I am submitting a safety complaint regarding my 2020 Mercedes-Benz GLE 350 due to what appears to be a significant air conditioning refrigerant leak. The vehicle rapidly loses refrigerant shortly after being recharged. When the system is operating, a loud seeping/hissing sound can be heard from the HVAC system area, followed by complete loss of cold air within a very short period of time. This is
... sue appears to indicate a major refrigerant system failure and raises concerns about potential defects in the evaporator, condenser, hoses, or related AC components. The leak rate is excessive and ongoing. I am concerned this may be a broader manufacturing defect impacting other Mercedes GLE vehicles. The vehicle has been properly maintained, and the failure occurred under normal operating conditions. Given the frequency of similar complaints online involving Mercedes HVAC refrigerant leaks and evaporator failures, I believe this issue warrants investigation for a potential recall or manufacturer accountability. The failure impacts driver and passenger comfort and visibility in humid or high-temperature conditions due to inability to properly defog windows. I respectfully request that NHTSA investigate this issue for potential safety and defect trends affecting the 2020 Mercedes-Benz GLE platform.Read more

Purchased truck in May of 2026 from Chevy dealer. Truck had around 1500 miles on it. Within 1 week the truck was shifting, very roughly, for no apparent reason making it lurch. I brought it to a local Chevy dealer, who ran it, showing a fault. I got the truck back from the dealer, who said tech had looked at it, and cleared the "fault" that was showing up. less than 1 week after that, the truck st

I'm concerned the dealer I bought the truck from may have known about the problem when I purchased the truck, and this may be the 3rd time that the truck has been back to a Chevy dealer for this problem. I looked on line and found Chevy has has hundreds of thousands of recalls on this 6.2 liter L87 engine, and there is a class action suit involving this engine from previous years, as well as hundreds of complaints regarding the 2026 version of this engine.Read more

The product is the Sena Phantom ANC motorcycle helmet. There is a critical, life-threatening manufacturing and design defect regarding its internal structure. Beneath an extremely thin layer of cheek padding, there is a 2-centimeter-thick, rock-hard plastic internal chassis (honeycomb-like rigid structure) on both sides of the helmet. This rigid plastic component does not compress, deform, o
... r absorb energy upon impact. Instead, it creates a solid, non-yielding barrier directly against the rider's skull. During normal usage, this structure does not conform to the head and causes severe pressure sores and tissue irritation inside the cheeks within short periods. More critically, in the event of an accident, this hard plastic chassis acts as a dangerous focal impact point. Instead of dissipating crash forces like a standard EPS foam helmet liner, it transfers the entire impact directly onto the temporal/facial bones, posing an imminent risk of severe head trauma, skull fractures, and fatal brain injury. This rigid design completely contradicts basic helmet safety principles and ECE 22.06 / DOT energy-management standards. The defective helmet is currently kept in brand-new condition and is fully available for official laboratory inspection and impact testing upon request. An official investigation has also been initiated with the local Ministry of Industry and Technology regarding this batch. Sena must initiate an immediate global recall before this fatal flaw causes preventable rider casualties.Read more

[XXX] Fire occurred while driving [XXX] my Ram 2021 and it was like a mini blast with a lot of sparks as I saw in a rear-view mirror. Everything went out on the dashboard and the car stopped at the Top of Wolf Creek Pass [XXX] . I got out of the car and saw a small fire burning under the car. ... indicates an explosion that occurred simultaneously. Also, the fused edges of the round hole indicate a high temperature during this explosion and fire. I disagree that this fair damage [XXX] my Ram 2021 3.0 ecodiesel [XXX] is considered from Progressive insurance as simple "wear and tear." estimate only $1669,78. Garber Collision Pontiac repaired the Ram 2021 by 04.24.2026 and replaced the damaged wiring harness ($1161 self-pay) as required by Progressive. After this repair, the Ram 2021 was unable to move and was delivered to the Tucson Ram dealer on 05/04/2026. The dealer did not indicate the root cause of the damage in the estimate. The car was also returned to me by the dealer. When I asked the dealer on 05.12.2026 to indicate the root cause of the damage, the dealer replied that he was not aware of such a similar damage and indicated that the warranty on the car has expired and there are no open recalls. But The dealer's initial assessment from 05.13.2026 (estimates $9325.25) indicates the fire was caused by a failure of the Exhaust Gas Recirculation (EGR) cooler, which is a known issue (similar to recall VB1/19V-757) where the cooler cracks, leaks coolant, and causes a fire in the intake manifold. This is a component failure resulting in an explosion/fire, not typical wear. I need a diagnostic report that specifically mentions the EGR cooler failure as the source of the fire. I was driving home from work late at night in December 2025. A Signal came on that said pull to safety or something like that and then I lost power steering and brake control and my entire dash in my vehicle went pitch black. I was in the middle lane on route 290 and I was very lucky not to hit any other vehicles, but it was incredibly scary because I had no control of my vehicle and there were
... many cars and trucks on the road around me. I coasted off slowly to the side and I had to sit in 15° temperatures for an hour while I waited for AAA to come and tow my vehicle. They towed it to my home and were not able to get it off the ramp so they had to tow it to the dealership because the battery had died. The vehicle for a couple weeks before was slow to start, which I did not realize at the time was a sign that the battery was being drained. It was brought to the dealer and discovered that the DC DC converter and the BECM was the issue. My vehicle is open for inspection if you need it. The converter and BECM have been replaced after fighting with the dealer and with Landrover. Landrover is aware of the situation because they had to work with the dealer on their systems.Unfortunately, I am out almost $1000 because they made me pay for the replacement of the battery ( and the towing ) . NHTSA Safety Complaint Draft Vehicle: 2025 Ram 1500 VIN: [XXX] Mileage at First Incident: 3920 Mileage at Most Recent Incident: 13,000 (approx.) Complaint Summary: My 2025 Ram 1500 has a recurring safety critical defect that causes the truck to suddenly and unpredictably decelerate from highway speeds (around 70 mph to 55 mph) down to approximately 5 mph, even though the engine RPMs r
... emain around 4,000. This creates an immediate risk of being rear ended or causing a multi vehicle crash. Full Description of the Problem: While driving at normal highway speeds, the vehicle suddenly loses propulsion without warning. The truck slows rapidly to about 5 mph, dealership verbally described as “limp mode” to protect itself, despite the engine continuing to rev at high RPMs. During these events, the dashboard and infotainment system display incorrect and erratic information, including: - Transmission indicator switching into 4WD, Park, or 1st gear while the vehicle is moving - Navigation activating on its own - Screen freezing, glitching, or displaying inaccurate data This defect has occurred multiple times and is unpredictable, making the vehicle unsafe to operate. Repair Attempts: I have taken the vehicle to an authorized Ram dealership on multiple occasions for the same issue: - Repair Attempt #1: 03/20/2025 - Repair Attempt #2: 10/02/2025 - Repair Attempt #3: 10/30/2025 - Repair Attempt #4: 11/17/2025 - Repair Attempt #5: 05/12 2026 The dealership has replaced PCM harness, transfer case control module, DTCM, performed software updates and claimed the issue was resolved, but the defect continues to occur. They have also stated they “cannot replicate” the problem, despite multiple documented incidents. Safety Risk: This defect poses a serious risk of collision, especially at highway speeds. The sudden loss of propulsion and incorrect gear display could easily result in a rear end crash INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My 2022 Kia Carnival, approximately 80,000 miles, had intermittent dashboard warning lights and grinding brake noise for a few weeks. I brought the vehicle to Tasca Kia White Plains first time on 4/14/26 then again on 5/8/26. The dealership initially could not reproduce the issue, then later suspected an ABS sensor issue and ordered the part. On 5/12/26, before the repair was completed, I was dri

At that time, the brakes would not respond normally, and I could not immediately shift the vehicle into Park. Police and towing services were involved. The dealership later replaced an ABS sensor. However, when I reviewed the codes with the service manager, I was told the ABS sensor code did not appear on the date of the brake incident. I was also told that an ABS sensor failure itself would not typically cause hydraulic brake failure. I am concerned that the sensor replacement does not explain the brake failure and that this may involve a brake/ABS/electronic stability control safety issue.Read more

The engine failed due to complete and sudden oil loss. The vehicle is currently at Fremont Hyundai and available for inspection. Safety risk: the engine failed while driving on the highway with children in the vehicle. A check engine light appeared with no prior warning, the vehicle immediately lost power and stalled, requiring an emergency exit and tow. This is a direct collision risk at highway
... speed. The failure was confirmed by an independent mechanic who found zero oil in the engine and identified it as a known Hyundai defect. The vehicle was then towed to Fremont Hyundai, which is currently conducting diagnostics. The vehicle has been inspected by an independent mechanic and by Fremont Hyundai [XXX] ). No police or insurance involvement. There were NO warning lights, oil pressure alerts, or symptoms of any kind prior to failure. I purchased the vehicle in 2024 and maintained regular oil changes and checkups with receipts. The last oil change was performed a few months before the failure. No mechanic ever flagged low oil. The only warning was a check engine light that appeared seconds before the engine stalled. I have incurred $900 in towing costs as a direct result of this failure. Fremont Hyundai is now requiring me to pay $280 for a diagnostic, followed by $890 for a two-week oil consumption test. They have also stated that if the engine does not pass the initial diagnostic, they cannot help me at all. Given that the engine ran completely dry of oil, it is highly unlikely to pass. This means I am being asked to pay fees for a process the dealership already expects to fail, leaving me with no path to resolution and a car I cannot drive. The 2015 Hyundai Santa Fe is included in a class action lawsuit (Cho et al. v. Hyundai Motor Company) over this exact defect. Incident date: May 17, 2026
I am writing to formally report a persistent and dangerous manufacturing defect regarding the rear window and seating assemblies in my 2020 Chevrolet Silverado. I have already had GM replace the rear window in 2023, it needs replaced again now in 2026. The rear window seal is failing, allowing a significant amount of rainwater to leak into the cab. Because of this ongoing water intrusion, the stru
... ctural base and brackets of the rear seats have developed severe rust. Furthermore, I am deeply concerned that this constant moisture saturation is creating an unseen mold hazard within the cabin, which poses a direct health and respiratory risk to my passengers. This leak also risks damaging sensitive electrical components and wiring harnesses located under the rear seats and trim panels, potentially causing short circuits and sudden failures of critical safety systems while driving. Many other owners of this generation Silverado have reported identical issues, including cracked plastic frames around the sliding rear glass that cause the leaks. Because this defect compromises vehicle integrity, creates a risk of electrical failure, and poses health hazards from mold, I am requesting that the NHTSA investigate this issue as a potential safety defect requiring a manufacturer recall.Read more

I have a video of the gears and shaft here: [XXX] I saved the parts and will be happy to provide them for investigation if needed. I am reporting a serious safety concern with my 2024 Jeep Wrangler 4xe, VIN [XXX] . The apparent systems involved are the hybrid propulsion system, charging/high-voltage battery system, powertrain control system, and/or regenerative braking/deceleration system. The vehicle is available for inspection upon request. After prior battery/charging-related issues and recent Jeep recall service, the vehi
... cle displayed a “Service Charging System” warning and the check-engine light came on. While driving home, the vehicle did not decelerate normally when I removed my foot from the accelerator. It felt like the Jeep continued propelling itself or maintaining speed without accelerator input. I drove approximately one mile down my street at about 40 mph, and the vehicle did not slow down as expected unless I applied the brakes firmly. I had to brake harder than normal to slow the vehicle. This created a serious safety risk because the vehicle appeared to continue moving at speed without normal deceleration, which could have caused a crash or prevented me from stopping in time. After the incident, the vehicle also turn off. It took multiple tries and about 20 minutes to turn off. The “Service Charging System” message and check-engine light remained on. The problem has not yet been reproduced or confirmed by a dealer or independent service center, but I am arranging for the vehicle to be towed/inspected and do not intend to drive it due to the safety concern. The vehicle was recently inspected/serviced by a Jeep dealer for recall-related work before this incident. I am requesting that the dealer/manufacturer inspect the vehicle and preserve all DTCs, freeze-frame data, module update records, HV battery/charging diagnostics, and any available accelerator/brake/propulsion data. No crash or injury occurred, but the vehicle created a serious safety risk. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The vehicle displays an “Engine Maintenance Required” warning message shortly after the air conditioning is turned on. The warning does not usually appear immediately at startup but consistently appears when the A/C system is activated. The vehicle currently remains drivable, but the issue appears related to the engine cooling system and may involve the coolant bypass valve/flow control valve,
... which has been widely reported by other owners of similar model vehicles. The problem raises safety concerns because a cooling system malfunction could potentially lead to overheating while driving. The issue has been reproduced multiple times. A generic code scan at an auto parts store did not show stored engine codes. The vehicle was taken to a Toyota dealership and Toyota customer service was contacted regarding known coverage programs related to this issue, but the VIN was reportedly not included. Warning symptoms first appeared during normal driving and continue intermittently whenever the air conditioning system is used.Read more

The front upper grille deflector detached and hit my windshield and bounced off behind me during a drive at normal highway speeds. This created a random flying debris object that appears to have been a problem with the 2022 and later Sierra 1500 models; however, the recall only appears to address the 2022 model. It also appears quite common for the 2024 year model as well. Vehicle has never been

While driving, the vehicle suddenly and without warning experienced a severe, dangerous loss of acceleration. The dashboard immediately displayed a "Reduced Engine Power" warning message along with a "Service StabiliTrak" light, forcing the vehicle into an immediate electronic fail-safe "limp mode." This sudden, drastic reduction in engine power severely restricts the vehicle's ability to maintain
... safe roadway speeds, accelerate from a stop, or merge safely into traffic, posing an immediate rear-end collision hazard to myself and other motorists. Furthermore, the defect simultaneously disables critical active safety systems by shutting down the StabiliTrak and Traction Control networks. A diagnostic scan revealed code P2138 (Accelerator Pedal Position Sensors 1-2 Correlation Not Plausible). Investigation reveals that this is a widespread, systemic safety defect caused by design flaws in the vehicle's body. Rainwater and moisture from clogged factory sunroof drains or cowl seams leak directly down into the interior cabin wall, pooling into the primary instrument panel-to-body wiring harness block (X218 connector) in the driver's side footwell. This water intrusion causes green corrosion on the delicate pins, short-circuiting the 5-volt reference sensor circuit for the throttle controls while the vehicle is in motion. General Motors is fully aware of this exact defect and has documented it internally via Technical Service Bulletin (TSB) #10-00-89-017H (and related GM TSBs). Because this defect causes unexpected, immediate engine failure and disables traction safety networks while driving, it constitutes a severe safety hazard that should be escalated to a mandatory safety recall.Read more
