advisor also had to apply significant accelerator pressure to release the locked parking brake. A certified Mercedes-Benz technician inspected the vehicle thoroughly, including scanning for fault codes and checking for rust or mechanical issues, but ultimately advised that this behavior was considered “normal operation.” I find this explanation concerning. Having to apply forceful acceleration to release a locked parking brake presents a clear safety risk, particularly in confined spaces such as garages or parking structures, and could easily result in property damage or personal injuryRead more

Life-Threatening Acceleration Lag: The vehicle is experiencing severe power loss during critical driving moments, for example, while entering intersections or attempting to re-accelerate into moving traffic. When I attempt to speed up, the engine fails to respond initially, leaving the vehicle stranded in the path of cross-traffic. The engine finally engages only because I am forced to floor the a
... ccelerator pedal out of necessity, which then causes the tires to squeal when power eventually returns. Intermittent Starting Issues: The vehicle frequently fails to start smoothly on the first attempt; a known symptom associated with the fuel pump impeller deformation described in Recall 23V-858. VCM Shudder and Hesitation: I am experiencing the distinct vibration and rhythmic shudder associated with the Variable Cylinder Management system. This symptom is most prominent during deceleration or coasting; specifically, when I lift my foot off the accelerator without applying the brakes, the system triggers a noticeable shudder as it incorrectly manages cylinder engagement/deactivation. Suspected Catalytic Failure (Codes P0420/P0430): Both catalytic converters are exhibiting inefficiency codes simultaneously, which are highly atypical and occur well before the expected component lifespan. This clearly points to upstream defects. Specifically, the catalytic substrate has been poisoned by carbon and phosphorus deposits from the documented oil-burning defect and has suffered thermal damage due to the improper fuel mixtures caused by the defective high-pressure fuel pump (Recall 23V-858).Read more

My Hyundai Santa Fe 2019 Limited is burning oil. It started after the Turbo broke in October 2024. The turbo began leaking oil and then died. In between oil changes I need to put oil in the engine to avoid it being drained. I am
... able to go about 3k miles on Synthetic oil and by 2.5-3k it is almost empty. It has been doing this now since 2024. I just had my 90k mile check up complete and with that I got an oil change. This was done in November 2024. Syntehtic oil was used. On January 16, as I was driving my car mileage decreased, the car began to shake and the engine light came on. I was able to safely stop and have the car towed. It was found that after driving just 2k miles the oil had completely burned off. I currently have an appointment at the dealer for them to run diagnostics on why it is burning. I saw there was a class action lawsuit against other Hyundai's doing the same thing and as a result Hyundai has to replace the engine. I also have read numerous forums where everyone is reporting the same thing with this particular model of it burning oil. I wanted to report this in case there were other owners of the same car experiencing the same thing.Read more

This complaint concerns a 2016 Nissan Rogue purchased from a used-vehicle dealership and involves safety-related defects present at or shortly after sale, raising concerns regarding inspection adequacy, disclosure accuracy, and systemic sales practices. Within days of purchase, the vehicle exhibited active water intrusion through the moonroof, interior ceiling stains, mold spotting, and moisture
... exposure to electronic components. These conditions indicate prolonged or pre-existing water intrusion rather than post-sale wear. Service personnel later advised that the vehicle showed indicators consistent with prior water or flood-related damage. The vehicle also demonstrated multiple mechanical and electrical malfunctions affecting safety and drivability. Subsequent inspection determined that handling and pulling issues were not caused by wheel alignment, but by worn sway bar frame bushings and cracked CV axle boots on both sides, indicating suspension and drivetrain deterioration. Noise initially attributed to rear brake issues was later identified as wheel bearing failure after prior evaluations misdiagnosed the source. Wheel bearing degradation is a progressive condition consistent with pre-existing mechanical wear. Additional defects included keyless entry and ignition errors, electrical shorts affecting interior systems, malfunctioning lift-gate controls, and crash-sensor irregularities occurring during rain and highway driving. These conditions created an unreasonable risk of impaired braking response, electronic failure, or loss of vehicle control. The vehicle was represented as inspected, safe, and reliable at the time of sale. However, the defects manifested immediately and required repeated repair attempts during the warranty period, resulting in significant loss of use.Read more

Complaint: Safety Defect - Engine Failure Risk (Stalling/Fire) My 2019 Hyundai Santa Fe has the defective Theta II 2.0L Turbo engine (G4KH), subject of class action In re: Hyundai and Kia Engine Litigation and multiple TSBs. The engine has a known manufacturing defect causing premature connecting rod bearing failure, leading to catastrophic engine seizure. SAFETY CONSEQUENCE: This defect present
... s a clear, unreasonable safety risk: Sudden Stalling: Engine can seize without warning while driving, causing loss of propulsion and power steering/brakes, leading to a crash. Engine Fire: Failed engines can leak oil onto hot components, causing fires. MANUFACTURER'S INADEQUATE REMEDY: Hyundai's "Knock Sensor Detection System" software update (Campaign 966) only attempts to detect failure moments before it happens. It does NOT fix the underlying mechanical defect. The engine remains a ticking time bomb. PERSONAL EXPERIENCE: My vehicle has suffered multiple related failures (turbocharger, fuel injectors) costing over $5,700, confirming the powertrain's deterioration. I fear driving it, especially with my teenage daughter. The defect is well-known, yet Hyundai's settlements fail to protect subsequent owners like myself, leaving unsafe vehicles on the road. REQUEST: I urge NHTSA to investigate the ongoing safety risk these engines pose to all owners, compel Hyundai to issue a true safety recall for a permanent repair (e.g., engine long-block replacement), and mandate clear disclosure of this defect to all used car buyers.Read more

Dear Public Safety office, I am writing to request clarification regarding the proper operation of seat belt retractors. I want to ensure that I fully understand how seat belt mechanisms are intended to function during normal adult use. The reason for my inquiry is that I am experiencing inconsistent behavior between the driver side and passenger side seat belts in my new vehicle (Hyundai Palis
... ade Calligraphy 2026). I have taken the car to the dealership and they informed me that it's working as expected by Hyundai but I am still unclear. I want to confirm whether the behavior I am seeing aligns with safety requirements. Here is what I am observing: •The driver side seat belt behaves as I would expect: if pulled fully and released quickly, it locks momentarily, but it releases normally on the next pull. •The passenger side seat belt, however, locks and remains locked, preventing the occupant from pulling the belt out again and moving even a quarter of an inch. This only unlocks when they completely unlatch and relatch it. This occurs even when the belt has not been fully extended. My questions are: 1.Should a seat belt ever remain locked to the point where an adult passenger cannot pull the belt out again without unlatching it and not even move a bit? 2.Is it expected for the driver and passenger retractors to behave differently under normal use? I want to ensure that I understand the federal requirements correctly before pursuing further inspection or repair. Any guidance or clarification you can provide would be greatly appreciated. Thank you for your time and assistance. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

On or around 12/8/2025 the 2025 Hyundai again had the front collision avoidance system randomly setting off alarms when the vehicle was driving on the freeway and other vehicles were traveling alongside the 2025 Hyundai. There was no emergency braking occurring at this time. 12/9/2025- the 2025 Hyundai returned to Jim Click Hyundai and at that time the dealer determined the front camera was showing an obstruction when none was there. The 2025 Hyundai vehicle came to a complete stop in traffic. The vehicle ahead 2+ car lengths ahead swerved from the left lane to the right lane to avoid a box in the road. Driver of the 2025 Hyundai was applying the brakes when the 2025 Hyundai emergency braking system took over and the 2025 Hyundai came to a complete stop in traffic when there was no need for this. 1/2/26- 2025 Hyundai returned to Jim Click Hyundai and remains there while the Service dep't is driving the vehicle trying to diagnose the cause of the continued emergency braking problem.Read more

The transmission of this model car is very poorly programmed a low speeds. it is constantly jerking in and out of gears making it difficult to moderate speed. There is also a substantial delay between hitting the pedal and accelerating the vehicle. The issues with the transmission paired with the very delayed accelleration leads to very ineffective control of the vehicles speed, leading to what I
... believe is dangerous driving scenarios. Turning through inersections, braking at a light, accellerating out of the way of traffic, are all made much more difficult/unpredictible due to how unpredictible and inconsitent the transmission behaves. It feels impossible to 'learn' how it behaves. Here is a forumn of other users confirming similar issues: https://www.mdxers.org/threads/jerky-transmission.181658/ I believe this should be addressed via software update or recall as it i'm sure will lead to accidents/injuries.Read more

I am filing a safety defect complaint regarding my 2017 ford explorer. The B pillar has broke in half a couple of years ago and nearly struck a vehicle behind me and if the remaining trim piece detaches, it could become a serious risk of collision, injury or property damage. it states NHTSA works to prevent unsafe traveling which is why I'm writing this. I have contacted Ford customer service and
... dealerships multiple times with no luck of anything. I have had to continuously tape the area to door which is not ordeal nor a forever fix. The b pillar is held on by wires that are slowly breaking. The wires lead to a hole that is inside my door. the opening exposes from the outside, meaning if it rains or snows, it is getting inside the hole, into my car door. If the recall piece ever came in, my door has already experienced neglect and issues. I'm concerned this will lead to corrosion, electrical complications and additional mechanical failures. Also, if it is not taped up correctly, air blows directly into the car door, into the cabin while driving onto my feet causing extreme cold exposure and physical harm and making it hard to operate the vehicle. It's very painful and also not ideal, especially when I also have a full time job and 3 kids to worry about. I currently owe 12k still on the vehicle and want to replace it with a safer vehicle. I should not have to be miserable every single day and put me and my kids in danger and also let my vehicle deteriorate while i wait for this recall piece on top of having to pay the remaining balance of this vehicle. I respectfully request that nhtsa review this complaint and investigate before anymore physical, emotional, financially harm is done.Read more

The 2024 Toyota Corolla needs to be added to the list of vehicles currently being investigated for transmission/engine issues. The vehicle in question is currently exhibiting transmission anomalies consistent with those observed in several other vehicles under review, specifically: slipping, jerking, surging forward, hesitation to accelerate from a complete stop, loss of power, droning sound at l
... ow speed, odd knocking noise/vibration during idle, unresponsive gear changes, shuddering upon vehicle shutdown, and decreased fuel efficiency. The owner has incurred expenses exceeding $2,000 for rental vehicles and diagnostic services in an effort to resolve these persistent issues. However, as of January 21, 2026, Toyota has not acknowledged the existence of a problem, asserting that these conditions are within normal operating parameters and that the vehicle is performing as intended. A local mechanic has advised the owner that Toyota will not diagnose the issue, but they are likely to offer a post-warranty adjustment should the transmission ultimately fail. It has become increasingly apparent that Toyota is not conducting a thorough investigation into these matters and expects the customer to bear the expense beyond the scope of the standard warranty. Beyond the immediate safety concerns for the owner, there is a significant risk of collision should the vehicle experience a loss of control or erratic behavior. Thank you for your time and assistance in investigating this matter.Read more

Hyundai model Ioniq 5 (all years and trim styles) have a known issue with ICCUs (Integrated Charging Control Unit) that fail. This component controls charging of the 12 volt battery, and in turn electrical systems. Hyundai replaced my ICCU, but I am concerned it may fail again, as the underlying issue does not seem to be known or addressed. When the ICCU fails, the vehicle can suddenly lose the ab
... ility to move. This can be extremely dangerous if the car is in motion and unable to safely pull over. Luckily, my ICCU failed in the middle of a parking lot. Many Ioniq 5 owners report this issue, some multiple times on multiple ICCUs.Read more

I am submitting this complaint regarding an unresolved safety recall on my leased Toyota vehicle and the manufacturer’s failure to provide a timely or reasonable remedy, resulting in ongoing child passenger safety risks. Toyota’s recall instructs that no one sit in or use a child restraint in the second-row seats while the vehicle is moving until the remedy is performed. This effectively proh
... ibits use of the safest and recommended seating position for children. The vehicle is used daily for highway driving to transport my family, including three children aged [XXX] , [XXX] , and [XXX] old. Because of Toyota’s seating restriction, my [XXX] toddler must now sit in the third row. This prevents the driver from easily monitoring or comforting the child and has caused driver distraction during highway travel when the child becomes distressed, creating a foreseeable safety risk. Toyota states it is “preparing the remedy” but has provided no repair timeline. After multiple contacts and escalation, I was told it could take up to 60 days with no interim solution. Toyota suggested contacting dealerships for a loaner but advised that an equivalent vehicle was unlikely, availability was uncertain, costs might need to be paid upfront with later reimbursement, and the duration would be open-ended. This is not a reasonable interim safety solution while continuing to pay full lease obligations. I was also informed that returning the vehicle would require paying approximately $15,000 in negative equity, which I believe is unreasonable given that the safety defect and usage restrictions are due to a manufacturer recall. Toyota has not provided a repair timeline, a reasonable interim transportation solution, or a penalty-free way to exit the lease. I request that NHTSA review whether Toyota is meeting its obligations under federal safety recall requirements. The error message “Power Tailgate Problem” appeared on the dash. Following the error message, the power tailgate stopped working. It would not open or close with the remote, the button on the dash, or the button on the tailgate itself. I discovered water had been leaking in the tailgate and the trunk area of my vehicle as well as the back seat that was folded down in storage had gotten soaked.
... I took my van to the service center and they told me it was the seal above the door and that the leak had ruined the power module controlling the door. They repaired the issues. That was in April 2025. The same thing happened last week (January 2026). After looking into the error message and the tailgate problem, I’m finding that the door may open or close at any time on its own. The water in the electrical module may also start a fire. I have two small children and this van is our primary vehicle. I’ve reached out to the dealership from which I purchased the van, I reached out to a local Honda dealer and I contacted Honda customer service and no one is willing to fix this issue. I will be required to pay out of pocket again. I’m requesting a recall on this defect in design.Read more
