The engine in my 2017 Ford Escape EcoBoost experienced a known “coolant intrusion” defect in which engine coolant leaks into the engine cylinders. This defect is widely associated with the 1.5L and 2.0L EcoBoost engines and can cause engine misfires, coolant loss, overheating, corrosion, and catastrophic engine failure requiring engine or short-block replacement. The vehicle is available for i
... nspection upon request. The vehicle is driven by a college student traveling away from home. The coolant intrusion defect creates a serious safety concern because the engine can suddenly misfire, lose power, overheat, or fail unexpectedly while driving. A sudden engine failure could place the driver and others at risk, especially during highway driving or in remote areas. The issue developed shortly after the vehicle was purchased from a dealership in June 2025. The symptoms are consistent with the widely documented Ford EcoBoost coolant intrusion defect acknowledged by Ford through Customer Satisfaction Program 21N12. The vehicle has exhibited symptoms including coolant loss and engine performance concerns consistent with this known defect. The dealership has been notified of the issue. The dealership has been contacted regarding the issue. Ford has previously acknowledged this defect through Customer Satisfaction Program 21N12 relating to coolant intrusion in affected EcoBoost engines. The vehicle experienced symptoms consistent with coolant intrusion, including coolant loss and engine performance concerns/misfires. These symptoms are widely reported with the affect EcoBoost engines and has progressed to severe engine failure.Read more

The vehicle has experienced repeated failures associated with diagnostic trouble code P06DD. When the condition occurs, the vehicle is unable to exceed approximately 3,500 RPM and experiences engine stalling and loss of power while driving. The issue is intermittent but has occurred repeatedly. The vehicle has been presented to authorized Dodge/Stellantis dealerships on five separate occasions fo
... r diagnosis and repair. The oil pump was previously replaced for this issue at approximately 5,000 miles, yet the condition continues to recur. Most recently, the dealership advised that the issue was resolved and returned the vehicle to me. Within approximately 30 minutes of leaving the dealership and driving on the freeway, the check engine light returned and the same stalling and loss-of-power condition immediately reappeared. The defect creates a significant safety concern because the vehicle loses the ability to accelerate normally while operating in traffic and on freeways. When the condition occurs, the vehicle cannot exceed approximately 3,500 RPM stalls while driving, creating a risk of collision. The vehicle currently has approximately 21,000 miles and remains under manufacturer warranty. Despite multiple repair attempts, the defect has not been permanently corrected.Read more

The contact owns a 2018 Jeep Cherokee. The contact received notification of NHTSA Campaign Number: 25V011000 (Power Train), and the part was listed as available at the time. The vehicle was taken to a dealer where it was diagnosed to have the PTU/full transmission failure as listed in the recall notice; however, the vehicle was not repaired due to the dealer not being able to remedy the part. The
... part was ordered. The contact stated while driving the vehicle at an undisclosed speed on the highway, she attempted to accelerate but no power was given. She pulled off to the side, and authorities were called for traffic control. The vehicle was unable to move. When engaging into drive, the vehicle sounded like it was coming apart. The contact stated the vehicle was not able to engage its own brake. When the vehicle was towed, there was no evidence that the wheels were locked up. Shortly after that, the vehicle's PTU became fully inoperable, causing the vehicle to not be able to be placed in park or any other driver setting. The vehicle was towed to the dealer a second time. The tow company parked the vehicle at the dealer on a hill. With a failed transmission, this caused the vehicle's E-brake to disengage, causing it to roll off the hill and stopped rolling shortly after once on flat ground. What component or system failed or malfunctioned, and is it available for inspection upon request? Two components failed simultaneously: (1) the rear passenger side tire experienced a catastrophic blowout while traveling at 65 MPH on the highway, and (2) the Tire Pressure Monitoring System (TPMS) failed to detect or warn of any tire pressure abnormality before or after the blowout. Following the b
... lowout, the vehicle’s diagnostics screen displayed “All Systems OK” while the instrument cluster simultaneously showed a “Low Tire Pressure” warning — indicating a TPMS malfunction. The original blown tire has been retained and is available for inspection upon request. All four tires have since been replaced. How was your safety or the safety of others put at risk? A tire blowout at 65 MPH on a highway created an immediate risk, I briefly lost control of the vehicle as it became wobbly and unstable to steer, we were at at risk of collision, or rollover. Two minor children were passengers in the vehicle at the time. The simultaneous TPMS failure provided no advance warning, eliminating any opportunity to safely reduce speed or pull over before the blowout occurred. The vehicle had been in my possession for approximately two-three hours at the time of the incident. Has the problem been reproduced or confirmed by a dealer or independent service center? UNKNOWN Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? UNKNOWN Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warning lamps, messages, or symptoms appeared prior to the tire blowout. The TPMS provided zero advance warning. After the blowout had already occurred and the vehicle was stopped on the roadside, the instrument cluster displayed a “Low Tire Pressure” warning. However, simultaneously, the Vehicle Diagnostics screen displayed “All Systems OK” with a green check-mark.Read more

These DTCs now show as Historical because the vehicle has not been operated since the last fault was recorded. Their Historical status only means the system has not run a new check — it does not mean the defect is repaired. The vehicle has also exhibited brake‑logic abnormalities, including a persistent Brake‑ON signal at highway speeds, which violates Brake‑Throttle Override safety logic and affects ADAS functions. The powertrain shows signs of torque converter instability, mechanical drag, abnormal slip, and heat buildup documented across these dates. Two OTA updates were performed during periods of instability, but neither corrected the defect. The dealership has not provided documentation confirming the safety issues were resolved. What component or system failed or malfunctioned, and is it available for inspection? The forward collision avoidance (FCA) system malfunctioned. The front camera system appears to have triggered unintended hard braking beyond normal driver input. The vehicle is available for inspection upon request. How was safety put at risk? While approaching a vehicle in front of me, I was gradually slowing d
... own and applying the brakes normally. Without warning, the vehicle suddenly applied significantly stronger braking than intended. This abrupt and excessive braking caused the vehicle behind me to collide with my car, creating a dangerous situation and increasing the risk of injury. Has the problem been reproduced or confirmed? UNKNOWN. The issue has not been reproduced since the accident, and it has not yet been confirmed by a dealer. However, the incident is consistent with Hyundai Recall NHTSA Campaign Number 26V316000, which describes unexpected and premature activation of the forward collision avoidance system. Has the vehicle been inspected? The vehicle has been inspected by insurance representatives following the accident. It has not yet been inspected by Hyundai or a dealership specifically for this recall-related issue. Were there any warning signs before the failure? There were no warning lights, alerts, or messages prior to the sudden braking event. The braking intervention occurred without advance notice. Additional description of incident: At the time of the incident, I was approaching a vehicle ahead and applying the brakes gradually under normal driving conditions. The vehicle then suddenly applied much stronger braking than expected, consistent with automatic emergency braking intervention. This abrupt deceleration caused the vehicle behind me to rear-end my car. This behavior matches the defect described in Hyundai Recall 26V316000, where the forward collision avoidance system may activate unexpectedly and apply excessive braking.Read more

All brake systems warning lamps illuminated while driving on the highway, the vehicle was pulled off the road and shut off, brakes were visually inspected and all fluid levels were examined and everything was intact. Vehicle was driven again after a breif time and everything was fine. A few weeks later all warning lamps were displayed, the vehicle was brought to a Honda dealership the following mo
... rning and a cracked fuel pump was diagnosed and the technician believed that there was no coolant in the vehicle and that was filled. The technicians suggested a scope of the engine to rule out a leaking head gasket but I could not afford it and monitored my coolant levels, had a pressure test done at a trusted mechanic and everything was fine for approx 3 months. Absolutely no coolant was observed on my driveway, the coolant level remained optimal on visual inspection. The vehicle started to drive roughly and I suspected new spark plugs were needed, those were installed at the dealership. The issue was not resolved and I returned w/in 1 month and was told that 3/4 cylinders were blown in addition to the head gasket. The vehicle has 106k miles and has received all maintenance as described. The cost to repair is over $8,000. I believe this is a failure on Honda as this is a very well documented issue with this specific vehicle.Read more

Engine has coolant leaking into the third cylinder. This explains why my vehichle runs rough, coolant level drops with no sign of leaks underneath the vehicle and white exhaust smoke. Apparently its a known issue with the 2.0 L Ecoboost engines and is addressed in a technical service bulletin (TSB). To correct coolant intrusion into the cylinders you have to replace the long block engine assembly
... . The issue started in Texas about a year ago and a code appeared for misfire in cylinder 3. Spark plugs and ignition coil was replaced only for the code to appear again the next week. Twice a month we would have to top off the coolant. It wasnt until about 2 weeks ago when smoke was coming out of the exhaust and was advised it is not safe to drive.Read more
