The dealership's diagnostic tools are falsely reporting that the A11 safety recall update was successfully completed when it has not been, leaving my vehicle's camera and instrument panel vulnerable to failure. I have attached the service invoice as proof of the failed repair attempt. I have contacted Acura Customer Relations multiple times and advised that the recall is not complete. Prior to towing the vehicle to Hendrick Jaguar Land Rover Charlotte, I performed a live data diagnostic using a Land Rover specific iCarsoft LR V3 diagnostic tool which confirmed the 48V hybrid battery at 17.86% state of charge critically below the minimum threshold required to authorize a start sequence. The TCU, BCCM, and VIM all showed not fitted simultaneously a known cascade effect of a critically depleted 48V MHEV system. Recall D126 for DC/DC Converter failure was confirmed on my vehicle. The DC/DC converter manages power transfer between the 48V and 12V systems. Its failure directly caused the 48V battery depletion and subsequent cascade module failures. Safety risk: The vehicle became completely immobile with no warning. The immobilizer went offline preventing the vehicle from starting, stranding the occupant in a parking lot. A vehicle that shuts down without warning and cannot be restarted poses a serious safety risk to the driver and other road users. Additionally the DC/DC converter failure affecting the MHEV system represents a risk of sudden loss of hybrid assist during driving. After accelerating away from a stop sign, the vehicle was traveling at approximately 20-25 mph when, simultaneous with a warning message, it came to an immediate and abrupt stop. After several additional failed attempts to restart the vehicle, the driver allowed the vehicle to sit idle for approximately five minutes. The driver was then able to restart the vehicle and drive it, very cautiously and slowly, approximately two miles to a safe location. Once in a safe location, driver contacted the dealer who confirmed that the vehicle was unsafe to operate. The vehicle was towed to the dealership. On May 7, 2026, the dealer informed us that they had found “several different faults stored in the vehicle but those faults all basically contradict each other.” The dealer further informed us that they would need to contact Audi in Germany for guidance on how to proceed with diagnosis and repair. I just had the tires road force balanced on a haweka with lug-centric balance, and the steering wheel persists shaking from 55-65 mph. From what I have seen is this is a design flaw on this generation of 4 runners called the shimmy. For a newer vehicle this is not only dangerous, but unacceptable to not be considered for a recall when it is this widespread.

FIRST ISSUE-THIS REPORT - LOSS OF POWER / ELECTRICAL FAULT (4/10/26) On April 10, 2026, I backed out of a parking space into a drive lane in a commercial parking lot. As the vehicle was rolling, it suddenly lost power. I attempted to restart the vehicle, but it would not start. The vehicle continued rolling backward into the intersection. Two warning messages appeared on the dashboard -"Stop Safel
... y/Electrical Fault Detected" "Stop Safely – Loss of Drive".Photos attached. Because the vehicle could not be restarted or moved, it remained disabled in the intersection for approx two hours. The vehicle had to be towed to the dealership. When the vehicle was taken to the dealership, I was later told that no problem could be found. The dealership reportedly ran a computer diagnostic test & stated that nothing was wrong with the vehicle. They performed routine maintenance, charged more than $900, & returned the vehicle to me without identifying or repairing the cause of the loss-of-power event. SECOND ISSUE – INTERMITTENT SENSOR FAILURES - The vehicle's front & rear sensors operate intermittently. I scheduled a service appointment for May 20, 2026, to address this issue, but I was forced to cancel a I did not have transportation available. THIRD ISSUE (reported to nhtsa.gov) BRAKE SLAMING/SUDDEN STOP (5/2/26) On 5/2/26, the vehicle experienced a brake slamming/sudden stop while I was driving through an intersection. Following this incident, I developed neck pain & headaches. The vehicle was towed to the dealership for a second time. During discussion with the dealership, I was informed that the April 10, 2026 service visit had been documented as relating to a "check engine" light. I want to make clear that the warning messages displayed during the April 10 incident were not a check-engine light. This type of transfer case is not installed in all F-150 trucks, only the upper models like the Lariat or Platinum models. The dealership installed BG 792 plus they mixed in BG 328 additive per a universal guideline published by BG products. This mixture doubles the viscosity of the fluid recommended by Ford, which is Mercon LV (Low Viscosity). The guidance was intended for less complex or manual type transfer cases not the Borg Warner 44-67. The installed mix of BG 792 +328 could cause a slower engagement and inconsistent operation of the Traction-On-Demand system during wet or icy conditions. The Borg Warner 44-67 uses speed sensing and injects low viscosity fluid through portsinto a "wet clutch" pack, and the hydraulic pressure is modulated by a specifically programmed PID loop. The hydraulic system has not been tested with the BG mixtures, that doubles the viscosity of the fluid. The BG 328 additive is specifically designed for friction type clutching plates, and could cause damage to the "wet clutch" pack found in the Borg Warner 44-67. I have notified the dealership and BG products of the misinterpretation of the BG guidance but they have claimed that the fluid mix they used "should be ok". They can not, and/or will not provide the chemistry calculation or testing protocols for the mix used, especially with BG 328 added into BG 792.Read more

A dashboard warning appeared: “gas and brake pedals are being pressed simultaneously.” I was not, and had not been, touching the accelerator. I then heard engine RPM increase audibly, consistent with an acceleration command, while my foot remained fully on the brake. I felt forward thrust working against my braking. The warning cleared and brakes engaged. Vehicle stopped approximately half a car length from the vehicle ahead, narrowly avoiding rear-end collision. Automatic Emergency Braking did NOT engage despite conditions clearly meeting its criteria — stationary vehicle directly ahead, my vehicle approaching at 15-20 mph with no observable deceleration for 3-4 seconds. No collision warning sounded; no auto-braking intervened. Researching it on YouTube states that it’s the camera on my windshield getting too hot and it’s a known issue. My steering is sticking while driving on the highway while the engine is warm and going over 45 mph. We are seniors and generally only drive locally. I was driving down an empty two lane road the truck went from 9th to 10th gear. As it went into 10th the transmission shifted hard and then locked up. This caused my rear wheels to lock up and I skid off the road. Eventually it shifted back down into a lower gear where I could slowly drive into a safe place.
