I am reporting a serious safety issue involving my 2017 Ford F-250 diesel truck. While towing a 34-foot fifth wheel trailer and traveling from [XXX] to [XXX] , the vehicle experienced a sudden and dangerous failure near [XXX] . The engine warning light appeared, the vehicle went into low power mode, and then completely shut down on the highway. This created a serious safety hazard, particularl
... y while towing a large trailer at highway speeds. The vehicle was towed to a repair facility where it was determined that the high-pressure fuel pump had catastrophically failed. The pump had developed a significant crack and released metal debris throughout the entire fuel system. This required extensive repairs, including: - Replacement of the high-pressure fuel pump - Replacement of fuel lines - Full fuel system cleaning, including the fuel tank The total repair cost exceeded $14,000 USD (near $20,000 CAD). The mechanic indicated that this type of failure should not occur under normal operating conditions. Additionally, the Ford parts dealer advised that the failed fuel pump could be returned for credit, suggesting the failure may not be consistent with normal wear and tear. This failure occurred suddenly, without warning, and resulted in a complete loss of power while driving, which I believe represents a significant safety concern. I respectfully request that NHTSA investigate this issue as a potential safety-related defect involving high-pressure fuel pumps in diesel vehicles. Tesla is in a protracted battle with me over fees related to damaged property and illegal self-help repossession procedures. They have been formally notified that any trespass onto my property is not permitted because they said they sent armed repossession agents because of “angry customers” like me. I currently have a complaint for those issues filed with the Attorney General of Pennsylvania.
... Tesla’s recent attempts to repossess the vehicle were initiated approximately two hours after informing the Pennsylvania Attorney General’s office that Tesla had not contacted me in any way, that Tesla was not being factual. Tesla’s response to that was again to not contact me but to send me a form letter stating they intend to disable my vehicle remotely. I am a privacy-minded person, and part of choosing the vehicle was a modicum of privacy guaranteed by Tesla. Tesla publicly says they do not so this, I was told they do not so this in the course of buying the car, and recent news seems to state they say they can’t do it. They have yet to repair a windshield and cabin camera that seem to be damaged. In other words, they intend to disable and lock my car without my permission or knowledge, without verifying the contents of the car. In other words, if there happens to be an occupant at the time they illegally disable the car, they intend to kill them. I’m asking that my contact information be kept private because they have already threatened to use lethal force, make extremely reckless decisions regarding their procedures, and retaliated by initiating this process when I informed a government agency that they lied to them.Read more

Shifter cable bushing failure - 1st recall in 2019, took to Dodge, was replaced in April of 2020. October 2025, safety recall replacement for the 1st recall fix as the shifter cable bushing failure, and as of May 12th 2026, there is NO remedy. ... ft to either P, D, N and R, I have to manually get out of the car, open the hood, reach inside the hot engine to manually shift the gears.I contacted Stellantis Customer Service that handles recalls. The ONLY option they had for me for this October 2025 recall was to get my car fixed at a mechanic or Dodge dealership and send them the paid invoice so they could reimburse me for the money I spent. I told them that was unacceptable to me because 1.) how can you announce a national public recall in October 2025 and it's May of 2026 and STILL do and not have literally the small part (bushing size) that you've recalled? 2.) Dodge telling me to get the part fixed for $500 or more at a mechanic when the part that doesn't exist ANYWHERE that fits my Dart? Make it make sense! Meanwhile, everyday, I risk burning myself every time I have to reach into my hot engine to manually shift gears. This is dangerous and risks litigation if I burn myself anymore. Why is Stellantis allowed to get away with this? I refuse to pay and get reimbursed when it's a recall. Worse yet, how do I get it fixed if the part apparently isn't available? Again, make it make sense... Dodge is responsible to fix recalls on their dime, not mine.Read more

The Auto High-Beam system activates the high beams inappropriately during operation. The system triggers the high beams directly into the eyes of oncoming traffic and activates during early dusk conditions. The sudden, intense illumination blinds oncoming drivers. This creates an immediate glare hazard and significantly increases the risk of a head-on collision. I reported this specific failure t
... o the authorized dealership on three separate occasions, engaging three distinct service advisors during the active warranty period. The service department omitted these complaints from the formal repair orders. The first advisor stated the system was functioning normally. The second advisor stated the technicians completed an inspection and verified the system as fully operational. The third advisor acknowledged experiencing the exact same malfunction on their personal vehicle and instructed me to manually disable the safety feature. The system fails silently, displaying zero warning lamps or error messages prior to or during the malfunctions. The attached service invoices document a severe, underlying instability within the vehicle's electrical and Advanced Driver Assistance Systems (ADAS) network, culminating in a complete VSA modulator replacement. These records establish a confirmed administrative pattern of the dealership dismissing intermittent safety faults, officially logging statements such as "unable to verify issue" and "no problem found" on subsequent visits. This evidentiary paper trail substantiates the dealership's systemic suppression of my specific, repeated verbal reports regarding the Auto High-Beam safety hazard. The vehicle and the Auto High-Beam system remain available for inspection upon request.Read more

I am submitting this complaint regarding my 2021 Kia Telluride experiencing repeated and dangerous electronic system failures while driving. Kia of East Syracuse (NY) diagnosed the issue as related to the backup camera assembly and communication/network failures. While driving, the vehicle experienced loss of power steering assist, instrument cluster failure, speedometer malfunction, incorrect gea
... r indication, backup camera activation while driving forward, blind spot warnings activating incorrectly, flashing/blank temperature display, and multiple driver assistance system failures. The speedometer displayed 0 mph while the vehicle was in motion and later displayed approximately 40 mph while stopped at a traffic light. The vehicle also became stuck in reverse while backing out of my driveway and would not shift properly, leaving the vehicle stopped in the roadway. Multiple safety systems malfunctioned simultaneously, including Blind Spot Collision Avoidance Assist, Lane Following Assist, Rear Cross Traffic Collision Avoidance Assist, Smart Cruise Control, Parking Distance Warning, and Forward Collision Warning systems. The dealership documented multiple fault codes and stated the dashboard lit up “like a Christmas tree” while the instrument cluster stopped functioning entirely. These failures create a serious safety risk because critical driving information and driver assistance systems become unreliable or inoperative while the vehicle is moving. The failures occur unexpectedly and without warning. Photos and video evidence have been retained, including video of the backup camera activating while driving forward and the speedometer malfunctioning while the vehicle was in motion. I am asking NHTSA investigate these failures as a potential safety defect involving the camera system, instrument cluster communication systems, and CAN network communication failures in the Kia Telluride.Read more

About 6 months ago, while driving my kids to school, the transmission aggressively downshifted causing the truck to skid momentarily. This occurred on the highway so the transmission was most likely in 6th gear. The event was so abrupt, it felt like it either switched into Park or 1st gear and felt like we hit something. A wrench icon appeared and the fan was abnormally loud so I pulled over and t
... urned the truck off to see if there was any damage which there wasn't any. The truck wouldn't start so we were stuck on the side of a busy highway during morning rush hour traffic. When it finally started about 2 1/5 hours later, I hooked up a code reader which indicated codes P0706 and P0707. Ever since, this condition occurs after the truck's been driving for about 5-10 mins without exception. The truck seems to stay in the current gear that was engaged at the time that the wrench icon appears. So if the truck is a stop and the issue occurs, it'll be stuck in 1st and run at high rpm. If it happens when the gear is in 5th or 6th and you then come to a stop, it makes pulling out in traffic dangerous due to the lag while getting the truck up to speed. I've been out of work and couldn't spare the $1200 repair cost. I've had to avoid highways and speeds above 30 mph in fear of crashing or damaging the transmission. Even at low speeds, transmissions shifts are hard and abrupt so I'm concerned that there's transmission damage. There's a burning belt smell in the cabin so I'm also concerned that's there's advanced belt wear damage due to the high fan condition caused by this. I have to leave the truck running if I have to go anywhere since it won't restart for approx 2 1/2 hours after I turn the truck off. Other symptoms noticed while driving: the gear indicator below the speedometer glitches. It'll change to other gears on the indicator while not actually changing physical gears. The gear indicators will disappear then reappear randomly.Read more

While driving, the engine suddenly shut off completely at a traffic intersection. After turning the vehicle off for 5 minutes, it was restarted and driven to another authorized Mazda dealership (Crystal Lake, IL) for specialized diagnostic scanning. The second dealership successfully retrieved stored diagnostic trouble code (DTC) P061B, which is directly linked to safety recall 24V-815 (Loss of Drive Power from Software Errors). Crucially, the detailed diagnostic scan revealed that this code was historically stored with a timestamp dating back to December 2024. This proves that the safety defect was active and detectable during the first dealer visit 16 months prior, but was completely missed by the first dealership, leaving my family with an ongoing crash hazard. To address the failure, the dealership replaced the low-pressure fuel pump in the fuel tank and a mass airflow (MAF) sensor. However, Mazda Corporate customer experience has formally stated to me in writing that they are "not in a position to guarantee the concern will not reoccur." Because this safety-critical propulsion failure causes the vehicle to suddenly die in traffic and has a documented history of ineffective recall remediation, it poses an unresolved, severe crash risk. I have made efforts to have the vehicle inspected and repaired, yet the condition persists, resulting in continued frustration and unexpected expenses. I respectfully request the following: * reimbursement for repair costs related to the engine issue * Documentation regarding any recalls, technical service bulletins, or known engine defects associated with this model * Assistance in resolving this matter promptly and fairly I would appreciate a written response outlining the next steps to resolve this complaint. On March 21st, while driving my Hyundai vehicle, I noticed the vehicle began making a ticking sound. After stopping the vehicle, it slowly began catching on fire. Had I been properly notified of the recall and potential safety risks, I would have done everything in my power to ensure the vehicle was inspected, repaired, and safe to drive. Because I was unaware of the recall issue, the vehicle continued to be operated without my knowledge of the potential danger. The incident caused significant financial and transportation hardship, including loss of reliable transportation and additional financial strain. Hyundai later reached a determination regarding the matter, but I remain concerned about the serious safety risk and the fact that I did not receive proper recall notification before the incident occurred. I am submitting this complaint because I believe the recall notification process and handling of this matter should be reviewed further due to the serious safety risk involved.Read more

The infotainment system in my Volkswagen ID.4 is experiencing intermittent failures while driving. The issue primarily affects the vehicle’s connectivity with my mobile phone (Apple CarPlay / Android Auto), music playback, and the use of navigation applications such as Google Maps. In several instances, the central display has also gone completely black, temporarily becoming unresponsive without
... warning. The affected component is the vehicle’s digital infotainment system, including the central touchscreen, Bluetooth connectivity, and multimedia and navigation functions. This system is available for inspection upon request. While driving, the system randomly disconnects from my phone, which interrupts Google Maps navigation, music playback, and other phone-based functions. This is particularly concerning because I rely on Google Maps for daily navigation, as the vehicle’s built-in navigation system is not sufficiently accurate or reliable. In my experience, the factory navigation does not provide strong real-time traffic detail and sometimes routes me through significantly longer or less efficient paths. The only advantage of the built-in navigation system is that it displays information on the driver’s head-up/front display, which Google Maps does not support. These failures create a potential safety risk, as the sudden loss of navigation while driving can lead to distraction when attempting to reconnect the phone or restore system functionality. Additionally, interruptions in audio guidance and the occurrence of a completely black screen may further affect driving focus. The issue occurs intermittently and has happened multiple times during normal vehicle operation. It is not consistently reproducible. To date, the vehicle has not been inspected by a dealership, the manufacturer, police, or any insurance representatives in relation to this issue. In most cases, there are no prior warning lights or messages; the system simply disconnects or the display goes blaRead more

We recently bought this vehicle a few weeks ago. Since we’ve had it I have been noticing a strong smell of burning engine coolant. I have looked at similar models of the 2021 Range Rover sports and they do have recalls for issues with the engine coolant system. I assume this is the same problem the phev Range Rover sport is having. Today I have a low engine coolant message pop up on the dash.

While driving, the vehicle repeatedly triggered emergency braking without any obstacle present. The car continuously displayed warnings indicating pedestrians in front and behind the vehicle when no people were present. Due to these false detections, FSD became disabled during operation. The repeated sudden braking caused severe lower back pain, and I will be seeking medical treatment. This ve
... hicle is unsafe to operate and presents a serious safety risk to myself and surrounding traffic. I have full video evidence of the incidents.Read more

Vehicle: 2026 Honda Pilot – Mileage 20,847 Issue: Engine Failure, oil leaks, wheel alignment, air conditioning, major gas leaks from the engine, safety electronics alerts on front dash display are saying the vehicle is not safe to drive. History of issues: 03-11-2026: Car broke down while driving in [XXX] . 03-12-2026: Received call from Ira Honda service and was told they needed to replace th
... e engine. 04-04-2026: I was told the vehicle was ready to be picked up. I started driving home and found the vehicle was all over the road. I returned to Ira Honda and once again was given a loaner. Also, the engine was leaking oil. 04-09-2026: Ira Honda called and said one of the engine gaskets was defective and they needed to replace it. They said that now the alignment was back in spec. I was told that the 3 yr engine warranty begins anew. When I got home I looked more closely at the alignment document and saw that the alignment was still slightly out of spec. I called and was told that it was the best they could do. 04-10-2026: air conditioning not working and asked for them to fix the alignment. 04-13-2026: I again brought the vehicle in for service. But was told there were no loaners available. I arrived at 9:30am and at 1:00pm. The A/C was fixed and determined the frame was not set correctly when they replaced the engine. This was the reason the alignment was off by so much. So the alignment was fixed. 04-14-2026: Leaking oil. So again, I had to return to Ira Honda. No loaner was available. 04-18-2024: Saturday morning and we smelled gas in the garage. Started the car and it took about a minute to start. Asked if it was safe to drive, Ira service said yes. 04-20-2026: Started the vehicle up and same issue occurred. However, this time gas was leaking out. It leaked in our garage, on our driveway and all over the street. Again fixed. 04-29-2026: Car again failed to start. During startup the car leaked a large amount of gas. It is still at the dealership. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIONRead more

Cylinder 2 and 4 misfire, that will cause all the sensor lights to come on and the vehicle will stall out. Doesn't matter if driving 15 mph or 55 mph, it will just stall.

While cruising at 36mph, we decided to initiate the cruise control. Upon reaching a slow-down traffic ahead, the brakes were applied to slow the vehicle, however the cruise control did not disengage and the brakes did not stop the vehicle. The brakes were pressed three times and still the car could not stop. We were in a two-lane road with an opposite direction lane on the other side of the grassy
... median. We saw an opening in between the stopped vehicles and crossed over from right to left lane into the grassy median, hit a ditch, and did a sharp turn into opposing traffic, almost hitting another vehicle as they moved out of way, and stopped the vehicle on the opposite side of the road. When the vehicle hit the ditch crossing the median, the right front tire popped and helped in stopping the vehicle. Four family members including myself were in the vehicle and there were several cars already stopped in the lanes. This incident caused great safety concerns for my family and the vehicles around us. Had we hit the cars while crossing over, it would have caused a chain reaction of "hits" as the cars were already lined up and stopped. We brought the car to the Jeep dealership and they did confirm that the internal valve body failure and TCM fault contributed to improper communication within the transmission. They did replace the TCM and valve body assembly, reprogrammed the module and road tested the vehicle. The auto stop/start warning light came on after the incident. They performed a battery test for the main and auxiliary, and both failed load test and read at low voltages. Both batteries were replaced and fault codes cleared. Upon receiving the vehicle, I tested the cruise control component and it has been working for the past three months now.Read more

Driver's side seat belt lower attachment point. Has a flexible cable swage attachment to the lower seat belt and. This braided steel cable has failed at the swage. No accident involved. Just a fatigue failure over time. But since this is a safety system, I thought I should report it. It seems very similar to the earlier seat belt failures on the same GMC platform. The contact owns a 2023 Chevrolet Tahoe. The contact stated that while driving at 75 MPH, the vehicle shut off without warning. Due to the failure, the contact used excessive force to steer the vehicle off to the shoulder of the highway where the vehicle was then towed to a dealer. After the vehicle remained in the dealer's possession for over a week, the vehicle was diagnosed with an engine failu
... re. The contact was then informed that the vehicle was placed under recall NHTSA Campaign Number: 25V274000 (Engine) and that the engine would be replaced. Despite the engine repair, the contact stated that an abnormal, rattling sound would emit from the vehicle upon start of the engine. In addition, the contact stated that the vehicle would violently jerk upon depression of the accelerator pedal while at a complete stop. The failure started intermittently; however, the contact stated that overtime the failure became more persistent. The vehicle was taken to the dealer where they claimed that the transmission was four quarts low on transmission fluid; they then informed the contact that they could not guarantee that remedy would repair the failure. The contact stated that failure improved; however, she would still experience a slight hesitation upon depression of the accelerator pedal. The manufacturer was notified of the failure but offered no assistance. The vehicle was not repaired. The failure mileage was approximately 39,000.Read more

The electronic parking brake on my 2025 Toyota Grand Highlander intermittently fails to auto-release when shifting from Park to Reverse or Drive. Curiously, the system does respond to the gear shifter when returning to Park, as the brake will then engage or release as expected once cycled. This indicates a failure specifically in the transition out of the Park position. This intermittent delay or
... refusal to release prevents the vehicle from moving when expected, creating a potential safety hazard in traffic or during time-sensitive maneuvers. No dashboard warnings are present, and the issue occurs even when the driver's seat belt is buckled and the foot brake is fully depressed.Read more
