I am reporting multiple safety-related issues involving my 2016 Chrysler 200 S. On 02/06/2024, after an oil change at a dealership, the underbody skid plate bolts were not reinstalled. While driving, the skid plate fell and I lost control of the vehicle, creating a serious safety hazard. No warning indicators were present prior to this failure. The issue was identified immediately after leaving th
... e dealership and is not properly documented in service records. In July 2022, following a battery replacement by the same dealership, the vehicle repeatedly lost power and stalled, leaving me stranded multiple times. I paid for diagnostic testing, but there is no documentation confirming proper diagnosis or repair. I was later advised incorrect hardware had been installed. On 02/27/2026, I was told the check engine light was on and diagnostic codes were cleared without repair. There is no documentation of the codes or repairs performed. I was also informed on 11/19/2022 that parts were ordered for a safety recall. From that date until 02/27/2026, I was repeatedly told parts were unavailable. I scheduled repair and dropped off the vehicle on 03/13/2026. I received notice that repair had begun, but later discovered the vehicle had not been serviced at all. No explanation was provided. Additionally, service records are inaccurate. A vehicle history report shows service on 03/17/2026 at a dealership I have never visited. The vehicle was not serviced anywhere on that date. These issues involve loss of control, repeated stalling, improper repairs, failure to complete a recall, and inaccurate reporting. Documentation is incomplete or missing. Supporting documents are available.Read more

I am submitting a safety complaint regarding a **2017 Kia Sorento SXL** equipped with a **3.3L Lambda V6 engine**, which experienced **sudden and catastrophic engine failure** at approximately **137,000 miles**. Prior to failure, the vehicle exhibited **progressively worsening engine knocking and vibration**, indicating internal engine damage. Despite these warning signs, the failure occurred **a
... bruptly while driving at highway speed**, resulting in a **sudden loss of power**. This incident created an **immediate safety hazard**, as I was traveling in active traffic and was forced to quickly maneuver to avoid surrounding vehicles. The unexpected loss of propulsion significantly increased the risk of a collision. The nature of the failure appears to be consistent with **internal engine component failure (such as connecting rod bearing failure)**, which is not typical for a properly maintained modern engine at this mileage. Importantly, my vehicle was manufactured in **November 2016**, placing it within an early production period for this engine. There is evidence from technical service information and consumer reports suggesting a **pattern of premature internal engine failures** in this engine family, particularly involving **rod bearing wear leading to catastrophic failure**. This raises concern that the issue may represent a **systemic defect** rather than normal wear and tear. I am concerned that other vehicles with this engine may experience similar failures **without sufficient warning**, potentially resulting in dangerous driving situations, especially at highway speeds. I am requesting that NHTSA: • Review this incident as a **potential safety-related defect** • Evaluate whether similar complaints exist involving the **3.3L Kia Lambda V6 engine** • Determine if an **investigation or recall** is warranted I am able to provide supporting documentation, including maintenance records, diagnostic findings, and repair estimates upon request.Read more

On two separate occasions, my 2023 Nissan Rogue has experienced major loss of power while driving 80 mph. The first time, all of the dash lights were blinking and there was an awful noise coming from the engine. I could not drive over 10 mph. The vehicle had to be towed 200 miles to the nearest Nissan dealer in Billings, MT. They cleared the codes, test drove it and said everything was fine. The s
... econd time my vehicle had to be towed. 115 miles from the nearest Nissan dealer. Same issues, except this time there were no dash lights. Prior to all of this happening, my car had an open recall for the 1.5L VC-Turbo engine that my car has. Since the first time this occurred and Nissan "fixed" the problem, there has been no open recall. With the second occurance, Nissan had to replace the Throttle Actuator and Gaskets according to the invoice. Luckily, I had purchased my own extended warranty and that warranty covered $1,104.20 of the $1,204.20 (I paid $100 co-pay). I was charged $715 for towing from Nissan Roadside Assistance. The invoice claims the Nissan found the following diagnostic results: DTC P0507-00 Wwhich is directly linked to a fractured internal gear in the Electric Throttle Control Actuator, the subject of NHTSA Recall 26V081 and DTC P0196 which is directly linked to engine oil temperature performance, the precursor to engine bearing seizures addressed in NHTSA Recall 26V080. Nissan is currently excluding many 2023 models from the "official" recall list. My car has 67,350 miles on it, but this failure should be covered by Nissan's 10-year/120,000-mile extended powertrain warranty. The parts replaced (throttle actuator and gaskets) and the diagnostic codes are identical to those in the 2026 expanded recall populations. Denying coverage based on an "open recall" statis is a violation of the spirit of the NHTSA safety mandates for this engine. I am formally requesting/demanding reimbursment from Nissan North America for the $815 that I have been charged.Read more

While driving on the 60 East highway (approximately 65-68 mph), the vehicle did not accelarate and became completely non-responsive. Each time I pressed on the gas pedal, the RPM needle started trembling between 600-1100, which is much lower than where it should be while the vehicle was in Drive and traveling at this speed. This was an extremely dangerous situation as I had my wife and 5 month old
... daughter in the vehicle, and other vehicles were traveling significantly faster than me. I noticed some engine vibrations, and the digital gauge cluster screen presented information on the screen as if the vehicle was "off." It instructed me to push the gear lever into Park. It then instructed me to press the brake pedal and push the iginition button. All of this occurred as the vehicle was still coasting (likely around 40 mph). I pulled over on to the shoulder to see what occurred. After coming to a complete stop the engine cut out. I was able to restart the vehicle and drove approximately 10 minutes before it occurred again on the surface streets. At that point the ignition did not catch and it sounded very much like the engine was not able to turn on at all. The check engine light turned on at this point. I called AAA for tow services shortly thereafter, and the vehicle was dropped off at the dealership. I have not received any followup from the dearlership yet.Read more

I own a 2025 Toyota Tundra SR5 non-hybrid. The vehicle has transmission hesitation/delayed throttle response after a rolling stop. When I slow down but do not come to a complete stop, then press the accelerator again, the truck sometimes has a 1–2 second delay before it responds and begins moving/accelerating. After the delay, acceleration can come in suddenly, which makes the vehicle feel unpre
... dictable and dangerous. This happens in normal driving situations such as rolling through turns, slowing for traffic, approaching intersections, or merging into traffic. The delay creates a potential safety concern because the vehicle does not respond immediately when throttle input is applied, which could increase the risk of being hit by cross traffic or not being able to accelerate as expected. I had two close calls so far where the vehicle wouldn't accelerate through a cross traffic intersection. The issue is intermittent but repeatable and has occurred at least a dozen times over my one year ownership (purchased brand-new in March 2025). There are no warning lights present on the dash. The issues started to be apparent almost immediately and has been continuing. During a 5k mile service visit at Bobby Rahal Toyota in Mechanicsburg, PA, I explained my concern to a service advisor, and they advised me that Toyota is aware of this problem but there is not fix for it. The component that failed is the coolant bypass valve, which is part of the engine cooling system. The vehicle is available for inspection upon request. The issue was first indicated by an "Engine Maintenance Required" warning message that appeared on the dashboard. No prior symptoms were noticeable before the warning appeared. The warning came on suddenly during normal driving conditions. This
... failure poses a potential safety risk because the coolant bypass valve regulates engine temperature. If it malfunctions, it may lead to improper engine cooling, overheating, reduced engine performance, or sudden breakdown while driving. This could increase the risk of being stranded in traffic or experiencing a hazardous situation on the road. The problem has been diagnosed and confirmed by an authorized Toyota dealership, which identified the coolant bypass valve as failed and in need of replacement. The dealership provided a repair estimate of $775 prior to taxes and fees. The manager shared that it would be around $890+ after it is all calculated. The vehicle has not been involved in any accident related to this issue, but the malfunction was verified through dealer inspection. No police or insurance reports were filed, as this was a mechanical failure. Based on personal research, this appears to be a widespread issue affecting similar Toyota vehicles, including other Corollas and RAV4 models, where the same component has failed under similar conditions. In some cases, repairs have been covered by the manufacturer, indicating awareness of the issue. There are many groups on Facebook and Reddit where customers with the same make and model 2022 Toyota Corolla SE have had the same issue after 3 years of having the vehicle. In summary, while driving under normal conditions, the warning message appeared without prior indication. The dealership later confirmed that the coolant bypass valve had failed and required replacement.Read more

The component that malfunctioned was the transfer case, the vehicle was already repaired so an inspection isn't possible anymore. The vehicle behaved as normal until the accelerator was pressed enough to make the front wheels lose traction, in which the vehicle couldn't send power to the rear, because the transfer case was shredded up. When needing to accelerate quickly in traffic, the vehicle wou

The dealer reproduced the problem, but initially stated that it was most likely due to the salt on the road (it was winter). As spring arrived, the problem still persisted, in which the dealer checked the transfer case and found it was shredded. They replaced the transmission and transfer case under warranty. There were no warning lights, but accelerating quickly would cause the car to spin up the front tires uncontrollably. In corners, this meant severe understeer that could be dangerous in the wrong circumstances.Read more

I am filing a safety complaint regarding a 2022 Forest River Salem Cruise Lite travel trailer, purchased July 21, 2022. At the time of sale, the unit had an open federal safety recall (NHTSA Recall 22V-243) involving a fire/explosion hazard. The recall was not remedied prior to sale. The vehicle was presented for service multiple times, and the recall was not identified or addressed until May 20
... 25. At that time, the manufacturer directed that the recall repair be completed; however, the issue has not been resolved. The manufacturer’s owner portal continues to show the recall as open, despite NHTSA’s system not showing an unrepaired recall for this VIN. The vehicle has been out of service for an extended period and is currently unusable. Request: Investigation into failure to remedy a safety recall within a reasonable time, potential reporting discrepancies regarding recall status, and the sale of a vehicle with an open safety recall.Read more

Hello… this has happened at least four times.. the car goes to stand by/idle mode when the auto shut off is engaged. I was at a light and car went to idle and when it goes to green. Car just goes to idle mode. I had to put hazards on and restart car. People honking while I’m trying to restart car from this state. It’s like the car does not return on after the gas is engaged.
