The contact owned a 2011 Toyota Corolla. The contact stated that while he was driving at 40 MPH, another vehicle that was speeding caused a multiple vehicle collision. During the collision, the contact was in the middle lane when the speeding vehicle crashed into a vehicle in the left lane, and that vehicle merged violently into the middle lane, causing the contact to rear-end the vehicle in front
... due to the driver abruptly stopping. During the attempt to stop, the vehicle failed to decelerate sufficiently. Upon impact, the frontal air bags failed to fully deploy, as they only partially emerged from the housing. The failure experienced was a loss of braking effectiveness at 40 MPH that resulted in a rear-end collision, during which the frontal air bags failed to deploy. The vehicle was towed to a family member's home. The tire pressure warning was the only warning light on at the time of the crash. The driver was injured. Injuries sustained were lower back, right side ribs, and neck pain that required medical attention. The vehicle was deemed totaled by insurance. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 124,000.Read more

The same issue has happened twice. I'm driving on the highway, going maybe 60 or 65, and with no warning, the check engine light starts flashing and the car won't respond to the gas pedal. The car immediately starts to rapidly decelerate, and I can only try to get to the breakdown lane before I get rear-ended. I've been lucky (especially since my child was in the backseat both times). But when I s
... top the car, turn it off, then turn it on again, everything is fine. I can get back on the highway, and no problems occur. The codes are not stored, however, so I can't know what caused the problem. When this happened the first time (last year), I took it to the Honda dealer where I bought it, and they said they had never heard of this happening. When it happened the second time in January, I called Honda, spoke to an agent who gave me a case number (XXX), and said she has heard of something like this from other drivers, but the solutions were all different, so she couldn't say what would fix it for me. I did some internet research, and found other drivers who describe the exact same issue: [XXX] I've switched to premium gasoline ($$) and won't let it get lower than 1/4 full, and have an OBDII scanner in my glove compartment ready to go if it happens again. That said, this is a very dangerous defect, and I feel unsafe in my car. I am disturbed that a problem of with such potentially serious consequences has no solution that anyone can offer me. (Note: When I go to submit this, the final form leaves off the last # of the VIN. I tried to correct, but it happened again. The VIN is [XXX] , 2020 Honda CRV Hybrid. Best VIN ever.) Incident #1: Car suddenly and without warning had complete loss of power, with gas engine shutting off and only using battery power. Car was traveling at 45mph in morning traffic. Pedal was pushed to the floor, but car would only go a maximum of 20mph. Warning dash lights then appeared saying "Hybrid Malfunction/Pull Over Immediately/Turn off car". I was able to pull off road and turn off car and
... then turn back on. Car ran normally the 2 miles back home. Incident #2: 2 days later, while driving my kids to school the car again, suddenly and without warning had complete loss of power, with gas engine shutting off and only using battery power. Again, warning dash lights: "Check Hybrid System. Stop Safely!". I did so and car ran normally back to my house (distance approx: 2.5 miles). I immediately called KIA of Beaverton and they said to have it towed right away. Once at dealer, they were unable to diagnose any issues and said that no codes showed up, but also said that the car "deleted codes". Car was in shop for 2 days and determined "car operates as designed." I picked up the car. Incident #3: 7 days later, with kids in the car, again car had complete power loss with no warning. Warning lights again told of "Check Hybrid System. Stop Safely!" Pulled over (in heavy Friday rush hour traffic) and turned off car. Turned on again, but this time car drove for less than a quarter mile before complete power loss happened again with all warning lights. This time we took video of the car not having power, all the warning lights etc. as evidence. We turned off the car and upon turning it on, it instantly had no power. We pulled car over and had to park it that night at a local business for towing. Car was towed again to Beaverton KIA. Again, dealer said they were not able to replicate problem after driving 250 miles on it. Insisted we pick up car, despite numerous comm's saying how unsafe we felt taking it. Dealer claimed KIA told them to remove .5 Qt of oil.Read more

I am writing to formally file a complaint and request resolution regarding a serious safety issue involving my 2012 Ford Fusion SEL (VIN: [XXX] ). I have been a loyal Ford customer for over 20 years and have owned only Ford vehicles. Unfortunately, my recent experience with Ford Customer Care and my local dealership has been extremely disappointing and concerning. A few weeks ago, my 2012 Ford F
... usion suddenly lost power steering while my young son was driving home from school. This created a terrifying and dangerous situation that could have resulted in a serious accident or injury. Thankfully, he was able to safely pull over, and no one was hurt. However, this was clearly a major safety failure. After this incident, I contacted Ford Customer Care and my local Ford dealership to seek assistance. I was informed that the Customer Satisfaction Program related to the EPAS steering system had expired for my vehicle and that Ford would not provide further support. However, this directly contradicts the information shown in my Ford app. The Ford app clearly lists Customer Satisfaction Program Campaign 15N01 – “EPAS Steering Effort” and instructs me to schedule a free service with a dealer. I later learned that an update was performed in 2015, but the EPAS steering system itself was never replaced—only updated—and that the program was considered a one-time offer. It is extremely upsetting to now face costly repairs for a known safety-related issue that was clearly not resolved correctly the first time. The fact that my Ford app continues to show this campaign as available, while Ford Customer Care claims it cannot be seen in their system and provides no further explanation, is unacceptable. As a long-time Ford customer, I expect transparency, accountability, and support—especially when it comes to safety-related defects. A loss of power steering is not a minor inconvenience; it is a serious hazard INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Formal Complaint on Seized Headlight Sockets and Kia’s Obstruction of Essential Safety Maintenance 1. Incident & Discovery of Defect: I am the owner of a 2015 Kia Sorento. Recently, one of my headlights failed. Upon attempting a routine bulb replacement, I discovered a critical hardware defect: the bulb socket is physically seized. It cannot be rotated or removed by any standard manual force.
... This is not a simple wear-and-tear issue; it is a mechanical seizure of a component that must remain accessible for safety maintenance. 2. Kia’s Refusal of Technical Guidance: To resolve this without damaging the entire assembly, I contacted Kia America (Case #[XXX]) specifically requesting technical instructions or any written explanations to safely disengage the seized unit. Despite multiple attempts via phone and email, Kia explicitly refused to provide any technical guidance, flatly stating they are "not technically trained" or authorized to share such information. 3. Obstruction of "Right to Repair" & Safety Risk: I firmly believe that a vehicle owner has a fundamental "Right to Repair," especially for essential safety components like headlights. By withholding basic maintenance instructions, Kia is intentionally obstructing my ability to maintain a street-legal, safe vehicle. Driving with a non-functional headlight is a Critical Safety Hazard that severely impairs visibility and creates an immediate risk of fatal accidents—yet Kia has chosen to leave me in this dangerous situation. 4. Exploitative Business Practice & Systemic Failure: This appears to be a deliberate and Exploitative Business Practice. By failing to provide a manual and refusing technical support for a defective, seized part, Kia is effectively forcing consumers into a "forced monopoly" where we must pay for expensive replacements at authorized dealerships for what should be a low-cost, simple bulb change. 5. Request for Investigation: I request that the NHTSA initiate a formal investigation. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF IRead more

When reversing, the car sometimes abruptly slams on the brakes as if it is avoiding a collision even though no collision is
... close to happening. GM refuses to fix the problem.Read more

The car is a brand-new 2025 Ioniq 5 with only 2,949 miles. All of a sudden and without warning, it entered "limp mode" while driving and did not go past 25 miles per hour. I was close to home so luckily I was able to street park it safely. Called roadside assistance and the car completely powered off (died) in front of the tow truck. The dealership says it is an ICCU failure and need to be entirel
