The 2.4L Theta II engine in my 2018 Kia Optima is failing, exhibiting excessive oil consumption, with oil loss documented at over 1 quart per 1,000 miles, leading to secondary failures in the catalytic converter, clogged with oil carbon and breaking into pieces that may damage the cylinder walls, and the spark plugs, which are fouled with carbon, causing misfires. The engine and related components
... remain intact and un-repaired, pending Kia’s required 3,000-mile consumption test. Safety of drivers have been put at significant risk. The excessive oil consumption and potential engine seizure threaten sudden loss of power, especially on freeways where car fails to accelerate to city and highway speeds, increasing the chance of a collision. The clogged catalytic converter poses a fire hazard, (reported by NHTSA investigations into Theta II) where oil leaks or debris can ignite on hot surfaces. Risk is heightened during the mandated 3,000-mile test as well. Problem has been confirmed by both a dealer and an independent service center as recent at 1 July, '25, documenting oil loss and new engine recommendation. Symptoms began with noticeable oil loss during oil changes, starting at least 1 quart per interval on May 10, 2023 (mileage 43,374), with the loss increasing over time. Misfires and RPM loss during acceleration first appeared after the oil valve gasket replacement in late May 2025, and freeway pickup issues became apparent around mid-June 2025. The full engine failure was confirmed by Spitzer on July 01, 2025.Read more

While driving, the vehicle has been going into limp mode at varying speeds on the roadway or while pulling onto the road. The problem lies within the components of the electric throttle body. The system malfunctions and locks the throttle body closed, thus putting the truck into limp mode. Shutting the vehicle off, waiting, and restarting temporarily resolves this issue UNTIL the PCM detects a fau
... lt in the electrical components of the throttle body and sends the truck into limp mode. When this happens, it also triggers a warning message regarding hill start assist. This is an extreme safety issue as pulling onto the roadway or loosing power in traffic and not being able to pull off can lead to collisions especially around blind curves or in heavy traffic. There also isn’t always a location to safely pull off the roadway, potentially leaving the vehicle and its operator in the roadway, increasing the risk of collision and injuryRead more

While driving on the highway, the vehicle engine began to shake violently. The check engine light came on. The car was driving normally prior to this moment. The car has been well maintained and is in excellent condition. I looked it up and found tons of cases that reference a judgement directive from the Theta II Engine Class Action Settlement (Case No. 8:17-cv-00838-JLS-JDE), where the warra
... nty coverage has been extended due to known engine issues. The KSDS software update (SC260) was installed in 2023, by Kia of Orange Park. The vehicle now has approximately 127,121 miles. We towed the vehicle to Daytona Kia who diagnosed the engine as failed, but declined to initiate a warranty claim or submit the case to Kia Corporate because diagnostic trouble code P1326 was not present. I purchased this diagnostic for nearly $200 from Daytona Kia and they verified the second cylinder failed. They blamed it on a sticking fuel injector, but did not verify that this was the issue. Only stating that they believed this to be the case in their review. They did not conduct testing to rule out rod bearing failure or to verify fuel injectors were to blame for the issue. I asked for that. They checked to see if the service manager would be willing to do a BCS test on the vehicle. It was denied. I asked for definitive verification of the cause of the failure. They said I would have to pay for additional diagnostics to start that process. I contacted Kia corporate and asked them to intervene. Kia has denied my warranty claim, stating that the failure either doesn’t qualify under their extended warranty or that the lack of formal documentation disqualifies my case. In a recent interaction with the dealership, they also cited the lack of a specific diagnostic trouble code (DTC) as a reason not to proceed with warranty repairs…despite the fact that many vehicles with this engine fail without triggering that code, and the software update (SC260) was already applied.Read more

The MMI (multi-media interface) frequently reboots during driving, resulting in a black screen and the inoperability of key systems such as the backup camera, rear obstacle detection, navigation, and cell phone tethering (for hands-free operation). These reboots require user input, including entering a PIN to log into the user account (if set up) and repeatedly tapping on the MMI, which can be hig
... hly distracting while driving. By its very nature, the reboot catches the driver by surprise and requires diverting attention from driving.Typically, this user input is provided while stationary, immediately after starting the vehicle. This issue constitutes a safety concern, as reboots can occur during reverse parking maneuvers, rendering the backup camera and obstacle detection sensors inoperable until the system reloads. Additionally, reboots completely disable the navigation screens and necessitate multiple user inputs to restore functionality, which can be hazardous while driving. Many states have enacted hands-free laws and requirements that mandate the use of dash-mounted systems to comply with these regulations. The reboots render such systems inoperative. Numerous reports on online user forums suggest that this issue is widespread and affects the same model year vehicles. For instance, see [XXX] . Several Technical Service Bulletins (TSBs) filed with the National Highway Traffic Safety Administration (NHTSA) indicate that available software updates should be applied to address infotainment-related issues. Relevant TSB numbers include: TSB 2067021, TSB 2076404, and TSB 2069112. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am filing this complaint regarding a known and documented issue affecting my 2012 Toyota Camry (VIN: [XXX] ), which has developed a transmission shudder and vibration between 20–50 mph. This problem is directly aligned with Toyota Technical Service Bulletin T-SB-0079-15 and Customer Support Program ZF4, both of which describe issues with the torque converter and ECM software. My vehicle has 1
... 16,670 miles, which falls within the mileage eligibility of the support program (ZF4: 150,000 miles), though it is just outside the time limitation (8 years). I reached out to Toyota (Case #: [XXX]) to request goodwill assistance due to the safety-related nature of the defect and the fact that it is a well-documented product flaw, but they declined support solely based on time — not on mileage or vehicle condition. The vibration affects the transmission and drivability, particularly during city and highway acceleration, creating potential hazards when merging or navigating traffic. I believe this qualifies as a design flaw with potential safety implications. I respectfully request that NHTSA review Toyota’s handling of this issue and determine whether further consumer protection or a recall is warranted, especially for owners who were not adequately notified or who fall just outside the support window despite clear symptoms. I experienced issues with braking including shuddering brakes. I took the vehicle to a repair shop who looked into any active recalls and there were none, I had the electronic brake control module recall completed in 2019. The shop replaced the front brakes and resurfaced the back which did not fix the issue. They then replaced the vacuum pump which did fix the issue I was responsible for all cost
... s. I recently noticed my vehicle is covered under special coverage N182202780 through 2027 which covers the cost of the vacuum pump replacement. Chevy is refusing to reimburse for the costs because the work was not done at their dealership despite not having a recall that States that. I would like the issue addressed theough a formal recall and reimbursement of repair costs as Chevy is obviously aware there is a safety issue related to braking due to failing components.Read more

This car burned through oil like you would not believe. I was unaware of the issues surrounding the engine in my 2013 Chevrolet Equinox until my engine stalled and completely shutdown while I was driving to work in January 2025. I was very lucky I was the only one on the road at the time and was able to move the car to the shoulder but had I been on the highway, I could’ve died in a wreck that w
... ould cause. Huge safety issue. The low oil pressure light came on right before but hadn’t done so at all prior and I had been super deligent with all oil changes. It truly just burned up all that oil and the engine to this day has yet to start again. Purchased the car used in Santa Ana, CA in. February 2024. Mechanics had looked it over thoroughly during purchase and before we took it across country and found zero issues yet 11 months later the engine was toast. Now I am stuck with a piece of garbage in my driveway that I still owe money on. This has to be a violation of consumer laws now knowing that GM has been aware of this issue for over a decade!Read more

I have had issues with my dual-clutch transmission slipping gears in multiple ways while driving; most commonly while in drive and going downhill the tachometer will aggressively rev when just idling and without pressing the accelerator.
