Dash message "Stop safely vehicle will shut off soon" included a red exclamation mark in the middle with a red triangle and a turtle in the bottom right. A yellow reduced speed type indicator in the top right and a check engine light all at the same time. Also a chime sounded. This occured in town moving around 35 mph. Dealer stated this exact VIN had already received a rebuilt trans around 50k
... miles from another dealer. Took dealer 2+ months to repair, and had to call many many times to encourage them to push to get it completed. Manufacturer seemed to delay sending parts because they can - no good reason provided. First Power Inverter Module (PIM) was replaced, but that did not resolve the issue. Then dealer rebuilt entire transmission. Dealer lead tech stated that a roller bearing near the bottom of the trans appeared to have start failing, which then causes a short and or some kind of damage with the electrical "phasers". Multiple conversations with lead technician and Chrysler cares indicate clearly that Chrysler is NOT investigating the issue deeply (they did not want any of the failed components, nor did they ask for details from dealer - are they even trying to resolve root cause.... this has been occurring since the earliest models). Chrysler should have a financial incentive to fix to remediate warranty repairs. Why can't chat GPT find literally anything on the web about the root cause, why is it nothing other than reports of the issue can be found??? Nobody has done a root cause analysis. Is Chrysler paying to minimize bad press??? If Chrysler replaced entire trans with rebuilt one at 50k and now it failed again at 80k - when can I expect it to fail a third time?Read more

On Sunday, September 21st, 2025, my 2013 Kia Optima EX suffered sudden catastrophic engine failure while driving on the highway. The check engine light was flashing, the car misfired badly, lost complete power, and became unsafe to operate. The vehicle was then towed to Wagner Kia of Shrewsbury, MA. On Monday, September 22nd, Kia diagnosed the engine with holes in the intake valves (Cylinder 3)
... and multiple misfire codes (P2191, P0272, P0301, P0304, P0172, P0303). The dealership documented that major engine repairs would be required, including two intake valves, two exhaust valves, valve cover gaskets, hraf bolts, two ignition coils, and a valve job. This sudden engine failure created an immediate safety risk of stalling or losing power while in motion, which could have led to an accident. Despite Kia’s history of Theta II engine recalls and settlements, my VIN is not included, leaving this major engine defect unaddressed. I am requesting that NHTSA investigate and consider expanding Kia’s existing engine recalls to cover this engine failure, as it represents a serious safety issue consistent with Kia’s documented engine problems.Read more

I own a 2022 Chevrolet Suburban (VIN: [XXX] ) that is included in NHTSA Recall No. 25V274 / GM Recall No. N252494000 for engine defects in the 6.2L L87 V8 engine. My vehicle was inspected at an authorized GM dealer today, on 9/23/2025 (Don K in Whitefish Montana). Instead of replacing the engine, the dealer performed the recall “remedy” of switching to higher-viscosity oil, changing the oil c
... ap and filter, and providing an owner’s manual insert. However, the dealer service manager specifically told me that my engine will eventually fail due to the known defect and advised me not to drive the vehicle on long trips. He also told me to be careful driving if I have children, and it is only a matter of time before the engine fails, which will look like sudden shuttering, loss of acceleration and it will no longer run. He told me wait time to get a new engine after it fails is currently 8 months. This is alarming because the recall notice states that engine failure can cause sudden loss of propulsion without warning, which could be catastrophic going across multiple lanes of highway (from my home street), and entering and exiting my street via the 4 lane highway "suicide lane". I am very worried about this in the coming winter season with snow and ice. The current “oil upgrade” remedy does not resolve the underlying manufacturing defect in the connecting rod and/or crankshaft. Even after the recall service, my vehicle remains unsafe to operate, and the dealer’s own warning confirms this. I request that NHTSA review GM’s recall remedy to ensure that it adequately addresses the safety defect. A true remedy should involve replacement of the defective engine components, not just an oil change, or an immediate buy back of my automobile by the company so I can purchase another safer vehicle. My family and I do not feel safe driving this vehicle, and the manufacturer’s current approach does not eliminate the risk of catastrophic engine failure. The component that failed was the steering damper. This results in the vehicle violently shaking at freeway speeds after hitting a bump. This has resulted in me having to pull over to get the issue to stop on multiple occasions. In addition, it can be difficult to keep control of the vehicle when this is happening and there is no set way to make it stop once it has begun. It is unpredictable, as i

This is the 5th time that it has been taken in for the same issue. The component has failed 4 times since I got the car 3 years ago. I have taken the car to the dealership 5 times for the same issue. The first time I brought the car in for the issue, they replaced several parts, but not the steering damper. Within a month I brought the car back in for the same issue. Each time since then, they have replaced the steering damper along with other parts (Invoices will be attached to this report with part names and numbers). Each time the vehicle was taken in to the dealership, the mechanics were able to reproduce the issue. This is documented on each invoice I received. No warning lamps, messages, or any other warning signs were present prior to the issue occurring. The issue first occurred in February of 2023 (10 months after purchasing the car new). It was taken to the dealership for the first time on February 10, 2023. Accident On [XXX] at [XXX], I headed out of the Timberlane Estates neighborhood to go to volunteer at the NWR in Crystal River and was behind 3 other cars going out of the same neighborhood. As I started to turn from [XXX] (25 mph) to [XXX] (decelerating to about 10), I had to fight the steering wheel trying to straighten it out but instead the car continued left and hit a car at the stop sign.
... This shook me up quit a bit. When I got out, the driver of the hit car and I traded information. We both have State Farm Insurance. Called State Farm to report the incident and took pictures of the front of both cars and of the skid marks. Called my husband and as we waited for the tow truck, we realized that the CV joint (?lower all joint) was totally out, which also caused the tire to flatten as well. We realized from the skid marks that this occurred as I was starting my turn. That’s why I lost control. There were no injuries, though I was shook up thru the next day and sore in my right wrist, shoulder and neck. Car went to Jim Hudson Collision Center Crystal River, FL. Car was inspected by them and the State Farm Insurance adjuster. The attached photos are from the Hudson Collision Center. I have a few taken at the accident site if you want them too. My 2018 Jeep Wrangler JL Sport is experiencing multiple premature engine failures that present a serious safety risk: •Vehicle overheats while driving. •Recurrent misfire on Cylinder #5 (DTC P0305). •White exhaust smoke with a sweet coolant smell on cold start. •Coolant loss despite recent refill. •Oil contamination with metallic particles found during inspection. •Valvetrain noise fro
... m the right bank, suspected intake camshaft wear. These conditions create a risk of engine seizure or sudden stalling while driving, which could cause a crash. Manufacturer awareness: Jeep/Mopar has issued bulletins acknowledging these exact problems, including: •TSB MC-11018482-0001 (May 2025): Intake camshaft/rocker wear causing misfires (P0301–P0306). •Customer Satisfaction Notification ZD8 (2023): Coolant inlet tube fastener issue causing coolant leaks and overheating. My vehicle’s symptoms directly align with these known manufacturer issues. Despite this, Jeep/Mopar has denied goodwill repair coverage. Safety Concern: This defect is not just a performance issue — it creates a high probability of sudden engine stall or seizure, endangering me, my passengers, and other drivers. I am requesting NHTSA investigate premature engine failures on the Jeep Wrangler JL (3.6L Pentastar) and require Jeep/Mopar (Stellantis) to address this safety hazard.Read more

2022 QX60 equipped with a Panoramic Moonroof. While in Florida 850 miles from home, I was traveling alone at about 35mph when I heard a loud explosion. It sounded like a gun shot at close range. I kept driving but observed my surroundings and there was no traffic, no construction, nothing overhead, no warning lights. When I got to my destination a couple miles away, I inspected the vehicle, fu
... lly expecting to see a bullet hole or shattered rear window from a bullet. What I found was the front moonroof had shattered and mostly still intact. It had blown up and out and not inward as it would if it were hit with a projectile. I contacted the nearest Infiniti dealer in Mobile Alabama 100 miles away, but they couldn’t look at it for at least 3 weeks. I covered the moonroof with heavy plastic and duct tape to keep the elements out of the vehicle and keep the moonroof intact as much as possible to help with Infiniti’s investigation. The cullet that had fallen into the sunshade was vacuumed up and kept also. After a tense 850 mile drive home, at times in torrential downpours, I contacted our local Infiniti dealer who wanted to inspect the vehicle. They informed me that their investigation could not determine a cause, but they did know it was not a warranty issue and denied it. I had the moonroof replaced by a local glass repair shop and then had to take it to a body shop to have the headliner dropped so cullet could be removed. I talked to numerous Infiniti representatives throughout this process, and everyone asked if I filed an insurance claim, I had not. Insurance should not have to pay for a manufacturer's defect that is still under warranty. As an Engineer for a tier one auto supplier that manufactures automotive safety glass for most major automobile companies, I can say with a high degree of certainty this was due to a manufacturer's defect or a design flaw. Due to the circumstances at the time, Nickel sulfide (NiS) inclusion is the most likely cause.Read more

The indicator controls on the steering wheel are unreliable. Soon after purchasing the car, I returned it for warranty repair because sometimes the right indicator fails. When pressing the indicator area in the steering wheel, one can usually feel a button click, but at times it’s like pressing a brick wall, no give and no click. It seemed ok after the repair, but now it’s back to doing exact
... ly the same thing. In addition, sometimes when I press near the bottom of the right indicator area, the left indicator turns on instead. It’s bad enough that I often can’t indicate when turning as I can’t always keep track of directions when the buttons turn upside down and the visuals on the buttons are backwards. It’s much worse that this bad design is so poorly implemented. Now writing this, I noticed when uploading the warranty invoice that they said the indicator works as intended. This after I gave them a video I took, while driving, of my fingers repeatedly pressing the indicator area as hard as I could, while describing how the indicator wasn’t turning on. I see you don’t allow videos to be uploaded here, but I still have that video and can supply it if needed.Read more
