On April 29, 2026, my 2024 Mazda CX-90 Turbo S Premium Plus experienced a steering malfunction while I was entering a right turn. The vehicle had Mazda steering recall 6524A / NHTSA Campaign 24V-022 marked closed/completed before this crash. ... or prior warning immediately before the incident, the steering wheel suddenly required abnormal effort, felt stuck, and would not turn normally. I was in complete shock from the sudden steering issue and abnormal resistance. The vehicle then struck the curb, left the roadway, and traveled down an embankment/grade of approximately 15 feet into a residential yard. The crash involved significant driver-side/front-left damage. The front driver-side wheel/tire separated and was found near the vehicle. The driver-side airbags deployed. Scene evidence included curb/edge impact damage, debris, and a deep ground-level rut/indentation below the roadway. This incident placed me and others at serious risk because the vehicle left the roadway and entered a residential property. The vehicle and failed components are available for inspection upon request. To my knowledge, the only prior inspection or repair related to the steering system was performed by the Mazda dealership in December 2025, when recall 6524A was marked closed/completed along with other recalls. The vehicle has not yet been independently inspected for this crash-related steering concern. Before the crash, the vehicle had multiple issues and spent significant time at the dealership for recall and non-recall repairs. One ongoing issue was a brief grinding/rubbing noise when accelerating from low speeds, which existed before the dealer work and continued afterward. Mazda/dealer indicated this low-speed grinding/rubbing noise was a known reported issue with no fix at that time.Read more

I have replaced the flex pipe, the catalytic converter two times, and both the upstream and downstream oxygen sensors. I am reporting a software-induced driver distraction defect in Tesla’s Full Self-Driving (Supervised) system, specifically the post-disengagement “Why did you intervene?” dialog introduced in software 2026.2.9.8 and modified in 2026.2.9.9 and 2026.2.9.10, running FSD v14.3.2. Every time the driver disengages FSD via steering, brake, or accelerator input, a modal dialog appears on the center
... touchscreen asking the driver to categorize the intervention. It presents four options (Navigation, Preference, Discomfort, Critical) plus a voice memo prompt. The dialog cannot be dismissed or deferred. It remains on screen indefinitely until the driver reads the prompt and taps a selection. This creates a serious safety hazard: The dialog appears the moment the driver has resumed manual control, often in response to an unsafe FSD action requiring immediate attention to the road. Forcing the driver to read text and make a multi-choice touchscreen selection during this high-cognitive-load transition directly conflicts with NHTSA’s Driver Distraction Guidelines for in-vehicle devices. The dialog occupies a large portion of the center display and obscures the navigation map and surrounding-vehicle visualization, removing situational awareness at the moment it is most needed. Because it cannot be dismissed, drivers are coerced into touchscreen interaction while driving, including in active traffic, intersections, merges, and lane changes. The forced-choice design also incentivizes drivers to tap any option to clear the screen, defeating the stated data-collection purpose and increasing eyes-off-road time. Tesla provides no setting to disable this dialog. I request NHTSA investigate this as a driver distraction defect and require Tesla to provide a dismiss option, defer the prompt until Park, or remove the forced-interaction requirement. The vehicle was towed that evening to local GM dealer - Yark Chevy in [XXX] . Upon inspection on May 5, 2026, the dealership informed me the engine had seized and would be replaced by GM. They were unable to provide a timeline for this repair. I have the tow expense of $260.70 and now the uncertainty of when I might have a vehicle back. Despite being unable to use the vehicle, I have continued to make monthly payments, resulting in considerable financial hardship. In an effort to resolve this matter, I consulted with a Maryland MVA investigator, who advised me to contact Kia directly and report that the vehicle appears to have been sold with a pre-existing defect. I have also communicated with the selling dealership, Eastern Motors; however, I was informed that due to changes in management, they are unable to provide further assistance and consider the matter outside of their control. Additionally, I have previously reached out regarding this issue and have not received a response. Given the timing of the failure, the repeated unsuccessful repair attempts, and the extended period the vehicle has been out of service, I respectfully request that Kia conduct a thorough review of this matter. I am respectfully requesting Kia’s assistance in resolving this matter. Confirmed, the problem has been reproduced and confirmed by the Honda of Kirkland service team. The vehicle has been inspected by the service team at Honda of Kirkland. There were no warning lamps, messages, or other symptoms of the problem prior to the issue occurring. The problem started about a year or so ago and has progressively gotten worse over time. The vehicle has maintenance done on a regular basis and has never been in an accident. Honda of Kirkland and American Honda Motor Co., Inc. have denied a goodwill repair for this issue. The rear sub frame has completely rusted through, on both driver and passenger sides. This makes the vehicle extremely unsafe to drive. It is a 2014 q5 with 160k miles. ... ke was coming from the door column that houses the seat belt. We had water in a cooler so he poured some on the area and I called 911. While waiting on the fire department the suv was still smoking but he couldn’t get the column cover off to see where it was coming from.Then fire starts flaming up through the melting plastic column so he took more water and poured it on the fire and smothered it with a towel until it went out. The fire department came pulled the seat belt column off and cut the wires and taped them up, then checked the voltage to make sure it was safe for us to drive home since it was only about 15 minutes away and told us not to park it the garage or close to the house so we went straight home and didn’t drive it anymore. Damage is to the seat belt, seat belt column and the electrical harness that goes under the seat. The suv was towed from our house to the dealership Vann York in high point nc. They have looked at the damage and advised the insurance company who is not covering the cause of the fire only the damage to the other parts. No alterations have been made to the suv. The dealer found catastrophic internal damage: “fluid is burnt with metal debris… filter is packed with metal debris… torque converter has came apart and filled transmission with metal debris… it is catastrophic failure.” These components are available for inspection. The dealer also documented a low battery condition: “batteries. low state of charge.” Ford’s Neutral Tow Mode relies on electronic control modules that require stable voltage, but Ford provides no warning that low voltage can cause the transfer case to exit Neutral Tow Mode. This creates a safety risk because the transmission can be rotated internally without lubrication while being towed, which can lead to sudden drivetrain lockup or loss of control. The problem was confirmed by the Ford dealer, and the vehicle was fully inspected by them. The only warning symptom occurred after towing: wrench light, slipping, and no movement. There were no warnings during the tow itself. This failure mode is not disclosed by Ford and cannot be detected or prevented by the owner.Read more

... e 2018 Ford F150 with 10 speed transmission I discovered that the CDF hub has a defect that Ford knows about but will not repair. The hub has a bushing that moves blocking transmission fluid ports. I called a two transmission shops and they were well aware of the issue and were able to give a quote range over the phone because of the volume of these transmissions that they have seen. I took the truck to my local Ford dealer and they checked the issue and confirmed that the CDF hub was the issue and the transmission would require a total rebuild. The rebuild came in at $7,755 dollars plus tax. A replacement transmission was $9035.26 before tax or just over 10,000 after tax.
