On 8/14/24 I had the rear springs replaced due to a broken spring. Now today 2/25/26 I was inform that I have a broken front spring. This vehicle is only 9 years old and we have had spring break in the front and back. This seems like a premature failure of a critical suspension component. Though fortunately nothing happened in the way a bodily injury the failure could very well lead to an inabilit
... y to properly control the vehicle if the broken spring were to slip and crash is possible. For example a tire could be punctured at a high rate of speed. I may not be able to maintain control with the broken suspension component. The vehicle springs should last more than 7 years before failure. I have owned many vehicles over the years a lot older than this one and with more miles and have never experience spring failure.Read more

While driving, the instrument cluster/odometer intermittently flashed and became difficult to read for ~15 minutes. After parking, the odometer continued flashing for ~10 additional minutes. After the flashing stopped, the vehicle displayed “ParkSense Unavailable – Service Required.” On a separate date, with no trailer connected and without pressing steering wheel buttons, the cluster displa
... yed a “Blind Spot Alert – Trailer Type (Conventional/Goose/5th) – Press OK to Select” prompt. These events were captured on video. Safety risk: driver information/warnings become unreliable while the vehicle is in motion, which is distracting and may reduce awareness of speed/vehicle status. No crash, injury, or fire occurred. A dealer inspected the vehicle and reviewed the customer video. The dealer documented stored electrical/communication/configuration faults consistent with the concern, including ECU configuration mismatch, lost communication with the trailer interface module, and communication bus off faults. The dealer performed software updates to related modules and road tested ~34 miles; the concern did not reproduce during the test drive. The vehicle also has an open safety recall (NHTSA 26V-059 / FCA 03D) for the trailer tow module with remedy/parts pending. Incidents occurred on 02/16/2026 and 02/18/2026 at ~3,000 miles, including after cold/rain conditions. One restart sequence did not immediately stop the flashing. Dealer scan recorded the following (stored) DTCs: TIPM B2204-87 (ECU configuration mismatch), TIPM U116B-00 (lost communication with trailer interface module), and ITCM U0077-00 (communication bus off). Dealer performed module software updates including CADM update per TSB 08-020-26 and PCM update per RRT-26-036. Recall remedy for 26V-059 / FCA 03D remains pending parts.Read more

Description of the Problem: •Component/system that failed: The engine starter, which is subject to an open recall issued September 23, 2025. The starter was not replaced during a service visit. The vehicle no longer exists due to the fire, but the VIN and service records are available for inspection. •Safety risk: The starter malfunction caused the vehicle to catch fire while parked, creating
... a serious safety risk to myself, my child, and others nearby. •Problem reproduction/confirmation: The fire and defect were confirmed by BMW’s inspection as originating from the starter irregularity tied to the recall. •Inspections: The vehicle was inspected by BMW North America’s engineering team after the fire, and insurance representatives confirmed the total loss. •Warning signs prior to failure: There were no warning lamps, messages, or symptoms indicating the starter defect before the fire. Additional context: The vehicle was serviced at an authorized BMW dealership on December 3, 2025. I was not informed of the recall, and the starter was not replaced. The vehicle caught fire on December 19, 2025, resulting in a total loss. While my insurance resolved the ACV, this incident was caused by BMW’s recall-related defect and procedural failure. I have escalated this matter to BMW North America and am requesting a resolution that restores me to a comparable position (replacement vehicle or financial support). Official confirmation: BMW North America confirmed in a confidential letter that the fire originated from the starter defect tied to the open recall. NHTSA SAFETY DEFECT REPORT Target: Waymo LLC / Alphabet Inc. Subject: Suspected ADS Perception Defect – Wheelchair Users Location: Chicago, IL (60611, 60601, 60602, 60610, 60654) Waymo’s Automated Driving System (ADS) appears to exhibit a defect in Object and Event Detection and Response (OEDR) performance concerning pedestrians using power wheelchairs and other mobility aids. Wheelchair use
... rs present a lower visual profile and atypical movement patterns. If misclassified as stationary objects or not detected in time, the ADS may fail to brake or yield appropriately. In high-density urban intersections, this creates an unreasonable risk of crash, serious injury, or death. Waymo has previously recalled vehicles for ADS object-recognition and classification failures. These prior defects raise concern that vulnerable road users with non-standard physical profiles remain insufficiently validated within the Operational Design Domain. Pursuant to 49 U.S.C. Chapter 301, I request that NHTSA initiate a Preliminary Evaluation into Waymo ADS OEDR performance as it relates to wheelchair users in dense urban environments.Read more

The stirring wheel consistently sticks and jerks in speeds 35 and over. The steering wheel constantly has to be adjusted to drive in a lane. It is unsettling situation. The issue creates a safety concern as it impairs precise vehicle control and is most concerning while making corrections at highway speeds. Multiple owners online are reporting the same symptoms, and there are indications that this may be caused by a faulty EPS gearbox or steering rack. When driving at highway speeds steering sticks. Steering wheel does not move without more effort than usual. Being unable to make small adjustments freely means I must constantly drive with two hands in order to drive straight forward. When I noticed this I searched Honda recalls and found the exact same issue is being recalled for later models. I am submitting this complaint to report a safety defect involving a 2018 Audi Q5 (Premium Plus, Technology Package). The vehicle’s MMI infotainment/navigation system intermittently shuts down or reboots during operation. When this occurs while the vehicle is in in reverse, the rearview camera display immediately goes blank and remains unavailable until the system completes its reboot cycle. Thi
... s failure has occurred during active reversing maneuvers, creating a significant safety concern. An incident already occurred that resulted in an impact to another object during backing and the camera display/MMI going blank loss just prior to stopping. The rearview camera is a required safety system under Federal Motor Vehicle Safety Standard No. 111. When the system powers off unexpectedly, the driver loses critical rear visibility information at precisely the moment it is most needed. The condition appears to be software-related, as the infotainment system restarts on its own. However, the temporary loss of the camera display increases the risk of collision with pedestrians, other vehicles, or stationary objects. I understand that recalls have been issued for similar rearview camera display failures in later model-year Audi Q5 vehicles. The symptoms I am experiencing are materially similar and may indicate a broader defect pattern affecting earlier production years. I have attempted to resolve the issue with Audi and their technicians and service management. The conclusion was they could not find any error messages or mechanical issues. They would not diagnose deeper or complete any further trouble shooting. The previous owner also attempted to resolve the safety defect under warranty and received similar attention with no solution. I respectfully request that NHTSA review this matter for potential investigation to determine whether the 2018 Audi Q5 should be included in any safety-related corrective action. Of note, the "CarPlay" use is not the cause.Read more

I was driving on the highway and experienced what felt like a brief power outage where my vehicle instantly dropped speed. I estimate it was approximately 10-15 mph. Everything appeared to return to normal at the time until approximately 5 minutes later when I exited the highway and it began making a loud noise. I drove a couple miles before getting concerned and thinking I don't want to risk b
... reaking down on the side of the highway going home or doing further damage to an expensive component such as engine or transmission. I used roadside assistance to tow my vehicle to my nearest dealership. The tow driver confirmed he heard the noise inside the vehicle while I stood outside and could not hear it. The dealership called me to approve the diagnostic fee and also asked questions about what happened. In hindsight, they had asked a very specific question regarding the noise sound and location which, in my opinion, shows they instantly suspected the issue that they would later report. After approving the fee, they sent a quote focused primarily around the rear differential housing being cracked for an estimate of $5221.85. I asked how this would happen and the service representative explained that the choke inside the differential broke and a piece of metal inside it cracked the housing. I began researching differentials in vehicles and specifically in Jeep Cherokees, I learned very quickly that this is a very common problem which explains how a representative was able to tell me the cause without seeing it. No warnings were ever issued on the dashboard and no symptoms were present prior to the sudden deceleration. There are no warnings on the dash after the event and the only symptom other than the noise while in motion is the acceleration/deceleration feels slightly less sensitive. Every potential cause stated by mechanics who have looked at this points to a manufacturing issue or quality issue. A complete failure could cause an accident harming people.Read more

We own a 2016 Jeep Cherokee and have been experiencing ongoing issues with the transmission and acceleration. The vehicle will sometimes hesitate or lag when pressing the gas pedal. There have been multiple instances where the vehicle did not move as expecte

This has nearly resulted in accidents due to oncoming traffic. The problem happens most often at normal city / street speeds, especially when accelerating from a stop or at low speeds. The vehicle feels like it struggles to shift or deliver power. We have also noticed hesitation at higher speeds, but the main issue is during everyday driving. No crash has occurred, but the hesitation and inconsistent response make the vehicle feel unsafe and unreliable.Read more

I am filing a formal safety complaint regarding my Mercedes-Benz Metris serviced at Mercedes-Benz Coconut Creek (Florida) under Recall VS2LAS561 – 9789VMCRC (Customer Service Case #20781216). On February 12th, I delivered the vehicle for recall service. It was returned on February 23rd without the recall properly completed. Invoice no. 328897 – tag T9514 lists replacement of four Continental
... 235/55 R17 tires and installation of four PRS sensor repair kits. These items were not installed. The tires were placed in the trunk, and only three used sensors are present; one wheel has no sensor. Additionally, the dealership provided 235/55 R17 tires, while the vehicle is homologated for 225/55 R17. Supplying non-approved tire sizes represents a safety concern. Since purchase (from the same dealership), I have reported tire pressure irregularities and abnormal lateral tire wear. During the most recent service, no tire rotation was performed despite documented uneven wear. Two weeks prior to the recall visit, I experienced a highway tire blowout while traveling with my family. When attempting to install the spare tire, I discovered the wheel lock removal key had been removed during a prior Mercedes service and not returned. I was unable to change the tire and required roadside assistance. The lug bolts had to be forcibly removed by an external shop, potentially damaging wheel components. The dealership has not properly resolved these safety-related issues. I do not feel safe operating this vehicle under current conditions. I request investigation into: • Recall execution compliance • Tire size compliance with vehicle specifications • Improper documentation of uninstalled parts • Removal of required emergency equipment (wheel lock key) These issues present potential safety risks to vehicle occupants and other road users. Pulling off from a stop sign on a hill, the car wouldn't accelerate and rolled back hitting a pedestrian and then jerked forward aggressively burning rubber on the tires. I smell the transmission burning. Luckily the lady didn't want to press charges.

While driving down the highway, my engine completely turned off. I was able to get to a safe location. When I tried turning the car back on, it would not turn back on. I took it to Kia, and they said I was covered under warranty due to their settlement. However, due to the salvage title of the car, they will not replace the engine, even though that is what their recall settlement covers.

Previous on board charger (GHCA) failed and was replaced in January of 2026. In February I noticed the car was charging slowly on a 48A home charger (car showed 5.4kw at 22A). I unplugged the charger, waited a minute, and plugged it back in. The car then reinitialized charging and showed 11kw at 48A. The car then started making audible electrical crackling noises. The vehicle has developed a pronounced knocking noise that was diagnosed by your authorized dealership as “piston slap.” I was informed by dealership staff that this is a known issue with this vehicle. However, because the vehicle is outside of warranty by time and mileage, I was told no repair assistance would be provided. My primary concern is safety. While there are currently no warning l
... amps illuminated, the engine is producing a consistent knocking noise. If this condition progresses to piston or engine failure while the vehicle is in operation, it could cause sudden loss of power or engine seizure. Such a failure at highway speeds or in traffic could create a serious accident risk to myself and others. Additionally, I was charged one hour of diagnostic labor, despite the dealership immediately recognizing the noise as piston slap without needing further testing. This raises concern that: The issue is well-known, The condition was identifiable without extensive diagnosis, Yet I was still billed for a full diagnostic hour. Given that this is acknowledged as a known issue, I am requesting: A formal review of this case by your product safety and quality department. Clarification on whether this condition has been the subject of technical service bulletins, extended coverage programs, or internal advisories. Consideration of goodwill repair assistance due to the potential safety implications. Reimbursement of the diagnostic charge, as no true diagnostic procedure appears to have been required. Written confirmation that continued operation of the vehicle in its current condition does not present a safety risk. I am prepared to provide service records, invoices, and any additional documentation needed. Given the potential safety implications, I respectfully request a response within 48 hours. I am hopeful the manufacturer will stand behind the safety and reliability of its product and address this matter promptly.Read more
