I am writing to formally report a significant mechanical issue involving the water pump in my 2018 Ford Explorer with the 3.5L V6 engine. This vehicle recently experienced a sudden failure of the internal water pump, a problem I have since learned is common across this model and others using the same engine design. In my case, the water pump failure occurred just outside of the vehicle’s extende
... d warranty coverage, leading to engine overheating and a substantial repair cost. What makes this issue particularly concerning is the design flaw in which the water pump is located internally, driven by the timing chain. When the pump fails, it can leak coolant internally, potentially leading to catastrophic engine failure without warning. This design not only makes the part extremely costly to repair (often $2,000–$4,000 or more), but it also introduces a serious safety risk—drivers may experience engine overheating or seizure while on the road, with minimal to no warning. This poses a clear hazard, especially at highway speeds or in high-traffic situations. I understand that many other Ford owners have reported similar failures, yet no recall or Customer Satisfaction Program (CSP) has been issued to address the problem. Given the frequency of these incidents and the potential danger involved, I respectfully request that NHTSA investigate whether this condition qualifies as a safety-related defect. I have included my vehicle details below for your reference: Make/Model/Year: 2018 Ford Explorer Engine: 3.5L V6 (non-EcoBoost) Mileage at Failure: 101345 Failure Description: Internal water pump failure leading to coolant leak and overheating Repair Estimate: $4511.19Read more

My name is [XXX] ,i am filing this report on behalf of My daughter [XXX] the driver of the vehicle that involved with the unintended acceleration on the 2009 Toyota Prius .On a good clear morning on [XXX] . My daughter drove the 2009 Toyota Prius to work- when she enter to the parking lot at the speed round 5 mile/hour . Just when she was making a left turn on to the entrance gate with her
... left foot on the brake pedal . On a sudden the vehicle RPM unintended rev up too high and the vehicle had an unintended accelerating to move forward aggressively causing lose control of the steering . Also she could not brake even though her left foot was on the brake pedal and pressing hard to try to stop the vehicle . . This dangerous incident causing her Prius to bumped into the electrical gate panel control box . The accident caused some damages to her Prius and to the entrance gate control box though no body was hurt . But it got us through the terrible experience because it could kill some one for sure . My daughter been driving this commute car daily in the past 5 years and she has the skill of 20 year driving with good record . This Prius of her still have all OEM equipments with no add- on parts-,no modifying part on any systems what so ever- and NO extra floor mat cause we been warned before about the possible of unintended acceleration causing by aftermarket floor mat to any vehicles. . We feel unsafe after the mystery accelerating problem on Toyota Vehicle that could take people lives . So the mystery unintended accelerating on car is REAL . I am currently checked and find out her Prius has 2 incomplete recalls (un-notice to us ) but none of them related to the unintended acceleration problem . We would like the NHTSA to take this problem seriously to save people life . Hi, my name is [XXX] and I leased a 2024 Hyundai Palisade Calligraphy last summer. I have been having issues with my car skidding on rough or uneven pavement and I brought the car in for service to check the issue. I was told by the dealership that my issue is "normal" and it's how the ABS system works. This is obviously ridiculous because no car is supposed to skid on regular roads with no snow
... or ice. The car skids and the wheel shakes and my brakes do not work for a few seconds. I have to basically slam on the brakes and pray the car stops. I told the dealership it is definitely not normal. They said they would check the car and call me. I got a call a couple hours later telling me the ABS system is working fine. When I googled the issue, I saw many, many complaints about this issue and a class action lawsuit that has been filed. I picked up my car and asked the representative to speak to a manager. He said they all leave at 4:00. It was 4:30. I asked him how Hyundai is ok with sending people like me back out on the road knowing the ABS system is faulty and causes loss of brakes? He replied "Until Hyundai issues a recall, there is nothing we can do." I have owned Hyundai cars in the past and have always admired them as a company. They offer great safety and great warranties. But now I ask why they are ignoring this obvious safety issue? What will happen when a person driving a Palisade loses brakes and God forbid hits a car or a person? Please make this right and fix these cars so that we can feel safe on the road. My children drive my car, though I won't be allowing that anymore. Please advise on next steps. My 2007 Toyota Tacoma has an open recall on the rear leaf springs due to the risk of fuel tank puncture and fire. However, Toyota and their authorized dealership are unable to perform this safety recall because the vehicle’s frame is too severely rusted. The dealership informed me that attempting the repair could further damage the vehicle, and they requested I sign a waiver releasing Toyota of
... liability if harm occurs during the recall process. They also informed me that replacement frames are no longer available, so the recall cannot be completed as I was interested to pay for this to get the recall safely completed This leaves me with a vehicle that presents a known, life-threatening safety hazard, which Toyota cannot resolve. The frame corrosion is due to a previously acknowledged defect for which Toyota offered limited-time frame replacements that expired before I owned the vehicle. I believe Toyota is violating its obligation to provide a remedy for this open recall. I am requesting that NHTSA investigate this issue, as it appears Toyota is leaving customers with dangerous, unrepaired vehicles and no available remedy.Read more

Automatic parking assist and reverse braking systems exhibit severe, recurring malfunctions causing collisions and injuries. On May 24, 2025, at approx. 4:10 PM, auto-park uncommanded the vehicle to strike a mailbox in a familiar spot. Dealer dismissed incident without diagnostics. Beyond this collision, the vehicle exhibits frequent, violent, unintended braking during reverse. This occurs when:
... Turning body to look back: System abruptly locks brakes, causing whiplash-like neck pain to me. Dealer stated this was "normal" for "driver seating" despite explanation. Encountering minor obstacles: System misidentifies leaves/garbage bins as threats, engaging full, hard braking. This causes extreme distress and has violently thrown passengers forward. My teenager son sustained a visible bruise for one week after hitting the console during one such incident March 2024 These system malfunctions create high risk of injury to occupants and pedestrians. The unpredictable braking events are a fundamental flaw. These issues were reported to the dealer within weeks of purchase and to VW Group of America Customer Care (800-822-8987) after the mailbox incident. They remain unresolved."Read more

I am filing a formal complaint regarding a known paint defect on my 2012 Toyota Sequoia, painted in Blizzard Pearl (paint code 070). The paint is peeling and delaminating from multiple areas of the vehicle, including the roof, hood, and rear panels, down to the primer and in some cases bare metal. This is not a cosmetic or age-related issue — it is the result of a manufacturing defect that Toyot
... a has already acknowledged in multiple models and years through Customer Support Program ZKG. This issue affects vehicles painted in Blizzard Pearl and Super White, due to poor adhesion between the basecoat and primer. It is a systemic defect that poses a long-term corrosion and safety concern, particularly as bare metal becomes exposed and vulnerable to rust. Toyota refuses to take responsibility, citing expiration of the CSP in December 2022. However, this defect is not time-sensitive—it emerges gradually and unpredictably over the life of the vehicle. It is unfair to place the burden of repair on consumers when the issue is clearly the result of poor materials and workmanship. I have contacted Toyota and provided photographic evidence. They acknowledged the vehicle is affected by the exact problem covered by the CSP, but declined to offer assistance because the program has technically ended. This is unacceptable, as the issue is part of a known national defect, not an isolated case. There are thousands of documented complaints for this exact same issue on forums, social media, and Toyota owner sites. I am requesting that Toyota: •Acknowledge this as an ongoing manufacturing defect regardless of warranty expiration, •Provide goodwill repair coverage or reimbursement for repainting, •Prevent further corrosion that could compromise vehicle safety and resale value. I am submitting this complaint so the issue can be formally documented and investigated, with the hope that Toyota will be held accountable and other owners will not be left to absorb theRead more

I then called Honda customer service (800-999-1009) support on June 23, 2025 at 1:17pm. I told the representative that the 2024 owners manual on page 84-85 did not have an amber brake indicator message among the 30 or so indicators in the manual. On page 493, the manual also mentions "If the brake system indicator (RED) comes on or blinks, there is a malfunction in the brake system" On page 494, the manual also mentions "if the brake system indicator (RED) comes on or blinks at the same time when the brake system indicator (AMBER), release the parking brake. The representative could not explain what a blinking amber brake system indicator meant and I should return car to the Honda dealer. Upon my insistence, my call was escalated and that a senior representative agreed that an blinking amber brake indicator explanation is not in the Owners Manual and could not provide me with an answer. I am reporting a safety defect related to the Vehicle Connectivity Module (VCM) in my 2017 Volvo XC90. The VCM is critical for emergency communications, including automatic crash notification and location sharing with emergency responders. This function is vital for occupant safety in the event of an accident. My VCM was serviced at an authorized Volvo dealer in Natick, MA in December 2024 due to
... intermittent connectivity issues. Despite this recent service, the VCM completely failed in mid-April 2025. When the module fails, the vehicle loses the ability to automatically contact emergency services and transmit its location after a crash, significantly increasing the risk of delayed or absent emergency response. I am aware that Volvo previously issued Safety Recall R39917 for 2017–2019 XC90s regarding VCM software errors that could prevent GPS location from being sent to emergency personnel. Volvo has refused to cover the repair, even though the failure directly impacts a safety-critical system that was already subject to a recall. This failure compromises the car’s core safety promise and leaves occupants at risk in emergencies. I urge NHTSA to investigate ongoing VCM failures in these vehicles, as the repairs have not resolved all safety concerns. Volvo should be required to address these failures to ensure driver and passenger safety.Read more

The MMI (multi-media interface) frequently reboots during driving, resulting in a black screen and the inoperability of key systems such as the backup camera, rear obstacle detection, navigation, and cell phone tethering (for hands-free operation). These reboots require user input, including entering a PIN to log into the user account (if set up) and repeatedly tapping on the MMI, which can be hig
... hly distracting while driving. By its very nature, the reboot catches the driver by surprise and requires diverting attention from driving.Typically, this user input is provided while stationary, immediately after starting the vehicle. This issue constitutes a safety concern, as reboots can occur during reverse parking maneuvers, rendering the backup camera and obstacle detection sensors inoperable until the system reloads. Additionally, reboots completely disable the navigation screens and necessitate multiple user inputs to restore functionality, which can be hazardous while driving. Many states have enacted hands-free laws and requirements that mandate the use of dash-mounted systems to comply with these regulations. The reboots render such systems inoperative. Numerous reports on online user forums suggest that this issue is widespread and affects the same model year vehicles. For instance, see [XXX] . Several Technical Service Bulletins (TSBs) filed with the National Highway Traffic Safety Administration (NHTSA) indicate that available software updates should be applied to address infotainment-related issues. Relevant TSB numbers include: TSB 2067021, TSB 2076404, and TSB 2069112. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

We ordered a car from Tesla on April 19th and received an email on May 1st stating the vehicle we had order had arrived, ready for delivery. We set up delivery for [XXX]. Picked the car up that day. On May 9th, we noticed that the “as configured” options on the Monroney sticker didn’t match the actual installed features on the vehicle. It was missing the Full Self Driving that was listed on
... the Monroney sticker (that we had not ordered). I immediately contacted Tesla and was told by Corporate sales support in California that it would be corrected since it was on the sticker at the time of delivery, it has to be included with the VIN purchased. Trusting them enough not to get the name of the gentleman I spoke with was my mistake. I have contacted Tesla corporate customer service, Tesla corporate sales support and the Council bluffs delivery center after I noticed the discrepancy between what was actually on the vehicle and what the Monroney disclosure stated. I have requested the full self driving capability that is listed on the Monroney sticker with the VIN attached to the car I purchased be added to the vehicle to match their disclosure under the “as configured” section of the sticker. I was told by corporate sales support in California that it would be added to the vehicle because of their mistake. I noticed it had been added to the wrong vehicle (we have 2 teslas). I contacted customer service and they directed me to council, bluffs. Preston Sherwood told me he suggested leaving it on the wrong vehicle, just cancel the monthly subscription and subscribe on the correct vehicle. So I did just that and my paid subscription expired yesterday on the wrong vehicle and the option went away. I contacted customer service and they said they would make it right, but it has to go through “sales support” then promised they would call me. No one did. The feature still isn’t on the vehicle as listed on the only physical legal document we were provided at delivery. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more
