Ongoing Fuel System Failures Related to Recall 24S78 – Request for Escalated Review To Whom It May Concern, My name is [XXX] , and I am the owner of two 2021 Ford F-350 6.7L Power Stroke Diesel trucks, both of which are affected by Recall 24S78 / NHTSA Campaign 24V957 concerning biodiesel deposit accumulation in the fuel system and a pending PCM software update. Vehicle Information: VIN [XXX
... ] VIN [XXX] Both vehicles have already experienced critical failures that directly align with the described defect in the recall, including: Failure of the CP4 high-pressure fuel pump Repeated clogging of the water/fuel separator filter Loss of power, extended crank times, and intermittent no-start conditions These issues occurred prior to the recall remedy being available and have compromised vehicle performance and safety. I am requesting: A full inspection and evaluation of the fuel systems under the scope of Recall 24S78 once the software becomes available. Coverage or reimbursement for repair or replacement of the CP4 pump and related components if found to be caused by biodiesel-related damage as described in the recall. A formal service record entry acknowledging these failures for future reference and eligibility under the recall. Escalation of this issue to a Ford regional service manager, if needed. Given the significant safety risk and the known failure history of the CP4 fuel system, I ask that Ford address this matter thoroughly and beyond a software update, if necessary. Please confirm receipt of this inquiry and advise me on next steps. My 2019 RAM 1500 Big Horn/Longhorn 3.6L V6 (VIN: [XXX] ) experienced a sudden and catastrophic engine failure without warning while driving. The vehicle shut off unexpectedly on the highway and would not restart. There were no prior dashboard warning lights, low oil pressure indicators, or error messages before the incident. A full diagnostic revealed severe mechanical failure in cylinder 1, incl
... uding piston-to-valve contact and 40% leak-down with compression at 50 psi. This damage rendered the engine inoperable and required a complete replacement. Prior to the failure: The vehicle was regularly maintained using synthetic oil, following the manufacturer's recommended intervals (between 6,500–10,000 miles). The oil life indicator was followed, and the truck was never low on oil or pressure. A Customer Satisfaction Notification for PCM (Powertrain Control Module) reprogramming was never completed due to the dealer’s system being down on two separate service visits. Despite following proper maintenance and attempting to comply with the manufacturer’s recall-like notice, the engine failure occurred without any warning or indication. The dealership and warranty provider have attempted to deny responsibility, blaming maintenance intervals without proof that it caused the failure. This is a serious safety concern as the engine shut down at highway speed, placing me and others at risk. I am requesting NHTSA investigate this issue and whether PCM logic, a known defect, or improper diagnostics at the dealership contributed to the failure. My 2022 Heartland Pioneer BH270 Travel Trailer experienced multiple electrical failures of the DC power system between October 2024 and April 2025. The converter and battery had to replaced twice. The original auto reset circuit breaker, and replacement auto reset circuit breaker, and a resettable circuit break were all replaced due to overload from the DC power system tripping the breaker. The ul
... timate cause of the electrical failure was found to be the wire connecting the travel trailer battery to the converter inside the unit had been installed improperly by the manufacturer. The wire has been installed around the strap for the fresh water tank. This installation improperly aligned the wire with the slide out mechanism causing the wire to be pinched each time the slide out was operated. This resulted in the wire sheathing degrading and shorting the wire. Electrical arcing occurred which melted the sheathing of the wire, fraying of the exposed wire, and corrosion of the exposed wire. This could have led to an electrical fire of the travel tailor at any moment the battery was connected and current attempting to run through the electrical system of the travel trailer.Read more

Very common problem it appears as there are several forum post from other accord owners 2013 and above that have all reported dash lights up with traction control, tire pressure, power steering, ABS and some instances Brake will illuminate on the dash board, and some have reported a combination of these lights or all of them. The culprit is a broken solder on the ABS module from my understanding
... and its something Honda is aware of but have not recalled. The part itself costs $1500, just for the part and does not include labor. This is a common issue that should be taken care of by honda as its a huge safety issue with the ABS, Traction control, power steering, tire pressure monitoring and brake not working when needed in an emergency situation. This is a huge safety risk that needs to be addressed. It also affects the value of the vehicle if not repaired.Read more

5/22/2025, Thursday morning I unplugged my car that was parked inside my garage. I sat down, tapped the pedal to turn on my “ignition mode” and within a few moments my windshield shot a massive flame in front of me and began sparking shooting glass from both heating elements. Glass shards landed on my hood and garage floor. My windshield was smoking and the amount of heat generated melted some
... of the glass around the area where the heating elements connect to the glass on the interior of my vehicle. My dealership fought me hard saying I had a crack that caused the fire and advised me to file an insurance claim for this “accident” and also continuing driving my vehicle that had just caught fire momentarily. Thankfully the fuse blew and that’s why the fire stopped but my concern is how much heat had to be generated to spontaneously combust my glass and then melt it! I refused to accept their response and after an entire day of arguing with VW customer support and the dealership. I repeatedly asked for a diagnostic of my windshield heating element module and the amount of electricity it was giving. The dealership declined to diagnose my car and removed the heated windshield fuse per my request. I spoke with the dealership owner who finally contacted their field service advisor who advised I should not drive my vehicle that caught fire and instead are going to further investigate my concern. I struggle to trust an actual investigation will be performed and hope they do not simply replace the windshield and ignore the safety hazard of my window shooting a flame and combusting glass.Read more

This car has a system called Forward Attention Warning (FAW) which monitors the driver's eyes using an in-cabin infra-red camera trained on the driver. When smart cruise control and lane following assist are both activated, FAW is forced ON by the car. If FAW thinks that the driver's eyes aren't on the road, it beeps, puts alerts on the panel visible to the driver and turns off all driving assis
... t features. The problem I'm reporting is that the FAW system is completely broken. Even though I'm looking straight forward, it says I'm not paying attention to the road, sounds loud alerts, then eventually shuts off all of my driving assistance features. I wear glasses, and sometimes sunglasses to reduce glare when driving in bright conditions, and I've tried this with my prescription sunglasses, my normal prescription glasses and even without any glasses. The system erroneously triggers in all cases. I believe that the FAW system false positives is a safety issue because: 1. The constant false alerts are very distracting, making the driver look at the panel in front of them to try to figure out what it's saying, taking their eyes off the road 2. Lane following assist is, itself, a safety feature, by disabling it due to these false positives from the FAW system, this is reducing safety in the vehicle 3. The FAW false positives will abruptly disable the smart cruise control, which is controlling the speed of the car, as well as the lane following assist, helping to keep the car in the lane. This abrupt disabling of these systems can cause the car to slow suddenly and depart from the lane if going around a curve. This can be very dangerous.Read more
