I am writing to report a recurring safety issue with my vehicle’s transmission. When attempting to accelerate into traffic, the engine revs high but fails to deliver power to the wheels, acting as if it is in neutral. This failure to shift into gear has occurred several times and poses a significant safety risk. I have maintained the vehicle regularly, including recent fluid changes. My researc

I respectfully submit the following additional information. In connection with this matter, the dealer in question asserted in writing that I "should have refused to participate" in the manufacturer-authorized "Z05 Service Campaign"—a measure designed to prevent potential damage—and characterized the program as nothing more than "free paint work." Service campaigns administered by manufacturers
... are conducted within the framework of federal safety regulations pursuant to Title 49, Chapter 301 of the U.S. Code; they are neither optional nor merely cosmetic in nature. Rather, they constitute systematic procedures intended to preserve vehicle integrity and support public safety. Statements suggesting that consumers should decline participation in such campaigns—or that they may bear financial responsibility for damage incurred during their execution—raise concerns regarding how these federally supervised programs are communicated and implemented and may discourage participation and undermine public confidence in the national vehicle safety campaign system. Although the dealership subsequently offered to replace the windshield at no cost, I have not received confirmation that the repair has been completed. This offer was made only after the vehicle remained at the facility for an extended period, during which I was repeatedly placed under pressure to assume financial responsibility for damage that occurred while the vehicle was under dealership custody. I was presented with two conditional options; however, in either scenario, I would ultimately be required to accept financial responsibility for the outcome. Ultimately, I was required to assume financial responsibility for issues arising during the work. On 2/23, I was told the loaner could be reported as "stolen," and I returned it immediately. After returning the vehicle, no alternative transportation was provided while my own vehicle remained unrepaired.Read more

I am submitting this formal complaint regarding an active safety recall affecting the seat belt system of my 2025 Hyundai Palisade XRT. Hyundai has acknowledged the existence of the defect. However, I was informed by a Hyundai customer service representative [XXX] ), under case number XXX, that there is currently no available remedy or repair for this recall. This situation is deeply concerning,
... as it involves a critical safety component intended to protect occupants in the event of a collision. Due to the nature of this defect, I do not feel safe operating the vehicle under any circumstances. Despite the acknowledged safety risk, Hyundai has not provided any adequate interim solution, such as a replacement vehicle or transportation assistance, leaving me effectively unable to safely use the vehicle. This places me in an unreasonable and unsafe position, as I am left with a vehicle that poses a potential risk to my safety without any available corrective action. I respectfully request that the National Highway Traffic Safety Administration investigate this matter and take appropriate action to ensure that Hyundai provides a timely and effective remedy, as well as appropriate support to affected consumers. The internal core of the Exhaust Heat Recovery System (EHRS), part of the center muffler/front pipe assembly (#28600-L5210), suffered a catastrophic failure. This allowed approximately 9 gallons of engine coolant to leak directly into the hot exhaust stream. Safety Risk: This failure creates a severe visibility hazard. During acceleration from stop lights and stop signs, the vehicle emits massive,
... thick clouds of white glycol smoke. The smoke is so dense that it completely obscures the driver's view of trailing vehicles and prevents following drivers from seeing the roadway or my vehicle's brake lights, creating a high risk of rear-end collisions. Additionally, the rapid loss of coolant caused the engine to overheat and lose power while in motion, nearly causing a stall in active traffic. Confirmation: The failure was confirmed by the service department at Washington Hyundai (Invoice #174281). This is the second failure of this specific component (previously failed in Jan 2025). The manufacturer issued TSB 23-EM-009H for this defect, acknowledging it affects 2020-2023 Sonata Hybrids, but no safety recall has been issued. Symptoms: The issue was preceded by a "sweet" burning smell and a sudden "Engine Overheating" warning on the dashboard. The failed component has been replaced by the dealer, but the manufacturer refused to replace the coolant-poisoned Oxygen (O2) sensors, leaving the vehicle in a technically compromised state. The failed unit is likely held by the dealer for warranty parts return.Read more

The engine stop/start system on my 2016 Honda Accord, VIN [XXX], has stalled and shut off without warning in active Los Angeles traffic dozens of times since September 2025. The vehicle loses all power mid-drive with no warning — no dashboard lights, no warning sounds — leaving me unable to accelerate or respond to surrounding traffic. The condition has progressively worsened and now includes
... repeated no-start failures requiring three documented tows. The component failing is the engine stop/start system. The vehicle is available for inspection upon request. My safety and the safety of others hav been put at risk repeatedly. I am a [XXX]. My wife, age [XXX], has [XXX] and has been a passenger during stalling events. Sudden unexpected stalling in active Los Angeles traffic creates immediate foreseeable risk of rear-end collision and loss of vehicle control. This risk has been experienced dozens of times over six and a half months. The problem has been presented to Honda of Hollywood, a certified Honda dealer at 6511 Santa Monica Blvd, Los Angeles CA 90038, across five separate visits. Each time the dealer concluded no problem could be found. On one visit I was charged nearly $2,000 for spark plugs and a gasket — work with zero diagnostic connection to the stalling complaint. The vehicle was returned to me in an unsafe condition after every visit. On [XXX] the tow truck driver who responded to my third documented tow independently experienced the failure, could not start the vehicle, witnessed the complete failure sequence, and declared the vehicle unsafe to drive. He is an independent third-party witness with no stake in this case. I have video documentation of the vehicle stalling in active traffic on multiple occasions. I have voicemails from Honda of Hollywood's own service advisor documenting their diagnostic failures. The contact owns a 2021 Toyota Rav4. The contact stated that while driving on the freeway at approximately 35 MPH, another vehicle approaching from the opposite direction attempted to make a left turn and collided with the contact's vehicle. The point of impact was the front driver's side wheel well and side bumper area, with additional damage noted on the front passenger side bumper. No warning l

Both vehicles came to a stop in the roadway at the intersection of [XXX] and [XXX] in San Jose, California. The contact's vehicle remained drivable and was driven to the residence; towing was not required. The airbags did not deploy. The driver sustained injuries, including a fractured vertebra and a fractured sternum, and received medical treatment. The driver believes that the injuries may have been prevented if the airbags had deployed. The vehicle was not diagnosed or repaired. The manufacturer was not made aware of the issue. The contact related the failure to NHTSA Campaign Number 23V865000 (Air Bags). The failure mileage was unavailable. I am reporting a recurring safety issue involving sudden loss of power while driving in my 2023 Ford Escape. The vehicle has experienced multiple incidents where, while accelerating or pulling into traffic, it suddenly loses power, hesitates, or fails to respond to the accelerator. In some cases, the RPM drops unexpectedly, and the vehicle will not move or respond for several seconds. After this
... delay, the vehicle may jerk or surge forward suddenly. This creates a dangerous situation, especially when entering busy roads or intersections, as it puts me at risk of being struck by oncoming traffic. This issue has occurred multiple times and has been brought to the dealership on several occasions. The vehicle has undergone multiple repair attempts, including major engine-related repairs such as cylinder head and cooling system work, as well as a fuel system component replacement. Despite these efforts, the problem persists. On multiple visits, the dealership has stated they were unable to duplicate the issue, even though the condition continues to occur during normal driving. The vehicle has been out of service for an extended number of days due to these ongoing issues. This defect is intermittent but poses a serious safety hazard due to the unpredictable loss of power and delayed acceleration. I am concerned about the risk of an accident or injury if this issue is not properly identified and corrected. I am requesting that this issue be investigated, as it appears to be a recurring defect that may affect other vehicles of the same make and modelRead more

"The 8-speed Dual Clutch Transmission and engine (Cylinder 4) failed on my 2022 Hyundai Santa Cruz SEL Premium AWD with 37,780 miles. Both components are available for inspection upon request. While merging onto the highway in Sport Mode, the vehicle experienced violent shuddering and noise so severe I believed I had multiple flat tires simultaneously — specifically both front and rear passenger

The dealer has NOT been able to reproduce or confirm the transmission issue despite multiple documented visits over the past year. However, AutoZone confirmed diagnostic code P0304 Cylinder 4 Misfire Detected on March 19, 2026 via OBD-II scan. The vehicle has been inspected multiple times by the Hyundai dealership over the past year. Hyundai Motor America has an open case on this vehicle and has escalated it to their buyback department. The Check Engine Light illuminated during the highway incident then self-cleared. Transmission slipping, shuddering at low speeds, high RPMs at low vehicle speeds, and failure to shift properly have been reported to the dealer for over one year. Safety Risk: The vehicle suffered a sudden loss of power while driving. Without any warning light or prior symptoms, the car began "pulling". I was advised by mechanics that the vehicle was unsafe to drive due to the severity of the misfire. After two expensive spark plug repairs in six months, the engine light finally triggered during a trip, revealing that the internal defect was significantly w
... orse than a simple maintenance issue. The problem began with a sudden, violent engine hesitation. A scan revealed a Cylinder 3 misfire (P0303), but there was no initial check engine light, leaving me unaware of the danger until the car began struggling to move. Over the next six months, the spark plugs were replaced twice. Despite these repairs, the car continued to struggle with acceleration and rough idling. A final diagnosis at Palmetto Ford confirmed coolant intrusion into the cylinders (TSB 22-2322). The engine is consuming coolant internally, which fouled the spark plugs and led to a mechanical failure leading to get over $11000 on repairs for a new engine.Read more

It was towed to a local shop, who was able to confirm that the transmission has an internal short. This is a very common occurrence with this vehicle. The transmission can not be repaired and it needs a new one. Each time the transmission would fail a dash light would come on with a P in a circle with an explanation mark next to it and an engine light. The vehicle experienced significant vibration at highway speeds and during braking within the first 6 months of ownership. The issue was confirmed by a dealership, and the rotors were resurfaced as a corrective action. The problem has now returned, and the rear brakes have worn down to metal-on-metal at approximately 35,000 miles. The rear brake pads are also unevenly worn, while the front brake
... pads remain in good condition. Multiple service representatives have stated they cannot explain this level of uneven rear brake wear. The brake defect causes strong vibration that impacts steering control and vehicle stability, especially at highway speeds and while slowing down. This creates a safety concern due to reduced braking performance and loss of consistent vehicle control. The issue has been confirmed by dealerships, but the root cause has not been identified or properly resolved. No warning lights or alerts were provided prior to the failure. This vehicle is subject to Safety Recall 24V-700 (Manufacturer Recall R24B2) due to a defect in the lithium-ion battery that can cause a fire during Level 3 DC Fast Charging. Since the initial notice, I have received four separate recall communications from the manufacturer, yet as of March 2026, no permanent remedy has been provided. The manufacturer’s only "interim" solution is an instruction
... to refrain from using the Level 3 DC Fast Charging (CHAdeMO) feature. This has effectively disabled a primary advertised function of the vehicle and creates a substantial safety risk should the port be used. This defect has rendered the vehicle unsellable on the secondary market and significantly impairs its daily utility, as I am unable to use charging infrastructure at my place of employment. The manufacturer continues to delay the final remedy with "coming soon" notifications, leaving me with an unsafe and substantially devalued asset for over a year.Read more

They repaired the leaks, however this did not resolve the consumption issue. - Second Visit (65,273 miles) - The oil change done by Mobil at 75,238 miles, before I brought my car in for the third time to Hyundai for the consumption issue, documented that my oil level was at the 'Low'(L) level, this is the case each time I get an oil change around 3,000 miles - The third time bringing my car to Hyundai my mileage was at 78,913, less than 3,000 miles since my last oil change. Before making the appointment my cars engine sounded like it was running rough so we took a look at the oil level, which was at L - The first two times I did the 1000 mile oil test, Hyundai reported that my oil level remained above Full when I returned for a check, shortly after, within an additional 2000 miles, my dipstick would show L. This car is well maintained as in; regular oil, changes, transmission flushes, rear differential fluid change, tire rotation every 5-7k miles, brakes and rotors changed nearly every 30k front and back, fixing all issues as they arise. I say all of this so that there is a clear picture of the car maintenance and the issues it is having despite the proper care. This car has had several issues from about 150k miles. When I first purchased the vehicle I noticed that it would stall when I would accelerate but did not think too much of it since it was brand new. A few years passed started to notice frequent oil changes were supposed to be
... made. Another few years turbocharger went out had to get it replaced. The engine consumes so much oil at first I thought it was because the turbocharger was out so when I got it fixed I thought that would fix that issue and it didn’t. I got an oil change March 2, 2026 and on March 21, 2026 I was on the highway and the truck started to shake really bad I exited the highway went straight to the oil place they said I had no oil on the dipstick I got ANOTHER oil change (expensive) and when I turned on the car to leave the car shook again and the engine light was flickering on and off and finally stayed on the dash board. Barely made it home. Got a tow truck to tow it to Hyundai dealership they told me they will not see it unless I pay the diagnostic fee of $200 and even then they think it’s the engine with everything I described to them. They said since there are no recalls on the Tucson that Hyundai will not do anything for me I would have to pay out of pocket for everything. Had to pay the tow truck again to tow it to my house ($250). My car has all the symptoms all the other cars have that they have recalled. This is a huge safety issue because i drive on the highway a lot and everyone knows a car with no oil is extremely dangerous.Read more

When driving home on the evening of 3/24, I was on the interstate (70mph) when my 2021 Nissan Rogue started to decelerate and would not gain speed. I got off at the next frontage road exit and when at the stop sign my check engine light came on and shortly after my car started sputtering when stopped and would barely gain speed. There was no prior warning lights or signs to indicate an issue. The
... car dealership was already closed so it was taken the morning of 3/25 where a diagnostic was performed. They said that they would need to replace the Engine Assembly - Long Block and that it wasn't covered since my vin wasn't affected. I called consumer affairs and I am outside of the extended warranty by 2000 miles so they will not assist. This has been a known issue wth Nissans enginea failing, and their extended warranty isn't long enough. My engine should not have juat failed with no warning.Read more

RE: Failed Radar Braking on a 2024 Hyundai Palisade What component or system failed or malfunctioned .. . . On Feb 8th, 2026 I was driving in to Lakeland FL on I-75 N, in the right hand lane, near a green stoplight following a car 2 lengths ahead. The radar cruise control was slowing me down, keeping up with the car in front. Another driver suddenly cut in front of me from the far left lane. T
... he cruise slammed on the brakes. Within seconds he took a hard right to catch an exit ramp. The radar cruise then accelerated at an extremely high rate, and no Braking, causing me to hit the car in front of me, which was stopped about a car length away at a red light. Our air bags, 3 on driver's side, 1 on the passenger side deployed and my wife and I appeared to have no injury. The Hyundai front end crumpled in, trapping us in the car. The police pried open the doors to get us out. The car was totaled. We rented a car through our insurance and continued our 4 day journey home. On the 3rd day my right leg started turning black and blue. By the 4th day my left leg started doing the same. On returning to Stevens Point I went to the ER. The Dr. said the bruises would take several weeks to clear. And it is not unusual for the bruising to appear several days later. On March 9th both my legs developed large hematomas. The right one just above the ankle, the left just below the knee. A week after that the right hematoma developed 2 deep open wounds and I had to go for wound care involving twice daily bandage changes and then a wound vac. The Dr. says it will be a long recovery. I would have been better off without the leg airbags. Two people in the front car claimed whiplash and were taken to the hospital for observation. The police said they did not appear to be injured, but better safe than sorry. We were trapped in the car and couldn't get out till the Police got there 20 minutes later. By then the ones in the front car had been taken away.Read more

The airbag SRS sensor on the passenger side cuts on even if someone is sitting in the passenger seat. Then it will randomly cut off when no one is in the passenger seat. I’m concerned that if I was to have a wreck then someone could die. I called the Subaru dealership when I first noticed it 2 years ago. They told me to bring it up there. They checked it out and said that no codes were showing.
... However, that sensor wasn’t showing at the time. They also told me that if it did come back on to bring it back up there and then proceeded to quote me a price of $1500.Read more

Subject: Safety Concern – Excessive Automatic Braking (2025 Hyundai Tucson) I am reporting a safety concern regarding the automatic braking system (Forward Collision-Avoidance Assist and Parking Collision-Avoidance Assist) in my 2025 Hyundai Tucson. The vehicle frequently applies sudden and aggressive braking in situations where there is no immediate collision risk. This occurs during normal d
... riving and low-speed parking scenarios, including when passing nearby objects, backing out of parking spaces, or maneuvering in tight areas. The braking is abrupt enough to create a potential safety hazard, including: - Risk of being rear-ended due to unexpected deceleration - Disruption of normal traffic flow - Reduced driver confidence and control This behavior appears overly sensitive and does not align with actual collision risk in many cases. I have attempted to adjust the system settings (including warning timing), but the issue persists. Additionally, the system resets upon restarting the vehicle, preventing a consistent mitigation of the problem. I am requesting: - A review of this system behavior as a potential safety defect - A software update or calibration option to reduce braking aggressiveness - The ability to permanently adjust or disable this feature Please advise on next steps and whether this issue is known or under review. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
