I was involved in a low-speed collision at a residential intersection after stopping at a stop sign and beginning to proceed. Going under 5 mph, I braked immediately, but contact occurred. Despite relatively minor front-end damage, the driver steering wheel airbag and driver knee airbag deployed. The passenger front airbag did not deploy, and no side or curtain airbags deployed. There was no sign
... ificant intrusion into the engine compartment or passenger cabin. I am submitting this report due to concern that the driver airbags deployed in a low-speed impact that did not appear severe enough to warrant deployment. Pictures attachedRead more

I am reporting a recurring, life-threatening electronic failure in my 2023 Hyundai Palisade. During this event, multiple critical safety systems failed simultaneously, including the headlights and horn, leaving me unable to warn other drivers. A "violent" surg
... e was required to clear the intersection, risking a high-speed collision. Similar spontaneous recurrences occurred on [XXX] and [XXX], and most recently on [XXX]. These incidents are characterized by "phantom braking," total loss of propulsion, and a complete lack of interior/exterior electronic response. Technical Evidence: Diagnostic scans revealed 16 distinct fault codes (including C161608, C181287, and U012687) involving the Chassis Control Module, Electronic Control Unit (ECU), and communication buses. Despite these logged codes, Hyundai and the dealership have repeatedly returned the vehicle to me, stating they "cannot replicate" the issue, thereby dismissing a documented electronic record of failure. Arbitration Status: I entered into BBB Auto Line Arbitration (Case #[XXX]). Hyundai has contested the claim by attempting to manipulate the "start date" of the defect to February 2024 to avoid Lemon Law statutes, despite dealer records confirming the first occurrence in August 2023. As of January 13, 2026, the dealership is again attempting to force me to take delivery of the unrepaired vehicle. This vehicle is a "phantom" safety hazard. The manufacturer’s inability to replicate a logged electronic fault during a brief test drive does not mitigate the extreme risk of sudden braking and power loss at highway speeds. I am requesting a formal investigation into these specific fault codes and the electronic integrity of this model. While driving approximately 63 mph on a busy roadway, my 2023 GMC Yukon experienced a sudden failure of the brake system, including loss of normal braking capability along with failures of the Electronic Stability Control (ESC) and ABS systems. The vehicle displayed warning messages stating “Brake System Failure” and “Service ESC” and entered a reduced-speed mode. Prior to this incident, t
... here were no warning lights, messages, or symptoms indicating a problem. During the failure, the brakes became largely unresponsive, placing me, my two minor children who were passengers in the vehicle, and surrounding motorists at serious risk of a high-speed collision. After extended deceleration, a minimal amount of braking power partially returned, allowing the vehicle to eventually come to a stop. The vehicle has not yet been inspected by a dealer, manufacturer, police, or insurance representatives, and the issue has not yet been reproduced or confirmed by a service center. The vehicle remains available for inspection upon request and has not been repaired. This incident occurred without prior warning and involved a sudden and dangerous loss of braking capability at highway speed.Read more

I have had numerous issues with fuel Injectors failing. The first incident was about 30,000 miles. This was fixed by the manufacturer not even 10,000 miles later, I once again had to return the vehicle, to be inspected by the dealership. Instead of replacing all of the fuel injectors at the same time they chose to only replace one. As soon as my car went over the 80,000 mile warranty limit, my fu
... el injectors went out once more. My mechanic son, has also changed several as well. I had to tow it, 100 miles to the Hyundai dealership, as I live in a rural area. I was told by a young lady, at the dealership, that it was be covered under the engine warranty, which is 100,000 miles. They weren’t even able to look at the car for a week charged me $250 for a diagnosis… After a week 1/2 dealership finally told me it would be over $3000 to repair.. NO! I had to pay to tow it back. I’ve been without a vehicle for five months. When the fuel injectors went out, it also damaged the high-pressure fuel rail, the high pressure fuel pump, and various sensors. This is dangerous, I live in a very rural area. It stopped operating out of the blue. It chugged/bumped a few times, and it was dead. This is inconvenient, and It’s dangerous, especially during the winter time where temperatures reach -20 regularly and no cell phone signals. I believe Hyundai is well aware, of the problem. In fact the service manager had mentioned that the parts were on backorder because apparently they are in quite high demand. No fuel injector should be going out in 15,000 or 20,000 miles, causing systemic failure. Hundai has made no effort to redesign failed parts. It’s quite disheartening, to replace/repair with a new defective part. That is going to go bad, in just a few thousand miles!! Im retired, on a fixed income. I bought new, so I wouldn’t have issues. Paying new prices, to drive junk. I’d just drop it off @ dealers, and walk away.. But the loan, is so upside down that isn’t even possible.Read more

Driving on the highway, I slowed down from 65mph to 45-50mph due to standing water ahead of me, immediately after hitting water, the engine stumbled. Soon after the engine stopped running. In hybrid mode only, was not able to maintain speed. I noticed the smell of fuel in the cabin, I noticed my vehicle range dropping rapidly. The odor of unburnt gasoline was very strong and I had begun to get a h
... eadache and get dizzy, I had to open the windows while it was pouring heavy rain. Given the area had a lot of standing water, I wasn't able to stop safely. I was able to limp the vehicle in hybrid mode to a nearby uphill exit. Once stopped, I immediately left the interior of the vehicle for fresh air. If there was no shoulder and I wasn't literally next to a highway exit, I would be in deep sh**. After stopping, I checked the airbox to see if water made it into the intake. It was bone dry. Walking the right side of the vehicle I saw fuel on the ground. I called for a tow truck and they dropped me off at home. When I looked underneath the car and found the fuel line hanging free. The clip holding the tank and chassis fuel lines together was missing. I went to the local Toyota dealership and the technicians said it was not a warranty issue because it's impact damage and that I should talk to my insurance about fixing the car because it was going to be $3500 dollars to drop the prop shaft, transfer case, transmission, and other parts to replace the line because "toyota does not sell the clip alone". The line was $168 dollars. I probed further and they brought the price down to $118. Online, the fuel line is $56 dollars. I checked the Land Cruiser forums and multiple people have experienced this issue under identical conditions, others have found the clip loose during regular maintenance. This appears to be a dangerous design flaw. My 2012 Tacoma and 2016 Land Cruiser do not have these issues since they use common steel spring clips to hold fuel lines in place.Read more

Hard braking first occurred on a major highway with no prior notice that this would happen in close call accidents. The cars traffic had to veer away from me in order to avoid hitting me. This helped in giving notice for some time, however the hard braking occurred again on a regular two lane small highway as I switched lanes, saw a car stopping suddenly and the brakes locked up. (This is the latest incident as dated below). There was no "Late" warning, the brakes just locked up. Being in fear of my and my families life I did further research and found that I could disable "brake assist" and only get warnings - which is what should be the default - BUT, this must be done each time I start the car. If my wife drives and forgets to do this each time I now fear for her life!!! This is not a simple matter. It is unconscionable and horrible this type of control over basic transportation is allowed to exist. This must be addressed and corrected! The vehicle has an ongoing safety-related defect involving the forward distance sensor. The distance sensor randomly turns off while driving, which disables critical collision-avoidance systems including Automatic Emergency Braking (AEB), Forward Collision Warning, and Intelligent Cruise Control. The failure is intermittent and unpredictable. The safety systems may appear functional and then shut
... off without warning, which creates a safety hazard because the driver cannot reliably depend on the vehicle’s crash-prevention features. The vehicle has been taken in for repair multiple times. The distance sensor has already been replaced once, but the problem continues to occur. In addition to the sensor issue, the vehicle has experienced multiple serious repairs, including replacement of the ECM, fuel pump replacement twice, battery replacement twice, engine oil pressure switch replacement, and other electrical-related issues. These repeated failures suggest an ongoing electronic or control system problem. At one point, the vehicle was held by the dealership due to the ECM being removed. A representative at Nissan of Murfreesboro suggested that I could pick up and drive the vehicle without the new ECM installed, stating it “shouldn’t” cause damage. I refused due to safety and risk of engine damage. Nissan Consumer Affairs has denied a buyback request and has only offered partial repair assistance, despite the safety system continuing to fail. The defect remains unresolved and continues to disable collision-avoidance systems while the vehicle is in operation. This issue presents a safety risk due to the loss of crash-prevention functionality without warning. I’ve had issues with the vehicle since I bought it in 2024. Nissan would only see my vehicle if I had a “video” since they have to have “proof” of the issue. Jan. 5, 2026 is the most recent time of me taking it in for the electrical light turning on and they said it’s internal failure of distance censor again.Read more

Subject: URGENT: Global Safety Defect Report – Jaguar Land Rover (JLR) Powertrain Control Module (PCM) and Fuel Pump Driver Module (FPDM) Failure. Description of Defect: I am writing to formally report a critical safety defect affecting Jaguar Land Rover (JLR) vehicles involving the Powertrain Control Module (PCM) and the Fuel Pump Driver Module (FPDM). These modules are experiencing spont
... aneous "reset" or "reboot" events during vehicle operation, leading to a sudden and total loss of propulsion. Safety Hazard: Affected vehicles are experiencing power loss at highway speeds, often resulting in the vehicle stopping abruptly in live lanes of traffic. This defect has directly led to near-misses and reported accidents. The loss of power frequently occurs without warning, rendering the vehicle a "sitting duck" on high-speed roads, creating an extreme risk of rear-end collisions and multi-vehicle pileups. Manufacturer Inaction: Despite internal technical bulletins (e.g., NHTSA TSB MC-10218879-0001) and mounting consumer complaints, JLR has failed to issue a global safety recall or provide a permanent hardware/software resolution. While the Chinese State Administration for Market Regulation (SAMR) has reportedly summoned JLR officials regarding these stalling issues, the manufacturer has yet to implement a verified fix or acknowledge the systemic nature of the failure. Demand for Action: We request that your agency launch an immediate investigation into the frequency and severity of these stalling events to compel Jaguar Land Rover to initiate a safety recall and protect the motoring public.Read more

How are you? The problem with our Hyundai involves the engine and engine control system. My vehicle is equipped with the same 2.0-liter Nu GDI engine that Hyundai recalled in other 2021 and 2022 vehicles for engine stalling, internal engine damage, and fire risk. Although my VIN is not currently listed, the engine itself is identical to the recalled engines and is available for inspection. Since
... early ownership, the vehicle has repeatedly hesitated, surged, and jumped when accelerating from a stop, especially at stoplights and intersections. This unpredictable behavior creates a clear safety risk because the vehicle does not respond consistently in traffic and increases the risk of a collision. These issues are ongoing and reproducible under normal driving conditions. The symptoms have occurred many times and were not isolated events. The vehicle has been presented for service and the issues described. No misuse or maintenance-related cause has been identified. Prior to and during these events, the vehicle has shown reduced power, inconsistent throttle response, and hesitation. At times, warning lights and engine behavior consistent with internal engine distress have appeared. These symptoms match those described by Hyundai, Consumer Reports, and NHTSA in recalls involving this same engine. Hyundai has already acknowledged that this engine defect can lead to stalling, seizure, oil leaks, and fire, and that warning systems may not fully detect the problem until damage has already begun. Because my vehicle uses the same recalled engine and exhibits the same failure characteristics, I believe it should be reviewed as part of a recall expansion or defect investigation.Read more

On 09/25/25 at 59,502 miles recall work was completed per NHTSA Recall No. 25V-274, L87 Engine Loss of Propulsion. Dealer paperwork shows that my truck passed the PicoScope inspection and was given the higher viscosity 0W-40 oil. On 12/10/25 at 63,904 miles, with the oil that was changed during the inspection; while traveling on highway NM-52 towing my travel trailer, I experienced a sudden loss o
... f propulsion resulting in my truck stopping in the middle of the roadway. No prior indications of any issues were noted, no check engine lights, no loss of oil pressure, when loss of propulsion occurred. While in the middle of the road I attempted to restart the truck with no start-up. Using gravity, the truck was slowly moved off of the highway backwards. Upon opening the engine hood, a strong odor of burnt engine oil was present. No oil was noted coming out from the engine. When removing the engine oil dip stick it was noted to be steaming and extremely hot to the touch. Due to the location the vehicle it took 2 days to be taken to the dealer. No local vendors were able to pick up the truck to take to the dealer resulting in it having to be personally trailered to the dealer. The vehicle was taken to Bravo Chevrolet and Cadillac in Las Cruces NM. The dealer evaluation of the truck determine that the engine had seized and was replaced under the extended warranty per the same listed recall. This engine failed after passing the required inspections and while utilizing the newly recommended engine oil. This failure could have resulted in an accident due to the nature of the failure, after being led to believe that there were no longer any issues or concerns after passing inspection. Why would there be a failure at that time if the engine passed inspection per the recall? Only cause would be that the remedy action is not properly addressing or correcting the root cause of the failures in these L87 engines.Read more

I am filing a safety complaint regarding a dealership-provided loaner vehicle that had an open safety recall at the time it was issued to me. On Tuesday 1/20, I received a loaner vehicle from Beaver Toyota in Saint Augustine, Florida while my personal vehicle is being held due to a recall. The loaner was a 2025 Toyota Sienna. I was not informed of any safety recalls at the time the vehicle was pro

After discovering the recall, I attempted to contact dealership management multiple times regarding this urgent safety issue. On 1/21, I contacted the General Manager through the dealership’s website. I also called the dealership on Wednesday at 1:37 PM and left a voicemail for the General Manager. I did not receive a response. On 1/22 at 12:24 PM, I called again and left another voicemail. I then followed up with an email requesting a return call due to the safety concern. As of this evening 1/22, I have received no response. Providing a loaner vehicle with an open safety recall, without disclosure, placed me and other occupants at unnecessary risk. On three occasions while the vehicle was idling, the dashboard screens and interior lights went completely black, and the steering wheel jerked abruptly in my hands. This sudden electrical failure creates a serious safety concern, particularly if it were to occur while driving at higher speeds. The dealer has not been able to reproduce the problem. There were no warning lamps, messages or other sy
... mptoms prior to the failure. There were no warning lamps after the problem occurred. The vehicle has been inspected multiple times by a Hyundai dealership. On one occasion, the dealership retained the vehicle for approximately two months but reported no definitive findings despite error codes being present. On a later visit following another failure, no error codes were recorded. The dealership replaced the head/stereo unit and confirmed no active recalls, yet the issue continues. This problem first occurred while the vehicle was covered under an extended warranty, but due to its intermittent nature, it was not resolved before the warranty mileage threshold was exceeded. I have since found numerous similar complaints online from owners of the same model year describing electrical blackouts and steering-related concerns. A Hyundai service manager advised me to dispose of the vehicle, which further reinforces my concern that the defect may be known but unresolved. I believe this issue represents a potential safety defect and warrants documentation and review. Thank you for your attention to this matter.Read more

My wife and I were driving to the dealer to have a safety recall service performed on her Jeep Wrangler 4XE. We had just merged onto the highway, I was in my car, behind my wife in her Jeep. Less than a mile onto the highway, my wife suddenly slowed down and moved to the right lane. As she moved over to the right lane, I saw her front tire violently wobbling back and forth. We slowed down to 3
... 0mph on the highway and after approximately 30 seconds the problem seemed to resolve. We limped our way to the exit (about another mile) and to the dealer about 1/4 mile off the highway. When my wife got out of the car she was still shaking, rattled by what had just happened. When we described what happened to the service center they responded "oh, you had the death wobble." After some research, I watched several videos about the death wobble, which confirmed what I had just seen. What if my wife had panicked and pulled over into another car in the right lane? What if she was on a small 2 lane highway and veered off the road and down an embankment? There were no warning lights or messages on the vehicle indicating any problem prior to this happening. This happened with ZERO warning and was terrifying to watch, let alone my poor wife experiencing first hand. The Jeep dealership has run diagnostics and report it is a bad steering damper. More than 3 weeks later the car remains at the dealership, awaiting repair of not only that, but also the safety recall fix for the hybrid battery. Communication from the dealership has been vague and suspicious in my opinion. They repeatedly have said they are waiting to hear from jeep on what to do next. My research indicates a previous class action suit relating to this "death wobble" issue on previous models. How does something this dangerous still happen on a 2023 model car with 19000 miles in 2025. I'm shocked at how casually the dealer and manufacturer have addressed this issue.Read more

I was driving on the highway with no one in front of me. The car suddenly slammed on the brakes and flashed "emergency breaking system" on my dash. My car went from about 75 mpg down to 50. The car behind me almost rear-ended me. He then proceeded to pass me and brake check me because he thought I had deliberately brake checked him. This incident caused the man behind me to get road rage and react
... dangerously to me. I hit the gas and it finally stopped breaking and let me gain speed up again. It was terrifying. This is not the first time this has happened but it was the first time in the highway. It has happened several times on side roads for no reason either. I have an appointment with Hyundai to figure out what they can do because I don't feel safe driving this car.Read more
