My 2025 Toyota Sienna is affected by the recall related to improper welding of the second-row seat rails. On November 19, 2025, I brought the vehicle to a Toyota dealership because they had advised me that it was not safe to continue driving due to the safety concern involving passenger seats. The dealership advised against driving it and kept the vehicle, and Toyota arranged a rental vehicle for
... me. As of now, several months later, there is still no repair remedy available and my vehicle remains at the dealership. I transport children and elderly family members, so passenger seat safety is very important to me. I am concerned about the long delay in providing a remedy for a safety-related issue and the extended loss of use of my vehicle.Read more

I am filing a formal safety complaint regarding my 2020 Hyundai Palisade due to severe and ongoing engine oil consumption that presents a safety risk, combined with an active federal airbag safety recall. For approximately one year, my vehicle has been consuming excessive engine oil. Approximately 1,500–2,000 miles after an oil change, the vehicle becomes dangerously low on oil. On January 19, 2
... 026, at 171,650 miles, the dealership initiated Hyundai’s official oil consumption test. At intake, the engine oil level was so low that nothing was visible on the dipstick. The dealership confirmed no external oil leaks were present and began the manufacturer’s oil consumption protocol. After driving approximately 1,000 miles, I returned on February 2, 2026, at 172,657 miles. The inspection showed the engine was 2.6 quarts low. The dealership stated combustion chamber cleaning was needed; however, Hyundai denied the repair based solely on mileage and the vehicle being out of warranty. This level of oil consumption presents a serious safety concern. Driving with critically low oil can cause sudden engine failure, stalling, or loss of power while traveling at highway speeds. This could create a crash hazard without warning. Additionally, my vehicle is subject to NHTSA Recall #26V034000 (Recall #292 – Ejection Mitigation), involving potential failure of the side curtain airbags for third-row passengers to deploy properly in certain crash scenarios. The remedy is not yet available. The combination of excessive oil consumption and an unresolved federal safety recall raises significant concerns regarding the overall safety and reliability of this vehicle. I am requesting that NHTSA investigate the engine oil consumption issue in the 2020 Hyundai Palisade, as excessive oil burning may indicate a broader manufacturing defect affecting engine integrity and driver safety. On Friday, March 13, 2026, while traveling at approximately 60 mph on the highway, my 2021 Chevrolet Suburban equipped with an L87 6.2L V8 engine experienced a sudden and complete loss of motive power. The failure sequence began with a dashboard warning prompting me to "press the start button again," followed immediately by the vehicle shifting into neutral without any driver input. Despite repeat
... edly pressing the accelerator, the vehicle continued to decelerate and could not re-engage drive. A second dashboard warning displaying "engine recall" then appeared mid-incident. Subsequently, the rear wheels locked, forcing the vehicle to coast to a complete stop on the hard shoulder without any brake input required from the driver. Upon coming to a full stop, the start button continued flashing with repeated prompts to press it, the vehicle lock emblem illuminated on the dashboard, and the vehicle became fully and mechanically immobile, unable to shift out of neutral or transition into any other gear. The engine could not power the vehicle at all. My safety and the safety of other motorists were directly and seriously endangered, as the failure occurred without warning in the middle lane of an active highway at highway speed, leaving the vehicle unable to maneuver or accelerate out of harm's way. A responding police officer confirmed the road hazard on scene and remained present to protect the vehicle and occupants while we awaited assistance. The AAA tow truck operator independently concurred upon arrival. The vehicle was transported to Classic Chevrolet, Grapevine, TX, the same day and is currently awaiting full technical inspection by the dealership's service team; an official mechanical diagnosis remains pending. The dealership service advisor has verbally confirmed awareness of an active engine and powertrain/transmission recall affecting this vehicle. This vehicle experiences multiple recurring safety-critical system failures while driving, creating a dangerous operating condition for both the driver and the public. 1. Dashboard / Instrument Cluster Failure While Driving While the vehicle is in motion, the dashboard display intermittently shuts off completely. When this occurs, I cannot see my speed, warning lights, battery status, or any othe
... r gauges. This creates a severe safety risk because I am effectively driving without any instrument information. The issue occurs while actively operating the vehicle on public roads. 2. Navigation / GPS Location Failure The vehicle’s built-in mapping system frequently shows my location several states away from my actual position. Even though the location is incorrect, the system continues to provide turn-by-turn navigation instructions based on the incorrect location. If these directions were followed, it could cause a driver to make unsafe maneuvers or become distracted while attempting to correct the navigation error. 3. False Automatic Emergency Braking Activation The automatic braking system frequently activates even when there are no vehicles, pedestrians, or obstacles present. The vehicle abruptly slows or brakes on open roads with nothing nearby. This creates a significant safety hazard because it could cause a rear-end collision if another vehicle is following. Repair History / Ongoing Issue: This vehicle has been taken in for repair multiple times for these issues. Despite attempted repairs, the problems continue to occur. The recurring nature of these failures indicates a systemic defect that has not been corrected. Safety Concern: These failures create multiple hazardous driving conditions: Loss of dashboard information while driving Incorrect navigation location and directions Unintended automatic braking events Allowing this vehicle to remain in service with these unresolved defects places both the driver and the public at risk. I am reporting a safety concern involving a 2018 Dodge Journey SE (VIN: [XXX] ) related to a failure in the vehicle’s emissions control system, specifically the catalytic converter and oxygen sensors. On January 13, 2026, the vehicle began producing a strong sulfur or “rotten egg” odor while driving, and shortly afterward the check engine light illuminated. Diagnostic testing revealed troubl
... e code P0420 (Catalyst System Efficiency Below Threshold – Bank 1). This code indicates that the catalytic converter is not properly converting exhaust gases, which can result in increased emissions and potential exposure to exhaust fumes. The vehicle was inspected by an authorized Chrysler, Dodge, Jeep, and Ram dealership in Maurice, Louisiana, which confirmed that the catalytic converter and oxygen sensors are failing and must be replaced. The vehicle remains available for inspection upon request. This vehicle was purchased for our growing family and is regularly used to transport our children, including a [XXX] infant and a [XXX] child. Since the emissions system issue began, occupants have noticed persistent exhaust-like fumes and strong sulfur odors while the vehicle is operating. Our infant has experienced recurring respiratory issues, and our [XXX] began experiencing respiratory symptoms after we purchased this vehicle. While we cannot definitively determine the cause, we are concerned that exposure to exhaust fumes from the malfunctioning emissions system may be contributing to these health issues. In addition, adult occupants have reported feeling unusually tired or fatigued after longer car rides in the vehicle, which raises additional concerns about potential exposure to exhaust gases inside the cabin. Because catalytic converter failures can allow harmful exhaust gases such as carbon monoxide and other pollutants to pass through the exhaust system improperly, we believe this issue presents a potential safety risk to vehicle occupants, p INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 URead more

I am filing a complaint regarding a suspension defect on my new Tesla Model Y Juniper. This raises serious concerns about the quality and safety of the suspension system. Based on information from other Tesla owners, this appears to be a recurring issue affecting multiple Model Y Juniper vehicles. A persistent suspension defect on a new vehicle may pose a safety risk and should be investigated. I request that NHTSA review this issue and investigate whether there is a manufacturing or design defect affecting the suspension system of the Tesla Model Y Juniper. 1. The vehicle’s transmission appears to be failing. I was driving my 2020 Toyota Highlander on the freeway at about 75 mph with my family in the vehicle when the car suddenly lost power and would not accelerate properly even though the engine was still running. Because of the sudden loss of power, I had to carefully move the vehicle to the shoulder of the freeway. At the time I did not realize
... it may have been related to the transmission. 2.Recently I brought the vehicle to an authorized Toyota dealership for a routine oil change and because I had started hearing a whining noise when accelerating, especially above about 30 mph. After inspecting the vehicle, the dealership informed me that the transmission needs to be replaced and provided a repair estimate of approximately $9,800. The vehicle currently has around 72,000 miles and the transmission is available for inspection upon request. 3.The sudden loss of power on the freeway created a dangerous situation because traffic around us was moving at highway speeds. My family was in the vehicle and I had to maneuver the car to the shoulder while vehicles were passing at high speed, which could have resulted in a serious accident. 4.The issue has been confirmed by the authorized Toyota dealership service department. When the incident occurred and we were stopped on the shoulder of the freeway, we contacted the highway patrol because we were concerned about our safety while stopped on the highway. 5.No warning lights or messages appeared on the dashboard prior to the loss of power. The only symptom noticed before bringing the vehicle in for service was a whining noise during acceleration.Read more

My 2015 Ford Transit 350 equipped with the 3.2L diesel engine experienced a failure of the high-pressure fuel injection pump at approximately 72,000 miles. The pump generated metallic debris which contaminated the entire fuel system. The vehicle began experiencing drivability problems including loss of power and intermittent stalling. The vehicle was diagnosed by a Ford dealership, where metal co
... ntamination was found in the fuel system. The repair required replacement of the high-pressure fuel pump, injectors, high-pressure fuel lines, and fuel rail, along with flushing and cleaning of the fuel tank and fuel system. The total repair cost was approximately $11,500. This failure appears to match the defect described in Ford safety recall 16S32 for certain 2015–2016 Ford Transit vehicles equipped with the same 3.2L diesel engine, where scuffing in the fuel injection pump can create metallic debris that contaminates the fuel system. My vehicle was built July 16, 2015, essentially within the same production period described in the recall, but my VIN was not included in the recall population. Because this failure required replacement of most of the fuel system components and created a sudden loss of power risk while driving, I am reporting this issue so it can be investigated and considered for inclusion with similar failures.Read more
