The contact owns a 2003 Toyota Camry. The contact stated that the vehicle would suddenly accelerate or lunge forward and had done so several times, both while idle, pressing the brakes, and while driving at various speeds. Suddenly accelerated downhill, and when approaching the inner section, the contact immediately turned the vehicle to avoid hitting another vehicle and ended up crashing into a n
... earby residence's garage, causing the vehicle to stop. The air bags did not deploy. The garage door was damaged, and the contact's vehicle's front-end was damaged. A police report was filed. The contact had stated they had experienced sleep issues and a fear of driving the vehicle since the incident. The vehicle was able to be driven to the contact's residence. The vehicle had not been driven since. The contact spoke with a certified mechanic who informed them it was a known issue and of NHTSA Campaign Number: 09V388000 (Vehicle Speed Control), which related to their experienced failure. The mechanic did not repair the vehicle. The dealer was contacted, and it was confirmed that the VIN was not included in NHTSA Campaign Number: 09V388000 (Vehicle Speed Control). The manufacturer was informed of the failure, and they too confirmed that the VIN was not included. The manufacturer referred the contact to the NHTSA Hotline to file a complaint. The failure mileage was approximately 75,000.Read more

On April 1, 2026 at approximately 7pm the vehicle had a complete engine failure exiting an interstate. As I was slowing to exit the interstate the truck, without warning, lost power and locked up the rear wheels. I was able to safely exit onto the off ramp, but did partially block the off ramp for 45 minutes waiting on a tow truck. Once I came to a complete stop the dashboard began displaying n
... umerous error codes, included "key missing, restart truck and shift to park, and start failure". The vehicle was part of the GM L87 engine recall but had previously passed the PICO test and was deemed OK to drive. I had the vehicle towed to the closest GMC dealership where it was diagnosed as an engine failure and the engine was locked up. The vehicle has been at the GMC dealership since the night of April 1, 2026 with no date given as to when it will be fixed. The vehicle had displayed no signs of any problems prior to the engine failure on April 1, 2026.Read more

The lane keep assist has never worked. It doesn't not keep you in the lane, it just chimes when you drift out of it. The bigger issue is the axles. I only owned the car for 1 month when the axles started loudly clicking with every turn. Honda is claiming this is just a cosmetic defect and it does not affect safety, but I don't believe that for a second. Clicking axles are NOT normal, and could o
... nly indicate signs of deterioration. Not only that, but myself and anyone else with this issue can feel that the drivability is affected. Turns and propulsion are slower as time goes on and the clicking gets louder. Honda knowingly continues to sell these cars for up to $60,000 and no longer offers a remedy for the faulty axles. They now plan to discontinue the vehicle and offer no further support for it, which is completely unnaceptable. This safety defect must be remedied as soon as possible. We should not wait until someone is dead.Read more

The rear bunk end door on this 2017 Jayco Jay Feather X17Z hybrid travel trailer does not fully latch or lock while traveling at highway speeds due to a known widespread manufacturing defect. Jayco factory installed canvas retainer strips with no pre-drilled holes and no washers causing the plastic retainer strip to crack and fail. This caused the entire rear bunk end frame to shift out of alignme
... nt making it impossible for the door latch to fully engage while towing at highway speeds. This is a documented defect affecting hundreds of identical units. Jayco was already replacing these under warranty proving they knew about the defect. Jayco subsequently redesigned the attachment system eliminating screws through plastic entirely — part numbers 0858182 and 0858183 — proving the original design was inadequate and dangerous. The bunk door not fully latching at highway speed poses a direct safety hazard to other drivers on the road.Read more

This vehicle has ongoing drivability issues that have not been resolved despite multiple repair attempts involving engine-related components and repairs. The issue began in November 2024 while the vehicle was under warranty with coolant and engine warnings. At that time, I reported coolant loss, warning messages, hesitation during acceleration, and abnormal vehicle vibration. I was initially told
... no issue existed. Shortly after warranty expiration, the vehicle required a major head gasket repair in 2025 after continuing to exhibit the same coolant-related issues. In 2026, the vehicle required an additional engine-related repair involving a camshaft seal in the same area of the engine that had previously been dismantled and rebuilt. Despite these repairs, the same drivability symptoms remain. The vehicle intermittently hesitates during acceleration, particularly from a stop or during merging conditions, where engine RPM increases significantly before the vehicle responds. The vehicle also exhibits significant steering wheel and seat vibration at highway speeds and during extended driving. These conditions create a potential safety concern because the vehicle does not consistently respond as expected during acceleration and becomes unstable and uncomfortable to operate at highway speeds. The hesitation creates concern when entering traffic, crossing intersections, or attempting to accelerate in situations requiring predictable vehicle response. The issue has been repeatedly presented to the dealership and repair attempts have been performed, including major engine work, but the condition remains unresolved. The repairs were treated as separate isolated issues rather than a continuation of the original condition first reported under warranty. The vehicle has been out of service for approximately 43 days across these repairs. No loaner vehicle was provided during those periods. The vehicle remains in its current condition with unresolved issues.Read more

... rectified. I currently have videos and pictures from when this started to this current time. There are also other customers that have the same complaints that I do. Below is a list of issues that have been going on since first complaint including continued first complaint. I do believe that this is an issue and is a danger due to the reverse camera glitching due to the screen, not being able to get out of the screen, sometimes can not leave a parking space due to the sensor says that there is something in the way where indeed there is nothing. Also can not pull out of drive way during snow due to the car automatically breaking for snow. The collision for the front is always going off when there is no need for it to go off as this can cause someone to unexpectedly stop due to this being something that is supposed to be a warning. If this could happen to the vehicle constantly without a solution then this could be connected to something much more serious. There are other issues that have gone on as well that I have slowly started to notice and pay attention to. I do believe that the amount I am paying for this car it should not have these issues with out explanation. Clearly this is common and on going. One thing can lead to another with these issues. On 4/27/26, my family and I were traveling south on Colorado C470 at 8 AM. Our 2021 Chevy Silverado 1500 had catastrophic engine failure at highway speed as we were passing an 18 wheeler. The failure caused the vehicle to lose power and engine, and the display said to put the truck in Neutral and restart the engine. After coasting to the shoulder, the engine would not restart. We were stranded on
... the highway during rush hour for an hour and 1/2 waiting for a tow truck. This scenario is identical to the safety risks outlined in NHTSA Recall 25V274 (and investigation PE 24-007). This failure resulted in the intake manifold being physically displaced and damage to the timing assembly. The dealership is attempting to deny coverage by claiming the "upper gallery" damage (timing/intake) is unrelated to the "lower end" issues covered under the special coverage/recall [ this is a Dealer Quote]. This logic is technically flawed: Consequential Damage: A catastrophic lower-end failure (e.g., a broken connecting rod or seized crankshaft bearing) at 80 mph creates the massive internal pressure required to shatter or "blow off" a plastic intake manifold. Timing Linkage: The timing chain is physically connected to the crankshaft. It is mechanically impossible to have a catastrophic crankshaft failure at high RPMs without causing secondary damage to the timing gears and "upper gallery". Recall Precedence: My vehicle exhibits the exact failure mode—loss of motive power due to bearing failure—currently being investigated by the NHTSA for 2019-2024 L87 engines.Read more

I am reporting a catastrophic failure of the rear seatbelt restraint system in my 2025 Mazda 3. The rear seatbelt guide geometry is badly designed, allowing the webbing to bunch and jam during normal operation. This physical obstruction prevents the seatbelt from retracting, leading to three severe safety hazards: 1) Total failure to restrain occupants. 2) Structural damage to the vehicle's safe
... ty cell. 3) Public hazard where the obstructed door rebounded open into oncoming traffic. Restraint Failure: On multiple occasions, the rear seatbelt jammed in the guide clip while in use. Due to the lack of retraction, a 200lb adult passenger was able to fully exit the vehicle while the belt remained buckled. Most alarmingly, the vehicle's safety logic failed to trigger any occupancy warning alarms. The system remained "satisfied" because the buckle was engaged, despite providing zero tension or restraint to the occupant. Structural Damage & Public Hazard: Because the belt failed to retract, it remained loose and obstructed the rear door's closing path. The door rebounded off the slack belt into oncoming traffic, resulting in a significant structural dent to the C-pillar (door jamb) immediately adjacent to the striker. This deformation occurred in a critical area for side-impact protection. Efforts to Resolve: The vehicle was inspected by a dealer, who claimed the system was functioning normally despite being shown the jamming. I escalated this to the Mazda North American Operations Executive Office. The executive team issued a final dismissal, refusing to provide technical answers or acknowledge the documented failure points. Crucially, they denied my request for a Regional Technical Manager (RTM) to perform an engineering review of the guide geometry and refused to inspect the damage to the C-pillar. By refusing a corporate inspection, Mazda has ignored a reported life-safety defect and its resulting structural damage. This is documented on a video providedRead more

The break pedal sticks down during high temperatures ( generally during summer months especially when park outside in the sun and high humidity) and causes the brakes to drag and the break pedal doesn’t come up by itself! Also the brake lights stay on. It is very dangerous to drive on this condition. I just want theNHTSA to be aware of this and I have seen a lot of complaints on line I contacted

2018 Kia Sorento, under 88,000 miles, with Theta II engine defect. KSDS update installed. Oil consumption issues requiring adding at least one liter of oil every 1,000 miles in between regular oil changes/ maintenance. Oil warning lamp does not illuminate at all or only blinks on for a second then stays off, even when oil is less than 1 liter. Noticed slight jerking/hesitation at acceleration fo
... r a few days, then experienced shaking and heard vibrations when breaking and idling. Within a few minutes, MIL appeared with audible warning, "Take this vehicle to a Kia dealership." Immediately parked car. Towed to Kia dealership. No record of DTC P1326 found, which would have qualified vehicle for new engine under PI1802 Y/Z Theta II Extended Warranty. Diagnostic found misfiring cylinder and fouled spark plug. Technician in the video stated "oil consumption." Due to absence of P1326 code, service manager suggests we replace fouled plug and return with the vehicle when the same issue happens again. This does not follow Kia's own guidelines in PI1802 Y/Z campaign for a couple of reasons. First, activation of the MIL in conjunction with the audio warning indicates the Knock Sensor P1326 code should have been generated and stored, but the warning system must have malfunctioned. Service dept. did not to inspect the knock sensor harness for damage or oil saturation which could have kept the code from registering. Second, Service dept. did not consider shaking and vibration as "engine noise," which would have required them to perform a BCT or test for connecting rod bearing noise, providing a pathway to request Techline PWA approve replacing the engine under warranty. Kia's Product Improvement Campaign for Theta II engines ought to prevent injury or death resulting from catastrophic engine failure. My vehicle presents clear evidence of engine failure progression at different stages. Rather than navigate the warranty via a Techline, Kia has advised me to continue driving.Read more

I was driving on the highway with my husband and our dog when out of nowhere the panoramic sunroof exploded. We were traveling home and could have been in an accident when it occurred because it sounded like a gunshot hit the car. If we had not been calm during the incident, not only our lives but the lives of everyone around us could have been affected. There were no warnings about this and whe
... n we purchased the vehicle in 2023 the dealership didn’t even say anything about a possiblity of this happening. I did research after it happened to make sure there wasn’t a recall we missed and I came across lawsuits because of this.Read more

While driving the vehicle under normal conditions, multiple advanced driver assistance and safety systems suddenly became unavailable without warning. The systems affected include Automatic Emergency Braking, Forward Collision Warning, Lane Keeping Assistance, and related safety features that rely on the forward-facing camera. The failures occurred at the same time and were indicated by warning m
... essages displayed on the instrument panel. The vehicle was inspected by an authorized dealer, who confirmed a diagnostic trouble code indicating an internal electronic failure of the forward multipurpose camera. The failed component is installed behind the windshield and is available for inspection upon request. The dealer stated that no accident, physical damage, misuse, or external cause was identified. After the camera failure was identified, the vehicle also briefly displayed a transmission system warning message that later cleared on its own. In addition, the center display screen has shown unstable behavior while driving, including unexpected resets and flashing back to the navigation screen. The sudden loss of collision mitigation, lane assistance, and warning systems reduces the vehicle’s ability to assist in avoiding crashes, particularly in emergency situations, and increases safety risk to the driver, passengers, and others on the road.Read more

My vehicle would suddenly downshifted to first, constantly seek for gears, loss of traction from not finding gears, and transmission died at 120k miles.
Incident date: Feb 10, 2026
The steering wheel has started to jerk left then comes center and goes right. (Likely sticky steering). Under 100k miles and happens mostly on the highway but has started to be on slower roads. No accident as of now, but common issue with this vehicle.

2025 Dodge Charger EV | VIN: [XXX] | Purchased: April 11, 2026 | Dealer: Thunder Chrysler Dodge Jeep Ram, Bartow, FL I am reporting multiple active, unresolved safety system failures that began within 24 hours of purchase and remain unrepaired after two dealer visits. On April 15–16, 2026, the dealer documented NINE fault codes across FIVE modules, including an active Electronic Throttle Modu
... le fault that would not clear — a direct risk of unintended acceleration or loss of speed control. Additional active alerts included ABS failure, brake system failure, airbag system failure, suspension failure, and blind spot system failure. The only repair performed was a software flash. No safety systems were physically repaired. Alerts returned May 2, 2026. Pre-sale service history (undisclosed at sale) shows five battery-charging failures, a July 2025 software fault, and a complete no-start event on December 18, 2025 requiring updates to four electronic modules. A post-purchase repair invoice (April 2025) also documents a Collision Thrust Angle Alignment — a frame geometry procedure performed after structural impact — raising concerns about airbag deployment zone integrity.
