May 7, 2026 National Highway Traffic Safety Administration 1200 New Jersey Avenue SE Washington, DC 20590 RE: Safety Defect Complaint – Brake System / Vehicle Control Concerns 2020 Chevrolet Malibu Dear NHTSA, I am submitting a safety complaint regarding persistent braking and vehicle control issues involving my 2020 Chevrolet Malibu. Prior to December 31, 2025, the vehicle exhibited jerk
... ing during braking, all dashboard gauges go to the left, warning lights, reduced power (20-25mph), and inconsistent braking behavior. This situation happens without warning, at any given time, even in the flow of traffic. An independent inspection indicated the brake system was in proper condition, suggesting a more complex underlying safety issue. The vehicle was taken to Reed-Lallier Chevrolet for diagnosis and remained there for approximately five months. During that time, I was not provided documentation confirming completion of a brake system diagnostic or a clear resolution path. Due to the extended delay without diagnosis, I no longer feel safe operating the vehicle. I am also concerned that prolonged inactivity without proper repair may have worsened the condition and increased risk of failure. Given the nature of the symptoms involving braking and vehicle control, I request review for potential safety-related investigation. Vehicle Information: 2020 Chevrolet Malibu VIN: Mileage:Read more

... forcement I am filing this formal safety defect complaint regarding my 2026 Subaru Forester. A pedestrian incident occurred on February 2, 2026, in which the vehicle failed to warn or brake automatically when a pedestrian crossed in front of my car. Additionally, the Subaru EyeSight system does not remain enabled between ignition cycles. Every time I start the car, I must manually re-engage the system. Consequently, Automatic Emergency Braking (AEB) is not active by default during ordinary driving and will reverts to off (not remain enable) when I am driving . I believe this poses an unreasonable risk to pedestrian and occupant safety. I am aware of prior investigations (e.g., NHTSA Action Number PE23010 and related lawsuits) involving Subaru EyeSight failures, including lack of braking for obstacles. Initially while taking a corner at low speed the truck made a clunk and grinding noise which resolved and then while going about 50 mph the grinding noise recurred which prompted stopping the vehicle on the side of the road and when almost to a stop, the wheels locked up causing a slight skid; the vehicle was in 4-wheel auto at the time and with change between 4wd and 2wd serveral times the front
... tires unlocked and i was able to drive it again. About 4 days later, while driving 70 mph, the vehicle started to make a grinding noise on the interstate, and I started to slow down, but unfortunately, I was on the left side of the highway on a narrow shoulder, and upon getting to a lower speed while in 2WD, my front axle locked up. I placed the vehicle in park, but due to safety concerns, I put the truck back in drive and tried to switch from 2WD to 4WD to unlock the axle, but unfortunately, the truck would not change gears. I then had to quickly get the vehicle to the other side during a time with fewer vehicles, while the left front tire was locked, and I had to drag it across the highway to ensure my safety. The left front tire remained locked, and after multiple attempts, the brake partially unlocked but continued to make a grinding and clunking noise. After some diagnostics, we determined the left front wheel hub was the issue, overheating and locking up. I reached out to mechanics for quotes on repair of said part, and upon reaching out to the dealer that I have used in the past for recalls and oil changes, I was informed there was a recall on said part/software that I was not aware of; the service rep used the last 8 of my VIN to look up my vehicle for the quote and provided the details about the recall. I use the RAM Guardian App which provides updates about recalls and upon checking it does not show a recall nor does it show a recall when i search my VIN and License Plate on the website. This seems to be a common issue based on customer reviews.Read more

2019 Subaru Impreza has experienced intermittent and now persistent failures involving the factory EyeSight driver assistance system. Prior to complete failure, the EyeSight system would randomly disable and re-enable while driving. Adaptive cruise control and related safety features became unreliable and would intermittently stop functioning. The issue worsened over time and eventually resulted
... in repeated “EyeSight Off” warnings and multiple system fault messages. Vehicle diagnostic scans showed repeated U1235 “Lost communication with EyeSight” faults across multiple modules, including ABS, steering, transmission, and instrument cluster systems. Additional communication and airbag indicator-related faults were also present. The windshield was recently replaced due to a crack located near the EyeSight camera area. Subaru service personnel stated that EyeSight systems are extremely sensitive to windshield specifications and calibration requirements, and that aftermarket windshields may prevent proper operation or calibration of the system. Subaru also indicated that EyeSight calibration equipment is highly specialized and expensive, and that repair costs may not be practical relative to the vehicle value. Despite attempts to reset the vehicle systems and inspect the issue, the EyeSight system continues to malfunction and remains disabled. Because EyeSight controls important driver assistance and collision-avoidance features including adaptive cruise control, forward collision warning, lane departure warning, and automatic emergency braking, this issue creates significant safety concerns. The intermittent nature of the failure, repeated communication faults, and inability to reliably restore system operation raise concerns regarding the long-term reliability and serviceability of Subaru EyeSight systems.Read more

I am deeply concerned about the risk of catastrophic engine failure and the potential for fire, which has been documented in similar cases. Despite having the Knock Sensor Detection System installed, my vehicle continues to exhibit these dangerous symptoms. I sought assistance from Landers Crain dealership and KIA; however, they declined to properly evaluate or service my vehicle along with KIA declining according to the KIA Regional Manager. I stopped all cruise control and drove the remaining five hours to my destination scared out of my mind. I called local dealership, country club Chevy, dodge, jeep, Hyundai etc. at 607432-9300 and told Gavin what had occurred. I was unable to get into a dealership while I was in [XXX] for the brief time I was there. On my way home I tried cruise control on I 95 in the right hand lane all the way over so that no traffic could be on my right side. It was a beautiful day. It happened again to me, the warning lights, the decrease and speed the yank to the right the shaking steering wheel and the sudden breaking while I was in the state of [XXX] . I turned off cruise control again. I didn’t have a problem after that. When I I returned home, , I took the car in to be looked at at the dealership . They said they couldn’t find anything wrong. I am submitting this complaint regarding Toyota Motor Sales, U.S.A. and my 2023 Toyota Tundra, which is currently subject to Safety Recall 25V767 involving catastrophic engine failure risk due to crankshaft bearing contamination within the V35A engine. Toyota has acknowledged that my vehicle is included in the recall and has further acknowledged that the engine will require replacement. However, d
... espite confirming the seriousness of the defect, Toyota has refused to repurchase the vehicle and has failed to provide any timeline whatsoever regarding when the engine replacement remedy will become available. Toyota’s position has simply been that the engine will be repaired “once a remedy is available,” without any estimated repair date or meaningful resolution for affected consumers. The recall itself states that the defect may result in engine knocking, rough running, engine no-start, and/or sudden engine stall. Toyota further acknowledges in its recall documentation that engine stall while driving can lead to loss of motive power and increase the risk of a crash. This is a major safety issue, not a minor inconvenience. I live and drive in [XXX] , where highway driving and high-speed traffic are unavoidable. Due to the risk of sudden engine failure or loss of power, my family and I no longer feel safe operating the vehicle under normal driving conditions. As a result, we have drastically limited use of the truck despite continuing to make monthly loan payments, insurance payments, and registration expenses. Additionally, the truck has experienced multiple other recall and repair-related issues since purchase. Several recalls have already required service visits, and additional unresolved recalls remain pending. The ongoing pattern of defects and unresolved safety concerns has substantially diminished my confidence in the reliability, safety, and value of the vehicle. Toyota acknowledges the engine requires replacement, Toyota provides no repair timeline INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACRead more

I've submitted a safety complaint #11733895. I wanted to give an update regarding GM Goodwill Assistance, which is 10% of $10,000 transmission replacement per Chevrolet. This is a known documented transmission defect from GM manufacturing, it is a major safety issue, the car was sold to me with GM/Chevrolet knowing about this issue, but they are putting the financial burden on the consumer. Diagno
... stic from Chevrolet confirms this defect and they basically are saying they won't help. This transmission should be 100% recalled for safety issues and I am just speechless it has not been. The random acceleration alone is extremely scary and totally unsafe. What do consumers have to do to get this recalled? Have a collision and kill or injure someone or myself? Something needs to be done. GM needs to take accountability and do the right thing RECALL THESE TRANSMISSIONS and fix the ones that need to be fixed.Read more

The electronic steering lock (ESL) system on my 2014 Mercedes-Benz C250 failed without warning, leaving the vehicle completely unable to start. When attempting to start the vehicle, the key would insert but the vehicle would not crank or start, and the steering lock malfunction prevented normal operation of the vehicle. The vehicle became stranded and inoperable. This failure created a safety ris
... k because the vehicle could not be moved and left me stranded unexpectedly. If this failure were to occur in an unsafe area or emergency situation, it could place the driver and passengers at risk. After researching the issue, I found numerous reports online from other owners of similar Mercedes-Benz C-Class models experiencing the same ESL/EIS failure. This appears to be a common issue affecting these vehicles. At this time, the problem has not yet been repaired. UNKNOWN if the manufacturer has inspected the vehicle. No warning lights or prior symptoms appeared before the failure. The malfunction happened suddenly and without warning.Read more

The cars speedometer screen and radio screen go black and sometimes flicker on and off. This makes it impossible to see the speed. Also the car stalls sometimes when this happens in traffic. The autonomous braking applies the brakes even when there is a large distance from other cars and objects to the point where it has almost caused me to get into an accident on numerous occasions. The driver at
... tention alert comes on and off over and over again even though I’m fully focused on driving and well aware of what is going on and this sometimes happens in a sequence where all of the warning lights illuminate on dash. Sometimes all of the warning lights display on cluster after it goes black and then car says to shift to park when display goes back on causing safety concerns for me in traffic. In addition my sunroof glass is making a loud noise like it’s going to crack.Read more

Yesterday evening we were driving and the orange check engine light came on. Followed by a red battery light, the red turtle and an exclamation point with a car and a speedometer symbol. It would not let me switch to electric e-save and kept me in hybrid mode. Regenerative braking would not work either. Once safely parked it was making a loud high pitched whining noise. It would also drop RPMs whi
... le idling and then they would rev up quickly. Once the tow truck arrived he tried to drive it up on the flat bed but it had no power to make it up the ramp and had to be pulled with the tow cable up the ramp. Another thing I have noticed as of the last 68c update is that when in electric mode and merging onto a freeway for example, it gets really sluggish at about 65mph and the gas engine won't kick in like it used to before the update. It doesn't go above 65 and slows down to about 62 even if my foot is pedal to the floor before ice kicks in. Makes it pretty dangerous to be merging with traffic going 70+.Read more

It was noted that the drivers door weather strip deteriorated even though the car is less than 3 years old/ 47,000 miles. The dealer and the Toyota customer care said it is likely related to external factors and not covered under warranty. I keep the car in a garage. We have had other cars that are well over 10 years old but do/did not have this problem. This can potentially lead to damage of othe
... r components. I see on the internet that other owners of this car have encountered this same issue . There have been 3 safety recalls on this model, related to air bag, instrument panel and rear view camera. These have been fixed. There have been at least 5 Toyota technical service bulletins ( TSBs) published on this model. These were fixed after I brought it up with the dealer/ manufacturer. I was not contacted by the manufacturer proactively. TSB 0017-24 : hesitation felt while accelerating from stop. A software update was performed. TSB 0060-25 : Fuel level logic enhancement. A software update was made. TSB 0028-25 : Shudder sensation at stop. A software update was performed. T-SB-0082-24: CV axle issue: it was fixed T-SB-0111-24: vibrating hood.I notice the issue but did not fix it since the fix involves placing a foam under the hood, which may not fix the underlying issue and might cause other issues. I believe these issues are related to the manufacturing oversight and especially more frequent because it is a first year model .Read more

I was driving down my neighborhood road about 20mph there were no other cars around on the road at all only mailboxes and cars parked in driveways. The car suddenly slams the brakes hard enough the tires were squealing and threw us forward thankfully me and my child had our seat belts on. It scared the heck out of us cause there was no warning and definitely not expected.
