The transmission/torque converter completely failed after being maintained while driving on the highway without any warning. The transmission began jerking and bucking, almost sending me into the oncoming lane while driving. Coupled with the lane departure avoidance system automatically turning my wheel, the vehicle became uncontrollable. Even though i removed my foot from the gas an tried braking
... , the transmission would not disengage for a few seconds causing me to almost lose control of the vehicle. Then the transmission just gave out and I was unable to pull off some where safe without issue. There were no warnings on the vehicle, no lights, nothing signaled an issue until after i was able to get the car to stop at the side of the road. The vehicle had to be towed due to not being able to drive at all. It has been inspected and I have been told the transmission just completely went out, possibly due to a leak in the linkage I was not aware of, and was not caught during my last inspection/ fluid change. Multiple items failed on the vehicle including the recently recalled back up camera/sensor issue that I was never advised about by the manufacturer.Read more

When braking(going down hill) the Bronco comes to a complete stop and the brakes feel good. Service writer tried looking up any recalls or complaints on this issue and couldn't find anything? On the BroncoG6 forum online, there apparently are other Bronco owners complaining about the same issue. One owner (online) stated that the dealer reprogrammed the ABS system and his truck hasn't repeated the creep anymore. There haven't been any brake, ABS or any warning lamps at all when this happens. ... ring Torque Sensor Fault I am submitting this complaint regarding an unresolved safety defect involving my 2011 Ford Escape XLT. This vehicle is subject to a federally recognized safety recall for loss of electric power steering assist: NHTSA Campaign Number 14V284000, Ford Recall 14S05, which involves the electric power steering (EPS) system and steering torque sensor faults that can cause the vehicle to suddenly lose steering assist. The safety issue covered under this recall is still present and represents an ongoing risk. The vehicle experiences a loss of power steering assist, which makes the steering wheel extremely difficult to turn and increases the risk of a crash, especially at lower speeds, in traffic, or during turns. On February 13, 2026, I had a scheduled appointment at Kingman Ford (Arizona) at 3:00 PM specifically to address this safety recall issue and the continuing steering assist problem. Kingman Ford declined service and refused to perform any recall-related repair, stating they could not provide service related to the recall. After the dealership refusal, I contacted Ford Corporate Customer Service directly. Ford Corporate also declined to provide any repair assistance, and stated they had no knowledge of any federal directives or requirements regarding safety issues connected to this recall. Because this is a federally recognized safety recall and the defect continues to occur, I am requesting that NHTSA document this as a recall remedy failure / unresolved safety defect, and review Ford’s refusal (and the dealership’s refusal) to correct a known steerinRead more

Tailgate was dropping open while driving, and sometimes would not latch when shut without slamming it several times. Took it to Gupton Dodge Dealership 37172 and they performed the recall. A few months later the same problems started occurring again, then after a while longer the tailgate actually would not open at all anymore. I took it back to the same dealership to notify them that their rec
... all repair failed. They told me the recall had already been done and it was working when it left their shop, so I would need to pay for any further repairs which would require replacing the entire actuator assembly. I brought the truck back home and removed the tailgate latch cover to assess the problem myself. I found that the original actuator was still on and had never been replaced in the recall work. I also found that by cleaning up the metal actuator parts with carb cleaner spray, and lubing it with dry lube and white lithium grease at critical movement spots, the actuator and tailgate latches started working perfectly again. I now realize that they very likely did the same thing for the recall, just cleaned and lubed it, and no parts replaced or added. I expect it will now work for another few more months before dust and moisture will cause it to need cleaning and lubing again in order to keep it working. Serious safety hazard, not just from dropped cargo! My wife closed tailgate once while my 6 year old grandson was standing next to her. As soon as she stepped away the heavy tailgate fell open hard, missing my grandson's head by inches. Dealers are just doing a quick, cheap, temporary fix to say they met their obligation, and are perpetuating the danger. Even if the actuator assembly is replaced with the OEM part, it will also fail from normal dust & moisture over time. The recall needs to require the actuator assembly to be REPLACED with an IMPROVED DESIGN that will not fail from normal dust and moisture, since it is not in a sealed compartment.Read more

Within the past two months, I have experienced multiple significant mechanical failures that raise serious concerns about the reliability and safety of this vehicle. Shortly thereafter, the fuel pump faile
... d. I became aware there was a recall related to the fuel system; however, when the dealership ran my VIN, it did not populate under the recall campaign. Despite this, the component still failed. Now, I am experiencing continued stalling and drivability issues, particularly when coming to a stop or when the automatic start/stop system engages at traffic lights. Based on the symptoms, I believe the secondary fuel pump may also be malfunctioning. A vehicle stalling in traffic creates a clear safety hazard. I rely on this vehicle daily, and repeated fuel system failures at this mileage are unacceptable. I am requesting the following: A full diagnostic review of the fuel system under manufacturer oversight Consideration for coverage or reimbursement of recent repairs Investigation into whether my vehicle should have been included in the applicable recall Assurance that this vehicle is safe to operate I have maintained this vehicle appropriately and did not expect major component failures at 55,000 miles. I am submitting this complaint regarding a recurring transmission issue in my vehicle. The transmission consistently jerks and shifts erratically, particularly in the morning during initial operation when at cruising speeds of about 50-75 mph. This behavior occurs when the transmission temperature is below approximately 100°F and happens on a daily basis. The gear changes feel harsh and abrupt,
... kind of like it doesn’t know what gear to be in its raising concerns about both drivability and long-term transmission reliability. I brought the vehicle to an authorized dealership for inspection. After evaluating the issue, the dealer informed me that the behavior was due to the vehicle being in “Sport Mode.” However, this explanation does not resolve the concern, as the severity and consistency of the jerking appear abnormal and occur regularly under the same temperature conditions. Given the persistent nature of this problem and its potential safety implications — especially during acceleration and low-speed driving — I am requesting that this issue be formally documented and investigated. Thank you for your attention to this matter.Read more

I am reporting a safety concern involving multiple loose windshield header bolts and cowl panel fasteners on my 2022 Mazda CX-9. The issue was discovered on 1/28/26 at approximately 44,000 miles. Several bolts securing the windshield header and cowl panel area were found loose. These components support parts located at the base of the windshield. Loose fasteners in this area raise concern about p
... otential windshield instability, movement of the cowl panel, and possible detachment or structural compromise over time. This condition could create a safety risk, particularly at highway speeds, including driver distraction from vibration or noise, potential windshield instability, and possible failure of components mounted near the base of the windshield. The Mazda dealer is aware of this issue. I was informed that they have repaired at least 15 similar vehicles, including those associated with a Roman Service member, indicating a possible pattern. Despite this, Mazda did not cover the repair cost. Given that multiple bolts were improperly secured and that the dealer has seen numerous similar cases, this appears to be a potential manufacturing or assembly defect affecting multiple vehicles. I am requesting that NHTSA review this issue for possible investigation and recall consideration.Read more

On February 11, 2026, at approximately 11:00 a.m., I picked up my Tesla vehicle from the Tesla Service Center located at 16955 Chesterfield Airport Road, Chesterfield, Missouri following battery repair. After paying for the service and being told the vehicle was ready, I discovered in the parking lot that the hood was not latched and was misaligned. The hood had been opened during service. Prior
... to drop-off, the hood latched properly and securely. Releasing the vehicle in this condition created a significant highway safety hazard. At highway speeds, an unlatched hood can deploy and completely obstruct forward visibility, potentially resulting in a catastrophic collision. When I immediately reported the issue to service management, responsibility was dismissed and attributed to a prior front collision history, despite the hood functioning normally before service. The service center did not promptly correct the unsafe condition. I was forced to leave the premises, purchase mechanical tools across the street, and physically resecure/realign hinge hardware myself in order to safely operate the vehicle. I have preserved video documentation of the hood condition at the time of release, along with service records and receipts. This represents unsafe post-repair vehicle release and failure of final safety verification. I have video evidence of my vehicle after I paid $1170 service fee and inspected the hood not latching. Having a newer truck with 15000 miles and the best warranty availability I would think RAM would stand behind their product and make it safe and dependable. But I was told by the service department this is a known problem that RAM does not have a fix for. Trucks 2023 and older have a fix not the 2024 and newer. Being late for work because if this is not acceptable either when it takes an hour for the light to go off to be out of limp mode as well. December 16 January 12 and 13 January 30-31 Service department recommended putting heat in my garage or installing a block heater but wernt sure if either would fix the issue.Read more

On February 2, 2026, while driving approximately 65 mph on the freeway in Wisconsin, the panoramic sunroof glass on my 2019 Toyota RAV4 XLE Premium (VIN: [XXX] ) spontaneously exploded. The ambient temperature was approximately 20°F. Crucially, the glass buckled outward, indicating the failure was caused by internal pressure or a structural defect, rather than an external impact (such as a rock)
... . There were no other vehicles immediately ahead of me that could have kicked up debris. Safety Risk: The explosion created a loud, gunshot-like noise that caused severe driver distraction at highway speeds. While no crash occurred and no injuries were sustained this time, the sudden structural failure posed a significant risk of loss of vehicle control. Status: No warning lights or messages appeared prior to the failure. The vehicle had no prior damage to the roof area. Photos of outward buckling available upon request INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
