You cannot wear dark sunglasses while driving this car, as the safety systems are turned off because the car fails to recognize your eyes, as it can't see them through the sunglasses. How does the research and development side of making this ioniq 5 not see this? This ultimately forces you to drive without sunglasses, causing an unsafe environment while driving. This is a bad design for the car.

**Vehicle:** 2021 Hyundai Palisade Calligraphy, ~75,000 miles. Purchased February 2025. **Problem 1 - Forward Camera Failure:** The forward-facing camera system controlling lane keeping assist, emergency braking, and lane departure warning has completely failed. Dashboard warnings appeared in recent weeks including "Check Forward Safety system" and "Forward Safety system disabled" before total fa
... ilure. This eliminates critical safety features designed to prevent accidents, significantly increasing collision risk. Camera unit behind rearview mirror available for inspection. **Problem 2 - Rear Shock Failure:** Rear shock absorbers have failed, causing excessive clunking and compromised vehicle stability. Rear suspension has been clunking since purchase in February. This creates dangerous driving conditions with reduced stability, increased stopping distances, and poor emergency handling. This is a known issue covered under Hyundai Technical Service Bulletin 22-22-001H. **Safety Risk:** Both failures create serious safety hazards - eliminating collision avoidance systems and compromised vehicle control/stability. **Professional Confirmation:** Local Hyundai dealer has confirmed both the camera system failure and rear shock absorber failure through diagnostic testing and inspection. **Components Available:** Both the failed camera system and rear shock absorbers remain available for inspection upon request. **Request:** Investigation and remedial action for these safety-critical defects, particularly given the rear shock issue is a documented manufacturing problem per TSB 22-22-001H.Read more

At approximately 30k miles (but after the 5 year warrantee had expired) the car started giving warnings about the brake system failing even though the brakes worked properly. Shortly after that, and without any warning indicators on the dash, the car began jerking when applying the brakes and down shifting at 35 mph. In the 4 months since the problem started, the jerking has become violent and oth
... er issues have begun. It also jerks when downshifting at 25 mph occasionally. It has a problem upshifting in gears 1-4, especially (but not only) when accelerating quickly. It seems that the gears are slipping. The engine revs higher because the gears are not properly shifting. My 18 month old child, my wife, and I have been put at risk driving in a vehicle that fails to shift properly accelerating through intersections and stopping at intersections. Both a Ford dealership and an independent auto repair shop have duplicated the problem and said the car needs either a new transmission ($8k) or to replace parts in the existing tranny that would cost $6k. Evidently there is a TSB from Ford on this issue, but not a recall. Ford has refused to offer financial assistance.Read more

I am the owner of a 2019 Kia Optima FE with the 2.4L GDI engine (Theta II). Despite following a consistent maintenance schedule with synthetic oil changes every 4,000 to 6,000 miles, the vehicle is experiencing premature engine failure. Specifically, the engine has internal valve damage, and the shop diagnosed the issue as failure of an internal valve component. The vehicle also has a leaking axl
... e inner boot, but the major concern is the engine. Kia has refused warranty coverage, goodwill assistance, or recall support despite this engine being part of a long-standing pattern of issues documented across many Kia and Hyundai vehicles with the Theta II platform. I still owe approximately $10,000 on this vehicle, and now face a potential $5,000+ repair due to a known manufacturing defect. Kia Consumer Affairs and the dealership both declined assistance, claiming my warranty has expired, even though this engine has been involved in class action lawsuits, extended warranties, and TSBs related to premature wear, oil consumption, and mechanical failure. I am submitting this complaint to formally report the issue and to urge the NHTSA to continue investigating Kia’s 2.4L Theta II engine failures, as they represent a potential safety and financial risk to consumers. Many drivers are left stranded or in serious debt due to an engine defect they could not have prevented.Read more

On May 5, 2025, my 2018 Hyundai Kona burst into flames in my garage and completely destroyed my Kona, Dodge Ram 1500 parked adjacent to it and my home along with its contents. Both vehicles and my home were deemed “a total loss” by the insurance company. The Kona engine compartment burst into flames after a short trip to the store. If the front door had not been locked, forcing my wife to go
... through the garage, we would have been unaware of the fire until it had reached the interior of the house setting off the alarms. As it was, I received smoke inhalation and a partially collapsed lung while attempting to put out the fire and while trying to put the burning Kona in neutral to push it out of the garage. The 2018 Kona is not safe as it can burst into flames under normal operation and without any warning potentially resulting in injury or death of the occupants or emergency response crews. Research indicates my 2018 KONA has no recalls; however, it has the same G4FJ engine and is manufactured at the same facility as several other Hyundai models that have been recalled for fires in the engine compartment. The Hyundai Veloster has a class-action settlement for engine failure & fires. Also, the Hyundai Tucson has an ABS module fire recall and shared electrical layout. Furthermore, Australia issued a Kona fire-related recall in 2019 due to brake tube routing near the exhaust system. Fire investigators have inspected the vehicle and determined the origin of the fire is the Kona. Hyundai inspected the Kona and failed to take any responsibility nor notify other 2018 Kona owners. There was no prior warning my 2018 Kona was having any problems. The only indication was a faint burning smell my wife noticed when she got out of the vehicle to collect the mail on the day of the fire. At the time, she contributed the burning smell to the neighbors burn pile. The insurance company has removed the Kona, and it should be available for inspection at their facility.Read more

The failure occurred in the sidewalls of the two factory‑supplied all‑season tires on the passenger‑side (front and rear) of my 2024 Hyundai Santa Fe. Both tires exhibit pronounced bulges on the outer sidewall of the front passenger tire. Tire size is 255/45 R20 105V M+S The rear OEM tire was replaced on February 10, 2025 and front OEM tire (with DOT codes visible) remain mounted on the v
... ehicle, but I must replace it to keep the vehicle operational. I noticed the bulging of the second tire today, July 29, 2025. Sidewall bulges indicate internal cord separation. A sudden loss of air pressure or blowout—especially at highway speeds—could result in rapid vehicle instability, loss of control, and a potentially serious crash, endangering occupants and other road users. No, the problem has yet to be inspected by the manufacturer, police, insurance or others. However there is mounting conserns and simular reports of the same problem occurring with other owners of 2024 Hyundai SantaFes with the same OEM tire. No warning lamps, messages or other symptons prior to the problem occuring. There has been no off‑road use, curb impacts, overloading, or manufacturing advisories at the time of discovery. My best assessment is a potential manufacturing or design defect in the OEM tire sidewall construction causing premature internal separation under normal use.Read more

1. What component or system failed or malfunctioned, and is it available for inspection upon request? Multiple safety-critical systems have failed repeatedly, including: •Engine (multiple shutdowns while vehicle was in motion) •Automated Emergency Braking (AEB) (activating without cause while reversing) •Brake Pedal (intermittently unresponsive with audible rattling) •HVAC (leaking fluid
... into the passenger-side floorboard) Yes, the vehicle is currently with the dealership (Stamford Hyundai, CT) and is available for inspection. 2. How was your safety or the safety of others put at risk? • The engine has shut off without warning while the vehicle was in motion, including in traffic, creating the potential for rear-end collisions. • The emergency brake has slammed on while reversing, without any obstacles present, risking whiplash and collisions with pedestrians or nearby objects. • The brake pedal becomes unresponsive, with a loose/rattling sensation, making it difficult to slow or stop the vehicle safely. • The HVAC leak creates slippery floor conditions, posing a secondary risk for occupants during ingress/egress. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? The dealership (Stamford Hyundai) acknowledged some issues but stated they were unable to replicate all of them. However, the brake pedal defect is consistently reproducible and occurs daily. Multiple videos and logs have been submitted to Hyundai’s consumer affairs team. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle is currently under inspection by Stamford Hyundai. Hyundai Consumer Affairs (case managers Noorpreet and Taruni) have been involved and have reviewed videos and documentation. No inspections by police or insurance. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No.Read more

After receiving a call from Mullinax Ford in Mobile, AL regarding a recall for our 2014 Ford F-150 Raptor (recall related to unexpected downshifting), I brought the vehicle to my workplace on [XXX], where a technician performed the software update (flash) on-site. Before the update, the vehicle was functioning without issue. That same evening while driving home, I experienced extremely hard downsh
... ifting, initially in lower gears, and worsening over time. The truck now violently downshifts, primarily into 4th gear, and sounds as if the transmission is slipping. I drove the truck again the next morning and the symptoms persisted. I contacted Zachary, the original representative at the dealership, who said it may be part of the “relearning” process. My husband drove it a week later after returning from a business trip and he confirmed the issue. It has progressively worsened to the point of being undrivable. We returned the truck to Mullinax Ford for evaluation on 7/29/25, and they claimed the failure was unrelated to the update and that the transmission needed to be replaced, at our expense. The truck unpredictably drops gears at high speeds, creating a sudden jerk that could lead to loss of control. This poses a serious safety risk to both the driver and others on the road. The dealership performed the recall software update and later diagnosed the truck with transmission failure, but is denying responsibility. No other third-party inspections have been done at this time. There were no warning lights or issues prior to the update. The issue started immediately after the recall software was flashed and worsened over the next 24–48 hours. The vehicle is currently at the dealership and available for inspection. We believe the issue stems directly from the PCM/TCM update applied under recall conditions. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The vehicle I rented — a Thor RV based on a Ford E350 — was handed over with a missing lug nut on one wheel and a dangerously loose lug nut on the opposite rear wheel. The component that failed is part of the wheel fastener system (lugs/nuts/bolts). It is not available for inspection now, as the vehicle has since been returned to the rental company. This situation posed a serious safety risk
... — had the remaining nuts failed while driving, the wheel could have detached entirely, potentially leading to a crash or injury. I noticed the issue during a basic inspection many miles after the pickup. I was forced to buy and install a replacement lug nut myself to make the vehicle roadworthy and safe to drive. I have the propf that the vehicule was handed over to me with a missing lug nut as the video I took during tje check in shows the missing lug nut. The issue was confirmed independently by me, and I informed the rental company immediately. They acknowledged the problem and promised to refund the cost of the part. However, it has now been over two months, and they have neither issued the refund nor provided any follow-up. The vehicle was not inspected by police, manufacturer, or insurance, as I managed to resolve the issue myself to avoid driving in unsafe conditions. There were no warning lights or messages — only a physical inspection revealed the problem. I am filing this report because I believe the company rented out a mechanically unsafe vehicle, and others could be at risk if this is not addressed.Read more

Started having issues with the power of the engine, white smoke from exhaust, and loss of coolant. Apparently there are multiple TSBs about this problem, and has been discovered to be an engine design failure for several models under Ford and Lincoln and it is still not a recall. This issue is known to ruin the head gasket and ultimately crack the cylinder head. If I had not spoke to my mechanic
... about the white smoke and power loss, he would not have recommended that I check my fluids and saw I was losing coolant untimely. Due to it leaking into the engine every time I drove my car. He also stated that he’s seen this issue before, found multiple sources around the country that have seen the same issue and basically fixing the issue won’t ultimately fix the problem. Because the engine design is defective, the problem will happen again. The engine would have to replaced. Which costs more than the car is worth. It’s very dangerous to have a faulty engine design and not do a proper recall or replacement plan.Read more
