Several months ago my wife was driving when the automatic brake system engaged when a car was turning in front of her. This engagement startled her and she felt the emergency braking was unnecessary considering the speed and distance the car was in front of her. This incident concerned her and made her lose confidence in the safety of the car. Then just recently we began encountering a almost
... consistent but sometimes intermittent brake System Error along with multiple trouble notices about problems with Road Departure Mitigation System, Lane Keep Assist System, Collision Mitigation Braking System, Adaptive Cruise Control, etc.. when using the car. We received an error code U3000-49 reporting an Internal electronic failure in the multipurpose camera unit. Online searches of this issue seem to be very common and concerning on how many are occurring considering this is a safety system. Our van has approximately 54,000 miles on it.Read more

To Whom It May Concern, I am submitting this letter to formally report a safety-related defect involving my leased Infiniti vehicle, serviced at Edison Infiniti in New Jersey on August 1, 2025. Despite reporting excessive tire wear and a persistent pull to the right, the dealership concluded that the alignment was within factory specifications and attributed the damage solely to a lack of tire rot
... ation. Based on this assessment, I paid out of pocket for a wheel alignment and replacement tires. However, the condition of the removed front tires tells a different story. The tires show extreme treadwear down to the internal cords, a clear indication of prolonged misalignment or defective suspension geometry. This level of degradation is not consistent with normal wear and tear on a vehicle with fewer than 17,000 miles. It compromised steering control and posed a serious safety risk, making it difficult to maintain lane position and safe handling. I believe this issue may reflect a broader defect in alignment or suspension geometry that could affect other vehicles of the same make and model. I have attempted resolution through the dealership and Infiniti Consumer Affairs without success. I respectfully request that NHTSA investigate this issue as a potential safety defect and advise on next steps.Read more

Make: Hyundai • Model: Santa Cruz • Year: 2025 • Trim: Limited • Mileage: 2600 miles • Date of Purchase: July 2025 Description of the Problem: I am reporting a safety issue with the Forward Attention Warning (FAW) system on my 2025 Hyundai Santa Cruz Limited, which unexpectedly disables Smart Cruise Control (SCC) during highway driving. The FAW system, which uses an in-cabin camera
... to monitor driver attentiveness, frequently misinterprets normal driving behavior—such as wearing polarized sunglasses or holding the steering wheel at the 12 o’clock position—as inattention. This causes SCC to deactivate abruptly, resulting in sudden speed changes that create a safety hazard, particularly at highway speeds (e.g., 65–70 mph). The issue occurs multiple times per drive, especially when wearing sunglasses, and persists even after attempting to disable FAW in the vehicle’s settings (Settings > Vehicle > Driver Assistance > Driver Attention Warning > Off). For example, while driving on [XXX], SCC deactivated without warning due to FAW detecting my sunglasses, causing the vehicle to slow unexpectedly and requiring immediate manual intervention to maintain speed. This has happened regularly, creating a risk of rear-end collisions or driver distraction. I contacted my Hyundai dealership and they stated that no software fix is available and the system is “working as designed.” Hyundai’s Technical Service Bulletin (TSB 25-GI-001H) acknowledges FAW issues with sunglasses and hand position but offers no solution. I also contacted Hyundai Customer Service (1-800-633-5151), but they provided no resolution. This issue is widely reported by other 2025 Hyundai owners (e.g., Tucson, Santa Fe) in online forums INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The air conditioning unit has failed and has continual problems, despite having repairs made through the dealership on four separate occasions. The problem regarding the AC has been reproduced and confirmed by both the dealer and another independent service center. Hyundai disabling critical vehicle safety systems if people wear sunglasses. I brought MY2025 hyundai santafe hybrid, this has driver attention monitoring system. Since the day i brought this vehicle i have been getting issues related to driver attention warning system. This error is being caused for no reason, this comes on when vehicle comes on and stays on (sometimes for days sometimes for week
... s and sometimes for hours). HYundai says this issue is because i have a dash cam in car, error wont go away even when dash cam is removed. Recently hyundai engineers said its because you may be wearing sunglasses. What? Wearing sunglasses will disable AEB and cruise control !!!OMG. I removed sunglasses and tried still wont work. Hyundai sent me list of things not to do, including not to laugh, not to shorten your eyes and not to wink. this is a major safety concern, not allowing drivers to wear sunglass which will disable safety critical systems.Read more

I purchased this vehicle as a Certified Pre-Owned vehicle from George Moore Chevrolet in Jacksonville, Florida, in October 2023. At the time of purchase, the vehicle had approximately 35,000 miles, and it now has approximately 60,000 miles. I have maintained all recommended service intervals and oil changes as required. On June 14, 2025, while stopped at a traffic signal with my foot on the brake
... pedal, the pedal suddenly sank to the floor, causing the vehicle to roll forward into traffic unexpectedly. I was able to place the vehicle in neutral and then park to prevent a collision. After turning the vehicle off and restarting it when the light changed, the brakes continued to fail. I carefully maneuvered to a nearby area behind a warehouse and allowed the vehicle to come to a complete stop by shifting into neutral. I contacted a tow truck through my State Farm roadside assistance program and had the vehicle transported to my local mechanic. Upon inspection on June 16, 2025, the technician determined that the brake lines were corroded and leaking. Initially, he suspected the master brake cylinder, but it was confirmed not to be leaking. He noted that the metal-braided portion of the brake line had completely deteriorated on one side and was severely compromised on the other. Both brake lines were replaced to restore braking function. Fortunately, I was able to avoid an accident, but this was an alarming experience that I have never encountered in any vehicle I have owned. My mechanic expressed concern about the extent of corrosion on such a relatively new vehicle and indicated this could represent a broader safety issue. I respectfully request that this matter be documented and investigated to ensure that other drivers are not placed at similar risk.Read more

I am the owner of a 2024 Ford Expedition, purchased new in March 2024. I am writing to formally report multiple instances of complete brake failure, which pose a significant safety risk. The most recent incident occurred on [XXX], at approximately [XXX], while I was driving northbound on [XXX] in Cupertino, California, with my wife and two grandchildren (ages [XXX] and [XXX]) in the vehicle. As I
... approached the [XXX] exit, I began decelerating. While applying continuous pressure on the brake pedal during the descent of the off-ramp, the brake pedal suddenly went soft and sank to the floor. The vehicle failed to slow down and began accelerating downhill, due to gravity, toward the intersection at [XXX] , where the light was red. This total loss of braking control lasted for approximately 5 to 10 seconds and nearly resulted in a serious collision. I was only able to regain control by releasing and reapplying the brake pedal and shifting the transmission into Park, at which point the brakes resumed normal function. A similar brake failure occurred earlier in 2024 under comparable driving conditions. At that time, I believed it to be an isolated mechanical glitch, and therefor did not take it to the dealer. I brought the vehicle to Frontier Ford in Santa Clara the following day ([XXX]) for immediate inspection. Despite ongoing efforts by the dealership and Ford engineering, the issue has not been duplicated or resolved. As of this writing, the vehicle remains at the dealership. I am filing this complaint so that the matter may be officially documented and thoroughly investigated as a potential systemic safety defect affecting this model. I urge you to investigate this issue promptly to help prevent similar incidents and ensure the safety of other drivers and passengers.
