I am a tall driver (6'4") and I need to wear sunglasses when driving most of the time. The eye tracking device seems generally incompatible with both of these conditions. When the eye tracker fails to recognize my eyes are looking at road it disables driver assist features such as lane centering which can occur unexpectedly and has almost caused me to wreck once. I have tried without sungla
... sses and squinting at the glare also causes the system to fail. I often have to lift my glasses and stare at the sensor to make the warning go away (in effect forcing me to take my eyes off the road. If sunlight hits it, it also fails. So in summary, this device does not work in the daytime. I have contacted the dealer, and they basically read the Hyundai response I've found online verbatim: summarized, you can't wear sunglasses, prescription glasses, be over 6 foot tall and several other conditions and expect the system to work. There is no way to disable the tracker. There is an option the dealer pointed to, but it is ignored when you turn on any of the assist features. Since this system is currently so unreliable, at best, this is an expensive feature I am unable to use. At worst, the distractions it is causing is going to cause an accident. This is an ongoing issue, date indicates first time issue was noticed.Read more

Supplement / update to prior NHTSA complaints regarding my 2021 Land Rover Discovery MHEV (VIN: [XXX] y prior complaints, Jaguar Land Rover has issued Recall 26V248 for DC/DC converter failure leading to loss of propulsion in 2021–2024 Land Rover Discovery MHEV vehicles. My vehicle experienced the same real-world failure condition: sudden loss of propulsion in traffic with my children in the veh
... icle. Jaguar Land Rover corporate has stated my VIN is not included in the recall population despite my vehicle being a 2021 Discovery MHEV and experiencing the same defect condition. I am submitting additional documentary evidence from Jaguar Land Rover Portland showing that during the repair process: * I was advised the vehicle would only run while connected to external jumper power and would shut off once that external power was removed. * Two days later, I was advised the installed battery would only charge to 6 volts and that another battery was needed to continue testing. These communications raise concern that this failure event involved broader electrical consequences and further support that my vehicle experienced the same defect condition described in Recall 26V248. The dealership ultimately completed repairs and charged me for the failure, while JLR maintains my VIN is excluded. I am requesting NHTSA review whether my VIN was improperly excluded from the recall population and whether the affected vehicle population should be expanded. On May 2nd 2026 I was driving my 2021 Tahoe high country with the 6.2 motor at 60 mph and started to slow down before my left hand turn. In the process of slowing down at about 40mph I heard a loud bang/thud noise. The car had completely shut down all motor related usage. I coasted into the shoulder on the side of the road the vehicle would not restart. I had my children and wife in the car. There

It was confirmed upon dealership inspection by the mechanic that an intake valve spring broke and locked up the engine. My 2018 Ford Edge (VIN: [XXX] ), equipped with a 2.0L EcoBoost engine, suffered a catastrophic engine failure due to internal coolant intrusion into the cylinders — a known defect covered under Ford Technical Service Bulletin TSB 22-2229. Approximately four months before the failure, I brought the vehicle to Red McCombs Ford in San Antonio after the check engine light illuminated. Despite this w
... arning, the authorized Ford dealer failed to diagnose the issue or recommend any repair, and advised me it was safe to continue driving. Shortly thereafter, while driving on a busy highway with my grandchildren in the vehicle, the engine suddenly lost all power with no additional warning. We were forced to coast to the shoulder, placing us and other motorists at serious risk of collision. The vehicle was towed immediately to McCombs Ford West (7111 Northwest Loop 410, San Antonio, TX 78238), another authorized Ford dealer in the same Red McCombs Automotive Group, where technicians confirmed the engine requires full replacement at an estimated cost of $7,415.89. A warranty claim was submitted but denied on the basis that I allegedly drove too long after the problem began — a determination I dispute, as the vehicle was towed immediately upon failure without further operation. The failed engine remains at the dealership, has not been repaired, and is available for inspection upon request. My 2024 Mazda CX-90 MHEV has experienced multiple life-threatening sudden engine stalls and a complete loss of motive power while driving in active traffic lanes. ... t technicians stated they could not replicate the issue and returned the car to me as safe. On March 30, 2026, the exact same failure recurred. While driving, the engine suddenly shut off completely at a traffic intersection. After turning the vehicle off for 5 minutes, it was restarted and driven to another authorized Mazda dealership (Crystal Lake, IL) for specialized diagnostic scanning. The second dealership successfully retrieved stored diagnostic trouble code (DTC) P061B, which is directly linked to safety recall 24V-815 (Loss of Drive Power from Software Errors). Crucially, the detailed diagnostic scan revealed that this code was historically stored with a timestamp dating back to December 2024. This proves that the safety defect was active and detectable during the first dealer visit 16 months prior, but was completely missed by the first dealership, leaving my family with an ongoing crash hazard. To address the failure, the dealership replaced the low-pressure fuel pump in the fuel tank and a mass airflow (MAF) sensor. However, Mazda Corporate customer experience has formally stated to me in writing that they are "not in a position to guarantee the concern will not reoccur." Because this safety-critical propulsion failure causes the vehicle to suddenly die in traffic and has a documented history of ineffective recall remediation, it poses an unresolved, severe crash risk. The vehicle is currently in my possession and available for inspection.Read more

I am submitting this on behalf of my daughter, [XXX] . On Friday, May 8, 2026, [XXX] was traveling southbound on Highway 89A approaching [XXX] . While still on the canyon road, her 2020 Chevrolet Equinox suddenly lost all power, including power assist to both steering and brakes. A sudd
... en loss of power steering and brake assist on this road presented a serious risk of loss of vehicle control. Fortunately, she was able to get the car stopped off of the roadway. However, she was then unable to place the car into Park or shut off the ignition. Multiple warning lights were illuminated across the instrument cluster. Her roadside assistance plan towed the car to the nearest repair facility, where on Monday, technicians were unable to replicate the issue. A previously stored DTC P3055 was found in the diagnostic system. This code relates to a known and documented issue with Chevrolet Equinox vehicles, for which Chevrolet has issued multiple Technical Service Bulletins, including TSB 21-NA-139, which identifies the Multifunction Power Supply Converter (GM Part #13510484) as the known culprit for this failure mode. On March 21st, while driving my Hyundai vehicle, I noticed the vehicle began making a ticking sound. After stopping the vehicle, it slowly began catching on fire. The fire ultimately spread and resulted in the vehicle becoming a total loss. This created a serious safety concern and placed me at risk. I later learned there was a recall associated with the vehicle. However, I never received any rec
... all notices because Hyundai reportedly had the wrong address on file. Had I been properly notified of the recall and potential safety risks, I would have done everything in my power to ensure the vehicle was inspected, repaired, and safe to drive. Because I was unaware of the recall issue, the vehicle continued to be operated without my knowledge of the potential danger. The incident caused significant financial and transportation hardship, including loss of reliable transportation and additional financial strain. Hyundai later reached a determination regarding the matter, but I remain concerned about the serious safety risk and the fact that I did not receive proper recall notification before the incident occurred. I am submitting this complaint because I believe the recall notification process and handling of this matter should be reviewed further due to the serious safety risk involved.Read more

Vehicle: 2026 Honda Pilot – Mileage 20,847 Issue: Engine Failure, oil leaks, wheel alignment, air conditioning, major gas leaks from the engine, safety electronics alerts on front dash display are saying the vehicle is not safe to drive. I returned to Ira Honda and once again was given a loaner. Also, the engine was leaking oil. 04-09-2026: Ira Honda called and said one of the engine gaskets was defective and they needed to replace it. They said that now the alignment was back in spec. I was told that the 3 yr engine warranty begins anew. When I got home I looked more closely at the alignment document and saw that the alignment was still slightly out of spec. I called and was told that it was the best they could do. 04-10-2026: air conditioning not working and asked for them to fix the alignment. 04-13-2026: I again brought the vehicle in for service. But was told there were no loaners available. I arrived at 9:30am and at 1:00pm. The A/C was fixed and determined the frame was not set correctly when they replaced the engine. This was the reason the alignment was off by so much. So the alignment was fixed. 04-14-2026: Leaking oil. So again, I had to return to Ira Honda. No loaner was available. 04-18-2024: Saturday morning and we smelled gas in the garage. Started the car and it took about a minute to start. Asked if it was safe to drive, Ira service said yes. 04-20-2026: Started the vehicle up and same issue occurred. However, this time gas was leaking out. It leaked in our garage, on our driveway and all over the street. Again fixed. 04-29-2026: Car again failed to start. During startup the car leaked a large amount of gas. The torque converter(automatic transmission) began failing causing the vehicle to stall when at a traffic light in heavy city traffic allowing for the possibility of being struck by other vehicles because of a restart delay. The only way to bypass the issue we learned was to put the vehicle in neutral when stopping. This reduced the stalling incidents but created other issues in heavy stop and go
... traffic. The vehicle was inspected by a transmission shop and they confirmed that the transmission was failing and the torque converter was causing the issue. There are no warning lights for transmission failure in these vehicles. In fact you can’t even check fluid levels on these transmissions. Only technicians can. A transmission failure on a 7 year old vehicle under normal wear and tear (113,000 miles) is unacceptable and we had to spend 5000.00 for a new transmission in order to remedy the obvious safety issues. Before the repair, my wife managed the stalling issue at 50+ years old by using neutral in between braking so the vehicle wouldn’t stall but imagine a teenager or elderly person dealing with that. I do have receipts for the repair. There was one interstate fender bender. We could not avoid because of people looking at accident on the side of the road. My wife was unable to shift the vehicle into neutral to keep it from installing in traffic and we ended up hitting somebody from behind. Very minor damage on the caravan to the front end.( I have a pic) the other vehicle was a large truck and received zero damage and was OK letting it go. State trooper at the accident people were watching said there were no harm done. Nobody at fault here , both parties agreeable , no injuries , so we elected not to turn in a police report.. This is just a credit card receipt for part of the repair total and we paid cash/check for remainder. Could look up if necessary.Read more

While cruising at 36mph, we decided to initiate the cruise control. Upon reaching a slow-down traffic ahead, the brakes were applied to slow the vehicle, however the cruise control did not disengage and the brakes did not stop the vehicle. The brakes were pressed three times and still the car could not stop. We were in a two-lane road with an opposite direction lane on the other side of the grassy
... median. We saw an opening in between the stopped vehicles and crossed over from right to left lane into the grassy median, hit a ditch, and did a sharp turn into opposing traffic, almost hitting another vehicle as they moved out of way, and stopped the vehicle on the opposite side of the road. When the vehicle hit the ditch crossing the median, the right front tire popped and helped in stopping the vehicle. Four family members including myself were in the vehicle and there were several cars already stopped in the lanes. This incident caused great safety concerns for my family and the vehicles around us. Had we hit the cars while crossing over, it would have caused a chain reaction of "hits" as the cars were already lined up and stopped. We brought the car to the Jeep dealership and they did confirm that the internal valve body failure and TCM fault contributed to improper communication within the transmission. They did replace the TCM and valve body assembly, reprogrammed the module and road tested the vehicle. The auto stop/start warning light came on after the incident. They performed a battery test for the main and auxiliary, and both failed load test and read at low voltages. Both batteries were replaced and fault codes cleared. Upon receiving the vehicle, I tested the cruise control component and it has been working for the past three months now.Read more

I lease a 2023 Tesla Model Y. The rear passenger side seat belt/buckle assembly had previously experienced issues involving unexpected disengagement/unlatching. The vehicle was previously brought to Tesla for service regarding the restraint system/rear passenger buckle area and Tesla represented that the issue had been addressed. On April 4, 2026, during an emergency braking event to avoid anothe
... r vehicle that abruptly cut in front of me and slammed on its brakes, I was forced to brake hard and maneuver to avoid a collision. During that braking event, the rear passenger side buckle released/unlatched while my child was properly secured in a child safety seat in that seating position. As a result, the entire child safety seat violently pitched forward and my child’s face struck the seat in front of her. This incident occurred after Tesla had already been given notice of restraint system concerns involving this seating position and after prior service attempts. The issue raises serious concerns regarding rear passenger restraint integrity and child occupant safety during emergency braking situations. Tesla was formally notified again regarding this incident on May 12, 2026. As of the date of this complaint, approximately one week has passed and Tesla has not responded or attempted contact regarding the reported child occupant restraint failure. The vehicle has not yet been reinspected by Tesla following this incident and remains available for inspection. I am requesting preservation of all vehicle telemetry, restraint system data, diagnostic information, and service records related to this vehicle and prior restraint system complaints/repairs.Read more

After doing some grocery shopping for about 20–30 minutes, I returned to my vehicle, put it in drive, pressed the accelerator, and received no response for approximately 3 to 4 seconds. I then pressed the brake and experienced the exact same issue — no response from the system for about 3 to 4 seconds. I decided to shut the vehicle down, wait approximately 10 seconds, and restart it. After re
... starting, the brake responded immediately (as expected), but there was still a significant delay from the accelerator of about 3 to 4 seconds. I then drove to another parking lot approximately 0.2 miles down the road, parked the vehicle, walked around the store for another 20 minutes, and returned to the vehicle. At that point, everything worked as intended. The accelerator and braking systems both responded properly. On Monday, May 11, I went to the Carvana Chrysler Dodge Jeep Ram location in [XXX] , where I was informed by the Service Manager that they would likely need to keep the vehicle for over a month to troubleshoot the issue with Chrysler before they could attempt a resolution, since the entire electrical system is effectively locked down and controlled by FCA electrical engineers. Also, a loaner was not an option if they kept the vehicle. Less than 5,000 miles on this vehicle with clearly a critical issue and almost no attempt to help the customer. I was on the highway heading home with my [XXX] son in the backseat and without warning the engine shut down and luckily we were on the exit ramp and away from heavy traffic. I pulled over to the side and tried to start it again but it won’t even crank. The cabin heater stopped working too, It was -9 Degrees outside that day and we started to freeze, luckily my wife was able to get to us in 20m
... inutes. The vehicle was towed to GM dealership and they confirmed the engine seized and will need a new engine under special coverage N252494003-03. This was after the recall was completed for this vehicle. It had only 61392KMS on it. We live in [XXX] and it gets extreme winters with below -40degree temperatures. It’s a remote area and there is no service on the highway for 200KMS when heading south to city, if this would have happened on the highway with no services and extreme weather we wouldn’t have made it out alive. We are a family of 5 with a [XXX] and a [XXX] and we bought this vehicle after knowing that the engine recall was completed and GM deemed it safe. Unfortunately that’s a lie and GM is putting peoples safety and life at risk. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am reporting Certification Fraud and severe safety violations by Central Florida Toyota. This vehicle was sold as "Toyota Gold Certified" on March 28, 2026, while having an active safety recall (NHTSA 26V203) with no available remedy. Furthermore, a professional inspection revealed a high-voltage hybrid system leak and structural damage (bondo/rust) on engine mounts, indicating a hidden prior co

While driving my 2016 RAM 1500 Eco diesel, the vehicle suddenly lost power and would not accelerate beyond 40 mph. The paint on my 2014 Kia Sorento (Snow White Pearl) is peeling and delaminating across multiple areas of the vehicle, including the roof and side panels. In several areas the paint has detached completely, exposing bare metal, and rust has begun forming on the roof. The condition appears to be progressively worsening and does not appear consistent with normal wear or isolated paint damage. The af
... fected areas are available for inspection. The peeling paint may create a road or visibility hazard if pieces detach while the vehicle is in motion. In addition, the exposed metal and developing rust on the roof and upper body areas may affect long-term structural integrity over time. This appears to be a widespread issue affecting similar Kia Sorento vehicles with Snow White Pearl paint.Read more
