Excessive Frame Flex/Failure& Structure I am reporting multiple serious structural and safety issues with my Grand Design RV that appear consistent with excessive frame flex and failure, as outlined in NHTSA Investigation PE24029. These include: Significant movement of front bedroom cabinets during travel, indicating internal structural instability of the walls. Large gaps in both the interior an
... d rear toy hauler doors, which compromise insulation and safety concerns. Cracked and misaligned front entry door, even after manufacturer replacement. Generator compartment door fails to lock properly, likely due to excessive frame flex/frame misalignment. Screws popping out of the front cap and a visible crack in the front cap corner. Bedroom slide failure due to internal twisting. Main slide floor detaching from the bottom. Persistent latching issues with the main entry door, even when the RV is perfectly level. These issues not only affect the usability of the RV but also pose serious safety risks while in transit. The structural instability could lead to doors opening unexpectedly, slideouts extending, or components detaching on the road—endangering both occupants and other drivers. I urge the NHTSA to include this case in its ongoing investigation and to consider a recall or mandatory corrective action for affected Grand Design RVs.Read more

The Connected Services system in my 2023 Ram TRX regularly displays large, full-screen alerts for emergency vehicles, tow trucks, and lane closures — often when there is no actual hazard present. These alerts are extremely distracting and dangerous. They: Block the entire screen, hiding navigation, camera, and HVAC controls and leaving the screen black until the user interacts with it multiple
... times to put it back on navigation. Require multiple presses to clear, forcing interaction while driving. Often reactivate within seconds, creating a loop of screen interference. Have triggered during merging, construction zones, heavy traffic, and when passing emergency vehicles or tow trucks, where focus is critical and lives are at risk. Occur falsely 80%+ of the time — no hazard is nearby. Cannot be disabled, according to Stellantis. These alerts violate basic safety interface principles by forcing unnecessary, repeated interaction in active driving conditions, forcing distracted driving. The system is overly aggressive, not user-configurable, and poses a significant distraction risk. Numerous owners on Ram forums have reported similar experiences, including repeated false triggers in the same areas. I am requesting NHTSA investigate this system. Stellantis should be required to: Offer a toggle to disable or limit alert pop-ups. Redesign the alerts to be non-obstructive and passive, or limit them to actual verified hazards. Until addressed, this is a serious safety issue affecting many drivers. I have video evidence of the issue occurring that I can provide.Read more

Today when trying to start the car, I hear a mechanical whirring and several notifications appear on my dashboard including the following: Check Engine, EyeSight Off Check Manual, Check Owner Manual, RAB Disabled. I looked online for similar issues and have discovered several instances of people with 2018 and 2019 subaru crosstreks (and other models) have had the same issue. Subaru has issued a Technical Service Bulletin (07-198-21), but this is a safety issue which they should address. My coworker has the same car and it has had work done for the same issue. Component or System That Failed or Malfunctioned: The component that failed was the partial-load ventilation valve, which is part of the engine’s crankcase ventilation system. This valve is responsible for regulating airflow and pressure within the crankcase during partial engine loads. A malfunction in this valve can lead to increased engine pressure, oil leaks, and potential damage to engine c
... omponents. The component is still available for inspection upon request. Safety Risk: The failure of the partial-load ventilation valve put my safety, and that of others, at risk in the following ways: •Engine stalling or reduced power while driving, especially during acceleration, which increases the risk of an accident. •In some cases, oil leakage caused by excess crankcase pressure could lead to engine compartment smoke or a potential fire hazard. •If the engine performance is compromised, it may suddenly lose power on highways or intersections, posing a serious risk. Problem Reproduced or Confirmed: Yes, the issue was confirmed. The technician verified the faulty valve during a diagnostic inspection after I noticed performance issues and warning messages. I discovered what appears to be severe rubbing on one of the fuel lines in my 2024 Toyota GR86. The car has just over 6,000 miles on it, and the rubbing is so severe that it has removed the printed writing on the line and smoothed out the hose to the point where it is flat. From what I can tell, the hose is rubbing against another fuel line, which must occur while the engine is running. Considerin
... g the car was manufactured in September 2024, I find this to be not only unacceptable but dangerous. If I had not caught this, it would have easily rubbed through and resulted in an engine fire, which could have resulted in the total loss of the vehicle. While reviewing my vehicle, I also noticed that a friend with a 2023 Subaru BRZ had the same issue. While his rubbing was not as severe, it is clear this is not an isolated incident and affects others. I am in the process of gathering photographic evidence from others who are impacted and so far know of at least two more cases beyond the above: a 2023 Toyota GR86 and a 2024 Subaru BRZ. I am reporting a safety concern with the KIA K5 Advance Driving Assistance Systems. The concern is the Driver Attention Warning (DAW) being a distraction to the driver due to consistent dinging and flashing light of the "taking a break" that according to manual could go off every 4 minutes, however, in my experience it can go off every 3 minutes (see attached documentation), which is excessive. Th
... e frequency that the light goes off has led me to be distracted and nearly hitting the car in front of me because I am looking down at the light. I'm at the point; I am afraid to drive the car because I don't know when this light will go off. According to the manual it can go off 4 minutes into a drive. When reviewing the manual, it fails to tell you what exactly is causes the light to go off and how long you are supposed to be taking a break for. I have taken it to the dealership three times, and they tell me it is working property. I have asked if the light can be shut off, but they tell me it cannot be shut off even though it says in the manual it can be. I have also worked with KIA corporate, and they tell me the newer KIAs it cannot be shut off. The older models also had attention level gauge that could show the driver where their attention level is, but this was removed. In conclusion, this "take a break light" is a major distraction to driver that cannot be shut off that could lead the driver taking their eyes off the road multiple times in a driver that can cause an accident.Read more

Formal Complaint – Hybrid System Failure, Safety Risk (VIN: [XXX] | Hyundai Case #[XXX]) Dear Sir/Madam, I am writing to file a formal safety complaint regarding a dangerous incident involving my Hyundai hybrid vehicle (VIN: [XXX] ) and Hyundai Motor America's failure to respond meaningfully to repeated requests for assistance. Incident Summary: On [XXX], while driving with my two young chil
... dren (ages [XXX] and [XXX] ), my vehicle suddenly displayed the following critical warnings: “Hybrid System Warning! Safely Stop and Do Not Drive” “Check Charging System” Immediately after these messages appeared, the car lost all power and came to a complete stop in the middle of the road. This created an extremely hazardous and distressing situation for me and my family. Although the vehicle is a few years old, it has very low mileage (38,345 miles), and I did not expect such a catastrophic failure. I submitted a report to Hyundai Customer Support on May 30, 2025 (Case #[XXX]) and followed up on June 9, 2025, but have only received an automated acknowledgment—no response, investigation, or support to date. Attachments: A video clip showing the warning messages at the time of the incident (I cannot upload the video clip since the option is not there, but I do have it if required) Requested Actions: A formal investigation into the safety and reliability of Hyundai's hybrid systems. A determination as to whether this incident qualifies under Hyundai’s warranty or federal safety defect regulations. Immediate contact from Hyundai with the next steps regarding inspection, resolution, or compensation. I am cc’ing Hyundai Motor America to ensure this matter is escalated appropriately and receives the attention it warrants. Thank you for your time and consideration. The right rear axle shaft assembly wheel bearing seal failed. The right rear ABS Sensor, aka (rotational Speed Sensor, wheel speed sensor) failed. These parts are available for inspection. The vehicle unexpectedly accelerated without me pressing the accelerator as I approached a red light at an intersection, putting my safety and others at risk. I had to slam on the brakes with all my might to get
... the vehicle to stop from going through the intersection. The road was wet, and the front wheels locked while the rear wheels continued to be driven by the engine without any input from the accelerator pedal. Eventually, the vehicle stopped, and the engine acceleration went back to normal operation. The problem has not been reproduced or confirmed by a dealer or independent service center. The vehicle and parts have been inspected by me, as the vehicle owner. The warning lamps suddenly illuminated on the instrument panel just prior to the incident. The warning lamps came on as I was approaching the intersection. The warning lamps that came on were the engine diagnostic indicator lamp aka Malfunction indicator lamp, the Anti-lock brake system warning light, the brake assist system warning light, the electronic stability program warning light. I also hear what sounded like the ABS Control Unit rapidly cycling just prior to the uncontrolled acceleration while nearing the intersection. I used diagnostic equipment and performed a visual inspection of the components, concluding that they needed replacement. Grease from the leaking right rear wheel bearing seal fouled the right rear wheel speed sensor. The wheel speed sensor cable develops cracks near the wheel hub. I believe these parts are defective and can cause erratic, uncontrollable, unsafe driving risks that could lead to accidents and injuries. The symptoms and warning indications went away after the parts were replaced. I still have these parts as I feel they should be inspected by NHTSA personnel to make a safety recall.Read more

My 2022 Hyundai Venue has a serious water intrusion issue that has gone unresolved for over a year. Every time it rains water pours into the cabin, especially on the passenger side leaving the floor soaked. I have to vacuum out water daily and use towels to prevent further damage. The vehicle has developed severe mold, which is a health risk, especially for my infant daughter who rides in the car.
... When the dealership first inspected it, they said it was the sunroof seal and would take several days to repair. Since the part wasn’t covered under warranty, I wasn’t offered a loaner and couldn’t be without a car. When I returned later, they said it likely wasn’t the sunroof after all, as there was no water damage at the top. They also said replacing the seal would be risky. After running their water tests, they claimed no water was entering the vehicle, despite clear evidence of ongoing leaks. To manage the constant water intrusion, I’ve had to regularly shop vac the car to remove standing water, line the interior with towels, and place multiple DampRid bags and buckets inside the vehicle weekly to try and reduce moisture. This has become a nearly daily routine, especially during rainy weather, and the problem continues to worsen. The interior constantly smells musty and feels unsafe for me and daughter. I am very concerned that the repeated water exposure may have damaged the electrical system or affected the airbags, especially the side airbags located in the door areas. I fear these safety systems could fail in the event of an accident. This is not an isolated problem. I have found multiple reports online of other Hyundai Venue owners experiencing the same leak in the same area. This suggests a larger factory defect that Hyundai is not addressing. See attached. I am asking NHTSA to investigate this issue, as it presents both a health and a serious safety risk. This defect should be evaluated for potential recall to protect other drivers and passengers.Read more

Just like the same era's Honda CR-V (the Honda Element's sibling car with many of the same components), the undercarriage rust on my vehicle is excessive: as described in the same era Honda CR-V's NHTSA Recall (23V-228). Large sections of my car’s rear frame are completely missing, as shown in the photographs. My car is now undriveable. I had it towed to two Honda dealerships at my expense and b
... oth refused to even look at the car's rust, when I explicitly asked them to determine if the car was safe to drive. Niether dealership would confirm. Honda corporate cited that they were unable to assess the photographs that I sent to it because it does not physically inspect the cars. Honda corporate also insisted that it could not do anything about any dealership’s behavior as they are allegedly completely independent from Honda of North America. Honda corporate also looked up the history of my Hondas that I have owned and Elements that I still own and actually asked me, if I purchased them just to try to have Honda fix them for free. Just like with the CR-V, a recall for the rear frame corrosion first occurred in Canada and then in the US: see NHTSA Recall 23V-228’s chronology. The Honda Element's Canadian recall is as follows: "Transport Canada Recall - 2021024 – HONDA". As of today, there is no NHTSA recall for rear frame corrosion for Honda Elements. I am submitting this complaint for this car now, and also contemporaneous will submit less detailed complaints under different VINs for my other Elements with rust problems: every Honda Element that I have ever owned, even one that had spent its entire 14 year life as a New Mexico, Arizona, and Texas car has had rust problems. Incident date: May 11, 2025
While driving my 2024 Ford Maverick on a highway at approximately 65 mph, the vehicle suddenly lost power to the front wheels while passing a slower-moving vehicle. This caused the truck to jerk and decelerate rapidly in the oncoming lane, forcing me to veer back into my original lane abruptly. In doing so, I narrowly avoided a head-on collision, but the sudden maneuver caused the driver behind me
... to swerve off the road to avoid a rear-end collision. As a result, that vehicle went into a ditch and struck a small embankment. Three people inside suffered injuries, one with a fractured wrist, another with whiplash, and the third sustained a concussion from the impact. Emergency crews responded to the scene, and all injured individuals were transported to the hospital for evaluation. Despite the severity of the incident, there was no physical damage to my Maverick, as I was able to regain control and come to a stop safely on the shoulder. The cause was later determined to be a failure of the front driver-side CV axle and associated drivetrain components. Ford service technicians confirmed the mechanical failure after inspection. There were no warning lights, dashboard alerts, or fault codes prior to the failure. Intermittent clicking sounds during acceleration and turns had been occurring since around 3,500 miles, but the dealership initially dismissed them as normal. The full failure occurred at approximately 4,200 miles. Ford has not issued a recall or technical service bulletin related to this, though the dealer acknowledged similar reports from other Maverick owners. The failed parts remain in possession of the dealership and are available for further inspection.Read more
