The vehicle is a 2026 Hyundai Palisade equipped with second- and third-row power seats subject to Recall #296. The power seat system may fail to detect contact with an occupant or object during operation, including folding and tilt functions. This creates a safety risk because the seat can continue moving despite contact with a person, increasing the risk of injury, particularly to children in th
... e rear seating area. This defect has been associated with a reported child fatality. The issue is known and confirmed by the manufacturer through the recall. I became aware of the condition in early March 2026. There were no prior warning messages or indicators before learning of the defect. At this time, no repair or remedy is available and no timeline for correction has been provided. The vehicle remains in service with this unresolved safety defect. Hyundai has been engaged in delay tactics rather than customer service. The condition has not been independently repaired or corrected. The vehicle is available for inspection upon request.Read more

My 2015 Hyundai Tucson with the 2.4L engine developed a loud engine knocking sound while driving. The knocking became worse and the vehicle became unsafe to drive due to the possibility of sudden engine failure. The vehicle is currently at a Hyundai dealership and has been there for over a month. The dealership denied the warranty claim stating that a different engine component failed instead of
... the connecting rod bearing. This engine is part of the known Theta II engine defect issue that has caused engine failures in many Hyundai vehicles. A sudden engine failure while driving could result in loss of power, difficulty steering or braking, and could increase the risk of a crash. I am filing this complaint because the engine failure symptoms match the known defect pattern, and the vehicle may be unsafe if the issue is not properly addressed. Hyundai Tucson with 113,000 miles and failing engine. I’ve been told repairs have amounted to $10,000. I was told my car has severe engine failure and must be replaced by Hyundai of the shoals in Tuscumbia, AL. Engine failure is dangerous as a fire may start from a metal piece and also loss of control of vehicle is possible. Hyundai has denied warranty as they say the exact part in theta 2 engine settlement is not failing. I have had to complete recalls on this car since the first payment I made on it. The engine knocking began a month ago and that’s about how long the dealership has had my car.Read more

I am filing a complaint regarding an unresolved and ongoing issue with my 2021 Hyundai Elantra N Line. The vehicle currently has approximately 80,100 miles. This issue began when the vehicle had under 60,000 miles. At that time, I brought the vehicle to a Hyundai dealership because the check engine light was on and I was informed it was related to the EVAP system, specifically the purge valve. I
... spent approximately five hours at the dealership and was told the issue had been repaired. However, shortly after leaving the dealership, the check engine light returned with the same issue. Since then, I have attempted multiple times to resolve this problem. I even purchased a new purge valve and had it installed by an independent mechanic. Despite replacing the purge valve, the check engine light remains on and continues to report EVAP system issues. I also had a mechanic perform a smoke test on the system, which did not reveal any leaks in the EVAP system. The mechanic stated they could not determine why the system continues to show a purge valve or EVAP-related code and suggested that only Hyundai may have the tools necessary to properly diagnose the issue. I contacted Hyundai corporate customer service for assistance, but they informed me that each dealership is privately owned and they could not intervene or assist further. This issue has now been ongoing for approximately two years. I continue to experience the check engine light and now the vehicle is beginning to have drivability problems including vibration and jerking during acceleration. This unresolved issue also affects my ability to pass vehicle inspections if the check engine light remains on. I am requesting assistance in resolving this ongoing issue and receiving proper diagnosis and repair support from Hyundai so that the vehicle can be repaired correctly.Read more

Vehicle: 2015 Subaru Forester Mileage at Failure: 85,000 miles Component: Powertrain – CVT Transmission My 2015 Subaru Forester is experiencing a premature safety‑related CVT transmission failure at approximately 85,000 miles. The vehicle developed a pronounced whining noise that increases with vehicle speed, consistent with internal CVT bearing or pulley failure. This is a known defect patter
... n in Subaru Forester CVTs and is not related to maintenance or wear. The noise is continually progressing and the risk of sudden catastrophic failure makes the vehicle unsafe to operate. An authorized Subaru dealer and the District Parts & Service Manager (DPSM) both confirmed that the CVT has failed and requires full replacement. This failure occurred well within Subaru’s extended CVT warranty mileage limit of 100,000 miles, and matches the defect pattern Subaru acknowledged when it extended CVT coverage nationwide for this model. However, Subaru is refusing to replace the CVT due to the 10 calendar year extension expiration. Subaru’s maintenance schedule for this vehicle does not require any CVT fluid replacement interval, so there is no missed maintenance that contributed to the failure. A failing CVT creates a safety hazard due to to the risk of sudden loss of power, hesitation, or inability to accelerate. I am filing this complaint so NHTSA is aware of this premature CVT failure and the associated safety concerns of SOA's refusal to replace the CVT under their extended warranty.Read more

On January 1, 2025, I discovered my vehicle had been stolen after finding broken glass in my driveway and immediately reported the incident to the police, who arrived within approximately 20 minutes; neighbors later provided camera footage of the theft, and I also observed other nearby vehicles with similar damage, including a broken rear window and another Hyundai Elantra that appeared abandoned.
... The following day, police informed me the vehicle had been recovered, and upon inspection I found the rear driver-side window completely broken and the ignition area under the steering wheel damaged. Couple months later I received a recall notice for the antitheft system on my 2020 Hyundai Elantra (VIN [XXX] ). When I brought the vehicle to the dealership on April 1, 2026, I was informed that the recall cannot be completed unless an additional part is replaced, at a cost of approximately $1,178. This part was damaged during a prior theft incident. My vehicle was stolen last year and later recovered, and it sustained damage during that incident. However, I understand that safety recalls are required to be repaired at no cost to the owner under federal law. I am concerned that I am being asked to pay for repairs in order to complete a safety recall, and that my vehicle remains vulnerable without the recall remedy being performed. I am also concerned about the safety and legal compliance of continuing to operate the vehicle under these conditions. I am attaching supporting documents as proof of the vehicle theft, along with the reports provided by the dealer. I experienced an unexpected Loss of Motive Power (LOMP) event. Late afternoon traffic was fast, estimate 75-80 MPH. I attempted to change lanes get to the righthand shoulder and was very nearly involved in a high-speed collision with a semi-petroleum tank truck. He applied his brakes to avoid hitting me and started to jackknife, but regained control and continued. The truck missed by inches.
... At this point I had NO remaining power and was essentially coasting until I managed to cross the remaining two lanes of traffic to the shoulder emergency lane. I reset the error codes and continued home with no further occurrences. The incident produced the following warning lamps: Electronic Throttle Control Failure, Start/Stop System Failure and Check Engine, then a few seconds later the Limp Mode/LOMP event. I carry an OBD Scanning Tool with me to be able to clear the error codes when I encounter them. They are typically a cascading event that can result in LOMP. I have experienced a total of 26 of these error lamp events since 4/19/22 which have resulted in 3 LOMP's. The remaining 23 events I was able to reset before the inevitable LOMP. This is the first LOMP that nearly caused a fatal accident, the remainder I was able to safely find a place to stop to perform the resets. There is an active Manufacturer Recall Number93C, NHTSA Recall Number25V-586 that may cover this issue. The Recall Status is "Incomplete, remedy not yet available." The Alfa Romeo dealership has noted the P0191-00 error and replaced the fuel pressure sensor on 2/23/23, but this did not solve the problem. Owner's forums, and now the Manufacturer/NHTSA recall have reported this as a known problem, so I have been clearing my own error codes in order to at least have a car to drive. In my mind, this nuisance has now become a potentially very dangerous condition.Read more

Based on everything we've documented, here's a tight, factual account written in the style NHTSA expects — first person, chronological, no fluff: "I own a 2016 Honda Pilot Touring AWD, VIN [XXX] , with 89,962 miles. The vehicle began producing a clunking noise from the engine. I brought it to Honda of Tomball [XXX] ), where the dealer diagnosed all main rod bearings as damaged and recommended
... full engine replacement. This failure is identical to the defect described in NHTSA Safety Recall 23V-751 (Honda Recall XG1/GG0): premature connecting rod bearing wear and seizure caused by improperly ground crankshaft crank pins. My vehicle is a 2016 Honda Pilot, a model year explicitly covered under the recall. However, my VIN was not included in the identified recall population, and Honda of Tomball has not offered to cover the repair under the recall. I am requesting that NHTSA investigate whether my vehicle's crankshaft contains the same manufacturing defect, and that Honda be required to extend recall coverage to my VIN. The repair cost for a full engine replacement is substantial, and the failure mode is mechanically indistinguishable from the acknowledged recall defect INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While driving my 2021 Cadillac Escalade on an interstate highway at approximately 70 mph with my family, including two minor children, the vehicle suddenly and without warning lost propulsion, shut off, and shifted into neutral. This occurred in active traffic and created an immediate safety hazard. A warning message appeared instructing me to restart the vehicle. After several minutes navigating
... traffic, I was able to restart the vehicle briefly and exit the highway. Shortly thereafter, additional dashboard alerts appeared indicating critical engine and transmission failure. After exiting the highway, the vehicle shut down completely at a traffic signal and failed to restart. The vehicle was immobilized in an active roadway until police assistance arrived to prevent a rear‑end collision and a flatbed tow was arranged. The sudden loss of propulsion at highway speed placed vehicle occupants and surrounding motorists at serious risk. The vehicle is available for inspection and was diagnosed as having a seized engine on March 9, 2026, by Diamond Cadillac in [XXX] . This failure did not occur without warning. Prior to this incident, I repeatedly reported excessive oil consumption to the selling dealer, Otto Cadillac, from whom I purchased the vehicle in April 2025 as a Certified Pre-Owned vehicle. On two separate service visits, I reported oil consumption exceeding two quarts between recommended service intervals and requested that this be documented. I was advised that the consumption was “within tolerance” and was later informed—incorrectly—that this tolerance applied to my engine configuration. Otto Cadillac performed a recall-related inspection and implemented the manufacturer-directed remedy consisting of an oil cap replacement and a change in oil viscosity to SAE 40. No mechanical repair or deeper diagnostic action was taken. The subsequent engine seizure confirms that these mitigation measures did not resolve the underlying defect.
