I am submitting a formal safety complaint regarding my 2021 Ford F-150 Lariat 5.0L V8, which is included in recall 25V455 (Ford campaign 25S75) involving low-pressure fuel pump failure that may cause engine stall while driving. Ford announced this recall in 2025, acknowledging a potential safety-related defect that can result in loss of propulsion. However, as of April 2026, no repair remedy has
... been made available for my vehicle. This extended delay leaves owners operating vehicles with a known safety defect and no practical means to correct it. An unexpected engine stall while driving creates a serious hazard, particularly in highway traffic, intersections, merging conditions, or when towing. Owners should not be left indefinitely without a repair after a manufacturer has identified a defect significant enough to warrant a federal recall. I respectfully request that NHTSA review the prolonged delay in remedy availability for recall 25V455 and require timely corrective action, clear owner communication, and appropriate interim measures where necessary. Please also review whether affected owners are receiving adequate notice regarding expected repair timing and risk mitigation steps while awaiting a permanent fix.Read more

I took my car in to Tomball Hyundai due to intermittent failure of the headlight switch and the wiper switch when both of these features are set to the ‘Auto’ setting. These are very intermittent issues, but as luck would have it, it was sprinkling on my drive to the dealership on [XXX] and I was able to capture the wiper issue via video on my phone which I shared with the service advisor. The
... n, fortunately, 2 days later, the service technician was able to verify the intermittent headlight failure. I was aware of a TSB (TXXH) which is meant to address this problem, however, Hyundai issued a superseded TSB a few months after the original, in which they deleted the wiper switch from the service. The 2015 Genesis multifunction switch is sold as a single integrated unit (P/N 93400-3M031), but Hyundai’s TSB 20-BE-009H-1 mandates a "hack" repair that leaves the failing wiper switch in place. It is my assertion that when Hyundai settled a class action lawsuit regarding this switch, they agreed to extend the warranty on the entirety of the Multifunction Switch Assembly (NOT just the Headlight portion of the switch) for 15 years and Unlimited mileage. The Hyundai National Customer Service Case Manager (Roobika, 714-912-2118 National Consumer Affairs) told me that my request of having the complete switch replaced was denied due to the vehicle being out of warranty and having 73,481 miles. *to be clear here - Hyundai HAS agreed to replace the Headlight switch only - their denial centers on the Wiper switch. Eager to find a solution, I suggested that I could cost-share. This was declined as well. I was only given the option of buying the whole switch myself and allowing them to provide the labor to install it. Furthermore, It is technically illogical to perform a warranty repair by disassembling an integrated unit to replace one half while ignoring the failure of the other half—especially when the labor required to access the part is identical. To Whom It May Concern, I am submitting this formal complaint regarding a malfunction in the air conditioning system of my 2024 Chevrolet Trax. Specifically, the evaporator and/or condenser has failed, resulting in inadequate cooling performance and potential system leakage. This issue is more than a matter of comfort—it presents a significant safety concern. Operating the vehicle in high temper
... atures, particularly in Texas, creates hazardous conditions inside the cabin. Excessive heat can lead to driver fatigue, dehydration, and reduced concentration. Additionally, the malfunctioning A/C system affects humidity control, which may cause windshield fogging and reduced visibility while driving. The problem has been consistently reproducible during normal vehicle operation. Symptoms include poor cooling performance, inconsistent air temperature, and possible unusual odors or sounds when the A/C system is in use. Notably, no warning lights or dashboard messages appeared prior to the failure, making it difficult to detect the issue early. At this time, the vehicle is available for inspection and can be evaluated by a dealership, certified technician, or manufacturer representative upon request. I request a thorough inspection to confirm the failure and determine whether this is due to a manufacturing or design defect. If necessary, I am also willing to have the vehicle reviewed by independent service professionals or relevant authorities. Given that this issue has occurred in a nearly new vehicle, it raises concerns about reliability, quality control, and potential widespread defects affecting other vehicles of the same model. I urge prompt investigation and corrective action to ensure driver safety and product integrity. Please advise on the next steps for inspection, repair, and resolution of this matter. I appreciate your prompt attention to this serious issue. The factory-applied clearcoat on my 2017 Chevrolet Tahoe is failing and delaminating. The clearcoat is peeling away from the base coat in sheets, exposing the underlying paint to the elements. The affected
... areas include roof and door panels. The failure began as clouding and hazing of the clearcoat and has progressed to active peeling and flaking. This issue appears to be a widespread manufacturing defect. Multiple class action lawsuits have been filed against General Motors alleging defective paint and clearcoat application on 2015–2020 full-size SUVs produced at the Arlington Assembly Plant [XXX] et al. v. General Motors LLC, Case No. [XXX], and [XXX] et al. v. General Motors LLC). Thousands of owners of Tahoes, Suburbans, Yukons, and Escalades from these model years have reported identical clearcoat failure. The alleged root cause is improper bonding between the clearcoat and base coat during the factory paint process. Repair estimates for a full repaint range from $3,600 to over $10,000. General Motors has not issued a recall or technical service bulletin to address this known defect. I was driving at 60mph. I heard a sudden pop that felt like it came from behind me / under the car. While I glanced to see if I ran over something (I did not), the car suddenly felt like it was about to stall. It was slowing down quickly and dramatically. My safety was in danger as I quickly dropped speeds in heavy traffic on a freeway without warning. I was able to maintain control, and I turned
... my hazards on. I noticed that I was maintaining speed around 35-40 mph (no matter how heavy I hit the accelerator), and I moved to the right lane and continued while wondering if my car would survive all the way home. Luckily I was only about 1 mile from my house at that point, so I proceeded at a low rate of speed and ultimately made it home. It was scary, sudden, and put me and others on the road in danger. A couple of warnings popped up saying "Electrical System issue" and something else that was related to electronics. The vehicle was towed to the dealership that night, and diagnostics and repairs began the following day. The problem has been confirmed by the dealer to be an ICCU failure that needs to be replaced. There were zero warnings prior to the failure--the car was working perfectly up until that moment. Anything is available for inspection, but as I mentioned it's being replaced as we speak so the broken / malfunctioning part wouldn't be there anymore if someone looked at it.Read more

Presumably, because the sale occurred on the exact date Recall #283 was issued, we were excluded from the manufacturer's October safety mailing. We unknowingly operated the vehicle for seven months without knowledge of the seatbelt defect, though we did receive subsequent notices for campaigns 9C9, 9C8, and Recall #292 (Ejection Mitigation). Combined, the active status of Recall #283 and Recall #292 leaves the vehicle with zero safe seating positions. Furthermore, internal records show the vehicle failed a computer diagnostic test ('NG' code) with a severely depleted 11.90V battery prior to purchase. The CPO sale was executed without resolving this documented fault. This correlates to a severe electrical issue where the parked vehicle spontaneously unlocks. (I also possess a video from the Hyundai technician confirming the battery issue). "Final Evidentiary Summary" - Chronological timeline of the transaction. "Palisade Inspection Form" - August 8, 2025, internal document recording the battery failure and outdated recall checks.
