NHTSA Complaint SummaryVehicle: 2017 Ford Escape SEEngine: 1.5L EcoBoostVIN: [XXX] Defect: Engine Block Design Flaw / Coolant IntrusionTechnical Description of the Issue:My vehicle is experiencing a well-documented manufacturing defect specific to the 1.5L EcoBoost engine. The engine block was designed with a "v-groove" between the cylinders that is prone to structural failure. This allows engine
... coolant to leak into the combustion chambers (coolant intrusion) and allows engine oil to contaminate the cooling system. My vehicle currently exhibits "milky," tan-brown sludge in the coolant reservoir, confirming that oil and coolant are cross-contaminating due to internal engine failure.Extreme Safety Hazards and Dangers:This defect creates several critical safety risks for the driver, passengers, and other motorists:Sudden Loss of Power at High Speeds: The mixing of oil and coolant destroys the engine’s ability to lubricate internal bearings. This can lead to a catastrophic engine seizure while the vehicle is in motion, causing an immediate loss of power and power steering, which creates a high risk of collisions on highways.Engine Fire Risk: When coolant enters the combustion chamber, it can cause severe engine misfires and overheating. Overheated oil or pressurized coolant spraying onto a hot exhaust manifold during a mechanical failure presents a significant fire hazard.Hydro-locking: If coolant floods the cylinder while the vehicle is in operation, the engine can hydro-lock, causing the engine to stall instantly and potentially throwing metal debris onto the roadway, endangering other drivers.Visibility Hazards: Excessive white smoke from the exhaust (caused by burning coolant) can suddenly obscure the driver’s rear-view visibility and the visibility of trailing motorists. Conclusion:Ford Motor Company was aware of this defect, as evidenced by Customer Satisfaction Program 21N08 and TSB 19-2346. However, because these programs have arbitrary expira INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (Read more

When slowing down in traffic or approaching a stop sign vehicle will stall then turn off. This will than cause the vehicle to abruptly stop. Emergency brake is then engaged. It takes 1-3 times to engage vehicle to restart using the push start button. This happens with in 2-4 blocks of a cold start or even after 15 minutes or more of driving. This vehicle has been in the Dodge dealer several time

When vehicle stalls it puts emergency brake on and alternator light comes on. After vehicle is re started you must pull over and turn vehicle off for few minutes to get alternator light to turn off. Volume of gas in tank seems to have NO bearing on when this vehicle stalls. This vehicle stalling happens more frequently when outside temperature is 50° and above. I have only had the vehicle 1 year and 4 months and I am already having to replace the wiper blades a for a second time. The fact that Chevrolet decided to put a wiper blades arm that no one makes adapters for except Chevrolet and the fact that I can’t just walk into an auto parts store and buy new blades is absolutely infuriating.

I have a Maverick (24 AWD XLT) and this issue has occurred twice. It happened on my vehicle the first time as I was trying to turn left, up an incline, with oncoming traffic - loss of acceleration. There was simultaneously a service light and power train malfunction message that went to my ford app. The vehicle had 5k miles on it. I took it immediate to the dealer. After testing they said the conn
... ection at the accelerator was loose (pigtail) and replaced. I just had the same failure occur yesterday (~22k miles) on my way home from work, except I lost acceleration on an inclined on-ramp trying to merge onto the highway. I had just slowed on the ramp to let a semi pass, and am thankful I didn’t try to get ahead of it. It wouldn’t have ended well. Still, I could see traffic jamming behind me, and folks were struggling to get on the highway because of my significantly reduced speed. Losing control of acceleration at a point where it is critical is very dangerous. I’ve never experienced a vehicle failure that felt like it risked my life. For this to occur twice in key moments of acceleration, with less than 25k miles is unsettling. Online there are many similar reports, not sure how this isn’t a recall yet.Read more

On April 13, 2026, I arrived home and parked the vehicle, engaging the traditional shifter into the "Park" position and turning off the engine. After exiting and standing on the ground to retrieve a purse from the cabin, the vehicle spontaneously and rapidly began reversing. As the vehicle gained momentum, I was struck by the driver-side door, which knocked me down and resulted in the vehicle
... running over me. The vehicle only stopped its rearward movement once it collided with a neighbor's parked car. Only after the vehicle began moving did the horn begin blaring, indicating a delayed rollaway alert. This incident resulted in a fractured ankle that required surgical intervention. Despite the traditional shifter being placed in "Park" and the engine being off, the vehicle failed to remain stationary, representing a catastrophic failure of the parking pawl or transmission engagement.Read more

While idling, the engine shuts off completely and requires the vehicle to be shifted into park and restarted before operation can continue. On multiple occasions, the engine has struggled to restart and cranked for several seconds before turning over. Honda of Tomball in Tomball, TX currently has it. This defect creates a significant safety hazard because the vehicle loses power unexpectedly while
... in traffic. The vehicle is immobilized until it can be shifted into Park and restarted, leaving us vulnerable to collisions or other accidents, particularly in busy intersections or congested roadways, and creates a serious safety risk to our family, children, and our daughter’s service dog. The vehicle has been presented to HOT multiple times for diagnosis and repair. This defect continues to happen. HOT has inspected this during multiple repair visits. To our knowledge, the manufacturer has not directly inspected the vehicle. (I have requested it with both Honda of Tomball and Honda of America.) No warning lamps or dashboard messages appeared prior to the stalling events. The failure occurs without warning while the vehicle is stopped in Drive. The primary symptom is sudden engine shutdown, followed by difficulty restarting in some instances. This issue began shortly after delivery of the vehicle and has continued intermittently. First documented time of problem was September 2025. Most recent is May 2026.Read more

Timeline Summary – 2022 Hyundai Palisade Safety Defects I first reported seatbelt buckles coming unlatched during normal use in early 2025. The issue was documented by the dealership on March 28th, 2025. A recall was later issued confirming a defect in the seatbelt buckle assemblies that may prevent proper latching and increase the risk of injury in a crash. From late 2025 through April 2026,
... I was repeatedly informed by Hyundai and dealership representatives that parts were unavailable, on backorder, and that no repair timeline existed, despite being told parts had been ordered as early as November/December. On April 2, 2026, I was again told there were no parts available and no estimated timeline for repair. After submitting a formal lemon law demand on March 29, 2026 and escalating the matter, Hyundai confirmed in writing that parts were limited and no defined repair timeline existed. Only after over a year of delay was I later informed that a part may be available. Additionally, there is another active safety recall affecting the vehicle for which no repair is currently available. This vehicle has had unresolved safety defects for over a year, involving critical systems, without a reasonable repair timeframe.Read more

Received a safety recall stating "Seat Back Reclinging Assembly May Not Securely Lock". Based on this information, it goes on to state that "a seat back that has not been secured in a locked position may fail to properly restrain occupants, increasing the risk of injury in the event of a crash at certain speeds." Currently there are no remedies available but it is advised that "no one sit in or us

In speaking to headquarters as well as mutliple dealerships, the only "solution" is to provide a rental car in the amount of $42/day and they would reimburse in 6-8 weeks. There is no current remedy date. That would mean there is an out of pocket cost of $1200+ a month, in which these funds are not available to our family. The panoramic sunroof glass suddenly shattered while the vehicle was being driven on the interstate, with no visible impact, object strike, or prior damage observed. This also created a significant safety risk due to driver distraction, panic, and the potential for more serious injury while the vehicle was in motion at highway speeds. The issue has not been reproduced or independently duplicated by a dealer or service center; however, it was inspected and documented by the insurance company and repair facility during the claims and repair process, which confirmed that full glass replacement was necessary. The vehicle or component has been inspected by insurance representatives and a repair shop, but it has not been inspected by the manufacturer or law enforcement. There were no warning lamps, dashboard messages, or any prior symptoms indicating a problem before the failure occurred, and the sunroof appeared to fail suddenly and without any advance indication while the vehicle was in motion.Read more

2018 Cadillac Escalade ESV Platinum, 6.2L V8 L86 engine, under 100,000 miles. Engine seized and was towed to an authorized GM dealership. Teardown confirmed a spun and twisted rod bearing resulting in complete crankshaft seizure, documented by the dealer as non-serviceable per GM, requiring full engine replacement. Component is available for inspection. Prior to seizure, the vehicle experienced a
... highway stall preceded by a harmonic balancer failure, belt system failure, and charging system collapse. The vehicle lost propulsion at highway speed, creating a crash risk to myself and other drivers. The failure mode, spun rod bearing with crankshaft damage, is physically identical to the defect documented in GM Safety Recall 25V-274, covering connecting rod and crankshaft manufacturing defects in 6.2L V8 L87 engines in 2021-2024 vehicles. The L86 and L87 are variants of the same Gen V small block architecture with the same bottom end design. I contacted Cadillac Customer Care and requested a Field Technical Specialist review. GM instructed the dealership to delay filing a technical assistance case until my third party warranty determination was made. GM subsequently refused to file any technical case, stating the L86 is not included in the recall and they have no plans to include it. The failure has been confirmed by an authorized GM dealership. I am requesting NHTSA evaluate whether the recall scope should be expanded to include 2018 L86 engines, and whether GM's refusal to conduct technical investigation while directing a third party warranty to absorb a potentially recall-related repair constitutes inadequate recall handling.Read more

The system did not alert me, did not activate, and did not apply emergency braking. I nearly collided with another vehicle as a direct result of this failure. The vehicle has a persistent diagnostic trouble code C28AB87 — CAN time-out for the front rad
... ar unit — which disables critical Advanced Driver Assistance Systems including Automatic Emergency Braking, collision avoidance, and Smart/Adaptive Cruise Control. This is the same defect that has been present since early in my lease. The vehicle has been presented for repair at two different Hyundai dealerships on three separate occasions: Attempt 1 — Oxmoor Hyundai, [XXX] , March 19-20, 2026. Defect diagnosed, unable to repair. Attempt 2 — Oxmoor Hyundai, [XXX] , April 1-17, 2026. Front radar unit replaced. Defect remained. Attempt 3 — [XXX] Hyundai, [XXX] , April 23, 2026 to present. Suspected bad connector, part ordered. Vehicle still at dealership. Cumulative days out of service: 24 days and continuing for the same safety defect. I have formally demanded a buyback from Hyundai Motor America under Ohio Lemon Law ORC 1345.72. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Despite being only 18 months old with under 26,000 miles, the vehicle suffered a catastrophic failure with no prior dashboard warnings. While driving at highway speeds, loud cracking noises occurred while the engine was running, though we were able to pull off safely. The incident resulted in a burning smell, fumes, and rapid battery drain; subsequent inspection revealed a sheared tensioner spring
... and a critical VDCU diagnostic code (P1A7795), indicating a "Check Generator Belt" error and a major hybrid power control system failure.Read more

My infotainment unit stoped working after an update that was pushed to the car by Hyundai. My battery was beginning to show signs that it needed replacing (not cranking in the cold temps or needing a few tries to start), so I went to Advance Auto to have the battery replaced. Once back in the car, everything was working fine (radio with sirius XM, back up camera, Navigation, Android and Apple car
... plan, the clock, everything). When I went to reset the clock, the car showed a software update was availabe and needed to be downloaded. Never before, had I seen this. So, I selected update. After 30 minutes of seeing it say downloading, it then started to say install. It gets to where it says, the radio needs to be restarted to complete the update and then gives error code 604. Now the telematics or infotainment works. I reached out to mapit, Hyundia and Bluelink who all say, "to bad for you, buy a new one for $1600". I did not ask for the update, I did not go looking for the update and I did not push the update to my vehicle, Hyundia did and me and the other 2000 owners who are going through this should not have to pay that kind of money to have it repaired. If they new they were shutting down 3G service, then don't push the update to my car. Hyundia has admitted the issue for Hyundia Tusons and a few other vehicles but not for 2015 model Sonata's. So for Tusons they send out 3 SD cards to fix the issue, but refuse to do so for all the cars they pushed an update to that as they called it Bricked the head of the unit. I am looking for help to get this fix without me paying $1600. If Hyundia recognizes the problem in some cars and not others thats an issue. One that we the public would love help addressing.Read more

Coolant hose on bypass valve came off. Dumped coolant and engine overheated. Tech said it just came off, service advisor said I “hit a bump”.

The tracking number assigned by NHTSA for this issue is 11733456 I am writing to update and escalate my previously filed safety complaint regarding my vehicle. I recently received my car back from the dealership after they informed me that all issues had been repaired and that the vehicle was safe to drive. However, after getting the vehicle back, it is not the same, and it is not safe to opera
... te. The vehicle pulls strongly to the left while driving, which creates a serious risk of losing control, especially at normal road speeds. In addition, multiple safety and driver‑assistance functions are not working as intended. Despite these issues, the dealership insists that everything is “within specifications” and refused to take further corrective action. Because of these unresolved defects, I am afraid to drive this vehicle. I have already experienced a traumatic incident related to this car, and returning it in this condition has caused significant fear and emotional distress. I cannot go through another dangerous situation. I believe this vehicle poses a genuine safety risk to me and to others on the road. I am requesting that NHTSA review my complaint again, including this new information, as the problem persists after inspection and attempted repair. A vehicle that pulls to one side and has non‑functioning safety systems should not be considered safe for normal use. Thank you for your time and attention to this matter. I am available to provide any additional details, documentation, or records you may require.Read more

Had an issue with driver assist radar when in complete control. Until you’re not! Brakes lock up skidding tires and causing seat belts to lock up. ... ed/slowed traffic due to emergency vehicles trying to get to scene of Naples wildfire. When I accelerated as traffic resumed at slow speed the car locks up brakes, squalling tires and slewing sideways. It startled me so much my heart was racing and I didn’t know if the car was still running or not. There was an 18 wheeler coming up behind and I was frozen not knowing what to do next. I don’t know if sensitivity can be adjusted or what it needs but my heart can’t take many more of these incidents. I will report to dealer as well. I know there is a Reddit thread about these same issues that my daughter found on researching this issue.Read more
