On January 26, 2026, at approximately 6:50 AM, I experienced a serious safety-related malfunction in my 2023 Toyota Mirai while entering traffic on Inglewood Avenue in Lawndale, California. After a brief pause, the display rebooted and turned back on, and immediately afterward the vehicle unexpectedly surged forward. At the time of the power loss, my foot was lightly on the accelerator pedal as I was merging. When the vehicle’s systems reinitialized, the car applied power consistent with the pedal position, resulting in a sudden forward movement. The accelerator was not fully depressed, and this was not a full-throttle event, but the surge occurred without warning while the vehicle was in active traffic. The sequence of events — loss of propulsion, apparent electronic system reset, followed by unintended propulsion upon re-engagement — created a hazardous situation while entering traffic and raises serious concerns about the vehicle’s electronic control and power management systems. The vehicle’s behavior was unpredictable at a critical moment when controlled acceleration was required, which could have resulted in a collision or injury. There were no warning lights or messages prior to the event. I am reporting this incident because it appears to involve a safety defect related to loss of propulsion and unintended surge while in motion. ... s flapping forcefully which created a highly unstable and dangerous condition. My husband pulled over on the side of the interstate to remove the tonneau cover to prevent it from fully detaching. He placed the cover into the truck bed. This incident occurred suddenly, with no warning. This was a near-miss incident with potentially catastrophic consequences. If the tonneau cover had fully detached, it could have become airborne and struck another vehicle. The crossbar that flew off, could have struck another vehicle, easily penetrating a windshield, causing another driver to lose control, or result in a multi-vehicle collision. The risk of serious injury or fatality was significant. The next day, on 1/23/26, we took the truck and damaged tonneau cover to our local Chevrolet dealership for inspection. The service manager visually inspected the cover and documented the failure. We scheduled an appointment for repair at their soonest availability, 2/4/26. We will not feel comfortable using a new cover unless a modification is made to adequately secure the cover to prevent this from occurring again. We also called the Chevrolet dealership where we purchased the vehicle from and spoke with the general manager to let them know what happened, as they have sold close to 100 of these Chevy Silverado EV’s. They said this has happened to others and they speculate that GM is already aware of this concern. We urge the NHTSA and GM to take immediate action. After purchasing the car, the engine oil low light came on after driving about 2,000 miles on a fresh oil change. We took the car to the Chevrolet dealer where they said the engine was over 2 quarts low on oil and recommended a fresh oil change. After another 2,000 miles of driving the low engine oil light came on again. We took it back to the same dealership and started the oil consumption test.
... After several months of oil checks, it was determined that the engine failed the test and service was recommended and would be covered under warranty. Extensive repairs were performed on the engine including piston replacement. After receiving the car back from repair, we continue to have the engine oil low life light appear after a few thousand miles of driving and have to add 2 quarts of oil to the engine. We contacted the Chevrolet dealership that performed the repairs and they said that prior warranty work would not be covered since we were now out of factory warranty. We were informed that we would have to go through the oil consumption test again and pay for any needed repairs. 2011 Mazda6 A serious safety defect occurred on my vehicle and matches NHTSA Recall 19V-323 / Mazda Recall 2818I (front cross member corrosion due to insufficient coating) despite this recall not being associated with my VIN. Front cross member cracked due to rust, causing loss of steering control, wheel misalignment, and scraping from the front wheel. This is exactly the failure described in the
... recall (breakage of the cross member and reduced steering control, increasing the risk of a crash). My safety and the safety of others were put at risk. Although my VIN is not currently associated with the recall, all conditions of the recall apply to my vehicle: -2011 Mazda6 within the affected production range -Defect is a manufacturing issue (insufficient paint coating) -Vehicle was registered in Missouri from 2023-2025, which is a listed state The only reason this recall was not associated with my VIN is because the vehicle was not registered in Missouri at the exact time of the 2019 recall campaign. This is an administrative gap, not a safety distinction. It should not be allowed to limit a manufacturer safety defect to specific states. Vehicles move across state lines constantly. A defect that causes loss of steering control does not stop being dangerous when a car crosses a border. Registration records alone did not reflect actual vehicle use or risk. If recalls are state-limited, manufacturers and NHTSA must continue tracking registration changes and associate the recall with the VIN as soon as a vehicle becomes registered in an affected state. Rust is progressive and age-related. Since this recall began in 2019 and it is now 2026, affected vehicles are more likely to experience failure, and many are now registered in different states than they were in 2019. I am requesting that NHTSA associate Recall 19V-323 with my VIN, investigate the adequacy of state-limited recall tracking, and take action to prevent this known safety defect from causing additional crashes.Read more

I bought a Subaru Forrester 2025 in March of 2025. I drove this car for almost 9 months to the day. On 12-19 -25 I was driving down the aisle in the Walmart parking lot at less than 10 mph. ... ted quickly and with great speed and since my front wheels were still turned toward the right, my car sped toward the cement encased parking sign at a high rate of speed. I was in disbelief. I backed up to be in the proper space and got out and surveyed the damages. Thankfully I was alone in the car and due to the handicapped parking signs ahead and to the right of the parking spot I did not cross the marked lot lines and no other vehicles were involved. The car was not driveable so I had the it towed to the dealership and found they did not have a body shop. Then engaged a body shop to repair. Estimate of damages is $6,820. My car is still there today. It is now Jan 25 and we are waiting on a few replacement parts. At first I thought I might have stepped on the gas instead of the brake as I was parking that night. But all common sense tells me that my foot was properly on the brake as I was finishing up my parking maneuver at a very slow rate of speed. I looked around the internet and see that these same circumstances were replicated by other drivers esp. on the 2012-2020 models of Forresters. I had no prior knowledge of what I consider to be a significant fault in this vehicle. I did receive widespread bruising to my chest from the seatbelt. Once my car is repaired I will take it back to the dealer to have several systems checked for errors. I am leery of driving it again. I am really not in a position to buy a different vehicle but it may come to that. Specific Recalls/Campaigns Not Addressed: I brought my vehicle to Fiesta KIA (Dealer Code 53232) to address the following open safety campaigns which they failed to remediate: NHTSA Recall #24V-757 (KIA SC326): "Instrument Cluster Screen May Fail." This is exactly the failure I am experiencing. NHTSA Recall #24V-443 (KIA SC302): ICCU Software Logic failure, which poses an immediate risk of loss of motive power while driving. Service Action SA563: Specific logic improvement for HUD and Cluster stability. Dealer Conduct (Safety Related): Despite the vehicle presenting with a "No Signal" instrument panel—a catastrophic safety failure—the Service Manager (Anthony) refused to provide the previously promised loaner vehicle. He suggested that because the vehicle was not purchased at his specific dealership, his obligation to facilitate this critical safety repair was diminished. While driving in traffic I heard a loud pop coming from the rear of the vehicle. A warning light came on on the dash board and the vehicle immediately slowed to about 10 mph. This was a dangerous situation as I was actively passing a vehicle on the highway and could have ended up colliding with oncoming traffic!

The drivers side seatbelt will not catch or retract. I went to an independent shop who contacted Jeep to order the part. They were told the part is on “extreme backorder” and it could be months before I could get my car repaired. My Jeep is not driveable. Based on the lack of availability of the part I feel this should be classified as a safety recall.

This is a new vehicle with around 800 miles purchased in early December, 2025. I started the vehicle and let it warm up at least 10 minutes. If they aren’t willing to fix this error I want them to buy me out cuz it’s not a small problem that could result in injury or death if not addressed. My vehicle was towed to a local mechanic and I was informed that the engine was blown and to tow my car to a Hyundai dealership as I was still under the 10 year/100k mile warra

The cleaning did not work and it was then found that a pin was sheared off inside the engine. My engine was then replaced under warranty but I still had to pay for the cleaning. A second mechanic looked over my paperwork after getting my car back 2 months later and noted that the engine failure could have been diagnosed without the cleaning as the cleaning would not have affected where the pin was sheared off. My 2017 Elantra had 90,315 at the time of this incident.Read more

Safety Risk & Pre-Existing Symptoms: Prior to the fire, the engine exhibited a "surging" idle and erratic RPM behavior, which was reported to the dealer. After the replacement of the aforementioned fuel and electrical components, the surging issue was immediately resolved, confirming the original symptom was a precursor to the fuel system failure. While no injuries occurred, the fire presented an immediate risk of occupant injury and total vehicle loss. 3. Disposal of Evidence (Spoliation): Despite an active liability dispute and an ongoing manufacturer investigation, the dealership (AutoNation Ford Valencia) admitted in writing that they disposed of the failed secondary fuel filter and the main wiring harness.
